Complex open elbow fracture Gustilo-Anderson type IIIB treated with the primary elbow arthroplasty: A case report.

Total elbow arthroplasty as a treatment option for open elbow fracture is relatively rare described. We reported a 39 years old polytrauma patient with complex open elbow fracture (Gustilo-Anderson type IIIB). The patient presented with large soft tissues defect on dorsal part of the left elbow, ulnar palsy due to the irreparable loss of the ulnar nerve, distal triceps loss due to the complete loss of the olecranon, loss of both humeral condyles with collateral ligaments and complex elbow instability. Only few similar cases have been published. Reconstructive surgery included repetitive radica

Elbow Arthroplasty: From Normal to Failure.

Total elbow arthroplasty is currently an established surgical treatment for several pathologies of the elbow. Although initially used primarily in the treatment of rheumatoid arthritis, indications for total elbow arthroplasty have expanded and now include trauma, primary and secondary osteoarthritis, fracture nonunion, and following neoplasm resection. Desired outcomes of elbow arthroplasty include decreasing patient pain, restoration of function and mobility, and prevention of or treatment for instability. In comparison with total elbow arthroplasty, radial head replacements are most commonl

Traumatic Divergent Elbow Dislocation with Associated Forearm Amputation in an Adult: A Case Report.

CASE: A 49-year-old right-hand-dominant man sustained an auger-related injury that resulted in open dislocation at the left wrist and palm, with complete amputation of the distal aspect of the forearm and the hand. The injury at the elbow included instability with an ulnar coronoid fracture, posterior dislocation of the ulna, and posterolateral dislocation of the radius. To restore stability of the forearm stump and elbow, we performed a complete resection of the radius, open reduction and internal fixation of the coronoid tip, a repair of the lateral collateral ligament, and transfer of the d

Optimizing Outcomes in Total Elbow Arthroplasty.

The use of total elbow arthroplasty (TEA) is projected to increase by more than 50% between 2020 and 2045. An aging population, contemporary prosthetic designs, and broadened indications are factors associated with this predicted increase. Although TEA can reliably improve pain and function, overall complication rates remain relatively high compared with other arthroplasties, making technical competence of utmost importance. Careful patient selection, preoperative optimization, and thorough counselling on the complication profile and the potential for mechanical failure following TEA are essen

Involvement of the medial and lateral epicondyles in distal humeral coronal shear fractures: Case series and literature review.

PURPOSE: Epicondyle involvement in capitellar and trochlear fractures is often considered a simple associated lesion that does not substantially change treatment or prognosis. Although theoretically predicted in reviews, case series almost never report elbow prosthesis use in comminuted coronal shear injuries associated with epicondylar fracture in the elderly. The purpose of this study is to focus on this underestimated injury pattern that can be a negative risk factor for treatment and prognosis. METHODS: We retrospectively reviewed all cases with coronal shear fracture of the distal humeru

Ideal functional outcomes for amputation levels.

This article provides a generalized overview of amputation classifications and the idealized outcomes for upper and lower amputations at their respective levels. The following levels are discussed: above knee/transfemoral, below knee/transtibial, above elbow/transhumeral, below elbow/transradial, and bilateral for upper and lower extremities. This classification defines a framework for clinicians to share with patients so that they understand the potential for their expected functional outcomes regarding mobility and activities of daily living, both with and without a prosthesis. Moreover, it

Sensory feedback add-on for upper-limb prostheses.

BACKGROUND AND AIM: Sensory feedback systems have been of great interest in upper-limb prosthetics. Despite tremendous research, there are no commercial modality-matched feedback systems. This article aims to introduce the first detachable and feedback add-on option that can be attached to in-use prostheses. TECHNIQUE: A sensory feedback system was tested on a below-elbow myoelectric prosthesis. The aim was to have the amputee grasp fragile objects without crushing while other accidental feedback sources were blocked. DISCUSSION: A total of 8 successful trials (out of 10) showed that sensory

Clinical long-term results of radial head arthroplasty in comminuted radial head fractures.

Modern radial head prostheses have recently become more common in the treatment of comminuted radial head fractures. The goal of this study was to evaluate how well the EVOLVE® modular metallic radial head implant prosthetic restores the functional range of motion and stability of the elbow. 30 patients with comminuted radial head fractures received an arthroplasty with an EVOLVE® prosthesis in our institution. 20 of those patients were available for long-term follow-up (mean > 10 years). The outcomes were assessed on the basis of pain, motion, and strength. The overall outcome was scored with

Overlengthening of the radial column in radial head replacement: a review of the literature and presentation of a classification system.

BACKGROUND: Radial head arthroplasty is a common procedure in elbow surgery. It has been shown to be of benefit for the patients, but there also are relevant complications that should be prevented if possible. One significant complication is overlengthening of the radial head prosthesis. In overlengthening, the head of the prosthesis overextends the physiological level of the native radial head and leads to overcompression in the radiohumeral joint. Rapid erosion and arthritic changes may then impede the clinical outcome. The incidence of overlengthening is not precisely known, but estimations

Loose Fit Monobloc Versus Cemented Bipolar Radial Head Prosthesis in the Treatment of Radial Head Fractures: A Comparative Study.

Background: Radial head arthroplasty is commonly used for unreconstructable radial head fractures, particularly in cases associated with elbow instability. Both loose-fit monobloc and cemented bipolar radial head prostheses are available, but comparative evidence regarding their clinical and radiographic outcomes remains limited. This study aimed to compare mid-term outcomes between these two implant designs. Methods: A retrospective observational study was conducted on 26 patients who underwent radial head arthroplasty for acute radial head fractures between November 2020 and October 2023. Fo

Long-term outcomes of a cemented bipolar radial head prosthesis: a large retrospective study.

BACKGROUND: In acute radial head fractures (RHFs), a radial head prosthesis can be considered if open reduction and internal fixation are not technically feasible. This study provides the largest long-term clinical and radiologic follow-up study of cemented bipolar Judet prostheses implanted to manage acute RHFs, either isolated and associated with other elbow fractures. We compare their functional and radiographic outcomes to test the hypothesis that the bipolar design can achieve similar results in both groups. METHODS: We reviewed data of 63 consecutive patients implanted with a bipolar pr

Early upper-extremity prosthetic fit in patients with burns.

Patients with upper-extremity amputations necessitated by burn injury have frequently faced delays in prosthetic fit. At the Regional Medical Center, Memphis, Tennessee, seven patients required amputations because of burns. These injuries were electrical in four cases, thermal and crush in one case, thermal in one case, and steam and crush in one case. Five patients had below-elbow amputations, one had a bilateral below-elbow amputation, and one had a bilateral above-elbow amputation. All patients were fitted with prostheses within 30 days of the last definitive surgery on the amputated extrem

3D printing-assisted fabrication of a patient-specific antibacterial radial head prosthesis with high periprosthetic bone preservation.

We present a novel fabrication and surgical approach for anatomical reconstruction of a fractured radial head using a patient-specific radial head prosthesis (RHP) made of polymethylmethacrylate (PMMA) bone cement. To this end, the use of PMMA bone cement for prosthesis fabrication was initially investigated using computational modeling and experimental methods. The RHP was fabricated through casting of PMMA bone cement in a silicone mold in the operating room before implantation. To enhance the precision of bony preparation for replacement of the radial head, a patient-specific surgical guide
