Atypical femoral fracture with early PET/CT changes from denosumab for metastatic breast cancer and transition from denosumab to zoledronic acid.

Atypical femoral fractures (AFFs) are potential complications arising from long-term use of bone modifying agents such as denosumab. We describe a case of bilateral AFFs that was treated surgically in a middle-aged woman receiving oncologic doses of denosumab for treatment of metastatic breast cancer for seven years. In retrospect, early cortical change was present on scout computer tomography (CT) and positron emission tomography-computer tomography (PET-CT) prior to her left AFF. This case highlights the potential for earlier opportunistic AFF radiologic screening during cancer surveillance.

Atypical femoral fracture and jaw osteonecrosis under high doses of denosumab, healed with the aid of low dose of denosumab: a case report.

Atypical femoral fractures (AFF) and osteonecrosis of the jaw (ONJ) are rare but serious complications associated with long-term use of antiresorptive therapies such as denosumab. Denosumab discontinuation to allow for bone healing is not possible because of the high risk of spontaneous multiple vertebral fractures. We present a case of concurrent AFF and ONJ in a patient previously treated with denosumab 120mg monthly. The patient was managed with a low-dose denosumab regimen tailored according to bone turnover marker monitoring, resulting in successful bone union and resolution of ONJ.

Denosumab mid-term densitometric gain in postmenopausal osteoporosis women in clinical practice: comment on "Variability of denosumab densitometric response in postmenopausal osteoporosis".

We aim to confirm previous observations among postmenopausal osteoporotic women who received denosumab in terms of densitometric changes. We performed an observational clinical practice scenario study assessing baseline bone mineral density and its change after 2 years of treatment in postmenopausal osteoporotic women. Most of our patients were severe and had previous fracture and had received several treatments before denosumab initiation. We observed a 10.06% (± 3.46) improvement in spine, and a 4.87% (± 4.78) improvement in femoral neck. Our findings are in accordance with other authors tha

Effects of denosumab, alendronate, or denosumab following alendronate on bone turnover, calcium homeostasis, bone mass and bone strength in ovariectomized cynomolgus monkeys.

Postmenopausal osteoporosis is a chronic disease wherein increased bone remodeling reduces bone mass and bone strength. Antiresorptive agents including bisphosphonates are commonly used to mitigate bone loss and fracture risk. Osteoclast inhibition via denosumab (DMAb), a RANKL inhibitor, is a newer approach for reducing fracture risk in patients at increased risk for fracture. The safety of transitioning from bisphosphonate therapy (alendronate; ALN) to DMAb was examined in mature ovariectomized (OVX) cynomolgus monkeys (cynos). One day after OVX, cynos (7-10/group) were treated with vehicle

Discontinuation of denosumab and associated fracture incidence: analysis from the Fracture Reduction Evaluation of Denosumab in Osteoporosis Every 6 Months (FREEDOM) trial.

Osteoporosis is a chronic disease and requires long-term treatment with pharmacologic therapy to ensure sustained antifracture benefit. Denosumab reduced the risk for new vertebral, nonvertebral, and hip fractures over 36 months in the Fracture Reduction Evaluation of Denosumab in Osteoporosis Every 6 Months (FREEDOM) trial. Whereas discontinuation of denosumab has been associated with transient increases in bone remodeling and declines in bone mineral density (BMD), the effect on fracture risk during treatment cessation is not as well characterized. To understand the fracture incidence betwee

Importance of prompt antiresorptive therapy in postmenopausal women discontinuing teriparatide or denosumab: The Denosumab and Teriparatide Follow-up study (DATA-Follow-up).

When teriparatide and denosumab are discontinued, bone mineral density (BMD) abruptly decreases. To compare rates of bone loss in postmenopausal women who discontinue denosumab or teriparatide and receive no additional prescription osteoporosis medications to women who discontinue these drugs followed by prompt antiresorptive therapy, we asked women concluding the Denosumab and Teriparatide Administration (DATA) study and its extension, DATA-Switch, to return for BMD measurements 1-2years after study completion. In these studies, women received 2-years of either teriparatide, denosumab or both

Pembrolizumab and Denosumab in Clear-Cell Renal-Cell Carcinoma.

BACKGROUND: Anti-PD1 immune checkpoint inhibitors (ICI) are effective in clear-cell renal-cell carcinoma (ccRCC). Preliminary data suggest inhibition of Receptor Activator of Nuclear Factor-κB Ligand (RANKL) signaling may potentiate ICI efficacy. We evaluated the activity and safety of the RANKL-inhibitor denosumab with pembrolizumab in people with pretreated advanced ccRCC. METHODS: This single-arm, multi-center phase 2 trial enrolled participants with metastatic or unresectable ccRCC progressing on or after VEGFR-targeted tyrosine kinase inhibitor (TKI) therapy. Participants received pembro

Idiopathic Hypoparathyroidism in an Osteoporosis Patient Receiving Denosumab Therapy.

Idiopathic hypoparathyroidism is a rare endocrine disorder characterized by impaired parathyroid hormone (PTH) secretion leading to hypocalcemia and hyperphosphatemia. We report the case of a 71-year-old female patient receiving denosumab for osteoporosis who was found to have asymptomatic hypocalcemia prior to her fifth denosumab injection. Lab results revealed a low ionized calcium level of 4.45 mg/dL, a low normal total calcium level of 8.2 mg/dL, and an inappropriately low PTH level of 17 pg/mL, with normal magnesium and vitamin D levels. Calcium levels were normal during the previous four

Metaphyseal band fractures in pediatric patients treated with denosumab.

OBJECTIVE: Denosumab, a potent antiresorptive agent targeting receptor activator of nuclear factor kappa-B ligand (RANKL), is increasingly used off-label in RANKL-mediated pediatric bone disorders, but concerns remain regarding rebound hypercalcemia and altered bone modeling and remodeling. This study describes clinical, biochemical, and radiological features of metaphyseal sclerotic band fractures in children on denosumab and assesses risk factors with focus on timing and occurrence of rebound hypercalcemia. DESIGN AND METHODS: We conducted a retrospective multicenter study. Clinical, bioche

Denosumab-Induced Hypocalcemia after Billroth II Gastric Bypass Surgery.

Hypocalcemia is a known risk following bariatric surgery and can contribute to the development of osteoporosis. Osteoporosis is commonly treated with denosumab, though denosumab can exacerbate underlying abnormalities in calcium homeostasis. We present the case of a 59-year-old female with severe hypocalcemia who had been treated with denosumab for osteoporosis three months before and had Billroth II gastric bypass surgery 15 years before, for bariatric purposes. Intravenous calcium supplementation was used to correct the initial electrolyte abnormality, and the patient was able to maintain ap

[A Case of atypical Femoral Fracture after Dosing Denosumab].

For the management of patients with castration-resistant prostate cancer with bone metastases, bisphosphonates and denosumab are used to prevent skeletal related events. Osteonecrosis of the jaw and hypocalcemia have been reported in patients treated with denosumab, but there have been few reports of atypical femoral fracture (AFF). Here, we report a case of AFF after dosing denosumab. A 69-year-old man with prostate-specific antigen (PSA) level of 13.08 ng/ml was diagnosed with adenocarcinoma of the prostate, cT3a, N0, M1b, with Gleason score of 4+4=8 and bone metastases to pubis and ischium.

Off-label uses of denosumab in metabolic bone diseases.

Denosumab (Dmab), a monoclonal antibody against the receptor activator of nuclear factor-κB (RANK) ligand (RANKL) which substantially suppresses osteoclast activity, has been approved for the treatment of common metabolic bone diseases, including postmenopausal osteoporosis, male osteoporosis, and glucocorticoid-induced osteoporosis, in which the pathway of the RANK/RANKL/osteoprotegerin is dysregulated. However, the imbalance of RANKL/RANK/osteoprotegerin is also implicated in the pathogenesis of several other rare metabolic bone diseases, including Juvenile Paget disease, fibrous dysplasia,

[Denosumab in clinical practice : beware before, during and after].

Denosumab is a very effective treatment of osteoporosis, easy to use and very well tolerated. Due to some associated risks, a close clinical follow-up is necessary. Before the first injection, it is necessary to correct hypocalcaemia or vitamin D deficiency when present. Calcemia has to be followed in case of renal insufficiency. Injections of denosumab should be done scrupulously every 6 months (± 3 weeks). Discontinuation of denosumab is associated with a severe rebound effect characterized by increased markers of bone remodeling, a rapid decrease of bone density values, and a risk of multip

Bone Loss After Denosumab: Only Partial Protection with Zoledronate.

A case series of six women with postmenopausal osteoporosis who had received continuous denosumab for 7 years and were then given a single infusion of zoledronate (5 mg) is reported. During denosumab treatment, bone mineral density (BMD) in the spine increased 18.5% (P = 0.006), and total hip BMD by 6.9% (P = 0.03). Post-zoledronate BMDs were measured 18-23 months after treatment, and there were significant declines at each site (P spine = 0.043, P hip = 0.005). Spine BMD remained significantly above the pre-denosumab baseline (+9.3%, P = 0.003), but hip BMD was not significantly different fro

Severe Hypocalcemia due to Denosumab in Metastatic Prostate Cancer.

Denosumab is a monoclonal antibody used for prevention of skeletal-related events (SREs) in patients with bone metastases from solid tumors. Hypocalcemia is a rare and dangerous side effect of the drug Denosumab. We present a case of a patient with metastatic prostate cancer who developed severe hypocalcemia after the administration of the drug. The patient's vitamin D levels were low when checked after administration of the drug, which likely predisposed him to the development of hypocalcemia. He was placed on high doses of oral and intravenous (IV) calcium and vitamin D without any appreciab

Severe hypocalcemia following denosumab injection in a hemodialysis patient.

Denosumab is a human monoclonal antibody directed against RANKL (receptor activator of NF-κB ligand) and is a novel treatment for postmenopausal osteoporosis, although its safety and efficacy in end-stage renal disease is unclear. We report the case of a 61-year-old female hemodialysis patient who developed severe hypocalcemia (total serum calcium, 5.37 mg/dL [1.34 mmol/L]) after receiving a single subcutaneous injection of denosumab. We review this medication's mechanism of action and the very limited data regarding its use in stage 5 chronic kidney disease. We advise against the use of denos
