Rapid DOTS expansion in India.

Since late 1998 the coverage of the DOTS strategy in India has been expanded rapidly. In both 2000 and 2001 the country probably accounted for more than half the global increase in the number of patients treated under DOTS and by early 2002 more than a million patients were being treated in this way in India. As a result, nearly 200 000 lives were saved. The lessons learnt relate to the importance of the following elements of the programme: (1) getting the science right and ensuring technical excellence; (2) building commitment and ensuring the provision of funds and flexibility in their utili

Supervised exercise therapy versus home-based exercise therapy versus walking advice for intermittent claudication.

BACKGROUND: Although supervised exercise therapy (SET) provides significant symptomatic benefit for patients with intermittent claudication (IC), it remains an underutilized tool. Widespread implementation of SET is restricted by lack of facilities and funding. Structured home-based exercise therapy (HBET) with an observation component (e.g., exercise logbooks, pedometers) and just walking advice (WA) are alternatives to SET. This is the second update of a review first published in 2006. OBJECTIVES: The primary objective was to provide an accurate overview of studies evaluating effects of SET

Digital adherence technology to improve medication adherence in tuberculosis patients: a systematic review and meta-analysis randomized control trials.

Medication adherence is critical for successful tuberculosis (TB) treatment, yet non-adherence remains a major barrier to TB control globally. Digital adherence technologies (DAT) have emerged as promising tools to support adherence, but their effectiveness remains variably reported across settings and intervention types. To evaluate the effectiveness of DAT compared to directly observed therapy (DOT) in improving TB medication adherence through a systematic review and meta-analysis of randomized controlled trials (RCTs). A comprehensive literature search was conducted across PubMed, Scopus, E

Correlation of sputum smear status with CD4 count in cases of pulmonary tuberculosis and HIV co-infected patients--a hospital based study in a rural area of Central India.

BACKGROUND: In HIV-infected patients, PTB (Pulmonary Tuberculosis) is still the commonest form of TB. The most cost-effective method of detecting TB cases among PTB suspects in high-prevalence countries is by sputum smear microscopy. World Health Organisation (WHO) states that sputum positivity decreases accompanying with atypical chest x-ray findings as CD4 count decreases. This expectation that infection with HIV would reduce the sensitivity of acid-fast smears, due to a decreased frequency of cavitary pulmonary MTB, has not been substantiated in a few studies done in the past. This study wa

Electronic tuberculosis surveillance systems: a tool for managing today's TB programs.

The World Health Organization (WHO) released the Stop TB Strategy in 2006, along with a revised version of the tuberculosis (TB) recording and reporting forms and register. These publications illustrate the need for an enhanced TB surveillance system that will include such key elements as rapid assessment of the quality of DOTS services; integration and response to the human immunodeficiency virus/acquired immune-deficiency syndrome (HIV/AIDS) epidemic; TB control challenges, such as increased smear-negative and extra-pulmonary TB and multidrug-resistant TB (MDR-TB); increased engagement of al

Management of tuberculosis in children in low-income countries.

Children become infected when they are exposed to infectious adults with smear-positive tuberculosis (TB). Most children become infected, but few progress to disease (TB). Children at greatest risk of developing disease are those younger than 5 years of age, HIV-infected and severely malnourished. TB is diagnosed in a child when the child has been exposed to an infectious case, has symptoms and a radiological picture suggestive of TB. Children are treated by the DOTS strategy, and can be treated with 6- or 8-month regimens. HIV-infected children are treated with the same regimens. Children und

The Singapore Tuberculosis Elimination Programme: the first five years.

The Singapore Tuberculosis Elimination Programme (STEP) was launched in 1997 because the incidence of the disease had remained between 49 and 56 per 100,000 resident population for the preceding 10 years. STEP involves the following key interventions: directly observed therapy (DOT) in public primary health care clinics; monitoring of treatment progress and outcome for all cases by means of a National Treatment Surveillance Registry; and preventive therapy for recently infected close contacts of infectious tuberculosis cases. Among other activities are the revamping of the National Tuberculosi

Motor imagery during action observation increases eccentric hamstring force: an acute non-physical intervention.

PURPOSE: Rehabilitation professionals typically use motor imagery (MI) or action observation (AO) to increase physical strength for injury prevention and recovery. Here we compared hamstring force gains for MI during AO (AO + MI) against two pure MI training groups. MATERIALS AND METHODS: Over a 3-week intervention physically fit adults imagined Nordic hamstring exercises in both legs and synchronized this with a demonstration of the same action (AO + MI), or they purely imagined this action (pure MI), or imagined upper-limb actions (pure MI-control). Eccentric hamstring strength gains were a

Using the 99DOTS (directly observed treatment, short course) application for improved adherence to tuberculosis treatment in Namibia: A mixed-methods study.

BACKGROUND: In Namibia, community-based directly observed tuberculosis (TB) treatment (C-DOT) has been effective for individuals living in settled villages, but it is problematic for mobile indigenous people who survive through hunting and gathering and have among the highest TB and multidrug-resistant TB rates and lowest adherence to TB treatment globally. OBJECTIVES: The 99DOTS (directly observed treatment, short course) programme was piloted in the Tsumkwe Healthcare Centre and the 25 villages in the catchment area from quarter 2, 2019 to quarter 1, 2020. A limited TB intervention in remote Taiwan: observations 10 years later.

BACKGROUND: A brief but comprehensive TB elimination program began in a remote region of Taiwan in 1997 involving five contiguous villages (n=2308). METHODS: An aggressive intervention included tuberculin skin testing and treatment of latent TB infection. Normal program data was collected and later analyzed forming the basis of an operational research study. RESULTS: An initial 31% reduction in active TB cases (81 to 56 over 4 years) (p=0.033) was observed and persisted until the end of the 10-year follow-up period despite no further intervention. In the control population, no sustained redu

Comprehensive clinical adherence interventions to enable antiretroviral therapy: a case report.

Adherence to antiretroviral therapy is key for successful treatment of HIV-infected persons. To enhance adherence, multilevel interventions are necessary. This is often a challenge, as this case of an HIV-infected man with a history of poor adherence and multiple virological failures shows. With a multidisciplinary approach, comprehensive intervention strategies were used to facilitate the patient's adherence to an enfuvirtide-based regimen. The interventions are described in detail and include adherence support with modified daily observed therapy, support regarding symptom management, and so

Progress toward tuberculosis control and determinants of treatment outcomes--Kazakhstan, 2000-2002.

INTRODUCTION: In Kazakhstan, during 1995-2002, the annual notification rate per 100,000 population for new cases of tuberculosis (TB) increased from 67.1 to 165.1. Beginning in 1998, public health authorities have used the national case management strategy (DOTS) promulgated by the World Health Organization (WHO) to control TB. Intended goals of DOTS include achieving a cure rate of >85% for persons with newly detected pulmonary TB sputum-smear-positive (PTB+) cases and having PTB+ represent >65% of all PTB cases among adults. Surveillance data collected during 2000-2002 were analyzed to evalu

Three-month weekly rifapentine plus isoniazid for tuberculosis preventive treatment: a systematic review.

BACKGROUND: Uptake of preventive treatment for tuberculosis (TB) remains poor. A 3-month regimen of rifapentine (RPT) plus isoniazid (INH) (3HP) could facilitate its scale-up. We conducted a systematic review to assess the effects of 3HP compared with daily 6- or 9-month INH monotherapy. METHODS: We searched the following databases to identify randomised controlled trials: PubMed, Embase, the Web of Science, Cochrane Central Register of Controlled Trials, three ongoing trial registers and conference abstracts up to 24 January 2017. Where possible, we pooled data using a random-effects model.

Determinants of weight gain among adult tuberculosis patients on dots during intensive phase in Wonago Ethiopia, unmatched case control study.

BACKGROUND: Weight gain during tuberculosis treatment is an important marker of the restoration of health. The relationship between change in weight among patients during anti-TB treatment in the intensive phase of follow-up and factors has not been well documented in Ethiopia, particularly in the southern Gedeo zone, despite the high burden of TB in the zone. Therefore, this study investigates the determined variables related to weight gain among adults of tuberculosis patients during the intensive phase in the Wonago District public health facility. Patient-cost studies on self-administered treatment (SAT) for drug-sensitive tuberculosis compared to facility-based directly observed treatment, short-course (DOTS): a protocol for a systematic review.

INTRODUCTION: Many patients with tuberculosis (TB) suffer from a huge economic burden, even though TB services are often provided free of charge at the point of care. Costs can create significant barriers, hindering patients' access to TB treatment. These costs include direct medical costs (such as consultation fees), direct non-medical costs (such as transportation costs) and indirect costs (such as wages foregone). This systematic review aims to synthesise the best available evidence on economic evaluations of patient-cost studies on self-administered treatment (SAT) for drug-sensitive TB co

Randomised controlled trial conducted in injecting equipment provision sites to compare the effectiveness of different hepatitis C treatment regimens in people who inject drugs: A Direct obserVed therApy versus fortNightly CollEction study for HCV treatment-ADVANCE HCV protocol study.

INTRODUCTION: Hepatitis C is a blood-borne virus (HCV) that can seriously damage the liver and is spread mainly through blood-to-blood contact with an infected person. Over 85% of individuals who have HCV in Scotland became infected following injecting drug use. Since people who inject drugs (PWID) are the main source of new infections, theoretical modelling has suggested that treatment of HCV infection in PWID may effectively reduce HCV prevalence and accomplish elimination. This protocol describes a clinical trial delivering HCV treatment within injecting equipment provision sites (IEPS) in
