Psychological, neurophysiological and therapeutic aspects of chronic pain: preliminary results with transcutaneous electrical stimulation [proceeedings].

Psychological, neurophysiological and therapeutic aspects of chronic pain are reviewed in the light of recent progress achieved in the respective fields (alpha-feedback training; gate-control theory; transcutaneous electrostimulation; percutaneous stereoactic radio-frequency cordotomy). The efficacy of selective large fibre stimulation has been evaluated in 39 spinal cord injury patients suffering from chronic intractable pain of 6 to 35 months's duration. Stimulation was applied daily for 6 consecutive hours. Pain reflief was assessed by verbal and visual analogue scales and McGill's pain que

Minimally invasive device to effect vocal fold lateralization.

Despite many operative procedures focused on vocal fold lateralization, none has achieved an acceptable level of dependability. Bilateral vocal fold abductor paralysis is treated by arytenoidectomy, cordotomy, suture lateralization, or partial cordectomy. Tracheotomy remains the gold standard for maximizing the airway and preserving phonatory function. We have developed a device that is minimally invasive, tunable, and reversible, with the potential for lateralization or medialization of the vocal process. The device consists of a polyethylene collar, a Vitallium cam, and a double-helix core f

Thyrocricotracheal separation with bilateral recurrent laryngeal nerve transsection: report of a survivor.

Thyrocricotracheal separation is an extremely fatal injury that has not been reported in the literature. Although timely and proper management of this injury is paramount to preserve the patient's life, airway, and voice, its rarity has resulted in a lack of consensus regarding the best management option. We report a case of thyrocricotracheal separation with bilateral recurrent laryngeal nerve transsection caused by a self-inflicted injury, which was treated with reanastomosis in conjunction with transverse laser cordotomy. The patient could achieve both decannulation and a serviceable voice

Repair of Posterior Glottic Stenosis with the Modified Endoscopic Postcricoid Advancement Flap.

Posterior glottic stenosis is a difficult clinical problem that frequently results in bilateral vocal fold immobility, dyspnea, and tracheostomy dependence. Charts were reviewed of all patients undergoing a modified endoscopic postcricoid advancement flap procedure for posterior glottic stenosis between October 1, 2003, and June 30, 2015. Age, sex, operative findings, complications, and outcomes were noted. Follow-up was available in 10 of 11 patients, 10 of whom were successfully decannulated. There were no complications. Regular diet was maintained in all patients. In patients with bilateral

Hypoventilation after high unilateral cervical chordotomy in a patient with preexisting injury of the phrenic nerve.

Unilateral cervical chordotomy for the relief of intractable pain is a well accepted procedure but is not without hazard. Postoperative respiratory failure is not an uncommon occurrence, but the likelihood increases with a number of factors, particularly preexisting pulmonary abnormalities or previous contralateral cervical chordotomy. Preoperative assessment of the pulmonary function of patients who are about to have cervical chordotomy is emphasized to predict and anticipate potential postoperative respiratory failure.

Deciphering the enigmatic symptoms of Pancoast tumors: Navigating the complex landscape of pain management-A case report.

Pancoast tumors, also known as superior sulcus tumors, encompass a diverse spectrum of neoplasms that infiltrate the apex of the chest wall, yielding distinctive clinical presentations. One of the earliest signs of tumor growth is pain radiating to the upper limb, stemming from peripheral nerve involvement, which can mimic joint pain or spinal radicular irritation. In this case report, we present the clinical history of a 64-year-old female smoker who had previously been recommended for orthopedic elbow surgery due to epicondylitis. Subsequent to the development of additional symptoms and rigo

Concurrent glottic and tracheal stenoses: restoration of airway continuity in end-stage malignant disease.

Six patients known to have inoperable esophageal carcinoma presented with stridor due to both malignant tracheal stenosis (with additional respiratory-digestive tract fistula in 2 patients) and bilateral vocal cord paralysis. Patency was restored by endotracheal stenting plus unilateral cordotomy. Four patients had immediate relief. Two patients required enlargement of the vocal cord incision. One of them declined reoperation and underwent tracheostomy. The stent function was uneventful, with no dislodgment or mucus impaction. The fistula seal was complete, with no aspiration through the newly

PAIN MECHANISMS - A REVIEW: II. Afferent pain pathways.

This paper reviews some major advances in our understanding of the organization of afferent pain pathways, and relates these and other findings to the limited success rate achieved by various surgical interventions used in the treatment of chronic intractable pain. First-order pain afferents, many of which use the transmitter substance P, may enter the spinal cord via both the dorsal and ventral roots. After terminating superficially in the dorsal horn, information may apparently ascend to higher centres via numerous pathways, some of which are contralateral some ipsilateral. The preferred rou

Immediate response to expiratory threshold load.

In anesthetized rabbits, cats and dogs the end-inspiratory volume referred to normal FRC (deltaVe.i.) of the first breath during expiratory threshold load (ETL) was greater than control deltaVe.i. The increase of deltaVe.i. was roughly proportional to the increase of end-expiratory volume relative to normal FRC (deltaVe.e.) caused by ETL. During squeezing of the rib cage or of the abdomen or after cordotomy at T1 the phenomenon occurred nearly to the same extent. This suggests that afferent impulses from the chest wall are not essential in determining the phenomenon. The first inspiration duri

Endoscopic laser medial arytenoidectomy for treatment of bilateral vocal fold paralysis.

Endoscopic laser medial arytenoidectomy for bilateral vocal fold paralysis has the advantage of preserving the structure and the position of the vocal fold, contrary to a transverse cordotomy or total arytenoidectomy. Our objective was to evaluate the functional results of this procedure. This is a prospective non-randomized study. Twenty patients were included: five patients had a tracheotomy preoperatively and 15 patients had dyspnea on exertion. Acoustic voice measurements, spirometric parameters and the voice handicap index 120 (VHI), were evaluated 1 week before surgery and 3 months after

Congenital bilateral vocal fold paralysis and Charcot-Marie-Tooth disease.

We present the case of a patient with Charcot-Marie-Tooth disease (CMT) type 1 with congenital bilateral vocal fold paralysis in order to emphasize the treatment options and long-term outcome. The case is reviewed with regard to presentation, differential diagnosis, and treatment. We also reviewed the literature to determine the frequency of congenital and childhood presentations of bilateral vocal fold paralysis associated with CMT, most specifically CMT type 1. We found only 14 children reported to have bilateral vocal fold paralysis associated with CMT, and only 1 of these cases was associa

Teflon granulomas and overinjection of Teflon: a therapeutic challenge for the otorhinolaryngologist.

We have found that Teflon granulomas and the overinjection of Teflon with the resulting laryngeal dysfunction, although uncommon, present a challenge to the otorhinolaryngologist, due in large part to the inflammatory reaction to the injected Teflon. This report addresses the management of 16 patients (12 women and 4 men) treated surgically for symptoms secondary to Teflon granulomas or vocal folds that had been overinjected with Teflon. The presenting symptoms varied and included airway obstruction, cough, choking, swallowing difficulties, and dysphonia. In each case the granuloma and Teflon

Pneumocephalus: effects of patient position on the incidence and location of aerocele after posterior fossa and upper cervical cord surgery.

The incidence of pneumocephalus and factors contributing to its occurrence were studied retrospectively in 100 consecutive patients who underwent posterior fossa or cervical cord surgery in the sitting, park-bench, and prone positions. Supine skull x-ray films taken immediately postoperatively were used to determine the presence of intracranial air. Surgery in the sitting position uniformly resulted in pneumocephalus (32/32 patients). Pneumocephalus also occurred frequently among patients in the park-bench (29/40) and prone positions (16/28). Intraventricular air occurred more frequently when

Current Treatment Options for Bilateral Vocal Fold Paralysis: A State-of-the-Art Review.

Vocal fold paralysis (VFP) refers to neurological causes of reduced or absent movement of one or both vocal folds. Bilateral VFP (BVFP) is characterized by inspiratory dyspnea due to narrowing of the airway at the glottic level with both vocal folds assuming a paramedian position. The primary objective of intervention for BVFP is to relieve patients' dyspnea. Common clinical options for management include tracheostomy, arytenoidectomy and cordotomy. Other options that have been used with varying success include reinnervation techniques and botulinum toxin (Botox) injections into the vocal fold

Successful emergency Lichtenberger lateralisation for immediate bilateral laryngeal immobility after total thyroidectomy: a CARE case report.

OBJECTIVE: This case report discusses a successful emergency Lichtenberger lateralisation procedure after immediate bilateral laryngeal immobility, occurring after total thyroidectomy. METHODS: A 63-year-old female with right-sided vocal fold paralysis due to compression by a multinodular thyroid goitre underwent total thyroidectomy, which resulted in immediate post-operative bilateral vocal fold immobility. The patient had acute-onset post-operative dyspnoea, was promptly re-intubated, and an emergency lateralisation Lichtenberger suture was placed over the right vocal fold and fixated on th

Posterior glottic fixation in children.

Vocal cord fixation from posterior glottic stenosis may produce progressive airway obstruction. Treatment involves scar excision with adjunctive procedures on healthy tissue, including arytenoidopexy, arytenoidectomy, cordectomy, and cordotomy. Twenty-five children with posterior glottic stenosis were studied; 6 had vocal cord fixation in the midline and 6 had limited vocal cord abduction. All were tracheotomy-dependent because of posterior glottic stenosis and secondary impairment of vocal cord mobility. All children were decannulated after scar incision and widening of the laryngeal framewor

[Description of complications after injection laryngoplasty with polydimethylsiloxane].

The injection of polydimethylsiloxane for medialization of the vocal folds has been used for more than 15 years with satisfactory long-term results. However there is little in the literature with regard to the complications and their treatment. We present 2 cases (1 unilateral laryngeal paralysis and 1 bilateral vocal fold atrophy) with unsatisfactory results after PDMS injection. The first case had chronic granulomatous inflammation induced by the PDMS and the second a periprothetic fibrotic reaction causing cordal rigidity. A superior cordotomy and extraction of the PDMS was performed in the
