Automated posterior cranial fossa volumetry by MRI: applications to Chiari malformation type I.

BACKGROUND AND PURPOSE: Quantification of PCF volume and the degree of PCF crowdedness were found beneficial for differential diagnosis of tonsillar herniation and prediction of surgical outcome in CMI. However, lack of automated methods limits the clinical use of PCF volumetry. An atlas-based method for automated PCF segmentation tailored for CMI is presented. The method performance is assessed in terms of accuracy and spatial overlap with manual segmentation. The degree of association between PCF volumes and the lengths of previously proposed linear landmarks is reported. MATERIALS AND METH

Subdural enhancement on postoperative spinal MRI after resection of posterior cranial fossa tumours.

In malignant brain tumours which may disseminate staging, usually by cranial and spinal MRI is necessary. If MRI is performed in the postoperative period pitfalls should be considered. Nonspecific subdural contrast enhancement on spinal staging MRI is rarely reported after resection of posterior fossa tumours, which may be mistaken for dissemination of malignancy. We investigated the frequency of spinal subdural enhancement after posterior cranial fossa neurosurgery in children. We reviewed 53 postoperative spinal MRI studies performed for staging of paediatric malignant brain tumours, mainly

[Angiographic findings in 86 confirmed tumours of the posterior cranial fossa (author's transl)].

The accuracy of vertebral angiography in the diagnosis of tumours was reviewed on the basis 368 selective angiograms. The greatest accuracy was achieved with space-occupying lesions situated centrally in the cerebellum in the region of the fourth ventricle or in the vermis. However, tumours growing eccentrically, but arising in this region, were occasionally missed. Least reliable is the angiographic diagnosis of tumours is the cerebellar hemispheres. Of these, only about two-thirds could be demonstrated. Only 60% of cerebello-pontine tumours were diagnosed from displacement of various structu

Hypertrophic olivary degeneration after posterior cranial fossa tumor resection in adults: illustrative case.

BACKGROUND: Posterior cranial fossa tumor resection can be complicated by a rare but potentially debilitating condition known as hypertrophic olivary degeneration (HOD). This condition is diagnosed using a high degree of clinical suspicion as well as T2/FLAIR MRI findings of nonenhancing, hyperintense enlargement of the inferior olivary nucleus. OBSERVATIONS: The authors present a unique case of HOD in a 49-year-old female 4 months after resection of a low-grade pilocytic astrocytoma from the cerebellar vermis. The patient displayed classic signs associated with HOD, including palatal tremor,

Acute Posterior Cranial Fossa Hemorrhage-Is Surgical Decompression Better than Expectant Medical Management?

BACKGROUND: To compare the in-hospital mortality and institutional morbidity from medical therapy (MT), external ventricular drainage (EVD) and suboccipital decompressive craniectomy (SDC) following an acute hemorrhagic posterior cranial fossa stroke (PCFH) in patients admitted to the neurosciences critical care unit (NCCU). Retrospective observational single-center cohort study in a tertiary care center. All consecutive patients (n = 104) admitted with PCFH from January 1st 2005-December 31st 2011 were included in the study. METHODS: All patients with a PCFH were identified and confirmed by

Hypothalamo-pituitary-adrenal axis integrity after cranial irradiation for childhood posterior fossa tumours.

BACKGROUND: The evolution of anterior pituitary deficits after treatment for pituitary tumours has been largely attributed to local irradiation, but may be influenced as much by tumour mass or surgery. Other than growth hormone (GH) insufficiency, the late endocrinopathies after survival from non-central brain tumours have been little documented. The aim of this study was to investigate the hypothalamic-pituitary-adrenal (HPA) axis in long-term survivors of cranial irradiation for childhood posterior fossa tumours. PROCEDURE: We studied long-term data in patients treated prepubertally for pos

Choroid plexus papilloma in the posterior cranial fossa: MR, CT, and angiographic findings.

In this study, we reviewed the magnetic resonance (MR, n=5), computed tomography (CT, n=3), and angiography (n=3) of six patients with pathologically confirmed choroid plexus papilloma (CPP) in the posterior cranial fossa. CPPs in the posterior cranial fossa have several features, including a propensity to arise at the foramen of Luschka with extraventricular extension, occasional peritumoral signal voids/cysts or calcification, weaker enhancement on MR or CT, and less strong tumor staining by the anterior or posterior inferior cerebellar artery or angiography.

[Orbicularis oculi reflex in computerized tomography verified lesions of the posterior cranial fossa].

The blink reflex elicited by electrical stimulation was studied in 60 patients with lesions of the brainstem and cerebellum (ischemic lesions, non-traumatic bleedings or tumors), which were diagnosed by computertomography. The relationship between localized morphological changes in computertomography and changes in blink reflex has been studied. 46 patients (77%) had a pathologic blink reflex. Usually the blink reflex was normal in patients with a tumor of cerebellum. In bleedings or tumors of the brainstem a good relationship existed between localization and extension of the lesion and the pa

A case of microcystic meningioma associated with acute subdural hematoma in the posterior cranial fossa.

A 53-year-old woman was brought to the emergency room with headache and progressive deterioration of consciousness. Radiological examinations revealed acute subdural hematoma extending along the cerebellar tentorium to the falx cerebri, and a mass lesion with hemorrhage in the left cerebellum, with acute hydrocephalus. Emergency tumor and hematoma removal with decompressive craniectomy of the occiput was performed. Histopathological diagnosis was microcystic meningioma. Postoperatively, the patient recovered to clear consciousness with sequelae of left cerebellar ataxia, cerebellar dysarthria,

Embryology of the craniocervical junction and posterior cranial fossa, part II: Embryogenesis of the hindbrain.

Although pathology of the hindbrain and its derivatives can have life altering effects on a patient, a comprehensive review on its embryology is difficult to find in the peer-reviewed medical literature. Therefore, this review article, using standard search engines, seemed timely. The embryology of the hindbrain is complex and relies on a unique timing of various neurovascular and bony elements. Derailment of these developmental processes can lead to a wide range of malformations such as the Chiari malformations. Therefore, a good working knowledge of this embryology as outlined in this review

Therapeutic strategy and long-term outcome of meningiomas located in the posterior cranial fossa.

The clinical and surgical findings of 41 consecutive cases of posterior cranial fossa meningiomas operated on between January 1987 and December 2011 at Saitama Medical Center/Saitama Medical University were reviewed. The 31 female and 10 male patients were aged from 19 to 74 years (mean 54 years). The tumors were located in the petroclival (N=15), craniovertebral junction (N=6), lateral tentorial (N=12), and cerebellopontine angle (N=8) regions. Mean tumor equivalent diameter was 4.3 cm (range 2-9 cm). Head pain (46.3%) and gait disturbance (26.8%) were the most common presenting symptoms, and

A huge occipital osteoblastoma accompanied with aneurysmal bone cyst in the posterior cranial fossa.

Osteoblastoma is an infrequent benign tumor and skull involvement is extremely rare. The occipital bone is much less frequently involved. We report an unusual case of a huge occipital osteoblastoma with aneurysmal bone cyst in the posterior cranial fossa of a 20-year-old young man. MRI scan and CT three-dimensional reconstruction revealed that the tumor (approximately 8.5 cm x 6 cm x 5 cm) occupied nearly half volume of the posterior cranial fossa. The cerebellum, the forth ventricle and brain stem were compressed remarkably and hydrocephalus was noted. The tumor mass was multiloculated cystic

[Monitoring of the auditory stem evoked potentials in removal of tumors of the posterior cranial fossa].

The material of intraoperative monitoring of the acoustic stem evoked potentials (ASEP) in 9 patients with tumors of the stem and in 28 patients with the parastem localization was analyzed. It was shown that changes in the amplitude of ASEP rather than latency were the decisive factor in the assessment of the functional state of the brain stem. It was found that even the pronounced changes of the bioelectrical activity recorded during the excision of the tumor could be reversible after a change of the strategy of the operative intervention and/or after performing the necessary therapeutic meas

Localised Wegener's granulomatosis presenting as an orbital pseudotumour with extension into the posterior cranial fossa.

Pseudotumour has rarely been reported to invade the intracranial cavity. We present a case of pseudotumour which extended intra-cranially into the posterior cranial fossa. The patient had circulating antineutrophil cytoplasmic antibodies in titres diagnostic for Wegener's granulomatosis. In the absence of clinical renal or respiratory tract disease, the case may represent a newly recognized sub-type of "localised" Wegener's granulomatosis.

[The efficacy of gamma teletherapy in the combined treatment of medulloblastomas in the posterior cranial fossa of children].

The results of radiation therapy conducted after various schemes were compared in 77 patients operated on for medulloblastoma of the posterior cranial fossa: 1. irradiation of the posterior cranial fossa area; 2. irradiation of this area and the entire length of the spinal marrow; 3. total irradiation of the CNS. Stages in a metastatic process necessitate irradiation in stages, i.e. irradiation of the posterior cranial fossa and the length of the spinal marrow followed by irradiation of the cerebral hemispheres one year after the operation. Total irradiation of the CNS shortly after the operat
