Pathogenesis of diabetic retinopathy--the missing link?

Release of angiogenic factors in response to the ischaemic retina is currently the most favoured hypothesis for the pathogenesis of proliferative diabetic retinopathy. Reducing the stimulus for angiogenesis by destroying the ischaemic retina also forms the basis of the most effective treatment of diabetic retinopathy by photocoagulation. Though ischaemia is undoubtedly important for neovascularization, there is recent evidence which cast doubts on ischemia being the sole mechanism for diabetic retinopathy. Many clinical observations viz. the protective effects of glaucoma, myopia, unilateral c

Ten years after the Diabetic Retinopathy Study.

The Diabetic Retinopathy Study (DRS) demonstrated that prompt argon laser photocoagulation, in comparison to indefinite delay in treatment, could reduce by more than 50% the risk of severe visual loss from proliferative diabetic retinopathy. The DRS also enlightened two generations of ophthalmologists about the value of the randomized controlled clinical trial as a way of assessing new and existing treatments for unsolved therapeutic problems. After the example of the DRS, laser treatment was shown to be effective in reducing vision loss in selected patients with branch retinal vein occlusion,

Hyperglycemia as a cause of diabetic retinopathy.

Diabetes mellitus is marked by hyperglycemia and a variety of other metabolic disorders. The significance of hyperglycemia in the pathogenesis of diabetic retinopathy has proven difficult to evaluate in patients. Diabetic dogs are known to develop retinal lesions morphologically identical to those typical of diabetes in man, provided hyperglycemia in the animal is allowed to persist at least for many months and usually for 3 to 5 years. The development of retinopathy in this animal model can be inhibited by careful improvement of diabetic (glycemic) control. Comparable retinopathy has recently

Diabetic retinopathy. Assessment of severity and progression.

This study describes a classification of overall retinopathy severity based on the presence and severity of lesions graded in the Airlie House classification of diabetic retinopathy, and demonstrates its use in assessing progression of retinopathy over a 6-year period in a group of insulin-taking patients. One hundred and ninety-one insulin-taking patients with diabetes of at least 5 years' duration were identified from their doctors' records in 1970-1971. Patients were seen again in 1972-1973 and 1976-1977. Stereoscopic color fundus photographs of 7 fields in each eye were taken at each visit

Unilateral proliferative diabetic retinopathy. I. Initial findings.

The combination of unilateral proliferative diabetic retinopathy (PDR) in one eye and nonproliferative diabetic retinopathy (NPDR) in the fellow eye was observed in 10.1% of patients with PDR. Most patients were between the ages of 50 and 70 years; the majority had adultonset diabetes mellitus. Unilateral aphakia and anisometropia of greater than 1 diopter were both infrequently seen. The incidence of glaucoma was unremarkable. Unilateral elevated-but not necessarily abnormal-intraocular pressure was found in a significantly larger proportion of eyes with NPDR (20%) than with PDR (12%). Most p

Hypomagnesemia, a risk factor in diabetic retinopathy.

The serum magnesium concentration was measured in 71 insulin-treated diabetic outpatients who had had the disease for 10 to 20 years. The patients were divided into two subgroups according to the severity of their retinopathy. As a whole the patients exhibited a definite hypomagnesemia (P less than 0.001) that was most pronounced in the subgroup having the severest degree of retinopathy (P less than 0.01). The subgroups were comparable regarding known risk factors implicated in diabetic retinopathy. Thus, hypomagnesemia appears to be an additional risk factor in the development and progress of

Genetic association of TIE2 with diabetic retinopathy and diabetic macular edema.

PURPOSE: To evaluate the associations of the TIE2 gene with diabetic retinopathy (DR) and diabetic macular edema (DME). METHODS: This study included a Chinese cohort of 285 non-proliferative DR patients and 433 healthy controls. The DR patients were classified further into those with or without DME. Thirty haplotype-tagging single-nucleotide polymorphisms (SNPs) in TIE2 were genotyped using TaqMan technology. Associations of DR and subtypes were analyzed by logistic regression adjusted for age and sex. Stratification association analysis by sex was performed. RESULTS: TIE2 rs625767 showed a

Choroidal Changes in Diabetic Patients With Different Stages of Diabetic Retinopathy.

Diabetic retinopathy (DR) is one of the long-term microvascular complications of diabetes mellitus (DM) and is considered a leading cause of vision loss worldwide. Chronic hyperglycemia can cause microvascular abnormalities to the retina and the choroid as well. The vascular tissue of the choroid supplies blood to the outer retina, photoreceptors, and retinal pigment epithelium. It plays an important role in the metabolic exchange of the retina. Many experimental studies reported that choroidal pathology in diabetic patients might play a role in developing DR. Choroidal thickness (CT) can refl

Genetic predictors associated with diabetic retinopathy in patients with diabetic foot.

INTRODUCTION    Early detection of diabetic retinopathy (DR) is crucial for preventing irreversible blindness. Recent studies identified some of the genetic factors involved in the pathology of DR, although their precise underlying mechanisms remain unclear. OBJECTIVES    This pilot study aimed to determine genetic predictors of DR among patients with type 2 diabetes (T2D) and diabetic foot (DF) based on pathogenetic pathways. PATIENTS AND METHODS    The study included 114 patients with T2D and DF (64 with DR, 50 without DR). Genetic analysis was performed for each patient and the following al

Experiences and Perspectives on Diabetic Retinopathy Screening and Treatment in Mexico: A Subanalysis of the Diabetic Retinopathy Barometer Study.

INTRODUCTION: Diabetic retinopathy (DR) and diabetic macular edema (DME) are leading causes of vision loss worldwide and represent a growing public health concern in Mexico. Despite the availability of effective treatments, including anti-vascular endothelial growth factor (anti-VEGF) therapies, adherence and persistence remain suboptimal, compromising outcomes. This study, part of the international DR Barometer project, examined the perspectives of patients, healthcare providers, and clinic staff to better understand challenges and opportunities in DR/DME care delivery in Mexico. METHODS: A

Awareness of Diabetic Retinopathy Among Known Diabetics.

Background and objective Diabetic retinopathy (DR) is the leading preventable cause of irreversible blindness in working-age adults worldwide. In Pakistan, with over 30 million people living with diabetes, specific patient awareness of DR as a distinct screenable complication remains poorly studied, particularly in urban tertiary care settings. The objective of this study is to determine the prevalence of specific DR awareness among known diabetic patients at the Pakistan Institute of Medical Sciences (PIMS), Islamabad, and to identify independent sociodemographic and clinical predictors using

Advances in diagnosing and treating diabetic retinopathy.

Diabetic retinopathy has traditionally been defined as a microvascular complication of diabetes mellitus; however, advances in retinal imaging, digital technologies and mechanistic insights have challenged this narrow view. Increasing evidence indicates that neuronal and glial dysfunction and degeneration, neuroinflammation, capillary non-perfusion and metabolic dysregulation can occur early in the disease process and can precede or coexist with clinically apparent vascular abnormalities. These insights support the emerging concept of diabetic retinal disease (DRD), which encompasses the full

Mitochondrial transport of glutathione in diabetic retinopathy.

Diabetes increases free radical production and impairs the antioxidant defense system; and increased oxidative stress-mitochondrial damage plays a central role in the development of diabetic retinopathy. Glutathione (GSH), a negatively charged molecule at physiological pH, is biosynthesized in the cytosol, and cannot pass through mitochondrial inner membranes. Interestingly, mitochondria contain ∼15 % of the total GSH and depend on their inner membrane solute carriers for its import from cytosol; in diabetes, retinal cellular and mitochondrial GSH (mtGSH) levels are downregulated. Our aim was

Diabetic retinopathy features in lund MetS rats.

The Lund MetS rat (BBDR.cg-Leprdb/db.cp/LundRj) is a novel animal model that has a congenic leptin receptor deficiency (LepR-/-) and males exhibit a variety of metabolic abnormalities mimicking the human metabolic syndrome, including hyperglycemia, dyslipidemia, severe obesity, and a type 2 diabetes-like condition from 14 weeks of age. However, whether Lund MetS rats (LM rats) develop diabetic retinopathy is still unknown. The purpose is to investigate the features of diabetic retinopathy in this model. In this study, male LM rats aged 15 and 30 weeks were analyzed for pathological retinal cha

Nuclear proteins and diabetic retinopathy: a review.

Diabetic retinopathy (DR) is an eye disease that causes blindness and vision loss in diabetic. Risk factors for DR include high blood glucose levels and some environmental factors. The pathogenesis is based on inflammation caused by interferon and other nuclear proteins. This review article provides an overview of DR and discusses the role of nuclear proteins in the pathogenesis of the disease. Some core proteins such as MAPK, transcription co-factors, transcription co-activators, and others are part of this review. In addition, some current advanced treatment resulting from the role of nuclea

[The Eye in Internal Medicine: Diabetic retinopathy].

Diabetic retinopathy has seen tremendous progress in diagnostic tools and treatment in recent 15 years. Sight threatening stages like proliferative diabetic retinopathy (PDR) and diabetic macular edema (DME) can be treated much more effectively now. The recognition of vascular endothelial growth factor (VEGF) as a driver of proliferation and macular edema has led to the development of VEGF inhibiting drugs such as antibodies (Bevacizumab), fragments of an antibody (Ranibizumab), or a so called VEGF trap (Aflibercept). Laser treatment is no longer a gold standard. Today laser therapy is part of
