[Simultaneous replacement of the aortic valve and the ascending aorta: its operative procedure, early and late postoperative results: especially for annuloaortic ectasia and dissecting aneurysm of the aorta].

During the past 18 years, from March 1972 to February 1991, 105 patients had replacement of the ascending aorta and aortic valve. Operative techniques used in these patients, early and late postoperative results of these patients were reported. One hundred patients had composite graft replacement, and 76 patients of them were treated by Bentall's technique or its modification. Anastomotic leakages after Bentall's operation were found mostly at the bilateral coronary ostia, and incidence rate of the leakages experienced in the first 22 patients who had one lane coronary-graft anastomosis, reduc

Ascending aorta over-angulation is a risk factor for acute type A aortic dissection: evidence from advanced finite element simulations.

OBJECTIVES: To assess whether ascending aorta over-angulation, a morphological feature recently found to be associated with acute type A aortic dissection, precedes dissection and how it affects wall stress distribution. METHODS: A baseline finite element model, previously created by a neural network tool from end-diastolic computed tomography angiography measurements in 124 healthy subjects, was modified to simulate the over-angulation accompanying aortic elongation, obtaining paradigmatic models with different ascending angulations (ascending-arch angle 145°-110°). The models were discretiz

Systematic Morphological Assesment of the Ascending Aorta Dissection: Application of Cardiovascular Pathology Consensus Statement in Tertiaty Care Hospital in Finland.

This study investigated the distinguishing characteristics between acute type A aortic dissection (ATAAD) and aortic wall dilatation. Utilizing systematic histopathology criteria from the Society for Cardiovascular Pathology and the Association for European Cardiovascular Pathology consensus statement, the analysis focused on degeneration, atherosclerosis, and inflammation in patients undergoing surgery in a Finnish tertiary care hospital. The study included 156 patients, those undergoing surgery for ATAAD (n = 116) and those with dilatation (n = 40). Despite similar clinical characteristics,

Single-cell transcriptomic analysis reveals differential cell subpopulations and distinct phenotype transition in normal and dissected ascending aorta.

BACKGROUND: Acute thoracic aortic dissection (ATAD) is a fatal condition characterized by tear of intima, formation of false lumen and rupture of aorta. However, the subpopulations of normal and dissected aorta remain less studied. METHODS: Single-cell RNA sequencing was performed including 5 patients with ATAD and 4 healthy controls. Immunohistochemistry and immunofluorescence were used to verify the findings. RESULTS: We got 8 cell types from human ascending aorta and identified 50 subpopulations including vascular smooth muscle cells (VSMCs), endothelial cells, fibroblasts, neutrophils, m

Ischemic Stroke Due to Spontaneous Dissection of the Brachiocephalic to the Common Carotid Artery with a Fatal Course after Expansion to the Ascending Aorta.

Spontaneous dissection of the brachiocephalic artery is rare, and there is insufficient evidence for optimal treatment. We herein report a case of ischemic stroke due to spontaneous dissection of the brachiocephalic to the right common carotid artery. The patient was treated medically but died suddenly 18 days after the onset because of aortic dissection. Although almost all reported cases of spontaneous dissection of the brachiocephalic artery have had good outcomes with medical management, it is important to note that sudden development of aortic dissection might occur, even without initial

Use of ASD closure device for the sealing of false lumen entry in the ascending aorta after dissection Type A surgical repair.

We present a case of a persistent false lumen after ascending aorta replacement due to Stanford Type A dissection treated by endovascular means. The main entry tear was occluded with an atrial septal defect closure device, sealing the false lumen. A total of five additional stents were implanted to centralise and secure the flow in the true lumen. CT scan at 6-month follow-up showed excellent results with a decreased total aortic diameter and thrombosed false lumen.

[Redissection of the Aortic Root Three Months after Replacement of the Ascending Aorta for Acute Stanford Type A Dissection;Report of a Case].

We report a case of redissection of the aortic root after graft replacement for acute aortic dissection using BioGlue to approximate the false lumen. A 49-year-old man underwent graft replacement of the ascending aorta for acute aortic dissection. In this operation, BioGlue was applied to the false lumen of the aortic root. Three months later, computed tomography revealed an ulcer-like projection and an intramural hematoma in the aortic root. Aortic root replacement was performed, in which operative findings showed a new intimal tear and a patent false lumen contained with hematomas and a frag

The 20-year outcome of ascending aorta replacement and total arch replacement comparison in DeBakey type I acute aortic dissection.

BACKGROUND: The mortality rate of acute DeBakey type I aortic dissection increases by 1%-2% every hour after the first presentation if left untreated. However, the long-term outcomes of ascending aorta replacement (AR) versus total arch replacement (TAR) remain unclear. This study evaluated and compared the long-term outcomes between AR and TAR. MATERIALS AND METHODS: This retrospective study included 398 patients with acute DeBakey type I aortic dissection who underwent surgical repair between March 2002 and October 2024. Patient data were extracted from medical records, and patients were st

[New applications of the coronary flow meter. Acute dissection of the ascending aorta with involvement of the supra-aortic trunks: a case report].

Coronary flow meter is commonly used in myocardial revascularization surgery since it allows an intraoperative assessment of blood flow through the different bypasses performed, ensuring an optimal result of the surgery. However, in cardiovascular surgery, the measurement of intraoperative arterial flows could be applied in other types of surgery other than coronary surgery. We present the case of a 68 years-old female patient diagnosed with Standford type A dissection who underwent urgent surgery, replacing the ascending aorta and aortic arch with an Evita Open Neo® prosthesis with direct rei

Smad2-dependent protease nexin-1 overexpression differentiates chronic aneurysms from acute dissections of human ascending aorta.

OBJECTIVE: Tissue activation of proteolysis is involved in acute intramural rupture (dissections, acute ascending aortic dissection) and in progressive dilation (aneurysms, thoracic aneurysm of the ascending aorta) of human ascending aorta. The translational aim of this study was to characterize the regulation of antiproteolytic serpin expression in normal, aneurysmal, and dissecting aorta. APPROACH AND RESULTS: We explored expression of protease nexin-1 (PN-1) and plasminogen activator inhibitor-1 and their regulation by the Smad2 signaling pathway in human tissue and cultured vascular smoot

[Follow-up of dissection of the right coronary artery and wall of the ascending aorta after directional coronary atherectomy. A case report].

We report on a patient in whom a restenosis after two PTCA procedures was treated with directional coronary atherectomy. In the second DCA maneuvre a spiral dissection, type D, developed from ostium to restenosis with a balloon shaped part in the wall of the aorta ascendens. This part disappeared nearly completely by next day. The dissection in the right coronary artery persisted three months later until a bypass operation and was not visible after two years.

Emergency rescue endovascular stent grafting of ascending aorta to relieve life-threatening coronary obstruction in a case of acute aortic dissection.

Myocardial ischemia associated with acute aortic dissection is frequently a fatal complication, and the emergent management still remains a challenge. We report a patient with life-threatening myocardial ischemia due to acute aortic dissection managed by rescue stent grafting of the ascending aorta. Coronary blood flow improved immediately with this endovascular procedure, hemodynamic status was ameliorated dramatically, followed by uneventful open repair.

Ascending aorta dissection associated with bicuspid aortic valve. Considerations 4 years after combined coronary artery bypass grafting and mitral valve replacement.

Aortic dissection etiology involve many factors that are difficult to identify clearly. We report a 47-year-old man who underwent a Bentall operation with reattachment of bypass grafts for a dissecting aneurysm (DeBakey type II) 4 years after combined triple coronary artery bypass grafting and mitral valve replacement. This case appeared to be associated with factors leading to dissecting aneurysm although it remains unclear which was more influential congenital bicuspid aortic valve or proximal anastomosis of venous grafts or both. This case suggests the need to consider appropriate timing in
