Baseline situational analysis in Bangladesh, Jordan, Paraguay, the Philippines, Ukraine, and Zimbabwe for the WHO Special Initiative for Mental Health: Universal Health Coverage for Mental Health.

INTRODUCTION: Mental, neurological and substance use conditions lead to tremendous suffering, yet globally access to effective care is limited. In line with the 13th General Programme of Work (GPW 13), in 2019 the World Health Organization (WHO) launched the WHO Special Initiative for Mental Health: Universal Health Coverage for Mental Health to advance mental health policies, advocacy, and human rights and to scale up access to quality and affordable care for people living with mental health conditions. Six countries were selected as 'early-adopter' countries for the WHO Special Initiative fo

Housing First for People With Severe Mental Illness Who Are Homeless: A Review of the Research and Findings From the At Home-Chez soi Demonstration Project.

OBJECTIVE: To provide a review of the extant research literature on Housing First (HF) for people with severe mental illness (SMI) who are homeless and to describe the findings of the recently completed At Home (AH)-Chez soi (CS) demonstration project. HF represents a paradigm shift in the delivery of community mental health services, whereby people with SMI who are homeless are supported through assertive community treatment or intensive case management to move into regular housing. METHOD: The AH-CS demonstration project entailed a randomized controlled trial conducted in 5 Canadian cities

High support housing for people with serious mental illness in Canada: a scoping review.

INTRODUCTION: Mental health supportive housing aims to provide accommodation and support services to people with serious mental illness (SMI). Various supportive housing models have emerged in Canada post-deinstitutionalization, with heterogeneous and limited available information on congregate-based high support housing models (HSH) that provide 24/7 onsite supports. A registered scoping review was undertaken to identify the models and outcomes of HSH for people with SMI in Canada, including those exiting homelessness. METHODS: Four academic databases (Medline, Embase, PsycINFO, CINAHL Plus)

The Baldovan Institution Abuse Inquiry: a forgotten scandal.

In this paper, I resurrect a long-forgotten inquiry into abuse and maladministration at an institution for people with learning disabilities, the Baldovan Institution near Dundee, that has lain buried in the archives for the past 60 years. I contrast the response to it with the very different response to the similar revelations of the Ely Hospital Inquiry more than a decade later. Whereas Ely opened up the institutional sector to greater public scrutiny and brought with it a formal commitment from the government to shift the balance of care away from the long-term hospital, Baldovan produced r

A cost-effectiveness analysis of opioid substitution therapy upon prison release in reducing mortality among people with a history of opioid dependence.

AIM: Although opioid substitution therapy (OST) immediately after prison release reduces mortality, the cost-effectiveness of treatment has not been examined. Therefore, we undertook a cost-effectiveness analysis of OST treatment upon prison release and the prevention of death in the first 6 months post-release. DESIGN: Population-based, retrospective data linkage study using records of OST entrants (1985-2010), charges and court appearances (1993-2011), prison episodes (2000-11) and death notifications (1985-2011). SETTING: New South Wales, Australia. PARTICIPANTS: A cohort of 16,073 peopl

Living with support in a home in the community: predictors of behavioral development and household and community activity.

The purpose of this article was to review studies of behavioral development and household and community activity among adults with mental retardation living in community residential services and to distill knowledge about the factors that influence outcome. Research points to behavioral development occurring across the full spectrum of disability but influenced by mental retardation syndrome and the acquisition of pivotal skills. However, engagement in household and community activities has been found to be strongly related to individual adaptive behavior. People with more severe mental retard

Evaluation of the dissemination of active support training in staffed community residences.

Our aim was to replicate Active Support, a staff training intervention designed to increase the assistance given to adults with severe mental retardation living in community residences in order to increase their participation in activities. Training was conducted in 38 residences, involving 303 staff members and 106 persons with mental retardation. Active Support resulted in significant increases in assistance and engagement in activity. Significant correlations between participant abilities, receipt of assistance, and levels of engagement were found. Active Support was found to be of greater

Acceptability of prison-based take-home naloxone programmes among a cohort of incarcerated men with a history of regular injecting drug use.

BACKGROUND: Take-home naloxone (THN) programmes are an evidence-based opioid overdose prevention initiative. Elevated opioid overdose risk following prison release means release from custody provides an ideal opportunity for THN initiatives. However, whether Australian prisoners would utilise such programmes is unknown. We examined the acceptability of THN in a cohort of male prisoners with histories of regular injecting drug use (IDU) in Victoria, Australia. METHODS: The sample comprised 380 men from the Prison and Transition Health (PATH) Cohort Study; all of whom reported regular IDU in th

Homeless shelter users in the postdeinstitutionalization era.

OBJECTIVE: To describe the psychiatric symptomatology and mental health service needs of homeless shelter users in Calgary, Alberta. Data were collected as part of a broad-based community action initiative designed to reduce the problem of homelessness. METHODS: A semistructured interview was conducted with a representative sample of 250 emergency shelter users. Mental health problems were measured through self-reports of 9 psychiatric symptoms known to be related to illnesses prevalent among homeless populations (depression, anxiety, and psychoses). The CAGE alcohol screen was also used. RE

Health of the chronically mentally ill who rejoin the community: a community assessment.

This article describes findings from a community assessment project on 25 chronically mentally ill individuals who rejoined the community following discharge from psychiatric hospitals. The focus was to identify health concerns or problems and health promotion patterns using Healthy People 2000 as a guiding framework. Findings suggest that most participants found relocation to the community easy. Stress, depression, and abuse were mental health problems reported by participants. Physical health problems included high tobacco use, low exercise level, poor oral health, high-risk sexual behavior,

[Social workers and physicians--partners in social psychiatry? Assumptions and reality in a social psychiatry model institution].

With the reform of psychiatric and psychosocial care services, new professional groups, apart from the traditionally medical professions, have increasingly found their way into everyday therapy. Social workers in particular have gained significance in complementary fields of treatment. An empirical survey of the everyday situation in a comprehensive community mental health service system focusing on the therapist-couple "physician" and "social worker" collaborating to guarantee continuity of care for long-term treatment, yielded three results: 1. The dual-therapistsystem was convincingly effec

Positive behavioral support of adults with developmental disabilities: assessment of long-term adjustment and habilitation following restrictive treatment histories.

The long-term maintenance of behavioral treatment effects is an important measure of clinical significance but is not reported with regularity in the published literature. The present report concerned therapeutic maintenance by evaluating five adults with developmental disabilities who had been exposed to multiple, restrictive procedures (food deprivation, mechanical restraint, electric shock) in a prior residential treatment facility and were transitioned to a new habilitation setting where these procedures were terminated in favor of alternative methods of behavior support. As revealed throu

Movement of individuals with complex epilepsy from an institution into the community: seizure control and functional outcomes.

The successful community placement of 15 medically fragile individuals with profound mental retardation and complex epilepsy was examined. We found that simplification of anticonvulsants, early intervention for seizures, and improved staff education resulted in (a) fewer seizures, (b) decreases in emergency room visits for status seizures and drug-related side effects, and (c) increased levels of functioning as measured by activities of daily living. This program allowed for a medically fragile epileptic population to be transferred from the institution to the community.

Reassessment of state hospital patients diagnosed with schizophrenia.

Schizophrenic patients in long-term care programs may not have been carefully diagnosed according to current criteria. As part of a clinical reassessment program at a state hospital, the author randomly assessed 72 patients who carried a diagnosis of schizophrenia. The diagnosis of schizophrenia by DSM-III-R criteria was confirmed in 45 patients. Various organic disorders were diagnosed in seven patients. Four patients had bipolar affective disorder, manic; one patient had schizoaffective disorder, depressed; one patient had a substance use disorder; and two had primary mental retardation or p

Predictors of social relations in persons with schizophrenia living in the community: a Nordic multicentre study.

BACKGROUND: Deinstitutionalisation has led to persons with serious mental illness spending most of their time outside psychiatric institutions. Not much is known about their social life. The paper presents the results of structured interviews with non-institutionalised persons with schizophrenia about treatment, care and social network. The network data are analysed from three perspectives: finding predictors of the number and of the quality of social contacts, and establishing the respective variables that characterise persons with high, and those with low, scores on both the quantity and qua

Decentralization of psychiatric services in Malaysia--what is the prospect?

Since the Government was implementing the policy of decentralization of psychiatric services, a large number of chronic schizophrenic patients failed community management because of the breakdown of family support. The rejected patients were admitted to the Old Persons' Home for protection because no other suitable places were available for them. In a follow-up study in one of the Homes, the one-year prevalence rate of treated psychiatric illness and schizophrenia was found to be 27.5% and 15.3% respectively. Lack of rehabilitation and community care facilities and inadequate staff in all cate

Predicting nursing facility transition candidates using AID: a case study.

PURPOSE: Although the nursing facility transition literature is growing, little research has analyzed the characteristics of individuals so assisted or compared participants to those who remain institutionalized. This article describes an analytic method that researchers can apply to address these knowledge gaps, using the Arkansas Passages nursing facility transition program as a case study. DESIGN AND METHODS: This study employed Arkansas Minimum Data Set 2.0 data for 111 transitioned individuals, a derivation sample of 1,000 other residents, and a validation sample of all residents from th

Occurrence of workplace violence and the psychological consequences of it among nurses working in psychiatric outpatient settings.

WHAT IS KNOWN ON THE SUBJECT?: There is a scarcity of studies concerning violence in psychiatric outpatient settings in the 2010s in spite of the deinstitutionalization of psychiatric services. Previous research on violence in psychiatric outpatient settings has failed to consider the association between the psychological consequences of violence, exposures to violence and background factors. WHAT THE PAPER ADDS TO EXISTING KNOWLEDGE?: It has been known for some time that exposures to violence are harmful for psychiatric inpatient nurses; the paper demonstrates that psychiatric outpatient nurs
