Plate Osteosynthesis for Segmental Forearm Fractures: A Comparison of Outcomes in Floating Elbow and Isolated Forearm Fractures.

BACKGROUND: Segmental forearm fractures are challenging to treat due to their instability. A floating elbow, characterized by ipsilateral fractures of both the humerus and forearm, is a rare but severe injury often caused by high-energy trauma and associated with significant soft tissue damage. The objective of this study is to compare the surgical outcomes of plate osteosynthesis in segmental forearm fractures, with and without the concomitant presence of a floating elbow. METHODS: A total of 35 adult patients who underwent open reduction and internal fixation for segmental forearm fractures

Treatment of Mason type II radial head fractures without associated fractures or elbow dislocation: a systematic review.

PURPOSE: There is no consensus as to the best treatment of Mason type II fractures without concomitant elbow fractures or dislocation. The aim of this systematic review was to compare the results of operative and nonoperative treatment of these injuries. METHODS: We systematically screened the databases of PubMed, EMBASE, and Cochrane Library until September 2011 for studies on nonoperative or operative treatment of Mason type II fractures. We defined successful treatment as an excellent or good result according to the Broberg and Morrey score, Mayo Elbow Performance Score, or Radin score. Ex

A rare paediatric 'floating elbow'; a supracondylar fracture with an ipsilateral Monteggia fracture: A case report.

INTRODUCTION AND IMPORTANCE: A paediatric floating elbow involves fractures of the supracondylar region of the humerus with ipsilateral fracture of the forearm bones. A floating elbow is very uncommon with an incidence of 3 to 13% of all supracondylar fractures. A concomitant supracondylar and Monteggia fracture is extremely rare with only six cases reported in the literature. CASE PRESENTATION: We present the unusual case of an eight-year-old girl with a concomitant ipsilateral supracondylar humerus fracture and open Monteggia fracture. Physical examination showed a neurovascularly intact li

Total elbow arthroplasty in patients who have elbow fractures caused by gunshot injuries: 8- to 12-year follow-up study.

INTRODUCTION: In this retrospective study, we aimed to present 8-12-year clinical and radiographic evaluation of total elbow arthroplasty in young patients who had open fractures due to gunshot injuries. MATERIALS AND METHODS: The study included a consecutive series of seven patients who had insertion of total elbow prosthesis (semi-constrained type) for the treatment of comminuted intra-articular elbow fractures resulting from gunshot injuries between 1994 and 1998. All patients were male and the mean age at the time of operation was 23 years. RESULTS: The average time from the original fra

Incarcerated Medial Epicondyle Fractures With Elbow Dislocation: Risk Factors Associated With Morbidity.

BACKGROUND: Incarcerated medial epicondyle fractures in association with elbow trauma are rare and an absolute indication for intervention. Because of the infrequent nature, outcomes following this injury are not well documented. We studied a large cohort of these injuries to determine factors associated with functional outcomes. It was hypothesized that a greater duration between initial presentation and time of surgery would lead to poorer outcomes. METHODS: A total of 32 patients aged 18 and under who underwent surgical treatment for an incarcerated medical epicondyle fracture at a level-1

Combined Ipsilateral Humeral Shaft and Galeazzi Fractures Creating a Floating Elbow Variant.

"Floating elbow" injuries of the arm traditionally represent a combination of humeral shaft and forearm fractures which require anatomic rigid open reduction and internal fixation of all fractures to allow for early range of motion exercises of the elbow. There are published variants of the floating elbow injury which include ipsilateral diaphyseal humeral fracture, proximal ulna fracture with proximal radioulnar joint disruption, and ipsilateral diaphyseal humeral fracture with elbow dislocation and both bones forearm fracture. We present the case of a 21-year-old woman whose left arm became

Fractures and Dislocations About the Elbow and Their Adverse Sequelae: Contemporary Perspectives.

Fractures and dislocations of the elbow can result in adverse outcomes. The elbow is a unique joint that allows for great mobility but is predisposed to instability, either simple or complex, in traumatic settings. Even simple elbow instability, in which no fracture is present, may be associated with tremendous soft-tissue injury. Surgical treatment is often required for complex instability in which various fractures are present. The treatment goals for fixation of elbow fractures and dislocations include stable fracture fixation, a stable concentrically reduced joint, and early range of motio

Anterior approach for operative fixation of coronoid fractures in complex elbow instability.

The coronoid process has been shown to play a critical role in ulnohumeral stability. Coronoid process fractures can occur in isolation or as part of a complex injury pattern. The most common complex pattern, known as the "terrible triad," includes a radial head fracture and elbow dislocation along with the coronoid fracture. Failure to address these fractures and ligamentous injuries can result in recurrent instability and progression to painful arthrosis. Both medial and lateral approaches to the coronoid have been popularized in recent literature, but there is no universally accepted approa

Recovery of elbow motion following pediatric lateral condylar fractures of the humerus.

BACKGROUND: Temporary elbow stiffness is often seen after a lateral condylar fracture of the distal end of the humerus in children. There are scant scientific data available to assess the expected time frame for return of elbow motion after these injuries. The purpose of this study is to provide a prospective, longitudinal evaluation of elbow motion in a large group of pediatric patients undergoing treatment for a lateral condylar fracture of the distal end of the humerus. METHODS: We prospectively evaluated 141 patients with lateral humeral condylar fractures at a mean age of 5.2 years and w

Case of bilateral Galeazzi fractures associated with dislocation of the right elbow.

We report an unusual case of bilateral Galeazzi fractures associated with dislocation of the right elbow and fracture of the right scaphoid caused by a motorbike accident in a 32-year-old man. Bilateral radiuses were fixed with plates, and the right scaphoid was fixed with a screw after closed reduction of the right elbow. A satisfactory result was obtained with a return to work and no radiographic problems 7 years after surgery. The mechanism of the injury was thought to be a high-velocity fall on the outstretched hands combined with extreme pronation of the forearms and extension of the wris

Sequential Opposite-pattern Supracondylar Fractures in the Same Elbow of a Child.

Supracondylar humerus fractures in children are most commonly of the extension type, whereas flexion-type injuries are rare. Sequential occurrence of extension- and flexion-type supracondylar fractures in the same elbow is extremely uncommon. We report a child who sustained a Gartland type II extension-type supracondylar fracture at 5 years of age, managed conservatively with above-elbow casting. He recovered fully and resumed normal activities. Ten months later, at age 6 years, he sustained a new fall on the same limb and presented with a Gartland type III flexion-type supracondylar fracture.

Floating elbow combining ipsilateral distal multiple segmental forearm fractures: A case report.

BACKGROUND: Floating elbow along with ipsilateral multiple segmental forearm fracture is a rare and high-energy injury, although elbow dislocation or fracture of the ulna and radius may occur separately. CASE SUMMARY: We report the case of a 37-year-old woman with open (IIIA) fracture of the right distal humerus with multiple shaft fractures of the ipsilateral radius and ulna with a history of falling from a height of almost 20 m from a balcony. After providing advanced trauma life support, damage control surgery was performed to debride the arm wound and temporarily stabilize the right upper

Distal Humeral Hemiarthroplasty Versus Total Elbow Arthroplasty for Acute Distal Humeral Fractures.

For acute distal humeral fractures not amenable to open reduction and internal fixation, total elbow arthroplasty has become an established alternative. However, lifelong activity restrictions designed to prevent early mechanical failure make this a poor option for some patients. This has led to a renewed interest in distal humeral hemiarthroplasty. Using modern implants and techniques, distal humeral hemiarthroplasty has shown outcomes comparable to those of total elbow arthroplasty at short- to mid-term follow-up, with an overall higher but different complication rate. Long-term data are nee

Medial elbow exposure for coronoid fractures: FCU-split versus over-the-top.

OBJECTIVE: The optimal exposure interval for anteromedial coronoid fractures is unknown. The purpose of this study was to quantitatively compare the osseous and ligamentous exposure of the medial elbow using the flexor carpi ulnaris (FCU)-Splitting and Hotchkiss Over-the-Top approaches. METHODS: Forty surgical approaches were performed on 20 fresh-frozen cadaveric elbows using a randomized crossover design. Access to key anatomic landmarks [anteromedial facet, coronoid tip, sublime tubercle/anterior bundle of the medial collateral ligament (MCL), posterior bundle of the MCL, and radial head]

Lateral elbow ecchymosis as a clinical sign of lateral humeral condylar fractures.

Given the appropriate clinical history and mechanism of injury, the presence of localized lateral elbow ecchymosis in a young child is usually the sign of lateral condyle fracture of the humerus. Recognition of this clinical sign helps in directing the diagnostic studies and empiric treatment toward a lateral condyle fracture of the humerus not apparent on initial radiographs.

Complex open elbow fracture Gustilo-Anderson type IIIB treated with the primary elbow arthroplasty: A case report.

Total elbow arthroplasty as a treatment option for open elbow fracture is relatively rare described. We reported a 39 years old polytrauma patient with complex open elbow fracture (Gustilo-Anderson type IIIB). The patient presented with large soft tissues defect on dorsal part of the left elbow, ulnar palsy due to the irreparable loss of the ulnar nerve, distal triceps loss due to the complete loss of the olecranon, loss of both humeral condyles with collateral ligaments and complex elbow instability. Only few similar cases have been published. Reconstructive surgery included repetitive radica

Elbow Arthroscopy for Posteromedial Impingement and Fixation of Olecranon Stress Fracture.

Posteromedial elbow impingement due to valgus extension overload often develops as a result of excessive valgus and extension force during repetitive overhead throwing activities. Impingement classically occurs in throwing athletes such as baseball, tennis, softball, or lacrosse players. If isolated, arthroscopic removal of the posteromedial olecranon osteophytes shows excellent postoperative satisfaction, return to sport rates, and return to previous level of activity. This Technical Note describes treatment of posteromedial elbow impingement syndrome and associated olecranon stress fracture
