Open reduction and internal fixation of a traumatic diastasis of the pubic symphysis: one-year radiological and functional outcomes.

The aim of this study was to review the number of patients operated on for traumatic disruption of the pubic symphysis who developed radiological signs of movement of the anterior pelvic metalwork during the first post-operative year, and to determine whether this had clinical implications. A consecutive series of 49 patients undergoing internal fixation of a traumatic diastasis of the pubic symphysis were studied. All underwent anterior fixation of the diastasis, which was frequently combined with posterior pelvic fixation. The fractures were divided into groups using the Young and Burgess cl

Intramedullary screw and plate combination technique for stabilization of anterior pelvic ring: when and how? - A technical note and case series.

BACKGROUND: Posterior pelvic ring restoration and stabilization are widely recognized as the primary goal in managing pelvic ring injuries. However, anterior pelvic ring (APR) stabilization is also essential in certain cases. Traditional methods, including open plating or intramedullary screw fixation alone, may be insufficient for addressing several challenging situations. The intramedullary screw and plate combination (SPC) technique for APR stabilization offers a feasible solution for these complex cases while achieving favorable clinical outcomes. Therefore, the present study aimed to intr

Post-processing of quantitative 4D-CT for initial evaluation of scapholunate Instability: Assessment of simplified approaches to data analysis.

OBJECTIVES: To evaluate the diagnostic performance of simplified post-processing approaches for quantitative wrist 4D-CT in the assessment of scapholunate instability (SLI). METHODS: A prospective monocentric case-control study included 60 patients with suspected post-traumatic scapholunate ligament (SLL) tears and persistent pain. Of these, 40 patients exhibited SLL tears, subdivided into two groups of 20 each: one group with completely torn ligaments and the other with partially torn ligaments. The remaining 20 patients, whose SLLs were intact, served as controls. 4D-CT and CT arthrography

The diagnosis of acute urethral trauma.

BACKGROUND: During trauma resuscitation, blind catheterization of an injured urethra may aggravate the injury by disrupting a partially torn urethra. In busy trauma centers, retrograde urethrograms (RUG) prior to catheterisation for all patients with unstable pelvic fractures presents a challenge during trauma resuscitation, and the procedure is not commonly practiced despite Advanced Trauma Life Support (ATLS) and World Health Organisation recommendations. The aim of this study was to determine the presenting clinical features of patients with urethral injuries and to predict major trauma pat

Successful internal fixation of a pelvic ring fracture during pregnancy: a multidisciplinary case report.

BACKGROUND: Severe pelvic trauma during pregnancy is rare and requires concurrent maternal and fetal management. Multidisciplinary coordination is essential, yet reports of definitive pelvic fixation with the fetus in utero are exceptionally uncommon. CASE PRESENTATION: A 28-week pregnant woman with no prior medical history sustained severe injuries (Injury Severity Score: 16) in a high-energy collision. Diagnostic imaging revealed a comminuted, displaced right hip fracture, sacral avulsion fractures, right-sided sacroiliac joint diastasis, and a large pre-sacral pelvic hematoma. Abdominal ult

From neck pain to neurological deficit: Cervical tuberculous spondylodiscitis unveiled: A case report.

INTRODUCTION: Cervical tuberculous spondylodiscitis is an uncommon but potentially devastating form of spinal tuberculosis. Its rarity, subtle clinical onset, and proximity to critical neurovascular structures make early diagnosis particularly challenging. Delayed recognition can lead to catastrophic outcomes, including spinal instability and neurological compromise. This case report aims to underscore the diagnostic pitfalls, highlight key imaging findings, and outline an effective management approach. CASE PRESENTATION: A 36-year-old woman presented with a one-year history of progressive ne

Postpartum sacral stress fractures and pubic symphysis diastasis as under-recognized structural causes of low back and pelvic girdle pain.

Pelvic girdle pain (PGP) is common after pregnancy, but clinical attention mainly focuses on benign, self-limiting ligamentous and muscular disorders. This narrative review highlights postpartum sacral stress fractures and postpartum pubic symphysis diastasis (PPSD) as under-recognized structural causes of severe postpartum low back and PGP and situates them within a postpartum pelvic ring instability spectrum (bony stress fractures and ligamentous disruption, with or without posterior ring injury). We narratively reviewed the English-language literature from 1980 to 2025 on sacral stress or i

Management of cavernous abdominal wall defects post radical cystectomy in adult exstrophy complex.

The exstrophy-epispadias complex represents a spectrum of genitourinary malformations ranging from simple glanular epispadias to an overwhelming multisystem defect, cloacal exstrophy. Neonatal total reconstruction of bladder exstrophy-epispadias complex is the treatment of choice. An adult patient presenting with untreated exstrophy is very rare. Malignant transformation, commonly adenocarcinoma, in such cases is a known complication due to mucosal metaplasia of urothelium. Management in such cases necessitates a radical surgical procedure that often results in a massive defect in the anterior

The pubic midline exposure for symphyseal open reduction and plate fixation.

BACKGROUND: Open reduction and plate fixation of the disrupted symphysis pubis is commonly performed through a horizontal Pfannenstiel incision. Certain clinical situations that complicate the soft tissue conditions of the lower abdomen may make the Pfannenstiel incision a less appealing option. We report on the use of a vertical pubic area midline skin incision in a series of patients undergoing open reduction and plate fixation of their traumatically disrupted symphysis pubis. MATERIALS AND METHODS: Institutional Review Board approval was obtained for a retrospective chart review of the cha

Hereditary intraosseous vascular malformation of the craniofacial region: an apparently novel disorder.

Primary intraosseous vascular anomaly, previously called intraosseous hemangioma, is a very rare malformation that is usually seen in the vertebral column and in the skull. It is exclusively described in sporadic cases and no hereditary component has yet been reported. The most commonly affected bones in the skull are the mandible and the maxilla, and life-threatening bleeding after a simple tooth extraction is frequently observed. Here, we report two consanguineous families containing a total of four affected patients manifesting primary intraosseous vascular malformation (VMOS (vascular malf

Follow up of infants with skull fractures by neurosurgeons due to the risk of growing fractures; is it needed?

INTRODUCTION: Growing skull fractures are a rare complication of paediatric skull fractures. Despite its rarity, a large proportion of resources go towards detecting this complication. This study aims to identify the factors associated with growing skull fracture development to determine which children require follow-up. MATERIALS AND METHODS: This was a single-centre retrospective study examining the referral data from all patients under one years old referred with head trauma between 2013 and 2023 (n = 246). Of these patients 189 sustained skull fractures, with two requiring surgery for a g

Caesarean Section for Orthopedic Indications.

BACKGROUND: The increasing number of late complications described after cesarean sections is prompting a reexamination of the indications for them in pregnant women. The high percentage of pregnancies terminated by preventive cesarean section for non-obstetric reasons also largely involves orthopedic conditions. A challenge for obstetricians is pregnant patients with orthopedic conditions both before and during pregnancy. Pregnant women with a history of orthopedic surgery require special attention. The lack of consensus in this area, physicians' fear of patients' claims and the skewing of pat

A sliding hip screw augmented with 2 screws is biomechanically similar to an inverted triad of cannulated screws in repair of a Pauwels type-III fracture.

OBJECTIVE: Pauwels III femoral neck fractures are highly unstable. These fractures are commonly treated with three cannulated screws or sliding hip screw (SHS) implants, however high rates of non-union persist. A hybrid SHS construct has recently been proposed. The objective of the study was to compare this construct to the familiar inverted triad of cannulated screws and to a single SHS. METHODS: Fourth generation biomechanical femur analogs were used to create a highly repeatable injury model. The hybrid SHS construct contained a SHS with two superior cannulated screws in an inverted triang

Radiographic changes of implant failure after plating for pubic symphysis diastasis: an underappreciated reality?

BACKGROUND: Implant failure after symphyseal disruption and plating reportedly occurs in 0% to 21% of patients but the actual occurrence may be much more frequent and the characteristics of this failure have not been well described. QUESTIONS/PURPOSES: We therefore determined the incidence and characterized radiographic implant failures in patients undergoing symphyseal plating after disruption of the pubic symphysis. METHODS: We retrospectively reviewed 165 adult patients with Orthopaedic Trauma Association (OTA) 61-B (Tile B) or OTA 61-C (Tile C) pelvic injuries treated with symphyseal pla

Isolated Syndesmosis Diastasis: Computed Tomography Scan Assessment With Arthroscopic Correlation.

PURPOSE: To investigate which method can predict tibiofibular diastasis more accurately among the tibiofibular interval at the ankle joint level or previous parameters taken 1 cm above the joint line. METHODS: An arthroscopic examination was performed in 78 consecutive patients with anterolateral ankle pain. Four different methods were performed to take measurements of the tibiofibular interval using an axial computed tomography (CT) scan under existing arthroscopic diagnosis. Three previously reported parameters were assessed at 1 cm above the joint level. In the first method, 2 measurements

Navigation-Assisted Posterior Open Reduction and Internal Fixation in a C-CLAMP Fashion for an Isolated C1 Fracture.

C1 fractures with an intact transverse ligament are usually treated conservatively. Patients who present with a progressive diastasis of bone fragments and a progressive articular subluxation mainly attributed to progressive lengthening of the transverse ligament (TAL) fibers can be treated with a C1 "C-clamp" fusion. A 75-year-old male who sustained a motor vehicle accident was neurologically intact. A computed tomography (CT) imaging demonstrated a Jefferson's type-C1 fracture with a slight lateral displacement of the C1 left lateral mass (LM) and a rotatory subluxation on the right. MRI sho

Treatment of syndesmotic disruptions with the Arthrex Tightrope: a report of 25 cases.

BACKGROUND: The complexity of syndesmotic injuries, often with both bone and soft tissue injury mandates an expeditious diagnosis and treatment to avoid unfavorable long term outcomes. Various methods of fixation of the syndesmosis have been reported. We present the largest series evaluating the Arthrex Tightrope for management of syndesmotic injuries. MATERIALS AND METHODS: Twenty-five patients with disruption of the distal tibiofibular articulation underwent treatment with an Arthrex Tightrope. In 21 cases, a single tightrope was placed, and in four cases, two tightropes were utilized. Asso

Use of the rectus abdominis muscle and fascia flap in reconstruction of epispadias/exstrophy.

Inferiorly based rectus abdominis muscle flaps and fascial flaps have been used to construct a firm abdominal wall without hernias and to provide coverage of the bladder, bladder neck, and proximal urethra in the secondary reconstruction of patients with epispadias/exstrophy complex. They have also been used to produce an elevation of the mons area, which is lacking in the typical exstrophy patient. Rectus fascial flaps have been the mainstay of abdominal closure when wide diastasis of the rectal muscles is present and when the lower abdomen lacks fascial support. We are pleased with the resul
