PURPOSE: To report the clinical course and management of graft-host interface Nocardia keratitis after Descemet membrane endothelial keratoplasty (DMEK).
METHODS: A 70-year-old man presented with a corneal epithelial defect, stromal edema, graft infiltrate, and graft-host interface infection 5 months after an uneventful DMEK performed for bullous keratopathy in the left eye. Corneal scrapings from the margin of epithelial defect showed gram-positive bacillus, and the organism was identified as Nocardia asteroides.
RESULTS: Intensive and appropriate topical and systemic antibiotic therapy res

PURPOSE: To describe a technique of using a femtosecond laser to assist with graft preparation for Descemet membrane endothelial keratoplasty (DMEK).
METHODS: A femtosecond laser was used to assist in graft preparation for 3 DMEK procedures. The donor cornea is mounted on a proprietary artificial anterior chamber. A partial thickness deep circular cut is made with the femtosecond laser through the endothelium, Descemet membrane, and posterior stroma. Graft preparation is then completed without the use of a corneal trephine.
RESULTS: In all cases, graft preparation was successfully completed.

PURPOSE: Descemet membrane endothelial keratoplasty (DMEK) is superior to penetrating keratoplasty (PK) in terms of visual rehabilitation, intraoperative safety and risk of rejection. Therefore, it seems reasonable to perform DMEK in eyes with endothelial failure following PK. We herein report our first clinical results.
METHODS: Nineteen eyes with endothelial graft failure following PK were treated with DMEK. The majority of these eyes (12) had limited visual potential. The major indication for DMEK was pain relief in patients with bullous keratopathy. Visual acuity (VA), central corneal thi

PURPOSE: To describe a new technique for unfolding of the Descemet membrane (DM) during Descemet membrane endothelial keratoplasty.
METHODS: DM and endothelium were stripped from donor corneas submerged in corneal storage solution. Donor DM diameters were 8.5 mm. The central 8 mm of DM was stripped from the recipient cornea. After staining with trypan blue to improve visualization, donor DM was inserted through a 2.75-mm incision. Unfolding was achieved with simultaneous digital pressure in the equatorial region and tapping of the corneal surface. This technique was performed in Descemet memb

We present a unique method of retrocorneal membrane removal with a femtosecond laser (FSL). A 22-year-old male who had undergone penetrating keratoplasty had a retained retrocorneal membrane and a double anterior chamber postoperatively. The membrane was dissected completely with the FSL and the free-floating membrane was removed. Histopathological evaluation confirmed the diagnosis of retained Descemets membrane (DM). There was improvement in uncorrected visual acuity from 20/300 to 20/50. Central corneal endothelial cell count was 810 cells/mm(2) preoperatively and 778 cells/mm(2) postoperat

Descemet's membrane detachment (DMD) is a rare but potentially serious complication of intraocular surgery, most commonly cataract extraction. Many predisposing factors of DMD have been postulated; however, none have proved to be significant. Most DMDs are surgically induced; however, the occurrence of this entity bilaterally, during or after otherwise uneventful cataract extraction surgery, might be related to an inherent abnormal adhesion between the stroma and Descemet's membrane or pre-existing endothelial abnormalities. Early recognition and proper management are important to preserve the

Descemet's membrane detachment is a condition with a wide range of etiologies. The most common cause is a localized detachment occurring after cataract surgery. We report a case of vancomycin injection-induced Descemet's membrane detachment as a complication following a routine cataract surgery and its management.

BACKGROUND AND OBJECTIVE: Descemet's membrane detachments are an uncommon complication after cataract surgery that can result in severe visual loss. A combined technique of intracameral gas injection and transcorneal suturing for the repair of Descemet's membrane detachments is described.
PATIENTS AND METHODS: In this interventional case series, four cases of Descemet's membrane detachments with associated corneal edema observed following cataract surgery were successfully repaired using a combined technique of intracameral gas injection and transcorneal suturing.
RESULTS: In all cases, Desc

We report a case of successful attachment of a detached Descemet's membrane using an intracameral injection of sulfur hexafluoride (SF6). Three months after cataract surgery, a 77-year-old woman was seen for almost total detachment of Descemet's membrane. Surgical repair with a through-and-through mattress suture was unsuccessful. One month later, Descemet's membrane was apposed by injecting 0.25 ml of 100% SF6 into the anterior chamber. Six months later the central corneal thickness measured 0.48 mm, and Descemet's membrane was attached. We believe this to be the first reported case of succes

A 39-year-old woman with progressive left unilateral glaucoma associated with corneal edema, iris changes, and peripheral anterior synechia formation underwent a left trabeculectomy and iridectomy in an attempt to control the intraocular pressure. Shortly after surgery the edema worsened despite low intraocular pressure measurements. A repeat trabeculectomy combined with a full thickness keratoplasty was performed 11/2 years later. The posterior corneal surface was found to be covered by a partially doubled layer of endothelial cells with ultrastructural features resembling epithelium. Similar

PURPOSE: According to some pioneer surgeons, lamellar endothelial keratoplasty techniques (EK), including Descemet membrane endothelial keratoplasty (DMEK) and Descemet stripping automated endothelial keratoplasty (DSAEK), yield excellent clinical results. However, there is a lack of studies with high levels of evidence and results of large national keratoplasty registers are contradictory. Therefore, two large cohorts of DMEK and DSAEK procedures are compared to a cohort of penetrating keratoplasty (PK).
METHODS: The study reports 868 keratoplasty procedures at a single centre (694 eyes with
