[Early detection of asymptomatic dysbaric osteonecrosis of the shoulder after type 1 decompression sickness: a case report].

INTRODUCTION: Dysbaric osteonecrosis is a rare illness in professional divers and compressed-air workers. The correlation between dysbaric osteonecrosis and previous decompression sickness with osteoarthromuscular pain (type 1 decompression sickness) remains a controversial subject. The probability for ischemic lesions detected with MRI to turn into osteonecrosis after decompression sickness is still not established. EXEGESIS: The authors report the case of a military diver declared definitely medically unfit to dive after the occurrence of advanced dysbaric osteonecrosis of the shoulder, eig

Recovery from alpha coma after decompression sickness complicated by spinal cord lesions at cervical and midthoracic levels.

A modified alpha pattern activity occurred in the EEG of a 21-year-old man during recovery from a diving injury. It persisted for 10 days but finally the EEG became normal. There was clinical and neurophysiological evidence of additional separate lesions in the cervical and mid-thoracic spinal cord. The latter resulted in spastic paraplegia but the patient made an otherwise good recovery.

Recurrent decompression sickness and late repermeabilization of patent foramen oval closure prosthesis: a diver's dilemma-case report.

BACKGROUND: Decompression sickness (DCS) is a well-known risk associated with scuba diving, particularly in people with right-to-left shunt, such as patent foramen oval (PFO). Herein, we present a unique case of late PFO permeabilization after closure. CASE SUMMARY: A 26-year-old male diver was diagnosed with DCS following a dive at 36 m. He underwent PFO closure with a STARFLEX® prosthesis. Ten years later, the patient was presented with recurrent manifestations suggestive of DCS. The performed diagnostic work-up detects a permeabilization of the implanted prosthesis, and he was treated with

Demonstration by Infra-Red Imaging of a Temperature Control Defect in a Decompression Sickness Model Testing Minocycline.

The prevention, prognosis and resolution of decompression sickness (DCS) are not satisfactory. The etiology of DCS has highlighted thrombotic and inflammatory phenomena that could cause severe neurological disorders or even death. Given the immunomodulatory effects described for minocycline, an antibiotic in widespread use, we have decided to explore its effects in an experimental model for decompression sickness. 40 control mice (Ctrl) and 40 mice treated orally with 90 mg/kg of minocycline (MINO) were subjected to a protocol in a hyperbaric chamber, compressed with air. The purpose was to mi

Successful treatment of decompression sickness complicated with acute hepatic infarction and acute kidney injury: a case report.

Decompression sickness is a disease caused by abrupt pressure change and presents various symptoms. To date, acute kidney injury associated with decompression sickness has been reported frequently, but there is no report of hepatic infarction associated with decompression sickness. We report a case of acute kidney injury and acute hepatic infarction treated with hyperbaric oxygen (HBO2) therapy and dialysis in a patient with severe decompression sickness after work diving.

Cutis Marmorata skin decompression sickness is a manifestation of brainstem bubble embolization, not of local skin bubbles.

"Cutis Marmorata" skin symptoms after diving, most frequently in the form of an itching or painful cutaneous red-bluish discoloration are commonly regarded as a mild form of decompression sickness (DCS), and treated with oxygen inhalation without reverting to hyperbaric recompression treatment. It has been observed that the occurrence of Cutis Marmorata is frequently associated with the presence of a Patent Foramen Ovale (PFO) of the heart, and indeed, with a properly executed contrast echocardiographic technique, these patients have an almost 100% prevalence of PFO. Only occasionally, Cutis M

Mechanism of action of antiplatelet drugs on decompression sickness in rats: a protective effect of anti-GPIIbIIIa therapy.

Literature highlights the involvement of disseminated thrombosis in the pathophysiology of decompression sickness (DCS). We examined the effect of several antithrombotic treatments targeting various pathways on DCS outcome: acetyl salicylate, prasugrel, abciximab, and enoxaparin. Rats were randomly assigned to six groups. Groups 1 and 2 were a control nondiving group (C; n = 10) and a control diving group (CD; n = 30). Animals in Groups 3 to 6 were treated before hyperbaric exposure (HBE) with either prasugrel (n = 10), acetyl salicylate (n = 10), enoxaparin (n = 10), or abciximab (n = 10). Bl

Case report: Reassessing guidelines for safe resumption of diving after spinal decompression sickness: insights from a challenging case.

BACKGROUND: Recreational divers who have experienced Spinal Decompression Sickness (DCS) often aspire to return to their diving activities. Traditionally, it is recommended to observe a waiting period of several months before contemplating a return to unrestricted diving, particularly when clinical symptoms are absent, spinal cord Magnetic Resonance Imaging shows no anomalies, and the evaluation for Patent Foramen Ovale (PFO) returns negative results. METHODS: This article presents a compelling case study involving a 51-year-old recreational scuba diver who encountered two episodes of spinal

A case of severe multi-system decompression sickness requiring critical care, aggressive fluid resuscitation and hyperbaric oxygen therapy.

An experienced recreational diver developed a severe multisystem life-threatening decompression sickness following a series of eight dives over four days, the profiles of which were all within acceptable limits. The diver had marked haemoconcentration and required large volume intravenous fluid replacement during initial treatment. Similar presentations have previously only been described following deep dives, fast ascents or where mandatory decompression had been missed. The management of this case, including the involvement of a hyperbaric unit co-located to an intensive care department, and

Technical diving accidents in France: a 15-year retrospective study reporting a high prevalence of musculoskeletal decompression sickness.

INTRODUCTION: Technical diving, involving rebreathers and/or helium-based gas mixtures for deeper and longer dives, may influence risk and clinical presentation of injuries due to helium's properties, equipment constraints, or exposure conditions. This study aims to describe the specific characteristics of this accidentology. METHODS: A retrospective study was conducted across five French coastline hyperbaric units. Medical records of technical divers presenting with decompression sickness (DCS), immersion pulmonary oedema (IPO), or gas-toxicity between 2010 and 2024 were reviewed. RESULTS:

Hyperbaric oxygen pretreatment according to the gas micronuclei denucleation hypothesis reduces neurologic deficit in decompression sickness in rats.

During sudden or too rapid decompression, gas is released within supersaturated tissues in the form of bubbles, the cause of decompression sickness. It is widely accepted that these bubbles originate in the tissue from preexisting gas micronuclei. Pretreatment with hyperbaric oxygen (HBO) has been hypothesized to shrink the gas micronuclei, thus reducing the number of emerging bubbles. The effectiveness of a new HBO pretreatment protocol on neurologic outcome was studied in rats. This protocol was found to carry the least danger of oxygen toxicity. Somatosensory evoked potentials (SSEPs) were

Does Decreased Diffusing Capacity of the Lungs for Carbon Monoxide Constitute a Risk of Decompression Sickness in Occupational Divers?

Long-term alterations of pulmonary function (mainly decreased airway conductance and capacity of the lungs to diffuse carbon monoxide (DLCO)) have been described after hyperbaric exposures. However, whether these alterations convey a higher risk for divers' safety has never been investigated before. The purpose of the present pilot study was to assess whether decreased DLCO is associated with modifications of the physiological response to diving. In this case-control observational study, 15 "fit-to-dive" occupational divers were split into two groups according to their DLCO measurements compar

Fluoxetine stimulates anti-inflammatory IL-10 cytokine production and attenuates sensory deficits in a rat model of decompression sickness.

Despite "gold standard" hyperbaric oxygen treatment, 30% of patients suffering from neurological decompression sickness still exhibit incomplete recovery, including sensory impairments. Fluoxetine, a well-known antidepressant, is recognized as having anti-inflammatory effects in the setting of cerebral ischemia. In this study, we focused on the assessment of sensory neurological deficits and measurement of circulating cytokines after decompression in rats treated or not with fluoxetine. Seventy-eight rats were divided into a clinical (n = 38) and a cytokine (n = 40) group. In both groups, the

Case of Broca's Aphasia as an expression of cerebral decompression sickness with advanced non-invasive monitoring during HBO₂ treatment.

INTRODUCTION: We present a case of severe CNS decompression sickness (DCS) that included Broca's aphasia in a recreational diver, offering insights into the application of advanced non-invasive monitoring during hyperbaric oxygen treatment (HBO₂). CASE REPORT: This 64-year-old male diver experienced the onset of expressive aphasia, vertigo, and right-sided hemiparesis about 30-120 minutes after surfacing following a rapid ascent after a deep dive. DISCUSSION: Initial monitoring with bispectral index (BIS) and density spectral array (DSA) revealed significant asymmetry between cerebral hemisp

Bispectral index with density spectral array (BIS-DSA) monitoring in a patient with inner ear and cerebral decompression sickness.

Bispectral index with density spectral array (BIS-DSA) monitoring during hyperbaric oxygen therapy of a case with inner ear and cerebral decompression sickness is described. During the initial treatment, a particular DSA pattern was found, which resolved after four treatments. Clinical resolution of the symptoms accompanied this improvement. The particular BIS-DSA pattern described in this case is concordant with a potential hypo-perfusion of the cortex related to decompression stress. This case suggests that BIS-DSA monitoring may be an easy, cost-effective, and viable form of neuro-monitorin

Hyperbaric oxygen preconditioning alleviates spinal cord injury by inhibiting pro-inflammatory astrocyte polarization in a rat model of decompression sickness.

Decompression sickness (DCS) spinal cord injury (SCI) is a pivotal concern in diving activities, and prevention is the key. Our previous study found that hyperbaric oxygen (HBO) preconditioning reduced neurological deficit in DCS SCI rats, but the underlying mechanisms remain unclear. This study examined pro-inflammatory astrocyte polarization in DCS SCI rats and determined whether HBO exerts its protective effect by inhibiting this polarization. Here, we show that bubble contact activates nuclear factor kappa-B (NF-κB), which in turn drives astrocyte polarization toward the pro-inflammatory p

A retrospective review of divers treated for inner ear decompression sickness at Fiona Stanley Hospital hyperbaric medicine unit 2014-2020.

INTRODUCTION: Inner ear decompression sickness (IEDCS) is increasingly recognised in recreational diving, with the inner ear particularly vulnerable to decompression sickness in divers with a right-to-left shunt, such as is possible through a persistent (patent) foramen ovale (PFO). A review of patients treated for IEDCS at Fiona Stanley Hospital Hyperbaric Medicine Unit (FSH HMU) in Western Australia was performed to examine the epidemiology, risk factors for developing this condition, the treatment administered and the outcomes of this patient population. METHODS: A retrospective review of

[Fewer cases of decompression sickness in Västra Götaland. Eight years of statistics show a reduced number of consultations and treatments].

During the period 2005 to 2012, a total of 340 consultations (phone calls and/or visits) regarding possible decompression illness were recorded at the two hospitals with recompression chambers in the Västra Götaland region, Sweden. An analysis of the data showed a trend towards fewer consultations and recompression treatments. A similar trend has been observed in many other countries. Possible reasons for this reduction in the number of cases are discussed. Most of the patients only present relatively mild signs and symptoms at the time of consultation. This, together with the fact that the nu
