Bridging the Coverage Gap: State Medicaid Limitations for Cardiac Rehabilitation and Risk to Disadvantaged Communities.

BACKGROUND: Cardiac rehabilitation (CR) is a guideline-supported, evidence-based intervention that improves cardiovascular outcomes, yet access remains inconsistent for Medicaid beneficiaries because coverage policies vary across states. PROJECT RATIONALE: We evaluated Medicaid reimbursement for outpatient CR services across all 50 U.S. states to identify policy variation that may contribute to inequitable access for vulnerable populations. PROJECT SUMMARY: A cross-sectional survey review of state Medicaid fee schedules, provider manuals, coverage policies, and direct communication with Medi

An Update on Management of Syndesmosis Injury: A National US Database Study.

We conducted a study to determine the incidence of syndesmotic injury and stabilization within various ankle fracture patterns along with the rate of syndesmotic implant removal. All data were obtained from a publicly available for-fee healthcare database, the PearlDiver Patient Records Database. Current Procedural Terminology (CPT) codes were used to search the database for cases of fixation of lateral malleolus, bimalleolar, and trimalleolar fractures in addition to syndesmotic fixation and implant removal. Open reduction and internal fixation (ORIF) was performed for an ankle injury in 54,7

Anemia is not a predictor of free flap failure: a review of NSQIP data.

BACKGROUND: Thrombosis is a common cause of flap failure in microvascular tissue transfer, which questions the effects of anemia on this outcome. This article seeks to contribute a large, multi-institutional data analysis to this debate. METHODS: Free tissue transfer patients were identified in the National Surgical Quality Improvement database through a specified Current Procedural Terminology algorithm. Bivariate analysis compared anemic and nonanemic groups with respect to flap failure and other outcomes. Multivariable logistic regression was used to determine risk factors for flap failure

Understanding the Bone Mass Measurement Act.

The Bone Mass Measurement Act (BMMA) set forth regulations to provide for uniform coverage under Medicare Part B for bone mass measurements for services provided on or after July 1, 1998. The BMMA authorizes Medicare coverage of "medically necessary approved measurements" performed for a "qualified individual" who falls into at least one of five diagnostic categories: an estrogen-deficient woman at clinical risk for osteoporosis; an individual with vertebral abnormalities; an individual receiving long-term glucocorticoid (steroid) therapy; an individual with primary hyperparathyroidism; and an

Hospital discharge data is not accurate enough to monitor the incidence of postpartum hemorrhage.

INTRODUCTION: Postpartum hemorrhage remains a leading cause of maternal morbidity and mortality worldwide. Therefore, cumulative incidence of postpartum hemorrhage and severe postpartum hemorrhage are commonly monitored within and compared across maternity hospitals or countries for obstetrical safety improvement. These indicators are usually based on hospital discharge data though their accuracy is seldom assessed. We aimed to measure postpartum hemorrhage and severe postpartum hemorrhage using electronic health records and hospital discharge data separately and compare the detection accuracy

Transplant Business Best Practices: An American Society of Transplant Surgeons Business Practice Services Committee Opinion on Reimbursement, Cost Accounting, and Professional Coding in Modern Transplant Practice.

Financial stewardship in transplantation requires integrated management of three distinct reimbursement domains: organ acquisition, transplant hospitalization, and physician professional services. Of these, organ acquisition is often the most complex and least intuitively understood, despite its major financial implications. This article outlines business best practices for transplant programs. Organ Acquisition Cost Centers should be managed as cost recovery structures rather than profit centers, with attention to Medicare Cost Report mechanics, living donor cost reporting, organ statistics,

Orbital floor fractures: outcomes of reconstruction.

Since the 1950s, myriad materials have been used to reconstruct orbital floor fractures. Technological advances have afforded new materials for reconstruction. Recent comparisons of materials have not been reported. Retrospective chart review was performed using current procedural terminology coding for orbital floor fractures treated between 1991 and 2009. A total of 510 charts were reviewed; 317 adult patients met criteria. Forty-seven of these patients underwent bilateral floor explorations, yielding 364 orbital floor fractures. Mean age was 33.7 years. Motor vehicle collision, assault, all

Evolution of carotid stenting: payment issues.

Carotid stent placement received assignment of two Category I Current Procedural Terminology (CPT) codes in 2005, based on the collaborative efforts of 10 medical, surgical, and radiological specialty societies. One code is used to report stent placement with embolic protection, the other without embolic protection. The codes are unusual for interventional procedures because they include all associated catheterizations, diagnostic imaging, angioplasty, and radiologic supervision and interpretation. The Centers for Medicare and Medicaid Services (CMS) issued a coverage policy for carotid stenti

Does the reporting of gastro-intestinal endoscopy meet the minimal terminology standard at King Edward VIII Hospital?

BACKGROUND: The Minimal Standard Terminology (MST) was developed to standardise endoscopic reporting. This study is aimed at assessing current reporting at a tertiary hospital and whether it meets this Minimal Standard Terminology. METHOD: This was a retrospective observational study of upper endoscopy reports between January and December 2014. The data extracted were compared to the current reporting standard listed in the MST. To assess and grade the quality of reporting we developed a Gastrointestinal Reporting Score, which incorporates MST variables in addition to demographic details, ind
