INTRODUCTION: The incidence of conversion from a laparoscopic to an open approach during nephrectomy is reported at 6-8%. Conversion to an open procedure may be necessary to control haemorrhage or allow progress in dissection but the well established benefits of minimally invasive surgery (MIS) are obviously lost. Hand-assisted laparoscopy (HAL) also offers the benefits to the patient of MIS. We have used HAL to convert from the pure laparoscopic approach during difficult nephrectomies, rather than converting to traditional open surgery.

BACKGROUND: Small bowel obstruction (SBO) arises on various backgrounds, and the surgical procedure is often modified intraoperatively as needed. Single-incision laparoscopic surgery (SILS) is less invasive than conventional multiport laparoscopic surgery (MPS) and reported to be equally safe and efficient. We have been applying SILS to SBO requiring surgical treatment, and we conducted a retrospective study to clarify the role of SILS in the management of SBO.
METHODS: Thirty-four consecutive patients were identified for inclusion in the study through a review of hospital records of patients

OBJECTIVE: To identify preoperative factors associated with surgical time and conversion of the laparoscopic cholecystectomy (LC) to open surgery in subjects with acute cholecystitis (AC).
METHOD: We developed a cross-sectional study that included 99 subjects older than 17 years with definitive diagnosis of AC who had undergone to LC. Preoperative variables such as clinical data, laboratory markers and ultrasound findings as wall thickness, the size of the major calculus and the presence of: perivesicular fluid, multiple cholelithiasis, biliary mud or microlithiasis were registered. We consid

BACKGROUND: Robotic surgery has been proposed as an approach to mitigate open surgery, which is associated with increased morbidity and worse outcomes when compared to minimally invasive cholecystectomies. The study objective was to determine the effect on conversion rates and outcomes following the adoption of robotic surgery for benign gallbladder disease in a high-risk population.
METHODS: Patients ≥ 18 years of age who underwent cholecystectomy for benign gallbladder disease from January 1, 2013 to April 18, 2025 at a Veterans Affairs hospital were retrospectively identified. Primary outc

BACKGROUND: Although endovascular aneurysm repair (EVAR) for abdominal aortic aneurysm (AAA) is widely used worldwide, the fact that it is associated with increased rates of reintervention has been considered a problem. This study aimed to analyze the outcomes of primary open AAA repair and open conversion with explantation of stent grafts after EVAR.
METHODS: In this retrospective study, we enrolled 1,120 patients (open repair, n = 664; EVAR, n = 456) who underwent AAA repair at Kobe University from 1999 to 2019. Of the 664 patients who underwent open repair, 121 (patients who underwent prim

BACKGROUND: Pre-operative prediction of a difficult laparoscopic cholecystectomy (LC) can help the patient as well as the surgeon prepare better for the intra-operative risk and the risk of conversion to open cholecystectomy.
METHODS: In 105 eligible patients who underwent LC during May 2001 to January 2003, patient characteristics, clinical history, laboratory data, ultrasonography results and intra-operative details were prospectively analyzed to determine predictors of difficult LC.
RESULTS: Of 105 patients, 12 (11.4%) required conversion to open cholecystectomy. Significant predictors of

Conversion from minimally invasive techniques to open surgery has mostly been considered as an undesirable event associated with intraoperative complications or poor preoperative planning. However, the impact of conversion to open surgery during adrenalectomy for large adrenal tumours remains unclear. This study investigates the outcomes of conversion from minimally invasive to open surgery for adrenal masses ≥ 60 mm with an additional focus on the identification of survival predictors in patients with large adrenocortical carcinoma (ACC). We retrospectively analyzed 97 patients who underwent

AIM: Aim of this study was to evaluate whether timing of laparoscopy lasting longer than two hours before converting to open surgery can worsen the postoperative course during laparoscopic cholecystectomy (LC) for acute cholecystitis (AC).
MATERIALS AND METHODS: Medical records of 1,161 patients who underwent urgent laparoscopic cholecystectomy for AC during the period 2001-2017 were retrospectively analyzed. A conversion to open surgery was performed in 70 (6%) patients. Among these, two groups of patients were identified: group 1 (n=51; 73%) included patients who underwent laparotomy within

BACKGROUND: Thoracoscopic surgery has been widely performed. However, sometimes open conversion becomes necessary to manage complications. We have been applying thoracoscopic technique to manage complications to minimize the rate of open conversion.
METHODS: Thoracoscopic suture technique in addition to some other technique has been used to control intraoperative bleeding, nerve injury and bronchial damage. Pulmonary artery bleeding could happen in thoracoscopic surgery. First step is to give compression to the bleeding point. Usually the lung nearby is used then a gauze is taken over. A piec

OBJECTIVE: Laparoscopic cholecystectomy (LC) has become the gold standard for the surgical treatment of gallbladder disease, but conversion to open cholecystectomy is still inevitable in certain cases. Knowledge of the rate and impact of the underlying reasons for conversion could help surgeons during preoperative assessment and improve the informed consent of patients. We decided to review the rate and causes of conversion from laparoscopic to open cholecystectomy.
METHOD: This study included all laparoscopic cholecystectomies due to gallstone disease undertaken from May 1999 to June 2010. T

INTRODUCTION: Single-incision appendectomy (SIA), including laparoscopic and laparoscopic-assisted techniques, offers superior cosmetic outcomes and is increasingly applied to both uncomplicated and complicated cases of acute appendicitis. In our practice, transumbilical laparoscopic-assisted appendectomy is the initial approach for all patients with acute appendicitis. This retrospective cohort study aimed to identify risk factors associated with conversion from SIA to multiport appendectomy (MPA).
METHODS: We retrospectively reviewed medical records of patients aged < 15 years who underwent

BACKGROUND: Reversal of Hartmann's procedure is technically demanding and occasionally performed via open surgery, which is associated with substantial morbidity. Minimally invasive approaches have expanded, yet comparative evidence between robotic and laparoscopic reversal remains limited. We conducted a systematic review and meta-analysis to compare perioperative outcomes of both approaches.
METHODS: Databases were searched through 20 July 2026. Studies reporting outcomes after laparoscopic and/or robotic Hartmann's reversal were included. Random-effects meta-analyses were performed with pr

BACKGROUND: Conversion from laparoscopic to open cholecystectomy (LC to OC) is an important surgical outcome that reflects procedural complexity and patient risk. While conversion predictors have been studied internationally, context-specific data from Iraq remain limited.
OBJECTIVES: To determine the conversion rate in an Iraqi cohort, identify independent preoperative predictors, develop a simplified risk score, and systematically categorize intraoperative conversion reasons.
METHODS: A retrospective cohort study was conducted on 945 consecutive patients who underwent LC between December 2

BACKGROUND: The use of laparoscopy for right hemicolectomy has gained popularity allowing the option of a totally laparoscopic intracorporeal anastomosis (IA) for intestinal reconstruction. This technique may alleviate some of the technical limitations that a surgeon faces with a laparoscopic-assisted extracorporeal anastomosis (EA).
METHODS: A retrospective chart review of 195 consecutive patients who underwent laparoscopic right hemicolectomy by four colorectal surgeons at three institutions from March 2005 to June 2014 was performed. Multivariate regression analysis was used to compare pos

BACKGROUND: Minimally invasive (MIS) cholecystectomies have become standard due to patient and hospital advantages; however, this approach is not always achievable. Acute and gangrenous cholecystitis increase the likelihood of conversion from MIS to open cholecystectomy. This study aims to examine patient and hospital factors underlying differential utilization of MIS vs open cholecystectomies indicated for acute cholecystitis.
METHODS: This is a retrospective, observational cohort study of patients with acute cholecystitis who underwent a cholecystectomy between 2016 and 2018 identified from

BACKGROUND: Conversion to open surgery forfeits any potential benefits of minimally invasive liver surgery (MILS), however, it remains uncertain whether the conversion itself introduces additional risk. The impact may differ depending on urgency (emergency or elective) and surgical approach (robotic or laparoscopic liver resection). This study aimed to evaluate outcomes of emergency and elective conversions in robotic liver resection (RLR) and laparoscopic liver resection (LLR).
PATIENTS AND METHODS: Data from 34 international centers of patients undergoing converted MILS procedures (stratifi
