Rapid acquisition of daptomycin resistance in a Corynebacterium striatum osteoarticular infection: case report and discussion on antimicrobial resistance.

BACKGROUND: Corynebacterium striatum, a commensal of the skin microbiota, can cause invasive osteoarticular infections that are challenging to diagnose and treat due to biofilm formation. Daptomycin, owing to its potent activity against gram-positive bacteria and favorable toxicity profile, is frequently used as empirical therapy in the management of postoperative bone and joint infections. However, cases of acquired resistance remain rare and poorly understood. CASE SUMMARY: We describe a case of chronic T11-T12 spondylodiscitis in a paraplegic patient. During the first surgical intervention

Lower Limb Necrotizing Soft Tissue Infection Associated with Corynebacterium minutissimum in an Adolescent: A Case Report and Surgical Management.

Corynebacterium minutissimum (C. minutissimum) is usually considered a skin commensal and is best known as the causative organism of erythrasma. Although invasive infections caused by this organism are rare, its clinical significance can be difficult to determine when it is isolated from wound specimens. Here, we present the case of a 14-year-old boy with progressive ulceration, swelling, malodor, and soft tissue destruction of the left lower leg. Intraoperative exploration revealed areas of nonviable fascia and adjacent soft tissue with purulent exudation, and a diagnosis of necrotizing soft

Case Report: From lung to peritoneum: genomic and immunologic insights into a rare dual-site Corynebacterium striatum infection.

Corynebacterium striatum is an emerging multidrug-resistant pathogen in immunocompromised patients. We report a 75-year-old male with paranoid schizophrenia on long-term clozapine who developed sepsis after gastric perforation repair. Within two days, genetically highly homologous Corynebacterium striatum strains were isolated from bronchoalveolar lavage fluid and ascitic fluid. Whole-genome sequencing revealed only 54 Single Nucleotide Polymorphisms (SNPs) differences, identical ermX-mediated resistance, and susceptibility to vancomycin and linezolid. Targeted therapy combined with drainage l

Hemophagocytic lymphohistiocytosis and histiocytic necrotizing lymphadenitis secondary to hemodialysis catheter-related bloodstream infection caused by Corynebacterium Striatum.

BACKGROUND: We herein described the coexistence of hemophagocytic lymphohistiocytosis (HLH) and histiocytic necrotizing lymphadenitis, alternatively known as the Kikuchi disease (KD), secondary to hemodialysis catheter-related bloodstream infection (BSI) caused by Corynebacterium striatum. CASE PRESENTATION: A patient on maintenance hemodialysis had developed persistent fever and Corynebacterium striatum was subsequently identified from the culture of both catheter tip and peripheral blood. During mitigation of the BSI, however, his fever was unabated and ensuing workup further found thromboc

TRIM21 negatively regulates Corynebacterium pseudotuberculosis-induced inflammation and is critical for the survival of C. pseudotuberculosis infected C57BL6 mice.

Corynebacterium pseudotuberculosis, a facultative intracellular bacterium, is an important zoonotic pathogen responsible for chronic inflammatory diseases. TRIM21, an E3 ubiquitin-protein ligase, plays pivotal roles in inflammation regulation. However, its role during C. pseudotuberculosis infection is unclear. Here, we found that TRIM21 expression was significantly increased in C. pseudotuberculosis-infected macrophages. Following infection by C. pseudotuberculosis, we observed a significantly higher number of bacteria and a higher degree of LDH release from Trim21-/- macrophages compared to

A Rare Case Report of Non-toxigenic Corynebacterium diphtheriae Bloodstream Infection in an Uncontrolled Diabetic With Peripheral Vascular Disease.

Corynebacterium diphtheriae usually causes respiratory diphtheria, which is considered as a disease of toxemia but never bacteremia. Over the last few decades, cutaneous diphtheria has been increasingly reported owing to the emergence of the non-toxigenic strain, which causes locally necrotic and ulcerative lesions. Bacteremia is very rare, but the existing evidence in the literature suggests that the organism can rarely cause invasive infections such as septicemia, endocarditis, and osteoarthritis. Here, we present a rare case of C. diphtheriae causing bloodstream infections in an elderly dia

Monomicrobial bone and joint infection due to Corynebacterium striatum: literature review and amoxicillin-rifampin combination as treatment perspective.

Corynebacterium striatum is a ubiquitous colonizer of human skin and mucous membranes. It is increasingly involved in infections, especially with prosthetic devices or in immunocompromised individuals. Microbiological diagnosis is challenging and bacterial resistance is a major concern. We performed a retrospective study of monomicrobial bone and joint infections (BJI) due to C. striatum in two referral centers from April 2012 to July 2017. We collected the patients' clinical and microbiological characteristics and outcomes. We also performed a literature review of BJI due to C. striatum. We i

Corynebacterium striatum Infection in Incision and Thoracic Cavity with Concurrent CRKP Colonization Following Lung and Bladder Tumor Resection.

Corynebacterium striatum, a commensal of skin and mucosa, is an emerging opportunistic pathogen. We report a rare case of postoperative incision and thoracic infection caused by C. striatum with concurrent pulmonary colonization by carbapenem-resistant Klebsiella pneumoniae (CRKP). Following resection of lung and bladder tumours, the patient developed fever, pleural effusion, and poor wound healing. Sputum cultures suggested CRKP colonization, while pleural fluid cultures identified C. striatum as the true pathogen. Clinical pharmacy consultation was pivotal in resolving this diagnostic dilemm

Hematuria as a Diagnostic Clue to Non-cirrhotic Hyperammonemia Due to Corynebacterium urealyticum Urinary Tract Infection: A Case Report.

Hyperammonemia is an important cause of altered mental status in older adults; however, non-cirrhotic hyperammonemia in the absence of underlying liver dysfunction is easily overlooked. Hyperammonemia secondary to urinary tract infection (UTI) caused by urease-producing bacteria is relatively rare, and reliable clinical clues for early diagnosis have not been fully established. We report the case of a 95-year-old woman with Alzheimer's disease residing in a nursing facility. She had macroscopic hematuria for two days without any change in consciousness. On the day of admission, she was found u

Cutaneous infection by non-diphtheria-toxin producing and penicillin-resistant Corynebacterium diphtheriae strain in a patient with diabetes mellitus.

Diphtheria is a potentially fatal infection, mostly caused by diphtheria toxin (DT)-producing Corynebacterium diphtheriae strains. During the last decades, the isolation of DT-producing C. diphtheriae strains has been decreasing worldwide. However, non-DT-producing C. diphtheriae strains emerged as causative agents of cutaneous and invasive infections. Although endemic in countries with warm climates, cutaneous diphtheria is rarely reported in Brazil. Presently, an unusual case of skin lesion in a Brazilian elderly diabetic patient infected by a penicillin-resistant non-DT-producing C. diphthe

Failure of killed Corynebacterium parvum in induction of protection in C57BL/6 mice against Babesia rodhaini challenge infection.

We examined the survival rate of C57BL/6 mice challenged with Babesia rodhaini 21 days post-killed Corynebacterium parvum (KCP) inoculation in comparison with ICR mice. All KCP-inoculated ICR mice survived the challenge infection, while all KCP-inoculated C57BL/6 mice died. To account in part for the difference in protective effects of KCP, stimulatory effects of KCP on splenic phagocyte chemiluminescence (CL) response were investigated in C57BL/6 and ICR mice. The KCP-inoculated ICR mice exhibited significantly higher CL values than ICR mice without KCP inoculation. No significant difference

Cellulitis Caused by Mixed Infection with Group A Streptococci and Non-Toxigenic tox-Gene-Bearing Corynebacterium diphtheriae: A Case Report.

INTRODUCTION: Cutaneous diphtheria has re-emerged in Europe, often presenting as skin-limited disease. Uncertainty around diagnosis, infection control, and toxigenicity testing can delay appropriate management. CASE PRESENTATION: A 62-year-old woman experiencing homelessness presented with a 2-week history of a painful lower-leg eruption. This evolved from vesicles to bullae and shallow ulcers with a characteristic green-grey pseudomembrane, unresponsive to oral flucloxacillin. Swabs of the ulcer grew Corynebacterium diphtheriae and concurrent group A β-haemolytic streptococci, while throat s

Daptomycin resistance of Corynebacterium jeikeium due to pgsA gene mutation acquired during daptomycin treatment in a patient with infective endocarditis.

Daptomycin is an alternative agent for invasive infections caused by Corynebacterium species in patients with intolerance to vancomycin. Although it has recently been recognized that treatment failure with daptomycin due to the emergence of resistant strains during treatment is not uncommon in Corynebacterium striatum infections, it has been unclear whether there is a similar risk in Corynebacterium jeikeium infections. In the presenting case, daptomycin treatment for C. jeikeium endocarditis in a liver transplant recipient with neutropenia failed due to the rapid emergence of the resistant st

Even a single positive blood culture may matter - A case of prosthetic valve infective endocarditis caused by Corynebacterium kroppenstedtii.

Corynebacterium is a skin commensal bacterium that can contaminate blood cultures. It is however also a rare cause of infective endocarditis (IE). Here we report a case of Corynebacterium kroppenstedtii aortic prosthesis IE in a 76-year-old man where only a single blood culture bottle was positive initially. C. kroppenstedtii is a very rare cause of IE, only reported two times previously. The diagnosis in our case was confirmed by repeated blood culture positivity and eventually by detection of DNA from C. kroppenstedtii on heart valves after valve exchange surgery. At surgery an aortic root a

Delayed, deep Corynebacterium surgical site infection following scoliosis corrective surgery in a patient with osteogenesis imperfecta: a case report.

BACKGROUND: Scoliosis is a common spine deformity. Most cases are idiopathic. However, it can be predisposed by certain congenital and neuromuscular conditions, such as osteogenesis imperfecta (OI). Surgical site infection (SSI) is a possible complication of scoliosis surgery. Corynebacterium jeikeium (C. jeikeium) is a rod-shaped, Gram-positive bacterium. It is part of the normal skin flora and has been implicated in various infections, especially in the immunocompromised. To date, only one case of SSI following scoliosis surgery has been reported to be secondary to C. jeikeium. In this repor
