Zero-Profile Spacer Versus Cage-Plate Construct in Anterior Cervical Diskectomy and Fusion for Multilevel Cervical Spondylotic Myelopathy: Systematic Review and Meta-Analysis.

BACKGROUND: Anterior cervical diskectomy and fusion with plate-screw construct has been gradually applied for multilevel cervical spondylotic myelopathy in recent years. However, long cervical plate was associated with complications including breakage or loosening of plate and screws, trachea-esophageal injury, neurovascular injury, and postoperative dysphagia. To reduce these complications, the zero-profile spacer has been introduced. This meta-analysis was performed to compare the clinical and radiologic outcomes of zero-profile spacer versus cage-plate construct for the treatment of multile

Tubular diskectomy vs conventional microdiskectomy for the treatment of lumbar disk-related sciatica: cost utility analysis alongside a double-blind randomized controlled trial.

BACKGROUND: Conventional microdiskectomy is the most frequently performed surgery for patients with sciatica caused by lumbar disk herniation. Transmuscular tubular diskectomy has been introduced to increase the rate of recovery, although evidence of its efficacy is lacking. OBJECTIVE: To determine whether a favorable cost-effectiveness for tubular diskectomy compared with conventional microdiskectomy is attained. METHODS: Cost utility analysis was performed alongside a double-blind randomized controlled trial conducted among 325 patients with lumbar disk related sciatica lasting >6 to 8 wee

Cervical Disk Replacement Versus Anterior Cervical Diskectomy and Fusion: Effect of Procedural Variant on Patients With a Prolonged Preoperative Duration of Symptoms From Disk Herniation.

INTRODUCTION: Patients with a prolonged preoperative symptom duration (PSD) in the setting of cervical disk herniation (DH) may suffer inferior outcomes after surgical intervention. Comparison between anterior cervical diskectomy and fusion (ACDF) versus cervical disk arthroplasty (CDA) in this at-risk population has not yet been conducted. METHODS: Patients undergoing ACDF or CDA for DH with a PSD > 180 days were selected. Six-week (6W) and final follow-up (FF) patient-reported outcome measures (PROMs) as well as magnitude of postoperative improvements (∆PROM) were compared between cohorts u

Risk factors for subsidence and loss of segmental lordosis segmental lordosisfollowing 1-3-level anterior cervical diskectomy and fusion for degenerative disease: A time-to-event analysis.

BACKGROUND: Thousands of anterior cervical diskectomy and fusion (ACDF) procedures are performed annually. Increased interbody size offers better immediate segmental lordosis and disc height restoration but may increase the risk of subsequent subsidence. Our objective was to identify factors for subsidence and segmental lordosis loss following ACDF. METHODS: The charts of patients undergoing 1-3 level ACDF at a single-institution were queried for demographics, pre- and postoperative radiographic alignment, bone health (Hounsfield units on CT), procedural details, and interbody characteristics

Study on the efficacy and safety of the combination of Shi's manual therapy and percutaneous endoscopic lumbar diskectomy for lumbar disc herniation with radiculopathy: study protocol for a multicenter randomized controlled trial.

BACKGROUND: Lumbar disc herniation (LDH) is a common chronic musculoskeletal disorder that seriously affects quality of life. The percutaneous endoscopic lumbar diskectomy (PELD) technique was developed to address spinal nerve root compression through direct visualization of pathological findings while minimizing tissue destruction upon exposure. It is an effective and safe treatment for LDH. However, recurrent LDH is a major concern after lumbar discectomy for primary LDH. A considerable number of clinical studies have reported that patients with LDH with radiculopathy could benefit from manu

Multilevel Anterior Cervical Diskectomy and Fusion with Zero-Profile Devices: Analysis of Safety and Feasibility, with Focus on Sagittal Alignment and Impact on Clinical Outcome: Single-Institution Experience and Review of Literature.

BACKGROUND: In multilevel degenerative conditions posterior approaches are often preferred, but anterior approaches provide comparable clinical results and better alignment. Anterior plating entails higher rates of soft tissue injuries and dysphagia, particularly in multilevel cases. This study evaluates efficacy and safety of zero-profile devices in 3- and 4-level anterior cervical diskectomy and fusion, analyzing patients' clinical and radiologic long-term outcomes. METHODS: We prospectively enrolled 24 patients with cervical spondylotic myeloradiculopathy who underwent 3- and 4-level anter

Recurrent lumbar disk herniation.

Recurrent lumbar disk herniation is the most common complication following primary open diskectomy. It is defined as recurrent back and/or leg pain after a definite pain-free period lasting at least 6 months from initial surgery. Careful neurologic examination is critical, and laboratory tests should be ordered to evaluate for infection. Imaging demonstrates disk herniation at the previously operated level. It is important to differentiate recurrent disk herniation from postoperative epidural scar because the latter may not benefit from reoperation. Treatment of recurrent lumbar disk herniatio

Stand-Alone Interbody Cage to Treat Cervical Facet Dislocation: A Case Report.

CASE: We present a 36-year-old man who sustained a traumatic C5-6 unilateral facet dislocation without neurological deficit. The patient was treated with anterior diskectomy and fusion using a stand-alone interbody cage. CONCLUSION: Current treatment of cervical facet dislocations (CFDs) in patients undergoing an anterior cervical diskectomy and fusion commonly makes use of plates with screw fixation and interbody grafts or cages to achieve spinal fusion after the diskectomy is performed. This report documents the successful treatment of a CFD with a stand-alone interbody cage.

Are There Differences in 2-Year Outcomes Between Two-Level Anterior Cervical Diskectomy and Fusion Versus Single-Level Anterior Cervical Corpectomy and Fusion to Treat Cervical Myelopathy? A Quality Outcomes Database Study.

BACKGROUND AND OBJECTIVES: There has been limited assessment between anterior cervical diskectomy and fusion (ACDF) and anterior cervical corpectomy and fusion (ACCF) on patient-reported outcome measures (PROMs) in the treatment of cervical spondylotic myelopathy. We sought to compare PROMs from two-level ACDF with single-level ACCF procedures. We evaluate these results in the context of minimum clinically important difference (MCID) thresholds, which have not been performed previously. METHODS: This retrospective analysis of prospectively collected data from the Quality Outcomes Database Spi

Open Versus Endoscopic Approach for Thoracic Disk Herniations: Equivalent Short-Term Outcomes With Significantly Different Costs.

BACKGROUND AND OBJECTIVES: Open thoracic diskectomy often requires significant bone resection and fusion, whereas an endoscopic thoracic diskectomy offers a less invasive alternative. Therefore, we sought to compare one-level open vs endoscopic thoracic diskectomy regarding (1) perioperative outcomes, (2) neurological recovery, and (3) total cost. METHODS: A single-center, retrospective, cohort study using prospectively collected data of patients undergoing one-level thoracic diskectomy was undertaken from 2018 to 2023. The primary exposure variable was open vs endoscopic. The primary outcome

Percutaneous Endoscopic Interlaminar Unilateral Ventral Dural Approach for Symptomatic Bilateral L5-S1 Herniated Nucleus Pulposus: Technical Note.

BACKGROUND: Percutaneous endoscopic lumbar diskectomy is a good treatment modality for lumbar disk herniation. However, when a patient complains of bilateral lower limb radicular pain with severe disk protrusion at the L5-S1 level, the transforaminal approach is often unable to resolve both lesions owing to anatomical limitations. It is also very difficult to resolve both lesions in an ipsilateral direction using the percutaneous interlaminar approach. We report our surgical technique and clinical results using a ventral dural approach of percutaneous endoscopic interlaminar lumbar diskectomy

SPORT lumbar intervertebral disk herniation and back pain: does treatment, location, or morphology matter?

STUDY DESIGN: Diskectomy candidates with at least 6 weeks of sciatica and confirmatory imaging were enrolled in a randomized or observational cohort. OBJECTIVE: This study sought to determine: (1) whether diskectomy resulted in greater improvement in back pain than nonoperative treatment, and (2) whether herniation location and morphology affected back pain outcomes. SUMMARY OF BACKGROUND DATA: Previous studies have reported that lumbar diskectomy is less successful for relief of back pain than leg pain and patients with central disc herniations or protrusions have worse outcomes. METHODS:

Surgical treatment of temporomandibular joint luxation.

Eminectomy with or without diskectomy was performed in 19 patients with temporomandibular joint luxation. Three of them were long-standing luxations (duration between three months and one year). The remaining 16 patients had recurrent luxations. The follow-up period was three years. No complications such as infection or permanent palsy of the facial nerve occurred. However, transient palsy of the temporal branch of the facial nerve, which resolved within three months was seen in 4 patients (21%). After treatment two patients had recurrent luxations. In the remaining 17 patients, two developed
