Depression in intensive care units.

There is thought to be a high incidence of depression in intensive care units (ICUs), both as a result of the psychological toll of serious illness and threat to life as well as the physiologic derangements caused by illnesses and treatments. The authors outline a strategy to classify ICU depressions into major depressive illness, adjustment disorders, dysthymic disorder, or organic affective syndrome. Other psychiatric disturbances which may simulate depression are also discussed. A summary of environmental, behavioral, psychotherapeutic, and psychopharmacologic treatments for these disorders

The core elements of neurosis: mixed anxiety-depression (cothymia) and personality disorder.

Although there has been great diagnostic activity within the conditions formally included under the general rubric of neurosis in the last 20 years, there is little evidence that the many new diagnoses (i.e., generalized anxiety disorder, panic disorder, social anxiety disorder, and dysthymic disorder) have helped clinicians and improved the health of those diagnosed. This is largely because of the extensive comorbidity between these disorders negates much of their attempted separation and it is argued that the core of neurosis is a mixed anxiety-depressive disorder, or cothymia, combined with

Depressive disorders in childhood. II. A longitudinal study of the risk for a subsequent major depression.

As part of a longitudinal nosologic study of major depressive disorder, dysthymic disorder (DD), and adjustment disorder with depressed mood ( ADDM ) in a school-aged cohort, the predictive validity of each diagnosis was examined. Using all available data on the course of the disorders, the criterion was the first subsequent major depressive episode. Major depressive disorder and DD signaled a similarly high risk of a new bout of depressive illness. For the children who recovered from their first episode of major depression and then had their second one (40%), the free interval did not exceed

Longitudinal study of the influence of life events and personality status on diagnostic change in three neurotic disorders.

It has been known for many years that diagnosis within the neurotic spectrum of disorders is temporally unstable and also that life events can be major precipitants of change in symptoms. Reasons for this instability could include inherent inadequacy of current diagnostic practice, the influence of life events as an agent of diagnostic shift, and an innate course of disorder with features dependent on the stage at which disorder presents (e.g., development of panic to agoraphobia). These possibilities were examined in a prospective study that was initially a randomised controlled trial. Two hu

Pharmacotherapy of geriatric depression.

Dysthymic disorders and major depressions both occur in the elderly. Aggressive but carefully chosen and monitored pharmacologic treatment is indicated in elderly patients with even modest symptoms of depression. Careful attention must be given to the presence of other medical disorders and treatments, physiologic changes of aging, decreased dosage requirements, and side effects. The available antidepressants are compared in terms of their effects on neurotransmitter reuptake mechanisms and the likelihood of producing sedation, anticholinergic effects, and cardiotoxicity. The low anticholinerg

Suicide attempts in the Hungarian adult population. Their relation with DIS/DSM-III-R affective and anxiety disorders.

Prevalence of suicide attempts and their relationship with DIS anxiety and affective disorder diagnoses were investigated in a Hungarian adult community sample. Despite the high suicide mortality rate, the rate of suicide attempts was similar to that reported in other studies using similar methods. Suicide attempts occurred more frequently among women and previously married persons. Although the presence of any lifetime anxiety and/or affective disorder increased the rate of reported suicide attempts, the effect of co-morbidity, recurrence and chronicity might be considered significant predict

Attention deficit hyperactivity disorder and treatment outcome in opioid abusers entering treatment.

Symptoms of DSM-IV attention-deficit hyperactivity disorder (ADHD) were determined in patients entering methadone maintenance treatment. The relationship of ADHD to psychiatric and substance abuse comorbidity, attention testing, and treatment outcome was analyzed; 19% of patients had a history of ADHD, and 88% of these had current symptoms. Continuous Performance Testing indicated poorer attention in patients with ADHD. The only substance use disorder more common in the ADHD group was clonidine. From dysthymia to treatment-resistant depression: evolution of a psychopathological construct.

Dysthymia is a psychopathological construct historically described and often reconsidered through the centuries. Its first description is dated back to 400 b.C., when Hippocrates proposed his theory about the 'black bile' and the melancholic temperament. The concept of dysthymia (dys-, 'ill', thymia-, 'emotions') has been largely elaborated in the XIX and XX centuries by Burton, Cullen, Schneider, Kretschmer, Akiskal and other authors, and recently re-formulated in the various editions of the modern Diagnostic and Statistical Manual of Mental Disorders under different diagnostic labels: neurot

Depressive disorders in childhood. I. A longitudinal prospective study of characteristics and recovery.

In the context of a prospective, longitudinal, and controlled nosologic study, the characteristics and diagnostic validity of major depressive disorder, dysthymic disorder, and adjustment disorder with depressed mood were examined in a school-aged cohort. The entities were distinguishable on several dimensions such as age at onset and pattern of recovery. Time to recovery from onset was most favorable for the adjustment disorders (90% remission rate during nine months) and least so for the dysthymias (89% remission rate during six years). Major depression and dysthymia were similar with respec

College students' attitudes toward finding a mate to escape depression: sex differences.

College student respondents (n = 128) were compared on their consideration of finding a mate or significant other as a strategy for a hypothetical teacher ("Terry") displaying symptoms of dysthymic disorder (DSM-IV criteria). The sex of "Terry" was manipulated by descriptive pronoun (he or she) only. Analysis indicated differences based on sex of Terry and sex of respondent. Respondents favored finding a mate for the hypothetical male more than the female. Men, more than women, supported finding a mate or significant other for the teacher.

Effects of fluoxetine on fine motor performance in dysthymia: an 8-week, nonrandomized, open-label study.

BACKGROUND: Present findings on psychomotor retardation in dysthymia are inconsistent and changes in psychomotor performance during antidepressant treatment have not been investigated in this population to date. OBJECTIVE: The present study aims to explore the psychomotor effects of an 8-week regimen of fluoxetine in dysthymic patients. METHODS: Dysthymic patients (both inpatients and outpatients of the Psychiatric Hospital Sint-Norbertus, Duffel, Belgium), presenting over a period of 2 years, meeting the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision cr

Factitious disorder comorbid with borderline personality disorder and dysthymia: from medically unexplained physical symptoms to functional neurological disorder.

INTRODUCTION: In this study, we introduce the concept of comorbidity between factitious disorder (FD), borderline personality disorder (BPD), dysthymia (DY), medically unexplained physical symptoms (MUPS) and functional neurological disorder (FND) characterising patients who may tend to exaggerate physical or psychiatric symptoms of presentation to a general or psychiatric hospital with a constellation of signs that do not receive confirmation from further clinical and instrumental assessments. The similarities between these syndromes and the constant presence of borderline personality in the

Rating dysthymia: an assessment of the construct and content validity of the Cornell Dysthymia Rating Scale.

BACKGROUND: Mason et al. developed the Cornell Dysthymia Rating Scale (CDRS), a 20-item clinician-rated inventory, and hypothesized that it may be superior to the commonly-used Hamilton Depression Rating Scale (HDRS) in assessing the symptoms of dysthymia, a form of chronic depression. The purpose of this study was to compare these instruments in an outpatient sample of dysthymic patients. METHOD: The CDRS and the HDRS and other inventories (including the Hopkins Symptom Check List (SCL)) were administered to 110 patients meeting DSM-IIIR diagnosis of dysthymia. RESULTS: There was a signific

Biopsychobehavioral correlates of insomnia. IV: Diagnosis based on DSM-III.

DSM-III diagnoses were established for 100 patients with chronic insomnia. Principal and additional diagnoses on axis I and II (psychiatric and personality disorders) were extremely prevalent. Only five patients had a principal diagnosis on axis III (physical disorders). The most common diagnoses on axis I were dysthymic, anxiety, somatoform, and substance use disorders. Compulsive personality disorder or trait was the most common axis II diagnosis. Ninety-five patients had multiple diagnoses. This study confirms that patients with chronic insomnia have a high prevalence of psychopathology and

Simulation study to demonstrate biases created by diagnostic criteria of mental illnesses: major depressive episodes, dysthymia, and manic episodes.

OBJECTIVES: Composite diagnostic criteria alone are likely to create and introduce biases into diagnoses that subsequently have poor relationships with input symptoms. This study aims to understand the relationships between the diagnoses and the input symptoms, as well as the magnitudes of biases created by diagnostic criteria and introduced into the diagnoses of mental illnesses with large disease burdens (major depressive episodes, dysthymic disorder, and manic episodes). SETTINGS: General psychiatric care. PARTICIPANTS: Without real-world data available to the public, 100 000 subjects wer

The overlap of depressive personality disorder and dysthymia, reconsidered.

This paper reviews empirical literature examining the diagnostic overlap of depressive personality disorder and dysthymia. Although some articles have suggested that depressive personality disorder lacks distinctiveness from dysthymia and may be one type of affective disorder, a large proportion of the literature may, in fact, support its distinctiveness as a personality disorder. Additional research is needed to determine the features of the two disorders that best distinguish between them, so that depressive personality disorder can be further evaluated for its validity and usefulness in the
