Background/Objectives: A Cochlear implantation (CI) is well-established auditory rehabilitation for severe to profound sensorineural hearing loss (SNHL), yet outcomes vary widely among implantees. Even with advancements in surgical methods and device technology, CI is still commonly applied as a generally uniform procedure, with limited attention to the underlying cause of SNHL. This review aims to summarize current evidence supporting etiology-based personalization of CI and to examine how etiology influences electrode selection, implantation timing, and clinical outcomes. Methods: We reviewe

Objective: Electrode tip fold-over (TFO) represents a complication of cochlear implantation. TFO may lead to insertion trauma and malfunction of the cochlear implant (CI), often requiring re-implantation. It is mostly associated with pre-shaped CI electrodes. Methods: Our aim was to provide a systematic literature review to examine the current rate of electrode TFO with different commercially available electrode types and compare it with our clinical experience. We performed a retrospective analysis of 370 paediatric CI patients up to 19 years of age, who underwent surgery at our department bet

PURPOSE: To evaluate the feasibility of surgical implantation of electrical components needed for the operation of biomimetic artificial intraocular lenses (IOLs) for presbyopia correction in phakic non-human primates. This included positioning a ring electrode into the posterior chamber sulcus, securing a circuit board-mounted implant under the superior rectus muscle, and placing a battery pack in the orbit. The full implant allows for recording and transmission of electrical signals within the ciliary muscle during accommodation.
METHODS: The implant consists of a microcontroller, a biopote

Implantable neurostimulators are key devices for treating neurological disorders such as Parkinson's disease and epilepsy by delivering electrical pulses to specific neural tissues. The electrochemical impedance at the electrode-tissue interface significantly influences therapeutic effects and impacts the quality of closed-loop brain signal acquisition. While the real-time and accurate impedance is difficult to measure in vivo, since reference electrodes cannot be implanted for a long time. This study aims to quantify the differences in electrochemical impedance spectroscopy of various electro

INTRODUCTION: Cochlear implant outcomes can be limited due to immunologically mediated intracochlear foreign body responses, resulting in new bone growth and fibrosis. Minimal consideration has been given to the possible role of immunological memory in modulating this response in sequentially implanted patients. We hypothesize the first implant primes the contralateral ear to respond more robustly to sequential implantation, leading to earlier increases in electrode impedance.
METHODS: This is a retrospective cohort analysis of clinical impedance measurements from 79 subjects with sequential

The electrically evoked compound action potentials (ECAPs) amplitude-growth function (AGF) slope correlates with spiral ganglion neuron (SGN) density in the cochlear implanted cochlea. Electrode insertion angle and medial-lateral distance covary from base to apex; in some human ears, SGN survival varies from base to apex, making it difficult to parse out contributing factors to the ECAP AGF slope. Evoked compound action potentials were analyzed on each electrode and compared to post-operative computerized tomography scans. When controlling for medial-lateral distance, insertion angle does not

OBJECTIVE: The present study primarily aims to study and model the impedance dynamics following cochlear implant (CI) surgery in humans. Secondarily, to observe how the modulation of a single-dose topic dexamethasone alters this response.
DESIGN: CI impedance and impedance subcomponents were measured in a day-by-day basis between CI surgery and its activation ( 1 month). Impedance dynamics were mathematically modeled. Furthermore, we conducted a randomized double-blind placebo-controlled clinical trial to evaluate the effect of dexamethasone application.
RESULTS: The model showed good fittin

OBJECTIVES: Removal of a cochlear implant and its intracochlear electrode array is sometimes necessary, potentially causing cochlear explant trauma. Explantation typically occurs years post-implantation by which time reactive tissue has formed around the electrode. We aimed to create an in-vitro electrode explant model to examine explant forces and intracochlear trauma across multiple electrode types and insertion depths.
STUDY DESIGN: An in-vitro model using gel to represent tissue surrounding the electrode was developed. Pre-curved electrodes and straight electrodes at different insertion d

HYPOTHESIS: Robotics-assisted cochlear implant (CI) insertions will result in reduced intracochlear trauma when compared with manual, across multiple users.
BACKGROUND: Whether intracochlear trauma and translocations are two factors that may contribute to significant variability in CI outcomes remains to be seen. To address this issue, we have developed a robotics-assisted insertion system designed to aid the surgeon in inserting electrode arrays with consistent speeds and reduced variability. This study evaluated the effect of robotics-assisted insertions on the intracochlear trauma as compa

The cochlear implant is one of the most successful medical prostheses, allowing deaf and severely hearing-impaired persons to hear again by electrically stimulating the auditory nerve. A trained audiologist adjusts the stimulation settings for good speech understanding, known as "fitting" the implant. This process is based on subjective feedback from the user, making it time-consuming and challenging, especially in paediatric or communication-impaired populations. Furthermore, fittings only happen during infrequent sessions at a clinic, and therefore cannot take into account variable factors t

INTRODUCTION: While cochlear implantation may have a positive effect on tinnitus, it is not effective in reducing tinnitus in all patients. This may be due to different patients requiring different strategies of electrical stimulation in order to obtain a positive effect on tinnitus. It is, therefore, important to identify the most effective stimulation strategies to reduce tinnitus. The simplest possible strategy is stimulation by only one electrode. In this study, we investigated tinnitus suppression by electrical stimulation via a single electrode of the cochlear implant.
METHODS: We perfo

HYPOTHESIS: To explore the results of a new slim modiolar electrode array (SMA) with respect to intracochlear placement and trauma evaluated by detailed radiologic imaging and histology.
BACKGROUND: Hearing and structure preservation is the goal of cochlear implantation for advanced hearing outcomes. Currently, this is most consistently achieved with thin lateral wall electrodes. Modiolar electrodes are located nearer the modiolus and may provide some electrophysiological advantages, but have a greater tendency for causing insertion trauma.
METHODS: The SMA was implanted in 20 fresh-frozen h

OBJECTIVES/HYPOTHESIS: Cochlear implant surgery now aims to preserve residual low frequency hearing. The current research explores whether fluctuations in the electrical impedance of cochlear implant electrodes may act as a biomarker for pathological changes that lead to the delayed loss of residual hearing.
STUDY DESIGN: Secondary analysis of a double-blinded randomized trial, where methylprednisolone was administered intravenously before cochlear implantation with a view to preserving residual hearing.
METHODS: Seventy-four patients with residual hearing after cochlear implant surgery were

OBJECTIVE The authors' group recently published a novel technique for a navigation-guided frameless stereotactic approach for the placement of depth electrodes in epilepsy patients. To improve the accuracy of the trajectory and enhance the procedural workflow, the authors implemented the iSys1 miniature robotic device in the present study into this routine. METHODS As a first step, a preclinical phantom study was performed using a human skull model, and the accuracy and timing between 5 electrodes implanted with the manual technique and 5 with the aid of the robot were compared. After this pha

OBJECTIVE: Electrocorticography (ECoG), used as a neural recording modality for brain-machine interfaces (BMIs), potentially allows for field potentials to be recorded from the surface of the cerebral cortex for long durations without suffering the host-tissue reaction to the extent that it is common with intracortical microelectrodes. Though the stability of signals obtained from chronically implanted ECoG electrodes has begun receiving attention, to date little work has characterized the effects of long-term implantation of ECoG electrodes on underlying cortical tissue.

Micro-electrodes and micro-electrode arrays (MEAs) for stimulating neurons or recording action potentials are widely used in medical applications or biological research. For medical implants in many applications like brain implants or retinal implants there is a need for flexible MEAs with a large area and a large number of stimulation electrodes. In this work a flexible MEA with an embedded flexible silicon dummy CMOS-chip facing these challenges has been designed, manufactured and characterized. This approach offers the possibility by connecting and addressing several of these MEAs via a bus
