Ecthyma gangrenosum due to Pseudomonas aeruginosa sepsis as initial manifestation of X-linked agammaglobulinemia: a case report.

BACKGROUND: X-linked agammaglobulinemia (XLA, OMIM#300,300), caused by mutations in the Bruton tyrosine kinase (BTK) gene, is a rare monogenic inheritable immunodeficiency disorder. Ecthyma gangrenosum is a cutaneous lesion caused by Pseudomonas aeruginosa that typically occurs in patients with XLA and other immunodeficiencies. CASE PRESENTATION: We report the case of a 20-month-old boy who presented with fever, vomiting, diarrhea, and ecthyma gangrenosum. Blood, stool, and skin lesion culture samples were positive for P. aeruginosa. A diagnosis of XLA was established, and the c.262G > T muta

Ecthyma gangrenosum in a previously healthy pediatric patient and associated facial paralysis and persistent hyperplastic primary vitreous.

BACKGROUND: Ecthyma gangrenosum is an infective lesion of the skin and mucosal membranes. It is most commonly caused by Pseudomonas aeruginosa, and the most important risk factors are malignancy and neutropenia. However, it has rarely been reported in children who were previously healthy. Persistent hyperplastic primary vitreous has been described as the persistence of the fetal hyaloid vascular system. Acute otitis media with facial paralysis is an infrequent association. CASE REPORT: We report the case of a 5-month-old boy hospitalized because of fever, otorrhea and necrosis on his body. He

Reconstructive procedure of lower lip defect due to ecthyma gangrenosum--a rare complication of acute lymphoblastic leukemia.

Ecthyma gangrenosum is a rare invasive cutaneous infection mostly caused by Pseudomonas aeruginosa in immunosuppressed patients, especially during neutropenic states. Embolic lesions usually accompanying with septicemia of P. aeruginosa are typically painless, round, and centrally necrotic with a surrounding erythematous halo. Facial involvement occurred only in 6% of the patients, which may result to complicated defects for reconstruction. In this article, we aimed to present a case with relapse of acute lymphoblastic leukemia presented to our clinic with 50% defect of the lower lip due to ec

The Treatment of Contagious Ecthyma in Lambs with a Local Anaesthetic/Antiseptic Wound Formulation Lowers Serum Amyloid A Responses.

Contagious ecthyma (CE) is a widespread, highly contagious zoonotic skin disease of small ruminants caused by the Orf virus (ORFV), leading to substantial economic losses and welfare concerns. There is no specific treatment, with topical antiseptics and oral or parenteral antibiotics often administered for preventing secondary infections, risking antimicrobial resistance. This study assessed the effect of treating CE in lambs with an antibiotic-free topical anaesthetic/antiseptic formulation (Tri-Solfen®; T-S; Medical Ethics, Australia/MultiSolfen®; M-S; Dechra, UK). Serum amyloid A (SAA), a m

Ecthyma gangrenosum and severe neutropenic sepsis caused by Staphylococcus aureus infection in a previously healthy child: a case report.

Ecthyma gangrenosum (EG) is a potentially lethal skin infection mainly caused by Pseudomonas aeruginosa, but other causative pathogens have also been reported. EG usually occurs locally and often arises in immunocompromised patients. The fatality rate can be extremely high if a systemic infection leading to sepsis occurs. EG and severe sepsis caused by Staphylococcus aureus infection are extremely rare in healthy children. However, upon occurrence, disease progression can be rapid, and the mortality rate is high. This current case report describes a previously healthy child with no underlying

An Infant with Measles Developed Ecthyma Gangrenosum Caused by Coagulase-Negative Staphylococcus: A Rare Case Report from Somalia.

INTRODUCTION AND IMPORTANCE: Ecthyma gangrenosum is a skin lesion that can be caused by either bacterial hematogenous seeding or a primary skin infection. Despite being the most frequent causal agent, Pseudomonas aeruginosa is not the only bacteria that has been involved. Other types of bacteria may also be implicated in the etiology of EG: cocci bacteria, both gram-positive and gram-negative. CASE PRESENTATION: Here, we report the case of a 10-month-old male infant who developed ecthyma gangrenosum after a measles infection. At the time of admission, the patient had a high fever of about 40.

Ecthyma gangrenosum following toxic epidermal necrolysis syndrome in a 3-year-old boy-a survivable series of events.

Toxic epidermal necrolysis syndrome (TENS) is a severe but rare skin reaction leading to epidermal desquamation of greater than 30% of the TBSA. It is most commonly precipitated by the administration of medication. Frequent complications of this syndrome include local wound infections, respiratory, mucocutaneous, and ocular complications. Ecthyma gangrenosum (EG) is a rare disease characterized by a milliary seeding of the cutaneous tissue with Gram-negative bacteria; it is most commonly seen in immunocompromised individuals. Here we report a 3-year-old boy who developed EG subsequent to TENS.

[A patient with acute Philadelphia-chromosome-positive mixed phenotype leukemia developing ecthyma gangrenosum while undergoing combined imatinib mesylate chemotherapy].

A 67-year-old woman with acute Philadelphia-chromosome-positive mixed phenotype leukemia developed bilateral periorbital ecthyma gangrenousum (EG) subsequent to periorbital edema while undergoing combined imatinib mesylate (imatinib) chemotherapy. Although initial periorbital edema was considered an imatinib side effect, the lesion deteriorated rapidly with high fever in the neutropenic phase, and the woman died of septic shock. Cultures from blood and exudative fluid grew Pseudomonas aeruginosa, after which EG was diagnosed. EG is a well-recognized emergent cutaneous infection most commonly a

The Many Lives of Staph: Staphylococcal Scalded Skin Syndrome in Two Brothers Secondary to Staphylococcus aureus-Associated Ecthyma Gangrenosum.

Staphylococcal scalded skin syndrome (SSSS) is a rare, infectious entity whereby the systemic distribution of Staphylococcal toxins results in the development of multiple symptoms, the most notable of which is a desquamative rash. Ecthyma gangrenosum, a skin infection most frequently occurring in immunocompromised individuals, is similarly uncommon. Herein, we present the case of two young siblings presenting with SSSS following the development of ecthyma gangrenosum. Prompt treatment of the Staphylococcal infection led to resolution of symptoms and recovery. Our case highlights the need to be

In vitro activity of VEGF-E produced by orf virus strains isolated from classical and severe persistent contagious ecthyma.

Proliferative orf virus infections in adult sheep have increased in Italy in the past few years: these extreme cases are frequently fatal and difficult to differentiate from other infectious diseases of sheep such as blue tongue. A probable explanation for the proliferative and highly vascularized nature of the lesions was found in the expression of the VEGF-E gene encoded by the orf virus. To investigate a possible role of the viral VEGF in the pathogenesis of severe persistent orf virus lesions, the activity of four VEGF-E variants was compared by an angiogenesis in vitro model. Similar angi

First identification of ORF virus causing contagious ecthyma in Morocco (MOR20): Genomic, phylogenetic, and sequence variants analyses for vaccine design.

The ORF virus induces a zoonotic contagious ecthyma disease, affecting small ruminants such as sheep and goats. ORF virus has not been identified in Morocco, and there is no vaccination protocol against contagious ecthyma. In this study, we analyzed the genome sequence of a new strain isolated in Morocco (MOR20) from a flock of sheep showing suspicious signs of Sheepox virus infection. ORFV MOR20 strain was isolated after 2 initial blind passages on Heart cells. The cytopathic effect was characterized by aggregation, swelling and detachment of cells, appearing 4 days after infection. The virus

Ecthyma Gangrenosum-Like Lesion in a Patient With a Leukocyte Adhesion Defect: Pseudomonas aeruginosa, Staphylococcus aureus, and a Rare Agent, Fusarium.

Fusarium species can cause serious infections, including superficial and invasive ones. In immunocompromised individuals, they can cause skin infections such as ecthyma gangrenosum and disseminated disease through lymphatic spread. In this case report, we present a nine-year-old girl with ecthyma gangrenosum whose primary disease was a leukocyte adhesion defect (LAD). She developed a burn on her right lower extremity after spilling hot tea. Pseudomonas aeruginosa, Staphylococcus aureus, and Fusarium spp. were isolated from a wound culture taken from her ankle. While Pseudomonas aeruginosa is g

Ecthyma Gangrenosum Secondary to Methicillin-Sensitive Staphylococcus aureus in an Atopic Child with Transient Neutropenia: A Case Report and Review of the Literature.

In addition to Pseudomonas aeruginosa, other organisms including Staphylococcus aureus have been reported to have associations with ecthyma gangrenosum (EG). There are very limited reports of Staphylococcus aureus EG causing systemic symptoms in an immunocompetent child. We present the case of an atopic child with transient neutropenia developing characteristic skin lesions of EG. Culture of the skin wounds yielded methicillin-susceptible Staphylococcus aureus (MSSA), and incisional biopsy of the skin lesions revealed aggregates of Gram-positive cocci at the subepidermal area and necrotic vasc

Concomitant diagnosis of immune deficiency and Pseudomonas sepsis in a 19 month old with ecthyma gangrenosum by host whole-genome sequencing.

X-linked agammaglobulinemia (XLA, OMIM#300300) is a rare monogenic primary immunodeficiency caused by mutations in the Bruton tyrosine kinase (BTK) gene. XLA is characterized by insufficient immunoglobulin levels and susceptibility to life-threatening bacterial infections. We report on a patient that presented with ecthyma gangrenosum and septicemia. Rapid trio whole-genome sequencing (rWGS) revealed an apparently de novo hemizygous pathogenic variant (c.726dupT; p.Ile243TyrfsTer15) in the BTK gene. Metagenomic analysis of rWGS sequences that did not align to the human genome revealed 770 alig
