Viral infections of the CNS in children: imaging features.

In this paper, we review the imaging findings associated with the major viral infections of the CNS in infants and children. We approach these infections by grouping them into several categories: congenital infections in neonates, aseptic meningitis and encephalitis, acute disseminated encephalomyelitis and other postviral syndromes in older children, and HIV infection.

Arteriovenous malformation of the vein of Galen in a newborn diagnosed with color Doppler examination--a case report.

A fourteen-day-old newborn infant presented with severe heart failure due to arteriovenous malformation of the vein of Galen. It was diagnosed by two-dimensional, pulsed-wave and color Doppler echocardiographic imaging. The latter method showed the afferent vessel to be the anterior cerebral artery entering an aneurysm of the vein of Galen at its posterior aspect. Information for surgical anatomic definition appears to be adequately provided by color Doppler examination, which permits avoidance of preoperative angiography.

[Pulsatile echography (ultrasonic cerebral tomosphygmography) in carotid and vertebral pathology. Surgical applications].

Ultrasonographic cerebral tomosphygmography (U.C.T.S.) is a method for determining cerebral pulsatility by centimeter-thick sections. The prototype uses a 2 MHz ultrasound wave emitted in the direction of cerebral structures. The reflected wave is captured by centimeter-thick sections, the mean determined and a pulsatile index defined. This index varies according to the regions studied (cortico-subcortical, superficial sylvian, deep sylvian and vertebrobasilar). Physiologic pulsatility was determined from a series of 150 healthy subjects. This technique was used to evaluate effects of carotid

The spectrum of cranial ultrasound and magnetic resonance imaging abnormalities in congenital cytomegalovirus infection.

Congenital cytomegalovirus (CMV) infection can lead to severe neurological sequelae and (progressive) sensorineural deafness. Neonatal imaging data is mainly based on cranial ultrasound (US) and computed tomography (CT). The additional value of magnetic resonance imaging (MRI) was assessed in congenital CMV infection. The eleven infants studied had a gestational age between 34 and 41 weeks and a birth weight between 1000 and 2780 grams. All but 2 of the infants presented with microcephaly and jaundice at birth. The diagnosis was confirmed postnatally in all infants by isolation of the virus or

[Clinical manifestations of schizencephaly and its sonographic diagnosis].

Schizencephaly is defined as a cerebral malformation of the CNS with various clefts of the cerebral cortex. We report on two patients referred to our department with neurological abnormalities. In both cases the cranial sonography already provided for the clinical picture of schizencephaly. A MR-scan confirmed the diagnosis. In addition one of the patients proved to have a migrational disorder. The analysis of these cases and the relevant literature point out how difficult the etiologic differentiation is and, on the other hand, how various the manifestations of the malformation can be. The im

Ultrasonographic findings in the periventricular region in premature newborns with antenatal periventricular leukomalacia.

Cranial ultrasonography was carried out at least three times a week during the first 16 days of life in all 53 premature newborns born within study period. If a cyst of more than three mm was detected in the periventricular region by fourteenth day of life, it was judged to be antenatal PVL. Periventricular echodensity was classified into mild, moderate or severe periventricular echodensity (PVE). Among 53 newborns, 11 babies (20.8%) were diagnosed as having antenatal PVL. Cysts were observed in the first two days in 9 cases (81.8%) out of 11 babies, and in bilateral periventricular regions in

Sonography of the brain in infants: role in evaluating neurologic abnormalities.

Three primary imaging techniques are currently available for evaluating CNS disease in the infant: sonography, CT, and MR imaging. No one technique is optimal in all situations. An integrated approach may be the most appropriate and cost-effective. Sonography plays a role in screening for and following up intracranial hemorrhage in premature infants, in diagnosing congenital malformations of the brain, and in the initial diagnosis and follow-up of infants with hydrocephalus. Sonography can be used to diagnose periventricular leukomalacia and hypoxic/ischemic injury, but it is relatively insens

Management of patients with asymptomatic neck bruits and carotid stenosis.

The role of extracranial occlusive lesions in the pathogenesis of ischemic stroke is well established. The management of asymptomatic carotid bruit or stenosis is still highly controversial, however. This article reviews in detail the currently available data in order to assist clinicians in the management of patients with asymptomatic neck bruits or stenosis.

Demonstration of perivascular echogenicities in congenital cytomegalovirus infection by colour Doppler imaging.

Two children with congenital cytomegalovirus infection and intracerebral echogenicities were investigated by computer sonography and colour Doppler imaging (CDI). By simultaneous imaging of brain tissue and CDI, blood flow within the stripe-like echogenicities of the basal ganglia was demonstrated. Using CDI the echogenicities were identified as the walls of thalamostriate vessels.

[Diagnosis of an aneurysm of the great cerebral vein of Galen using duplex sonography].

The combination of two-dimensional sonography of the skull and pulsed Doppler measurements (= Duplex-scan-technique) was used to detect an aneurysm of the vein of Galen in a three months old infant, who presented with clinical signs of congestive heart failure. Excellent correlation of sonography with computed tomography and intravenous digital subtraction angiography could be demonstrated. A short review of the pathological and clinical findings of this lesion is given. The value of Duplex-scanning in the diagnostic approach of arterio-venous malformations of the brain in comparison with othe

[Neonatal tricuspid insufficiency--a contribution to the differential diagnosis of the cyanotic newborn infant].

A cyanotic newborn is described to illustrate and discuss the rare transient tricuspid insufficiency together with functional pulmonary atresia. Etiologically the manifold causes of perinatal asphyxia are to be taken into consideration. The clinical appearance corresponds to type A of transient myocardial dysfunction according to the classification of Rowe together with respiratory distress syndrome and heart failure. Pathogenetically the disease is based on the different histological consequences of myocardial ischaemia of the right ventricle in connection with a delayed fall of the pulmonary

Does Increased Echogenicity in the Caudothalamic Groove Always Indicate Hemorrhage in Early Infancy?

We aim to evaluate the diagnostic significance of caudothalamic groove (CTG) echogenicity on cranial ultrasonography (CUS) by examining its correlation with magnetic resonance imaging (MRI) findings, including susceptibility-weighted imaging (SWI). This retrospective study screened 131 neonatal intensive care unit neonatal intensive care unit infants with diagnostic-quality CUS and MRI (including SWI) performed between January 2019 and April 2022. Among these, 34 had CTG echogenicity on CUS; 18 of the 34 had no corresponding CTG hemorrhage on SWI and formed the analytic cohort. The final cohor

Head Ultrasound in Neonatal Hypoxic-Ischemic Injury and Its Mimickers for Clinicians: A Review of the Patterns of Injury and the Evolution of Findings Over Time.

Hypoxic-ischemic injury (HII) of the neonatal brain and resulting clinical hypoxic-ischemic encephalopathy remains a significant cause of morbidity and mortality in the neonatal population. Ultrasound (US) has emerged as a powerful screening tool for evaluation of a neonate with suspected HII. The pattern of injury on brain imaging has crucial implications in therapies and predicted neurodevelopmental outcomes. US has become increasingly effective at determining the pattern, timing, and extent of injury in HII as well as differentiating these findings from a host of diagnoses that can result i

The etiology of lenticulostriate vasculopathy and the role of congenital infections.

Lenticulostriate vasculopathy (LSV) refers to increased echogenicity of the penetrating vessels that supply the basal ganglia and segments of the internal capsule seen on cranial ultrasound. Initially identified in infants with congenital infection, LSV has now been associated with a variety of infectious and non-infectious conditions. Although robust epidemiologic studies are lacking, the available evidence does not support broad evaluation for multiple congenital infections when LSV is identified. We propose screening infants with LSV for congenital cytomegalovirus infection and ensuring tha

Meningitis due to Neisseria meningitidis serogroup B in India.

Invasive meningococcal disease has a fulminant course and high mortality. Neisseria meningitidis serogroup A is predominantly responsible for meningococcal disease in India and the developing countries. Group B meningococcus, which is prevalent in the developing world is uncommon in India. We herein report the second case of group B meningococcal infection from the country, two decades after the reporting of the first case. Ineffective vaccines against serogroup B warrant the need for close surveillance of this disease.
