[Bilateral Duane syndrome associated with crocodile tears, and congenital megacolon: a case study].

Duane's syndrome is a congenital form of strabismus characterized by horizontal eye movement limitation and specifically by globe retraction with palpebral fissure narrowing in attempted adduction. It is usually associated with ocular and systemic pathology. We report a case of 2-year-old girl presenting Duane syndrome associated with crocodile tear syndrome, preauricular tags, and Hirschsprung disease. Through this rare case, we discuss different pathogenic hypotheses of this syndrome and associated abnormalities.

[Bilateral meningiomas of the optic nerves associated with partial unilateral Duane's syndrome].

A 34 years old man, with right eye blindness since childhood, presented with a decreased vision of the left eye 28 years later. Apart from bilateral papillary atrophy, right blindness and decreased left visual acuity, neuro-ophthalmological examination revealed a partial Duane's retraction syndrome on the right. Optic canal X-rays and cerebral CT-Scan revealed calcified meningiomas of both optic nerves. The diagnosis was confirmed at surgery. Cases of bilateral optic nerve meningioma are reviewed. The difficulties of diagnosis, the usefulness of the CT-Scan, the poor prognosis of the disease,

Möbius syndrome redefined: a syndrome of rhombencephalic maldevelopment.

OBJECTIVE: To investigate the variable clinical picture of Möbius syndrome (MIM no. 157900) and to further understand the pathogenesis of the disorder. METHODS: A standardized questionnaire was submitted to 37 Dutch patients with Möbius syndrome. All underwent standardized neurologic examination with special attention to cranial nerve functions, motor skills, and facial and limb anomalies. RESULTS: Of 37 patients with facial paresis, 97% had bilateral and 3% had unilateral ocular abduction weakness. Further analysis showed isolated abducens nerve palsy in 9%, a conjugated horizontal gaze par

Duane's syndrome associated with crocodile tear and ear malformation. A case of stapes anomaly and its reconstruction.

Duane's syndrome is a congenital eye movement disorder characterized by marked limitation or absence of abduction, variable limitation of adduction, and narrowing of the palpebral fissure with retraction of the globe on attempted adduction. We treated a patient with bilateral Duane's syndrome associated with crocodile tear and external, middle, and inner ear anomalies. Exploratory tympanotomy showed an absence of the oval window and a rudimentary superior structure of the stapes that was connected to the abbreviated incudal lenticular process with connective tissue. A small hole was drilled in

Surgical management of enophthalmos in Duane syndrome.

Duane syndrome is a congenital disorder of eye movement characterized by partial or absent development of the sixth cranial nerve nucleus and/or the sixth nerve itself. Patients often undergo procedures, both surgical and nonsurgical, to correct ocular misalignment and improve the resulting visual problems. We present a case report of a 49-year-old woman with left-sided Duane syndrome who underwent aesthetic surgical correction of enophthalmos.

Management of vertical deviations secondary to other anatomical abnormalities.

BACKGROUND AND PURPOSE: To review the surgical management of five vertical strabismus syndromes secondary to anatomical abnormalities. These syndromes are: 1) craniosynostosis; 2) "heavy eye syndrome" of high myopia; 3) Brown syndrome; 4) upshoot-downshoot in Duane retraction syndrome (DRS); and 5) antielevation syndrome after inferior oblique anteriorization. METHODS: The syndromes are presented from evolving to well-accepted surgical management practices based on review of the current literature. RESULTS: Surgical management techniques discussed include: 1) excyclotorsion of the muscle con

Structure of SALL4 zinc finger domain reveals link between AT-rich DNA binding and Okihiro syndrome.

Spalt-like 4 (SALL4) maintains vertebrate embryonic stem cell identity and is required for the development of multiple organs, including limbs. Mutations in SALL4 are associated with Okihiro syndrome, and SALL4 is also a known target of thalidomide. SALL4 protein has a distinct preference for AT-rich sequences, recognised by a pair of zinc fingers at the C-terminus. However, unlike many characterised zinc finger proteins, SALL4 shows flexible recognition with many different combinations of AT-rich sequences being targeted. SALL4 interacts with the NuRD corepressor complex which potentially med

Fourth cranial nerve palsy and Brown syndrome: two interrelated congenital cranial dysinnervation disorders?

Based on neuroimaging data showing absence of the trochlear nerve, congenital superior oblique palsy is now classified as a congenital cranial dysinnervation disorder. A similar absence of the abducens nerve is accompanied by misinnervation to the lateral rectus muscle from a branch of oculomotor nerve in the Duane retraction syndrome. This similarity raises the question of whether some cases of Brown syndrome could arise from a similar synkinesis between the inferior and superior oblique muscles in the setting of congenital superior oblique palsy. This hypothesis has gained support from the c

Applications of molecular genetics to the understanding of congenital ocular motility disorders.

The congenital fibrosis syndromes (CFS), including congenital fibrosis of the extraocular muscles (CFEOM) and Duane syndrome (DS), are rare congenital strabismus syndromes that present with nonprogressive restrictive ophthalmoplegia with or without ptosis. Although historically believed to result from primary extraocular muscle (EOM) fibrosis, our laboratory's work is based on the hypothesis that these disorders result from distinct, but analogous, developmental defects of the oculomotor (nIII), trochlear (nIV), and abducens (nVI) nuclei. We have defined three inherited CFEOM phenotypes (CFEOM

Multiple congenital anomalies associated with Duane's syndrome.

A 33-year-old man had Duane's retraction syndrome, microphthalmos, mixed hearing loss, short fingers and toes, and horseshoe kidneys on both sides and a right epibulbar lipodermoid. The patient had undergone surgical procedures to correct a cleft lip and palate and a left undescended testis. His parents were first cousins. Results of a chromosomal study revealed a normal male karyotype. The association of multiple congenital malformation in this patient may be rare.

Congenital innervation dysgenesis syndrome (CID)/congenital cranial dysinnervation disorders (CCDDs).

Congenital loss of innervation to the extra-ocular muscles (EOMs) can have a profound effect on the target muscle. This has been well recognised in Duane's retraction syndrome. However, it has been less emphasised in other congenital oculo-motor disorders. Such congenital ocular motor defects have been expanded to include DRS, congenital fibrosis of EOMs, monocular elevation defect, Möbius syndrome, as well as several other non-ocular muscles supplied by cranial nerves such as facial muscles. Such loss of innervation to motor muscles can be unified as a defined clinical entity, which can be la

[Ocular coloboma associated with cervico-oculo-acoustic syndrome].

INTRODUCTION: Cervico-oculo-acoustic syndrome is an uncommon syndrome consisting of Klippel Feil syndrome, deafness, and Duane syndrome. We relate a new case of cervico-oculo-acoustic syndrome. CASE REPORT: We report the case of 15-month-old girl referred for esotropia of the right eye. Ocular examination revealed type 1 Stilling Duane syndrome in the right eye, in which we also noted microcornea, iris coloboma, and papillary and inferior chorioretinal coloboma sparing the macular area. The left eye was normal. The physical examination revealed Klippel Feil syndrome characterized by an extrem
