BACKGROUND: The use of transvenous pacing leads is associated with the risk of developing tricuspid valve (TV) dysfunction. This develops through several mechanisms including the failure of leaflet coaptation or direct damage to the TV or to its sub-valvular apparatus and can result in significant tricuspid regurgitation (TR). Multiple approaches to pacemaker implantation after transvenous lead extraction (TLE) or surgical TV repair have been described. Placement of pacing leads across the TV is generally avoided in such circumstances.

Fungal pacemaker endocarditis (FPE) is a rare but potentially fatal infection that can occur in patients with implanted cardiac devices. Diagnosing FPE can be difficult due to its subtle and non-specific symptoms, which often necessitate heightened clinical awareness to identify. Management typically involves removal of the infected device combined with antifungal therapy. We describe a 79-year-old female patient who developed Candida albicans endocarditis involving a pacemaker lead, successfully managed through coordinated surgical extraction of the device and systemic antifungal therapy.

When an implanted device is infected, complete explantation of the device system, including lead extraction, is usually required. Superficial problems with wound healing may be managed more conservatively, but distinguishing between a surface process and deeper infection can pose a clinical challenge. We present a case of poor wound healing after an ICD pocket revision procedure, and an allergic reaction to the suture material was found to be the cause. Diagnosis, management, and future implications of suture allergy are discussed.

Cardiovascular implantable electronic devices (CIEDs) such as pacemakers and implantable cardioverter defibrillators require the placement of a transvenous lead through the superior vena cava (SVC), which can be difficult if there is stenosis or obstruction of the SVC. Moreover, SVC syndrome may occur after the lead is inserted even if the SVC was intact before the implantation. Therefore, there is need of an appropriate strategy for handling stenosis or obstruction of SVC during lead placement. In addition, advances are being made in CIEDs that do not require transvenous leads, and thus CIEDs

BACKGROUND AND AIMS: Over the past decades, high-volume lead extraction centres gained broad experience in advanced transvenous lead extractions (TLE). However, access to advanced TLE techniques is limited, thus it would be beneficial to predict which extractions can be performed safely and effectively by simple procedures. We aimed to report on predictors of procedure complexity in clinically successful TLEs.
METHODS: Our retrospective study included 70 infectious TLEs, of which 65 were clinically successful. TLEs were defined as simple if only simple manual traction or a locking stylet was

Over the last decades, the implementation of new technology in cardiac pacemakers and defibrillators as well as the increasing life expectancy have been associated with a higher incidence of transvenous lead complications over time. Variable degrees of venous stenosis at the level of the subclavian vein, the innominate trunk and the superior vena cava are reported in up to 50% of implanted patients. Importantly, the number of implanted leads seems to be the main risk factor for such complications. Extraction of abandoned or dysfunctional leads is a potential solution to overcome venous stenosi

Infections are known complications of cardiovascular implantable electronic devices (CIEDs). We describe a case of a 62-year-old male who presented with pulseless electrical activity (PEA) cardiac arrest and respiratory failure. He had a history of cardiac resynchronization device and defibrillator (CRT-D) implantation for nonischemic cardiomyopathy. After resuscitation, he was found to have methicillin sensitive Staphylococcus aureus (MSSA) bacteremia on blood culture and large vegetations on the CRT-D lead and tricuspid valve found on echocardiography. The patient underwent extraction of the

Right ventricular perforation leading to cardiac tamponade can occur during the chronic phase after cardiac device implantation. Physicians who manage the pacemaker clinic must be alert to the wide range of symptoms and signs that can accompany delayed right ventricular perforation. Surgical rather than percutaneous lead extraction may be prudent.

OBJECTIVES: The aim of this study was to evaluate the utility of transesophageal echocardiography (TEE) during transvenous lead extraction (TLE) involving both conventional and laser lead removal.
BACKGROUND: TLE carries a small but measurable risk of serious adverse events. Few studies have examined the potential benefit of continuous monitoring with TEE during this procedure.
METHODS: Continuous TEE monitoring was performed in 100 consecutive patients (67% male; average age, 57 ± 17 years) who underwent TLE in the past 5 years. Lead extraction was attempted for 193 leads. The average time

We report three patients with screw-in lead perforation in the right atrial free wall not long after device implantation. All the patients complained of intermittent stabbing chest pain associated with deep breathing during the implantation. The "dry" epicardial puncture was utilized to avoid hemopericardium during lead extraction in the first case. The atrial electrode was repositioned in all cases and replaced by a new passive fixation lead in two patients with resolution of the pneumothorax or pericardial effusion. A literature review of 50 reported cases of atrial lead perforation was adde

A 57-year-old male with a dual chamber pacemaker and symptomatic, persistent atrial fibrillation (AF) accompanying a febrile illness presented for elective transesophageal echocardiography (TEE)-guided cardioversion. The patient was found to have a large 2.5 cm × 2.3 cm, mobile mass attached to the right atrial lead. Following device and lead extraction, he developed progressive shortness of breath which was attributed to his underlying arrhythmia. One month later, AF ablation was pursued and preablation TEE revealed a dilated main pulmonary artery with a 1.8 cm × 1.6 cm mass in the distal lef

INTRODUCTION: The optimal approach to the extraction of leads with large vegetations remains uncertain.
METHODS: High-risk patients with lead associated vegetations undergoing device extraction at Vanderbilt Hospital with concomitant use of the Penumbra Aspiration System (Penumbra Inc, Alameda, CA) are described. An 8.5 Fr Agilis NXT (Abbott Inc, St. Paul, MN) was advanced to the right atrium, through which a Penumbra Indigo Cat-8 catheter was advanced. Using intracardiac echocardiography, the Penumbra was positioned directly on the vegetation, suction was applied until adherent, and the Indi

Infective endocarditis involving cardiac device leads remains a serious complication in patients with intracardiac hardware, particularly in the setting of bacteremia. We present a case of a 47-year-old female with heart failure with reduced ejection fraction (HFrEF) and an automated implantable cardioverter-defibrillator (AICD) who developed Group B Streptococcus (Streptococcus agalactiae) bacteremia with evidence of both lead-associated and tricuspid valve vegetations. The case highlights diagnostic challenges in transthoracic echocardiography (TTE), multidisciplinary management, and the rol

Pacemaker lead infections are complications of pacemakers that occur infrequently (0.13-2.11 per 1,000 device recipients) but cause significant morbidity and mortality, especially in high-risk cohorts such as intravenous drug users (IVDUs). We report a 36-year-old male with pacemaker lead infection with methicillin-sensitive Staphylococcus aureus (MSSA). Despite the extraction of the entire pacemaker system and medical therapy based on the latest guidelines, reimplantation of a pacemaker in the setting of ongoing intravenous (IV) heroin abuse led to recurrent infection and 2.2 cm right atrial

BACKGROUND: Device reimplantation after extraction because of cardiac implantable electronic device (CIED) infection in pacemaker-dependent patients can be challenging in individuals with limited access options.
OBJECTIVE: The purpose of this study was to describe a straightforward, low lateral thoracic implantation technique for patients with a patent axillary vein but unavailable bilateral pectoral sites.
METHODS: Nine pacemaker-dependent patients (mean age 70 ± 13 years, 7 male) who underwent CIED extraction and low lateral thoracic reimplantation in whom bilateral pectoral sites were una

BACKGROUND: The number of lead extractions is growing because of the greater population and increasing age of individuals with a cardiac implantable electronic device. Lead extraction procedures can be complex undertakings with risk of significant mortality, and vascular tears in the superior vena cava are of greatest concern.
OBJECTIVE: The purpose of this study was to study whether a novel algorithm that analyzes pre-extraction computed tomographic (CT) images can determine the likelihood and location of lead-lead interactions and lead-vessel attachment within patients' venous vasculatures.

BACKGROUND: Nowadays, transvenous lead extraction (TLE) is considered an essential technique in lead management strategy. Since 2011, a multidisciplinary approach was undertaken in our centre involving electrophysiologists, cardiac surgeons and anaesthesiologists to improve cross- unit cooperation and minimize complications and mortality. The present paper reports procedural outcomes and complications of our lead extraction experience.
METHODS: We retrospectively collected and analysed data from all consecutive patients undergoing cardiac implantable electronic device leads TLE at the IRCCS C

BACKGROUND: A 'lead ghost' is a fibrotic or thrombotic cast that remains after transvenous lead extraction (TLE) and typically resides within the right heart. This case describes a paradoxical embolism through an unrecognized patent foramen ovale (PFO), most likely due to a transient right-to-left shunt that allowed the 'lead ghost' to enter the systemic circulation.
CASE SUMMARY: A 55-year-old man with ischaemic cardiomyopathy (ICM) underwent cardiac resynchronization therapy-defibrillator lead extraction for right-ventricular (RV) lead fracture. Intraoperative transoesophageal echocardiogra

The number of transvenous extraction procedures for leads of intracardiac rhythm devices increased recently and the most common reason for lead extraction is device infection. In some cases, combined use of two different techniques is necessary. Long-term results of this approach are not clear. In this case report, we present a patient with biventricular implantable cardioverter defibrillator who underwent percutaneous hardware removal due to recurrent pacemaker pocket infections. Mechanical dilator sheath removal and Needle's Eye Snare were used in the same patient. Although asymptomatic, tri
