Spatial quantification of alterations in ossicular microarchitecture related to middle ear disease.

The human auditory ossicles -malleus, incus, and stapes- are critical for hearing, yet the middle ear that houses them is frequently affected by inflammatory conditions such as chronic otitis media with and without cholesteatoma, leading to ossicular chain damage. In cases of severe degradation, reconstructive surgery often involves ossiculoplasty using sculpted autografts. However, the impact of inflammation and surgical sculpting on the in-depth microarchitecture of ossicles remains poorly understood. This study investigates the structural integrity of ossicles (incudes and mallei) affected

[72 cases of the auditory ossicle malformations but with normal findings in the tympanic membrane].

We evaluated 66 patients (72 ears) with auditory ossicle malformations, but with normal findings in the tympanic membrane, who underwent ossicular reconstruction and stapes surgery at our department between 1984 and 1997. There were 27 men (30 ears) and 39 women (42 ears). Classification of pathologic condition based on surgical findings showed separation of the incus-stapes joint in 43%, fixation of the malleus or incus in 16%, fixation of the stapes footplate in 26%, in incidence of fixation of the auditory oosicles that is higher than that previously reported. In 33% of the patients, conduc

Congenital cholesteatoma.

We present a British series of eleven patients with primary cholesteatoma, including one patient with bilateral disease. Eight children presented with a history of hearing loss, while one child had had recurrent otitis media and another had had earache. Operative findings were: in five ears, cholesteatoma confined to the antero-superior segment with intact ossicles, in a further four, cholesteatomas extending throughout the mesotympanum with ossicular erosion in one, and in two ears posterior disease throughout the middle ear and mastoid, which had eroded the ossicles in both cases. The five c

Glomus tympanicum chemodectoma: unusual radiological findings.

A 64-year-old Qatari female, with a one-year history of right otorrhoea and deafness, had a reddish-white mass projecting into the right ear canal, through the tympanic membrane, that proved histopathologically to be a paraganglioma. Computerized tomography (CT) of the temporal bones showed extensive destruction of the right mastoid bone, the posterior ear canal wall, and the sinus plate, with total disruption of the ossicles, simulating a malignant tumour, which is unusual for a middle ear paraganglioma.

Conductive Hearing Loss in Children.

A variety of congenital and acquired disorders result in pediatric conductive hearing loss. Malformations of the external auditory canal are invariably associated with malformations of the middle ear space and ossicles. Isolated ossicular malformations are uncommon. Syndromes associated with external and middle ear malformations are frequently associated with abnormal development of first and second pharyngeal arch derivatives. Chronic inflammatory disorders include cholesteatoma, cholesterol granuloma, and tympanosclerosis.

A case of congenital ossicular malformation with mild auricular deformity: embryological and surgical considerations.

A case of bilateral congenital conductive deafness with mild auricular deformity is presented, and the anomalous structure deformity of the ossicles was multi-focal in this case, and a previous report indicates that ossicular malformation in the ear with congenital conductive deafness is multi-focal when the external ear is only slightly deformed. Bilateral ossicular reconstruction with replacement of the stapes yielded satisfactory results.

Morphological and Morphometrical Aspects of the Auditory Ossicles in the European Badger (Meles Meles).

Given the scarce morphological data regarding the middle ear anatomy of this species, the paper attempts to describe the morphological and morphometrical data of the auditory ossicles in the badger. The study was performed using the standard morphological investigations and provides a complete morphological description of the ossicular assembly (malleus, incus and stapes) with some comparative features and attempts to provide a complete set of standardized metrical data for each ossicle. A more-detailed attempt to compare some functional aspects in the light of combined metrical ratios is also

Spectrum of Otological Manifestations in Treacher Collins Syndrome: A Case Series of 9 Patients.

Treacher Collins syndrome (TCS) is an uncommon congenital disorder predominantly involving craniofacial, orbital, and otological structures. The various ear malformations seen in 9 patients with TCS are described. TCS predominantly affects the external and middle ear structures, with inner ear structures being relatively spared, not unexpected given the dual embryological origin of the human ear. The external and middle ear malformations were categorized and graded as those involving the ear pinna, external auditory canal, tympanic cavity, ossicles, facial nerve canal, oval window, and bony la

Study on the Prosthesis Structural Design and Vibration Characteristics Based on the Conduction Effect of Human Middle Ear.

As a bridge from the sound signal in the air to the sound perception of the inner ear auditory receptor, the tympanic membrane and ossicular chain of the middle ear transform the sound signal in the outer ear through two gas-solid and solid-liquid conversions. In addition, through the lever principle formed by three auditory ossicle structure, the sound was concentrated and amplified to the inner ear. However, the sound transmission function of the middle ear will be decreased by disease, genetic, or trauma. Hence, using middle ear prosthesis to replace the damaged ossicles can restore the con

The middle ear of the pink fairy armadillo Chlamyphorus truncatus (Xenarthra, Cingulata, Chlamyphoridae): comparison with armadillo relatives using computed tomography.

The pink fairy armadillo Chlamyphorus truncatus is the smallest extant armadillo and one of the least-known fossorial mammals. The aim of this study was to establish if its middle ear is specially adapted to the subterranean environment, through comparison with more epigeic relatives of the groups Euphractinae (Chaetophractus villosus, Chaetophractus vellerosus, Zaedyus pichiy) and Dasypodinae (Dasypus hybridus). We examined the middle ears using micro-computed tomography and subsequent three-dimensional reconstructions. D. hybridus has a relatively small middle ear cavity, an incomplete bulla

[Cholesteatoma in children--sex differences].

It has been reported that cholesteatoma is more aggressive in children than in adults and that it affects boys more often than girls. We found particularly prominent sex differences in the clinical characteristics and behavior of cholesteatoma in children, by comparing them with those of acute otitis media (AOM), otitis media with effusion (OME) and adult cholesteatoma. We retrospectively analyzed the clinical records of children diagnosed as having cholesteatoma by comparing their courses with those of AOM, OME, chronic otitis media, microtia and congenital malformation of the ossicles. The b

Comprehensive Review of External and Middle Ear Anatomy on Photon-Counting CT.

Photon-counting CT (PCT) allows for improved spatial and contrast resolution compared with traditional energy-integrating detector CT. PCT offers markedly improved visualization of previously described structures, as well as those that were previously beyond the resolution of imaging. Although the anatomic details of the external ear and middle ear structures have been described previously, the rich detail of these structures has not been comprehensively reviewed in the radiology literature. The microarchitecture of the middle ear ossicles and bony protuberances are particularly well visualize

Isolated Schneiderian papilloma of the middle ear cleft.

A 60-year-old woman presented with pulsatile tinnitus in the left ear. MRI and CT imaging suggested a soft-tissue mass in the middle ear. Exploratory tympanotomy and biopsy confirmed Schneiderian (inverted) papilloma. Endoscopic and radiological assessment showed no evidence of sinonasal lesions. The patient proceeded to mastoid surgery with removal of the ossicles for disease clearance. The small number of cases published to date of isolated middle ear inverted papilloma suggest a high recurrence and malignant transformation rate and aggressive management is therefore warranted.

The expression of tenascin-C and tenascin-W in human ossicles.

The ossicles of the middle ear (the malleus, incus and stapes) transmit forces resulting from vibrations of the tympanic membrane to the cochlea where they are coded as sound. Hearing loss can result from diseases such as rheumatoid arthritis (RA) that affect the joints between the ossicles or degenerative processes like otosclerosis that lead to ankylosis of the footplate of the stapes in the oval window of the cochlea. In this study, immunohistochemistry was used to determine if the extracellular matrix glycoproteins tenascin-C or tenascin-W are expressed in the incudomalleolar and incudosta

Computerized tomographic evaluation of the middle ear and mastoid for posttraumatic hearing loss.

High resolution computerized tomography has been proven valuable in all aspects of temporal bone study. The importance of the procedure in evaluation of traumatic lesions, particularly of the middle ear, has been underemphasized. Damage to the middle ear may occur with blunt trauma, penetrating injury, or barotrauma. Conductive hearing loss and/or CSF otorrhea may result. Detailed evaluation of the ossicles is the hallmark of properly performed computerized tomographic evaluation.

Radiology of the ear in mandibulo-facial dysostosis--Treacher Collins syndrome.

Deformities of the external and middle ear with conductive deafness are common in mandibulo-facial dysostosis but cochlear function is nearly always normal. Twelve patients with the typical appearance and characteristics of the lesion are described. All had some degree of atresia of attic and antrum with absent or deformed ossicles but 4 had external auditory meatuses of normal calibre. The characteristic tomographic appearances of the ear are described.
