Heterogeneity of vascular dementia: mechanisms and subgroups.

Today, multiple, thromboembolically generated cerebral infarcts are regarded as the main pathogenetic pathway of vascular dementia (VAD), with multi-infarct dementia (MID) as its clinical counterpart. However, taking into account other vascular mechanisms that may influence the brain, such as vessel-wall damage (atherosclerosis, hyalinosis, amyloid angiopathy, or blood-brain barrier dysfunction), cerebrovascular insufficiency (disturbance of systemic circulation, perfusion vulnerability related to the vascular anatomy of the brain, or disturbance of autoregulation), and hyperviscosity, it is e

Lower cognitive performance in normal older adult male twins carrying the apolipoprotein E epsilon 4 allele.

OBJECTIVE: Given the strong association of the apolipoprotein E (apoE) allele epsilon 4 with late-onset Alzheimer dementia or multi-infarct dementia, we tested whether normal older adult men with at least one epsilon 4 allele demonstrate subclinical changes in cognition and perform more poorly on tests of cognitive function compared with subjects without the epsilon 4 allele. DESIGN: Matched-pair design of normal adult male (average age, 63 years) fraternal twins. SETTING: Subjects voluntarily participated on an outpatient basis at a research or medical center facility. PARTICIPANTS: Member

Organic personality disorder in dementia syndromes: an inventory approach.

Personality alterations were investigated in patients with dementia of the Alzheimer type (DAT), multi-infarct dementia (MID), and normal control subjects, using a standardized informant-rated personality inventory. Personality alterations were observed in all patients, and the two diagnostic groups exhibited many similar behavioral alterations. DAT patients had significantly greater alterations in maturity and exhibited less personal control compared with the MID patients; the latter manifested more apathy and remained more affectionate and easy-going. Few correlations were found between the

Vascular dementia: a historical background.

The history of vascular dementia can be traced back to cases of dementia postapoplexy described by Thomas Willis in 1672. During most of the 18th and early 19th century, "brain congestion" (due in all likelihood to the effects of untreated hypertension) was the most frequent diagnosis for conditions ranging from stroke to anxiety and to cognitive decline, and bloodletting became the commonplace therapy. The modern history of vascular dementia began in 1894 with the contributions of Otto Binswanger and Alois Alzheimer, who separated vascular dementia from dementia paralytica caused by neurosyph

Elevated platelet microparticles in transient ischemic attacks, lacunar infarcts, and multiinfarct dementias.

Platelets release microparticles (PMP) upon activation. Elevated levels of PMP were observed in patients with immune thrombocytopenic purpura (ITP), sometimes associated with a syndrome resembling transient ischemic attack (TIA), suggesting a thrombogenic potential for PMP. To determine if this association applies to TIA and other cerebrovascular accidents (CVA) without ITP, we studied PMP profiles in 71 patients with ischemic CVA: 28 with small vessel CVA (SCVA), either lacunar infarcts or TIA; 24 with large vessel CVA (LCVA); 19 with multiinfarct dementia (MID); 12 with Alzheimer's dementia

Clinical diagnosis of Binswanger's disease.

To aid in the prospective study of Binswanger's disease, a poorly understood form of vascular dementia, a standardised criteria for its antemortem diagnosis was proposed. These criteria include dementia, bilateral radiological abnormalities on computed tomography (CT) or magnetic resonance imaging (MRI), and at least two of the following three clinical findings: A) a vascular risk factor or evidence of systemic vascular disease; B) evidence of focal cerebrovascular disease; and C) evidence of "subcortical" cerebral dysfunction. These criteria were validated in two ways. First, by retrospective

[Juvenile arteriosclerotic leukoencephalopathy: anatomoclinical study of a case].

A 34-year old right-handed man was suffering from recurrent cerebro-vascular insults. CT-scans revealed several subcortical lacunar infarcts, and leukoaraïosis. Arteriography of the left and the right carotid arteries was performed respectively on the 4th and the 9th year of the disease, and did not elicit significant extracranial and intracranial vascular lesions. There were no arguments in favor of infectious, inflammatory, or auto-immune vascular diseases. The patient had tardive hypertension and dementia, and died at the age of 44. Pathological findings, limited to the brain and cervical s

Global rating, symptoms, behavior, and cognitive performance as indicators of efficacy in clinical studies with nimodipine in elderly patients with cognitive impairment syndromes.

Nimodipine is a calcium antagonist which improves learning and memory in brain-lesioned or aged animals (LeVere & Sandin, 1989; Schuurman & Traber, 1989). It also accelerates the recovery of experimentally damaged sciatic nerves (van der Zee et al., 1987) and reduces age-associated gait abnormalities in aging rats (Schuurman et al., 1987). Selective action on cerebral vessels has also been proven. Vasoconstriction was prevented or reduced with nimodipine under experimental conditions (Toward, 1981) and cerebral blood flow could be increased (Kazda et al., 1982). The drug has been tested in sub

Marked increase in cerebrospinal fluid ubiquitin in Creutzfeldt-Jakob disease.

We have established the radioimmunoassay for ubiquitin in the cerebrospinal fluid (CSF) and measured the ubiquitin concentration in CSF from 4 cases of neuropathologically verified Creutzfeldt-Jakob disease (CJD), 10 cases of multi-infarct dementia (MID), 7 cases of senile dementia of Alzheimer type (SDAT), and 18 controls. The normal values were determined to range from 7.3 to 21.0 ng/ml, 14.3 +/- 1.1 ng/ml in the mean +/- S.E.M. The CSF ubiquitin levels in the cases of MID and SDAT were 16.6 +/- 6.4 ng/ml and 21.3 +/- 6.1 ng/ml, respectively. In the cases of CJD, the CSF ubiquitin was marked

Apolipoprotein E epsilon4 association with dementia in a population-based study: The Framingham study.

Apolipoprotein E type 4 allele (apoE epsilon4) is associated with Alzheimer's disease (AD) in the late-onset familial form and in sporadic cases, but the age-associated risk in a randomly sampled elderly population is not established. We examined the association of apoE epsilon4 with AD and other dementias (mainly multi-infarct or dementia following stroke) in 1,030 persons aged 71 to 100 years in the population-based Framingham Study cohort. Kaplan-Meier survival analysis revealed that 55% of the apoE epsilon4/epsilon4 homozygotes developed AD by age 80, whereas 27% of apoE epsilon3/epsilon4

[New aspects of senile and presenile dementia].

Problems of research in dementia are presented. It is a matter both of structural disturbances in the texture of conditions which impair the texture of conditions of possible functions, as well as of actual functional disturbances of the brain. The numerous etiological and diagnostic aspects resulting from this show distinctly that the dementia does not exist. In the numerous dementias which appear as diseases of higher age, the distinction between true old age diseases (Alzheimer, Pick, progressive glial dystrophy) and the cerebrovascular dementias (multi-infarct-dementia, Binswanger's diseas

White matter damage is associated with matrix metalloproteinases in vascular dementia.

BACKGROUND AND PURPOSE: Vascular disease causes multi-infarct dementia (MID) or Binswanger's disease (BD), the latter of which is a progressive form of vascular dementia (VaD) associated pathologically with fibrinoid and hyaline changes in brain arterioles with injury to the white matter. Clinically, BD patients have long-standing hypertension with disturbances of gait and intellect. Because matrix metalloproteinases (MMPs) are important in cerebral infarction, we hypothesized that disturbances in the MMPs may be involved in VAD: METHODS: Brain tissues from 5 patients with VaD of the BD or mu

Outcome of shunt operation on urinary incontinence in normal pressure hydrocephalus predicted by lumbar puncture.

Bladder function in four patients with normal pressure hydrocephalus, one with dementia of Alzheimer type and five patients with multi-infarct dementia was studied by history and urodynamic tests (cystometry and Bor's ice water test). The bladder hyperactivity could be temporarily improved by a lumbar puncture and removal of 50 ml CSF and later abolished by a shunt operation in patients with normal pressure hydrocephalus while no changes occurred in the other patients. Urodynamic testing in connection with a lumbar puncture may be a reliable way to predict the outcome of a shunt operation in c

Presenile dementia--etiology, clinical profile and treatment response at four month follow up.

Dementia is the development of multiple cognitive deficits that includes memory impairment and at least one of the following--Aphasia, apraxia, agnosia or disturbances in executive functioning. The common causes of dementia among the elderly are Alzheimer's disease, vascular dementia, mixed dementia and Lewy body disease. The concept of reversible dementia was introduced in 1980 when a task force sponsored by National Institute of Ageing found 10-12% of dementia cases in older group to have reversible causes such as metabolic-nutritional, drugs, infections, psychiatric disorders etc. In our se

Clinico-tomographical aspects in the pathogenesis of vascular dementia.

This article discusses the role of some pathogenetical factors in the main morphological variants of vascular dementia and their correlation with certain clinical parameters. On the basis of computer-tomographical (CT) studies of 62 patients with dementia, it was possible to distinguish three groups of patients, according to the type of parenchymatous brain lesions: (1) 14 patients with "pure" leukoaraiosis (LA); (2) 23 patients with ischemic foci (IF), and (3) 25 patients with a combination of LA and IF. A comparative study of these groups confirmed the concept that in the development of LA a

Distribution of major dementias by race and sex in South Carolina.

Preliminary data from the newly implemented Registry for dementing illnesses was used to examine the distribution of four types of dementia in black and white residents of South Carolina. The data for 1464 subjects were abstracted by field research nurses in state mental health facilities. Overall, 649 patients (44.3%) were black and 765 (52.3%) were white. Women comprised 53.8% of all cases of dementia in this study. The overall distribution included 66% Alzheimer's disease (AD), 12% multi-infarct dementia (MID), 10% alcoholic dementia (ALC), nine percent other-medical and three percent other

Chronic information-processing changes in individuals with a first-ever clinical lacunar syndrome.

OBJECTIVE: The aim of this study was to examine whether chronic cognitive changes occur after a first-ever single clinical lacunar syndrome. BACKGROUND: Patients who have been diagnosed with a first-ever single clinical lacunar syndrome, on the basis of clinical rather than radiologic criteria, perform normally on general clinical neuropsychologic measures. METHOD: We examined information-processing abilities in 17 first-ever lacunar syndrome participants [lacunar anterior circulation infarct (LACI)], using 2 experimental tasks of information processing. Magnetic resonance imaging of the ageing brain.

Many changes occur in the brain with normal ageing. These have been extensively investigated by autopsy studies and more recently with high resolution MRI. Dementia is a major public health problem worldwide which affects the elderly population. Magnetic resonance imaging (MRI) is the imaging test of choice to identify underlying treatable causes of dementia, including normal pressure hydrocephalus, subdural haematoma and intracranial tumour. It is important to understand the normal changes which occur with ageing and these should not be interpreted as pathological lesions on MRI. Though there
