Theory-based design and field-testing of an intervention to support women choosing surgery for breast cancer: BresDex.

OBJECTIVE: Design and undertake usability and field-testing evaluation of a theory-guided decision aid (BresDex) in supporting women choosing surgery for early breast cancer. METHODS: An extended Theory of Planned Behavior (TPB) and the Common Sense Model of Illness Representations (CSM) guided the design of BresDex. BresDex was evaluated and refined across 3 cycles by interviewing 6 women without personal history of breast cancer, 8 women with personal history of breast cancer who had completed treatment and 11 women newly diagnosed with breast cancer. Participants were interviewed for views

Decision-making for bilateral risk-reducing mastectomy for an increased lifetime breast cancer risk: A qualitative metasynthesis.

OBJECTIVE: Previvor is a term applied to a person with an identified, elevated lifetime cancer risk but without an actual cancer diagnosis. Previvorship entails the selection of risk management strategies. For women with a genetic mutation that increases their predisposition for a breast cancer diagnosis, bilateral risk-reducing mastectomy (BRRM) is the most effective prevention strategy. However, BRRM can change a woman's breast appearance and function. The purpose of this qualitative metasynthesis (QMS) was to better understand the decision-making process for BRRM among previvors. METHODS:

Complex trauma to the shoulder girdle, including the proximal humerus, the clavicle, and the scapula: current concepts in diagnosis and treatment.

Fractures to the shoulder girdle are common injuries in an aging population. Many techniques and theories lie behind the treatment of such injuries. Knowledge and understanding of current concepts for diagnosing and treating proximal humeral, clavicular, and scapular fractures and the theory behind them will help surgeons make informed decisions with regard to patient care.

A Quantum Probability Approach to Improving Human-AI Decision Making.

Artificial intelligence is set to incorporate additional decision space that has traditionally been the purview of humans. However, AI systems that support decision making also entail the rationalization of AI outputs by humans. Yet, incongruencies between AI and human rationalization processes may introduce uncertainties in human decision making, which require new conceptualizations to improve the predictability of these interactions. The application of quantum probability theory (QPT) to human cognition is on the ascent and warrants potential consideration to human-AI decision making to impr

Resolved and unresolved bioethical authenticity problems.

Respect for autonomy is a central moral principle in bioethics. It is sometimes argued that authenticity, i.e., being "real," "genuine," "true to oneself," or similar, is crucial to a person's autonomy. Patients sometimes make what appears to be inauthentic decisions, such as when (decision-competent) anorexia nervosa patients refuse treatment to avoid gaining weight, despite that the risk of harm is very high. If such decisions are inauthentic, and therefore non-autonomous, it may be the case they should be overridden for paternalist reasons. However, it is not clear what justifies the judgme

Situational pressures on aviation decision making: goal seduction and situation aversion.

INTRODUCTION: The context in which a decision is made can shape the decision process in important ways. Of particular interest here are 'strong' situations, that is, contexts where the situation itself exerts an influence on decision making. Borrowing concepts from field theory and approach/avoidance theory, this paper explores the influence of strong situations on pilot decision making. METHOD: There were 28 pilots flying in Alaska who were interviewed using a critical incident technique where participants were asked to relate stories about challenging flying situations. A bottom-up qualitat

Predicting the sun-protective decisions of young female Australian beachgoers.

We tested the theory of planned behaviour with additional norms to predict the sun-protective decisions of young female beachgoers in Australia. Participants (N = 336) completed standard theory of planned behaviour measures and additional normative factors (group, image and personal norms). Attitude, subjective norm, and both personal and group norms predicted intention. Intention and group norm predicted follow-up behaviour. These findings provide further evidence that it is the more proximal sources of norms reflecting one's personal and referent group influences, rather than perceptions of

Differences in pain assessment and decisions regarding the administration of analgesics between novices, intermediates and experts in pediatric nursing.

This article describes a study examining the influence of expertise on nurses' pain assessments and decisions regarding pharmacological interventions in children. In an experimental design, novices (n = 271), intermediates (n = 222), and experts (n = 202) in pediatric nursing, various cases were presented. Each case consisted of a combination of a vignette and a video. Subjects were asked (1) to assess the child's pain intensity, (2) to specify their confidence in the assessment, and (3) to state whether or not they would administer a non-narcotic analgesic. The results indicated that expertis

Probabilistic analysis of decision trees using Monte Carlo simulation.

The authors describe methods for modeling uncertainty in the specification of decision tree probabilities and utilities using Monte Carlo simulation techniques. Exact confidence levels based upon the underlying probabilistic structure are provided. Probabilistic measures of sensitivity are derived in terms of classical information theory. These measures identify which variables are probabilistically important components of the decision. These techniques are illustrated in terms of the clinical problem of anticoagulation versus observation in the setting of deep vein thrombosis during the first

Decision analysis of treatment choices in the osteochondroses.

Physicians tend to decry the lack of data on which they can make decisions. This is commendable, and all should encourage the pursuit of better data and more precise analysis. But decisions must be made, and each physician must deal with what data are available and evaluate them against all the general uncertainties. Equally important are the values that we place on the outcome of treatment. Much of the disagreement among physicians about treatment protocols involves a difference in values. While this is not necessarily bad, it points to the need to consider explicitly the value we place on a

New insights into regulation of erythrocyte shape.

Two ideas explain the mechanism of shape regulation: the lipid bilayer coupled theory and the protein network scaffold theory. Recently, several important articles have been published on the former theory. However, many phenomena argue against the theory, including behavior of ghosts and triton shells, various types of manipulation of proteins, and fixation of the shape by the addition of large reagents outside the cell. Moreover, hereditary spherocytosis shows normal, uneven distribution of phospholipids, and hereditary and artificial defects of a membrane protein show spherocytes or elliptoc

To tilt or not to tilt; the decision-making process.

PURPOSE: This study's aim was to explore the complexities of how power tilt use is integrated within the context of daily life. Other studies have used mobility-type metrics of frequency, duration, and amplitude as measures of power tilt use in the context of daily life but results lack depth in explaining the complex interplay of participating in daily activities and power tilt use. METHODS: This grounded theory study explored how power tilt was used in daily life from the perspectives of people experienced with using power tilt and therapists experienced with prescribing power tilt. Between host country and homeland: a grounded theory study on place of dying and death in migrant cancer patients.

BACKGROUND: Migrant cancer patients face unique challenges in end-of-life decision-making. One key yet underexplored aspect is the decision-making process surrounding the place of dying and death. This study explores the factors influencing these decisions involving migrant cancer patients in Italy. METHODS: A constructivist grounded theory approach was employed. Data were collected through semi-structured interviews with 28 participants (patients, family members, cultural mediators, and other key informants, some from a previous grounded theory study). Why moral judgements change across variations of trolley-like problems.

In the standard 'trolley problem', respondents must decide whether to save a condemned group of individuals by sacrificing a safe bystander. Although respondents often are willing to sacrifice the bystander in some circumstances (e.g., by pulling a lever), they are loath to sacrifice the bystander in others (e.g., by pushing the bystander off a footbridge). This difference in responding has been explained via a Dual Process theory of moral judgements (DPT). DPT, however, is a classic boxes-and-arrows model that only makes directional predictions. The competitive advantage of institutional reward.

Sustaining cooperation among unrelated individuals is a fundamental challenge in biology and the social sciences. In human society, this problem can be solved by establishing incentive institutions that reward cooperators and punish free-riders. Most of the previous studies have focused on which incentives promote cooperation best. However, a higher cooperation level does not always imply higher group fitness, and only incentives that lead to higher fitness can survive in social evolution. In this paper, we compare the efficiencies of three types of institutional incentives, namely, reward, pu

The relationship between counterfactual thinking and emotional reactions to event outcomes: does one account fit all?

By enabling a comparison between what is and what might have been, counterfactual thoughts amplify our emotional responses to bad outcomes. Well-known demonstrations such as the action effect (the tendency to attribute most regret to a character whose actions brought about a bad outcome) and the temporal order effect (the tendency to undo the last in a series of events leading up to a bad outcome) are often explained in this way. An important difference between these effects is that outcomes are due to decisions in the action effect, whereas in the temporal order effect outcomes are achieved b

NitroGenius: a nitrogen decision support system. A game to develop the optimal policy to solve the Dutch nitrogen pollution problem.

A nitrogen decision support system in the form of a game (NitroGenius) was developed for the Second International Nitrogen Conference. The aims were to: i) improve understanding among scientists and policy makers about the complexity of nitrogen pollution problems in an area of intensive agricultural, industrial, and transportation activity (The Netherlands); and ii) search for optimal policy solutions to prevent pollution effects at lowest economic and social costs. NitroGenius includes a model of nitrogen flows at relevant spatial and temporal scales including emissions of ammonia and nitrog

Osseointegration and self-management decision making.

PURPOSE: This study explores how individuals living with limb osseointegration (OI) make decisions about their self-management, including how they seek, evaluate, and apply health information. MATERIALS AND METHODS: Using a constructivist grounded theory approach, ten participants with OI from Canada, the United States, and Australia were interviewed via Zoom. Transcripts were analyzed using constant comparative methods to identify themes and develop a theory of self-management decision-making. RESULTS: Participants described a multifaceted decision-making process across four stages of their

The uncertain 'best interests' of neonates: decision making in the neonatal intensive care unit.

Neonatology is a recently specialized area of medicine in which medical advances continue to save the lives of many ailing infants. However, like most areas of health care, it is not a perfect science, and hence not all infants can be saved, no matter how advanced the technology or how well-trained the physician. Inevitably, decisions regarding when to forego treatment must be made. It is essential to scrutinize the bases upon which these decisions are reached. This article examines the development of the scientific, legal and philosophical theories in Canada, the United States and the United
