Hospital Admission for Severe Pediatric Dissociative Disorder.

Although children with dissociative disorders (DD) are referred to mental health inpatient units, no research exists to endorse this. We studied the outcomes of patients with DD over a 5-year period on a national inpatient unit for children up to 12 years of age. Demographic, clinical, and satisfaction data were collected and compared with the data of other inpatients not having DD. Eight patients were identified, of whom six were female. All had several comorbidities. Mean Children's Global Assessment Scale scores improved from admission to discharge (from 31 to 61, respectively). Admissions

Trauma and dissociation: implications for borderline personality disorder.

Psychological trauma can have devastating consequences on emotion regulatory capacities and lead to dissociative processes that provide subjective detachment from overwhelming emotional experience during and in the aftermath of trauma. Dissociation is a complex phenomenon that comprises a host of symptoms and factors, including depersonalization, derealization, time distortion, dissociative flashbacks, and alterations in the perception of the self. Dissociation occurs in up to two thirds of patients with borderline personality disorder (BPD). The neurobiology of traumatic dissociation has demo

Psychotherapy and pharmacotherapy for patients with dissociative identity disorder.

There is a wide variety of what have been called "dissociative disorders," including dissociative amnesia, dissociative fugue, depersonalization disorder, dissociative identity disorder, and forms of dissociative disorder not otherwise specified. Some of these diagnoses, particularly dissociative identity disorder, are controversial and have been questioned by many clinicians over the years. The disorders may be under-diagnosed or misdiagnosed, but many persons who have experienced trauma report "dissociative" symptoms. Prevalence of dissociative disorders is unknown, but current estimates are

Dissociation and social cognition in schizophrenia spectrum disorder.

While there is emerging evidence that dissociation is linked with trauma history and possibly symptoms in schizophrenia, it remains unclear whether dissociation represents a symptom dimensions in its own right in schizophrenia and as such is uniquely related to other features of illness. To explore this issue the current study sought to find out whether dissociation was uniquely related to an index of social cognition closely linked to social functioning, namely affect recognition. We hypothesized that dissociation would be linked with affect recognition because symptoms of dissociation may un

Panic disorder and dissociation - comparison with healthy controls.

OBJECTIVE: According to recent findings, clinical symptoms of the patients suffering from panic disorder are related to dissociation. The aim of our study is to examine if the dissociation, can influence the intensity of psychopathology in patients suffering from panic disorder with or without agoraphobia and compare the levels of dissociation in the patients and healthy subjects. METHODS: 41 patients suffering from panic disorder (70.1% females) and 66 healthy controls (77.3% females) were included in the study. The patients were psychiatrically assessed and the subjective intensity of sympt

Dissociation in borderline personality disorder: a detailed look.

The objective of the present study was to assess in detail the whole spectrum of normal and pathological dissociative experiences and dissociative disorder (DD) diagnoses in borderline personality disorder (BPD) as diagnosed with the Revised Diagnostic Interview for Borderlines. Dissociation was measured comprehensively in 21 BPD outpatients using the Structured Clinical Interview for DSM-IV Dissociative Disorders-Revised, the Multidimensional Inventory of Dissociation (MID), the Dissociative Experiences Scale pathological taxon analysis, and the Somatoform Dissociation Questionnaire. The freq

Ritual homicide during dissociative trance disorder.

A case report is described of a man who developed a single period of recurrent episodes of dissociative trance disorder (DTD) during a family stress. The cultural environment was permeated by thoughts and beliefs in magic. During one trance state, the man performed an exorcism on the 6-month-old baby of his brother-in-law, and the baby died from cerebral hemorrhage. In the 5-year follow-up, no other episodes of DTD developed, and the man is serving an 18-year sentence.

Examination of the pathological dissociation taxon in depersonalization disorder.

In recent years, the pathologic dissociation taxon developed by Waller, Putnam, and Carlson (Psychological Methods 1:300-321, 1996) from a Dissociative Identity Disorder (DID) sample has been increasingly used in studies of dissociation in general. However, the taxon's convergence with dissociative diagnoses other than DID, as well as the taxon's central premise that pathologic dissociation is a categorical rather than a dimensional construct, remain areas of exploration. This report examines the applicability of the pathologic dissociation taxon to Depersonalization Disorder (DPD). The Dissoc

Childhood trauma and dissociative symptoms in panic disorder.

OBJECTIVE: Childhood trauma has been associated with increased risk for both panic disorder and dissociative symptoms in adulthood. The authors hypothesized that among individuals with a primary diagnosis of panic disorder, those experiencing depersonalization/derealization during panic attacks would be more likely to have a history of childhood trauma. METHOD: Rates of traumatic events were compared between panic disorder patients with (N=34) and without (N=40) prominent depersonalization/derealization during panic attacks. Symptom severity in the two groups was also examined. RESULTS: Cont

Dissociative symptoms in patients with obsessive-compulsive disorder.

To clarify the relationship between dissociative symptoms and obsessive-compulsive disorder (OCD), 100 patients with OCD were assessed with standardized instruments measuring symptoms of OCD, dissociation, and depression. Diagnoses of personality disorders and dissociative disorders were made using structured interviews. Compared with a previous study, OCD patients had dissociation scores slightly higher than normal controls and comparable to patients with other anxiety disorders. Patients with elevated dissociation scores had more severe OCD symptoms, were more depressed, and were more likely

PTSD, complex PTSD and the dissociative subtype of PTSD in patients with eating disorders.

INTRODUCTION: Posttraumatic stress disorder (PTSD) and dissociation are associated with eating disorders (EDs) and serve as markers of higher severity and comorbidity. However, the role of complex PTSD (CPTSD) and the dissociative subtype of PTSD (DPTSD) in EDs remains relatively unexplored. METHODS: Participants were 635 patients (60% female) admitted to a higher level of care (residential, partial hospital or intensive outpatient) for treatment of a DSM-5 defined ED, substance use disorder (SUD), PTSD and/or mood disorder. In addition to a semi-structured interview, patients completed self-

Forensic aspects of dissociative positive symptoms in trauma-related disorders and borderline personality disorder.

A psychotically motivated act or an act committed under impaired insight and control of action in the midst of an acute psychosis is the standard for lack of criminal responsibility. There is now increasing evidence that positive symptoms, particularly in the form of hallucinations and delusions, in trauma-related disorders and borderline personality disorder (BPD) are comparable to positive symptoms in psychotic disorders, posing a challenge for differential diagnosis and forensic assessment of the relevance of positive symptoms to insight and self-control. Due to the indistinguishability of

A Network Approach to Trauma, Dissociative Symptoms, and Psychosis Symptoms in Schizophrenia Spectrum Disorders.

BACKGROUND: Dissociative experiences commonly occur in schizophrenia spectrum disorders (SSD). Yet little is known about how dissociative experiences in SSD are related to SSD symptoms. Accordingly, we investigated the relations between dissociative experiences and SSD symptoms, focusing on symptoms bridging these 2 symptom clusters as well as their relation to reported trauma history. STUDY DESIGN: Network analyses were conducted on the responses of 248 individuals with an SSD who enrolled from multiple mental health centers in The Netherlands. Dissociative experiences were assessed via the

Clinical correlates of childhood trauma and dissociative phenomena in patients with severe psychiatric disorders.

In this present study, we aim to investigate the possibility of a link between psychotic disorders and traumatic experiences in a group of female patients diagnosed with psychotic disorders by comparing childhood trauma exposure with a group of non-psychotic psychiatric disorder attending the same pschiatric clinic. Secondary purpose of this study is to examine the clinical correlates of trauma exposure, dissociative phenomena and psychiatric symptomatology between these two group of patients. Two psychiatric sample groups, those with psychotic disorders-mostly schizophrenic-(n = 54), and thos

The impact of the therapeutic alliance on treatment outcome in patients with dissociative disorders.

BACKGROUND: Research has shown that the therapeutic alliance plays an important role in enhancing treatment outcome among individuals with a variety of disorders, including posttraumatic stress disorder (PTSD). However, the therapeutic alliance and treatment outcome has not yet been studied in dissociative disorders (DD). OBJECTIVES: The current study sought to investigate the impact of alliance on treatment outcome for DD patients. METHODS: Data from a naturalistic, longitudinal international treatment study of DD patients and their therapists were analyzed to determine if the alliance, as

Self-mutilating behavior in patients with dissociative disorders: the role of innate hypnotic capacity.

BACKGROUND: Despite the fact that the assumption of a relationship between self-mutilation and dissociative disorders (DD) has a long history, there is little empirical evidence to support this premise. The present study examined this relationship and investigated whether this commonality is associated with innate hypnotic capacity. METHODS: Fifty patients diagnosed with DD and 50 control subjects with major depression were assessed by using a self-mutilation questionnaire, Dissociative Experiences Scale, Traumatic Experiences Checklist, and the Eye-Roll Sign for their self-mutilating behavio

Anatomical and functional alterations in patients with functional/dissociative seizures and posttraumatic stress disorders.

BACKGROUND: Functional/dissociative seizures (FDS) involve alterations in emotional processing, agency, self-monitoring, and motor control. Post-traumatic stress disorder (PTSD) involves brain systems related to emotional processing, salience detection, memory, and cognitive control. FDS and PTSD converge on several clinical and neurobiological dimensions-including trauma-related vulnerability, emotional processing, dissociation, and alterations involving limbic and regulatory networks-while producing markedly different clinical manifestations. However, it remains unclear which neural abnormal

The Role of Disgust Emotion in Eating Disorders and Its Relationship with Dissociative Symptoms.

Background: Disgust is a central emotion in eating disorders, influencing both their development and maintenance. High sensitivity to disgust has been associated with restrictive and purgative behaviors, as well as with greater severity of eating disorder symptoms. Despite its importance, the different aspects of disgust are rarely examined in depth, limiting the understanding of its role in these disorders. Since the emotion of disgust is more closely linked to the body and food than other emotions, the aim of this paper is to understand its possible role in eating disorders and obesity. Meth
