Surgical technique for the management of corneal perforation in brittle cornea.

Brittle cornea syndrome is among the few special scenarios in ophthalmology that are a nightmare not only for the operating surgeon but also for the patient. Here, the thin and fragile corneas are unable to maintain the shape and structural integrity of the globe and are more prone to minor traumatic or spontaneous corneal perforations. Suturing a brittle cornea and closure of the corneal perforation in a brittle cornea are very challenging requiring the utmost care and special precautions. If proper measures are not taken during the surgery, it may be difficult to salvage the eye. Hence, it i

Corneal Perforation Self-Healing with an Iris Plug in the Cornea.

Keratitis treatment outcome is usually dependent on the targeted treatment of the cause. We want to present a small corneal ulcer of unidentified origin that progressed into corneal melting and resulted in corneal perforation. The patient had a widespread antibacterial and fungal treatment and an ophthalmological follow-up with slit-lamp examination and AS-OCT. The spontaneous iris plug in the cornea helped to solve the anterior chamber collapse and made permanent anatomical changes in the anterior part. A 45-week follow-up found a hyperdense stromal corneal scar with 556-µm-wide stable iris-c

Umbilical Cord Patch Transplantation for Corneal Perforations and Descemetoceles.

PURPOSE: To evaluate the clinical outcome of umbilical cord patch (UCP) transplantation for deep corneal ulcers with perforations and descemetoceles. METHODS: In this retrospective, noncomparative, interventional case series, 11 eyes of 11 patients with corneal perforation or descemetocele were included. The thickness and microstructure of UCP were measured. All eyes were treated with UCP and amniotic membrane transplantation for corneal reconstruction. Corneal ulcer healing, corneal thickness, anterior chamber formation, and best-corrected visual acuity (BCVA) were recorded and analyzed. RE

Improvement of Amniotic Membrane Method for the Treatment of Corneal Perforation.

In our retrospective study we evaluated the efficacy of an improved amniotic membrane (AM) roll-in filling technique (AMR) combined with multilayer amniotic membrane cover to treat corneal perforation and included 46 cornea perforations ≤ 3 mm in diameter treated with AMR and 20% C3F8 mixed gas filling of the anterior chamber. Anterior chamber depth, aqueous leakage, bubble maintenance time, and cornea morphology were monitored after each operation. The mean diameter of corneal perforation was 1.60 ± 0.55 mm (range 0.5-3) and the success rate of the AMR method for corneal perforation reconstru

Contact lens rehabilitation following repaired corneal perforations.

BACKGROUND: Visual outcome following repair of post-traumatic corneal perforation may not be optimal due to presence of irregular keratometric astigmatism. We performed a study to evaluate and compare rigid gas permeable contact lens and spectacles in visual rehabilitation following perforating corneal injuries. METHOD: Eyes that had undergone repair for corneal perforating injuries with or without lens aspiration were fitted rigid gas permeable contact lenses. The fitting pattern and the improvement in visual acuity by contact lens over spectacle correction were noted. RESULTS: Forty eyes o

[Urgent penetrating keratoplasty in perforated infectious corneal ulcers].

PURPOSE: To evaluate anatomical and functional prognosis of urgent penetrating keratoplasty in perforated infectious corneal ulcers. METHODS: Seven eyes of seven patients had an urgent penetrating keratoplasty for perforated infectious corneal ulcers. Anatomical success was defined by eradication of infection and preservation of eye from enucleation and phthisis bulbi. Corneal graft survival was defined by the presence of a clear graft. RESULTS: The patients'mean age was 40.5 years. Four eyes had bacterial corneal ulcer and three eyes had herpetic corneal ulcer. Anatomical success was obtain

Multilayer amniotic membrane transplantation in the treatment of corneal perforations.

PURPOSE: To determine if multilayer amniotic membrane transplantation (AMT) is useful in the treatment of corneal perforations, and in particular to assess to what extent efficacy is affected by perforation size. METHODS: Fifteen patients (15 eyes) with corneal perforations of different sizes were divided into 3 groups: group A (microperforation, 6 eyes), group B (0.5-1.5 mm, 4 eyes), and group C (>1.5 mm, 5 eyes). The corneal perforation was caused by autoimmunity-related ulcer (3 eyes), neurotrophic ulcer (9 eyes), infectious keratitis (1 eye), or postkeratoplasty ulcer (2 eyes). Two layers

Bilateral consecutive central corneal perforations associated with hypogammaglobulinemia.

OBJECTIVE: To describe the presentation and the clinical course of a patient with consecutive central sterile corneal perforations associated with common variable immunodeficiency. DESIGN: Case report. METHODS: Multiple corneal cultures and scrapings were performed in an effort to identify an infectious cause and all were negative. Corneal biopsy did not demonstrate any evidence of micro-organisms. An extended investigation failed to uncover a collagen vascular cause or atopy. RESULTS: Progressive sterile stromal thinning with intact epithelium in the left eye proceeded to perforation despi

Lamellar keratoplasty in the management of inflammatory corneal ulceration and perforation.

Corneal ulceration and perforation may occur in the course of several systemic and ocular inflammatory conditions. These serious complications may respond to systemic immunosuppression and conjunctival surgery, but tectonic keratoplasty is sometimes required. We report on 10 cases of corneal ulceration (of which 8 suffered perforation) in patients with rheumatoid arthritis, Stevens-Johnson syndrome and Mooren's ulceration which were treated by lamellar keratoplasty. Our microsurgical technique for lamellar keratoplasty benefited from the use of viscoelastic agents, cyanoacrylate adhesive and a

Corneal perforation in patients with vitamin A deficiency in the United States.

An emaciated alcoholic 52-year-old white woman presented with an old left corneal perforation and bilateral conjunctival and corneal xerosis. The serum vitamin A level was 0 mumol/L. Laboratory and radiologic findings were consistent with the diagnosis of a fat malabsorptive syndrome secondary to chronic alcoholic pancreatitis. Histopathologic evaluation of her enucleated globe revealed conjunctival epidermidalization, corneal perforation with prolapse and loss of intraocular contents, retinal detachment, and massive choroidal hemorrhage. A second patient presented with bilateral conjunctival

Corneal perforations. Changing methods of treatment, 1960--1980.

One hundred four nontraumatic corneal perforations or descemetoceles in patients admitted to the Wilmer Institute from 1960 to 1980 were studied retrospectively, with follow-up data obtained from records or from referral physicians for 87 of these perforations. Forty-nine percent of the cases were caused by bacterial corneal ulcers, 13% by exposure, 12% by chemical burns, 6% by fungal keratitis, 5% by herpes simplex keratitis, and 15% were undiagnosed. Since the start of tissue adhesive use at the Wilmer Institute in 1974, there has been an apparent trend towards a lower enucleation rate (6%)

Sodium hyaluronate and tissue adhesive in treating corneal perforations.

The injection of sodium hyaluronate through perforation sites prior to tissue adhesive application has extended the current use of this modality to large perforations and perforations in which intraocular tissue is incarcerated. Three clinical cases are described in which the conjoint therapy was used, and four monkey eyes were studied clinically and histopathologically to test for adverse drug interactions.
