Sarcopenic Dysphagia Reconsidered: A Systematic Review of Complex Interactions and Diagnostic Challenges.

Sarcopenic dysphagia is an emerging clinical concept defined as dysphagia resulting from generalized sarcopenia and sarcopenic changes in the muscles involved in swallowing. However, despite growing clinical interest, its diagnostic criteria, pathophysiological boundaries, and causal validity remain insufficiently established. This systematic review aimed to examine how sarcopenic dysphagia has been defined and assessed in the literature and to identify conceptual and methodological gaps. A comprehensive literature search was conducted across PubMed, EMBASE, the Cochrane Library, and CINAHL, c

Endoscopic and videofluoroscopic evaluations of swallowing for dysphagia: A systematic review.

OBJECTIVE: This systematic review was performed to compare the diagnostic accuracy of FEES and VFSS in evaluating swallowing in adults with dysphagia. METHODS: A search for articles published between January 1991 and March 2020 was carried out in the MEDLINE EMBASE, COCHRANE, SciELO, and LILACS electronic databases. Based on the eligibility criteria, six articles were included. RESULTS: FEES demonstrated a higher ability to diagnose pharyngeal residue, penetration, and aspiration compared with VFSS, and slightly better performance in detecting premature spillage. CONCLUSION: There were no s

Swallowing skill training approaches and underlying motor learning principles: A scoping review.

PURPOSE: While dysphagia rehabilitation has traditionally focused on muscle strengthening, there is an increasing shift towards harnessing motor cortical plasticity to modify swallowing motor output via swallowing skill training. We reviewed current swallowing skill training approaches and framed existing evidence in the context of motor learning principles. METHOD: Searches were conducted across six databases (Scopus, CINHAL, MEDLINE, PUBMED, Emcare, and Web of Science) and the grey literature. Two researchers independently screened studies for inclusion using Covidence. English publications

Upper Airway Sensory Testing in Dysphagia - Implications for Clinical Practice and Future Research Directions.

Adequate upper airway (oral, pharyngeal and laryngeal) sensation is crucial for safe and efficient swallowing and airway protection. Despite its importance, methods of upper airway sensory testing for individuals with dysphagia remain poorly defined. The aim of this study was to summarise and appraise current methods of upper airway sensory testing to inform dysphagia clinical practice and future research directions. A systematic review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Studies that reported a method of upper airway sensory t

Treatment Approaches for Swallowing Disorders in Children.

A swallowing disorder or dysphagia is defined as a disorder in the sequence of swallowing, during the oral or pharyngeal phase, which compromises the safety and/or efficiency of transit of the food bolus to the esophagus. The evaluation of neurodevelopment, nutrition, and preventive medicine actions are as important as the clinical evaluation of dysphagia, so they must be included and systematized in all pediatric evaluations; This evaluation can be divided into different parts: bedside swallowing evaluation, instrumental swallowing evaluation, and additional studies. The management of swallow

Consistencies among miscellaneous scales for evaluation of post-stroke dysphagia.

PURPOSE: Post-stroke dysphagia (PSD) is the most common type of dysphagia. Stroke patients with sustained dysphagia have poorer outcomes. The severity of PSD is assessed using miscellaneous scales with unknown consistencies. We aim to investigate the consistencies among miscellaneous scales, which could aid in the assessment of PSD. METHODS: A total of 49 PSD patients were enrolled. Functional Oral Intake Scale (FOIS), Dysphagia Severity Scale (DSS), Ohkuma Questionnaire, Eating Assessment Tool-10, and Repetitive Saliva Swallowing Test were performed. FOIS was performed by physicians, and DSS

The Introduction of a Protocol for Esophageal Screening in Videofluoroscopic Swallowing Studies: Exploring Clinical Impacts and Barriers.

PURPOSE: Esophageal screening is a valuable inclusion in videofluoroscopic swallowing studies (VFSSs). However, routine standardized esophageal screening does not always occur in clinical practice. This study introduced and evaluated an esophageal screening protocol at one Australian hospital. METHOD: Radiology, gastroenterology, and speech-language pathology endorsed an esophageal screening protocol, which followed a timed 20-ml International Dysphagia Diet Standardisation Initiative Level 0 bolus from mouth to stomach in an upright anterior-posterior position. Mealcoach: Contact Microphone-Based Meal Supervision For Post-Stroke Dysphagia Patients.

Meal supervision for post-stroke dysphagia patients significantly improves prognosis during rehabilitation. Aspiration often occurs during meals, which may further incur aspiration pneumonia. Therefore, it's necessary to know the patient's swallowing ability as well as the occurrence of cough. Recently, some researchers have detected swallowing or coughing with audio signals and have made remarkable achievements. However, the users need to stay in quiet environments or wear uncomfortable cervical auscultation devices because the signals generated by swallowing are weak. In this work, we presen

A narrative review on the role and main findings of the Videofluoroscopic Study of Swallowing in Parkison's disease.

PURPOSE: Dysphagia is a common symptom in patients with Parkinson's disease (PD), though it may go undiagnosed until severe complications arise. Dysphagia can be suspected on a clinical basis, but an instrumental assessment is mandatory to confirm its presence and evaluate pathophysiological aspects and severity of the swallowing impairment. Aim of this review is to inform the clinician and the radiologist on the importance and the main radiological findings of the Video-Fluoroscopic-Swallow-Study (VFSS) in patients with PD starting from the most recent literature data on the topic. Dynamic evaluation of swallowing in pediatric patients with dysphagia: The Brazilian perspective.

Dynamic Evaluation of Swallowing (DES) is an exam which provides invaluable information on the swallowing function. This review describes the requirements, protocols and processes for DES in regions of Brazil. Dysphagia is a swallowing abnormality, in one or more swallowing phases, with risk and/or occurrence of aspiration, damage regarding nutrition and/or hydration and possible need of nonoral feeding methods. DES is performed in an armored room, using fluoroscopy, with the patient in lateral and anteroposterior views, in an orthostatic position. Different consistencies and volumes are teste

Physical properties of a model set of solid, texture-modified foods.

Those suffering from swallowing disorders, or dysphagia, require texture-modified foods for safe swallowing. The texture is modified according to the severity of the disorder, as maintained by the guidelines outlining classes of texture-modified foods, ranging from viscous soups to soft, solid foods. As a basis for studies of bolus rheology and oral response of solid texture-modified foods, a set of well-defined, solid foods has been identified and characterized regarding texture and physical properties. Gelled food is compared to both the firmer timbale class and to the corresponding regular

Translating Dysphagia Evidence into Practice While Avoiding Pitfalls: Assessing Bias Risk in Tracheostomy Literature.

Critically ill patients who require a tracheostomy often have dysphagia. Widespread practice guidelines have yet to be developed regarding the acute assessment and management of dysphagia in patients with tracheostomy. In order for clinicians to base their practice on the best available evidence, they must first assess the applicable literature and determine its quality. To inform guideline development, our objective was to assess literature quality concerning swallowing following tracheostomy in acute stages of critical illness in adults. Our systematic literature search (published previously
