Extracorporeal shock wave therapy for the healing and management of venous leg ulcers.

BACKGROUND: Leg ulcers are chronic wounds of the lower leg, caused by poor blood flow, that can take a long time to heal. The pooling of blood in the veins can damage the skin and surrounding tissues, causing an ulcer to form. Venous leg ulcers are associated with impaired quality of life, reduced mobility, pain, stress and loss of dignity. The standard treatment for venous leg ulcers is compression bandages or stockings. Shock wave therapy may aid the healing of these wounds through the promotion of angiogenesis (the formation and development of blood vessels) and reduction of inflammation, t

Hope as moral accompaniment: a framework for palliative care practice.

Growing evidence underscores the importance of hope in shaping patients' experiences. In the context of illness, hope regulates emotions, supports resilience, helps patients face uncertainty, and sustains the belief that life remains meaningful and worth living, even in adversity. While its relevance is well documented, very few approaches actively cultivate hope for its possible indirect therapeutic impact at a psycho-physical level. Among them, some positive psychology contributions seems to be particularly significant. Drawing on Snyder's model, hope is conceptualized as the combination of

The pediatric Cogni-Famille protocol: family-mediated manual therapy achieves comparable outcomes at one-eighth cost in children with cerebral palsy-a franco-Cameroonian comparative study.

BACKGROUND: Cerebral palsy (CP) remains the leading childhood motor disorder worldwide, yet rehabilitation access is severely limited across sub-Saharan Africa due to resource scarcity, workforce shortages, and profound cultural stigmatization. Family-mediated approaches offer a promising pathway to bridge this gap. To our knowledge, this study represents the first evaluation of a pediatric adaptation of the Cogni-Famille protocol - originally developed for adult post-stroke rehabilitation - applied to children with spastic CP across two dramatically different resource environments. METHODS:

Thematic synthesis of qualitative studies on patient and caregiver perspectives on end-of-life care in CKD.

BACKGROUND: Although dialysis prolongs life for patients with end-stage kidney disease, 20% of deaths in this population are preceded by dialysis therapy withdrawal. Recently, there has been more focus on conservative (nondialytic) care as a legitimate option, particularly for elderly patients. This study aims to describe patients' and caregivers' perspectives on conservative treatment and end-of-life care in chronic kidney disease (CKD). STUDY DESIGN: Systematic review and thematic synthesis of qualitative studies. SETTING & POPULATION: Patients with CKD and caregivers. SEARCH STRATEGY & S

New horizons in personality disorders-from neglect to necessity in geriatric care.

Personality disorders, characterised by enduring and maladaptive patterns of behaviour, cognition and emotional regulation, affect 1 in 10 older adults. Personality disorders are frequently encountered in geriatric care considering their association with multimorbidity and increased health care utilisation. Patients with personality disorders often receive inadequate somatic health care due to (i) difficulties in expressing their actual symptoms and needs, (ii) challenging interactions with professionals, and (iii) non-compliance with medical treatment and lifestyle advice. Acknowledging perso

Evaluation of interventions in prehospital and in-hospital settings and outcomes for out-of-hospital cardiac arrest patients meeting the termination of resuscitation rule in Japan: A nationwide database study (The JAAM-OHCA Registry).

BACKGROUND: Out-of-hospital cardiac arrest (OHCA) is a global health burden with low survival rates. The termination of resuscitation (TOR) rule, widely adopted internationally, aims to preserve dignity, optimize resources, and protect healthcare providers. However, prehospital TOR is not implemented in Japan, presenting legal and practical challenges. This study analyzes temporal trends in prehospital and in-hospital interventions for OHCA patients with poor predicted outcomes. METHODS: This retrospective study analyzed data from the Japanese Association for Acute Medicine Out-of-Hospital Ca

Ethical problems in the management of myelomeningocele and hydrocephalus. The Milroy Lecture 1975.

Within the last generation the application of major advances in drug therapy, intensive care, transfusion techniques, surgery, anaesthesia, and radiotherapy, together with a vast expansion of knowledge due to increased investigative facilities, have led to an unprecedented, dramatic and beneficial increase in the number of persons who would previously have died, or lived with severe handicaps, but can now be cured. Techniques have also been developed to prolong the lives of many people who are now able to enjoy several extra years of productive and meaningful existence, and to alleviate and im

Transgender identity and psychiatric care in Italian prisons: a thematic analysis of systemic gaps and institutional challenges.

BACKGROUND: Transgender and gender-diverse (TGD) individuals face high levels of stigma, discrimination, and psychological distress, which are amplified in carceral settings. Italian prisons have introduced specialized sections for TGD inmates; however, limited empirical evidence exists on their function and the psychiatric, social, and institutional challenges emerging within them. METHODS: A case-informed qualitative design was applied to four TGD inmates housed in a specialized prison section in Northern Italy. Data were derived from routine psychiatric and endocrinological assessments, ad

DNA methylation profiles of bronchoscopic biopsies for the diagnosis of lung cancer.

BACKGROUND: Lung cancer is the leading cause of cancer-related death in most western countries in both, males and females, accounting for roughly 20-25% of all cancer deaths. For choosing the most appropriate therapy regimen a definite diagnosis is a prerequisite. However, histological characterization of bronchoscopic biopsies particularly with low tumor cell content is often challenging. Therefore, this study aims at (a) determining the value of DNA methylation analysis applied to specimens obtained by bronchoscopic biopsy for the diagnosis of lung cancer and (b) at comparing aberrantly CpG

Existing in a Battle Zone - The Lived Experience of Adolescents Subjected to Hospital Compulsory Care Due to Severe Anorexia Nervosa.

When untreated, anorexia nervosa is a life-threatening and disabling condition with the second highest mortality rate of all psychiatric disorders. In this study we undertook an in-depth exploration of adolescent girls' experience of inpatient compulsory care due to severe anorexia nervosa, aiming to better understand the complexity of the disorder and facilitate improvements in care standards. We used Lindseth and Norberg's phenomenological hermeneutic approach and conducted in-depth interviews with seven girls with a mean age of 15.3 years who had experienced inpatient psychiatric care in a

Designing an Indicator‑Driven, Value‑Based Architecture for Pneumonia Prevention in Japan: A Formative Policy Viewpoint on Adult Vaccination and Oral Care.

Japan's aging society concentrates pneumonia burden across communities, long‑term care, perioperative pathways, and hospitals. Adult pneumococcal and influenza vaccination are supported by trials and meta‑analyses and are often cost‑effective; yet, realized value depends on targeting, measurement, financing, and pairing with bedside prevention such as oral care and hospital‑acquired pneumonia bundles. This viewpoint proposes a formative, indicator‑driven architecture linking cost‑effectiveness to operations by aligning vaccination with complementary oral‑care prevention and value‑based payment

The tensions between risk and choice: Perspectives of Australian occupational therapists about informed choice when working with older people.

INTRODUCTION: Aged care in Australia is under scrutiny. In 2021, the Royal Commission into Aged Care Quality and Safety highlighted the need for increased informed choice. Occupational therapists working within residential and community aged care have a responsibility to provide informed choice to their clients. Knowing more about the perspectives of occupational therapists about informed choice with older people will provide insights into practice that may direct future training needs and best practice recommendations. This study therefore aimed to explore perspectives of Australian occupatio

Patients' perspectives of the skills and competencies of therapy radiographers/radiation therapists (TRs/RTTs) in the UK, Portugal and Malta; a qualitative study from the SAFE Europe project.

INTRODUCTION: The role of the Therapy Radiographer/Radiation Therapist (TR/RTT) is to provide radiotherapy to patients with a cancer diagnosis. This includes, not only administration of treatment, but also management of side-effects and provision of support/care. Despite this role being consistent throughout Europe, there is currently no standardisation of education for TRs/RTTs. The SAFE EUROPE project aims to standardize TR/RTT education to enable 'safe and free exchange' of TRs/RTTs across Europe. Consequently, this study aims to explore patients' perspectives regarding the current skills a

A qualitative, mixed-method approach to reaching consensus on function, fatigue, and fatigability outcomes in teens and adults living with spinal muscular atrophy.

BACKGROUND: Spinal muscular atrophy (SMA) is a neuromuscular disease caused by mutations in the survival motor neuron gene, SMN1. Loss of SMN1 function results in deficiency of the SMN protein leading to motor neuron death, muscle wasting, and progressive loss of motor function. Two disease modifying therapies have been approved for teens and adults in the United States, with many more potential treatments in the drug development pipeline. As treatment options for teens and adults with SMA increase, a validated core set of outcome measures is needed to assess motor function, perceived fatigue,

Narrative Medicine to Enhance the Well-Being of Caregivers in the Care of Pediatric Patients with Complex or Serious Illnesses: A Systematic Review.

BACKGROUND: Being the caregiver of a pediatric patient with a complex or serious illness can be emotionally intense and stressful. Caregiver burden significantly affects the biopsychosocial well-being of both the child and the caregiver. Interventions such as Narrative Medicine (NM) may help alleviate this burden by enhancing emotional resilience and strengthening support networks. OBJECTIVES: This review aimed to explore which NM interventions can reduce the emotional burden of caregivers of pediatric patients under the age of 18. METHODS: A systematic literature review was conducted to exa

Physiotherapy Intervention for Promoting Comfort in Palliative Care Patients: A Focus Group Study.

Background/Objectives: Population aging and the rise in chronic diseases challenge healthcare systems to adopt person-centered approaches, especially in palliative care (PC), where symptom management remains limited. Physiotherapy plays a key role in alleviating discomfort but faces inconsistent integration in Portugal due to lack of recognition. Variations in intervention methods hinder uniform care delivery, limiting timely patient access to comfort-focused treatments and knowledge. This study aims to deepen the understanding of physiotherapy's role in Portuguese PC to improve its integratio
