Two sets of nerve conduction studies may suffice in reaching a reliable electrodiagnosis in Guillain-Barré syndrome.

OBJECTIVE: Recent studies have advocated the use of serial nerve conduction studies (NCS) in the electrodiagnosis of Guillain-Barré syndrome (GBS). The current study aims to elucidate when and how frequent NCS can be performed to reflect the disease pathophysiology. METHODS: A prospective study of GBS patients documenting the initial and final electrodiagnoses following serial NCS performed at three time intervals: 1-2 weeks, 3-8 weeks and 8-12 weeks. RESULTS: Twenty-one patients were recruited over a period of 2 years. Electrodiagnosis within 2 weeks revealed 17 acute inflammatory demyelina

Postoperative Outcomes Between Positive and Negative Electrodiagnosis in Patients With Tarsal Tunnel Syndrome: A Retrospective Comparative Study.

BACKGROUND: The sensitivity of diagnosing tarsal tunnel syndrome with an electrodiagnostic study is just over 50%. Given this low reliability, many surgeons prefer to make a diagnosis solely from a physical examination, despite reported electrodiagnostic findings. Thus, to understand the clinical ramifications between these 2 methods of diagnosis, this investigation compared the postoperative outcomes following a tarsal tunnel release between patients with positive and negative preoperative electrodiagnosis (EDx). METHODS: This study retrospectively reviewed 53 consecutive patients who underw

Electrodiagnosis-based management of patients with radiculopathy: The concept and application involving a patient with a large lumbosacral disc herniation.

OBJECTIVES: The evaluation of patients with lower back pain (LBP) is based mainly on clinical examinations and imaging procedures that are subjective or anatomic in nature. The treatments, either physical therapy or osteopathy, lack evidence-based protocol and may be disruptive to the spine. Therefore, a neurophysiologic-based approach to managing such patients is necessary. METHODS: A 40-year-old female complained of LBP and radiculopathy for more than 12 years, a condition that was accompanied by numbness, tingling and weakness in the left leg. This study examined the effectiveness of using

Does the comparison of median-to-ulnar nerve sensory conduction add an additional value in electrodiagnosis of carpal tunnel syndrome?

BACKGROUND AND PURPOSE: Distal sensory onset latency (DSOL), conduction velocity (SCV) and nerve action potential (SNAP) amplitudes are used in electrodiagnosis of carpal tunnel syndrome (CTS) beside motor conduction data. The aim of our study is to search whether the comparison of median-to-ulnar nerve sensory conduction adds an additional diagnostic value in CTS or not. METHODS: Median and ulnar nerve were stimulated on wrist, and SNAPs were recorded on second and fifth fingers, respectively. Best cut-off points for the searched parameters and their diagnostic efficiencies were determined.

Utility of Preoperative Electrodiagnosis Together with Peripheral Nerve High-Resolution Ultrasound: A Complex Case Report of Neurofibromatosis Type I.

Our case report underscores the importance of electroneuromyography (ENMG) combined with peripheral nerve high-resolution ultrasound (HRUS) in the evaluation of neurofibromatosis type 1 (NF1). A 49-year-old woman affected by NF1 came to our attention because of new-onset left arm weakness and atrophy. Debulking of a cervicothoracic C7-T1 neurofibroma had been performed 8 years earlier. On current admission, magnetic resonance imaging disclosed increased lesion volume that was thought to cause the neurologic deficits by compressing the C8 root. Findings from intraoperative neurophysiologic moni

Single-fiber electromyography-based diagnosis of CACNA1A mutation in children: A potential role of the electrodiagnosis in the era of whole exome sequencing.

INTRODUCTION: A loss-of-function mutation in CACNA1A, which encodes P/Q-type Ca channels, causes various diseases. As most of the Ca channels at neuromuscular junctions are of the P/Q type, patients with loss-of-function CACNA1A mutations exhibit disturbed neuromuscular transmission. The associated jitters and blocking in such patients can be detected by single-fiber electromyography (SFEMG). CASES: We report two cases with different phenotypes, which were predicted to harbor loss-of-function mutations of CACNA1A, by using axonal stimulation SFEMG. One case involved a 2-year-old boy with epis

[Electrodiagnosis of neuromuscular diseases--a syndrome of monomelic amyotrophy with spinal hemiatrophy, anti-GM1 antibodies and abnormal F-waves (MASH)].

Electrodiagnosis rests upon sound anatomical and physiological bases of the peripheral nervous system, which are utilized for the dynamic process of differential diagnosis. If clinical and electrophysiological findings do not conform to any of the previously known diseases, there may be a chance of identifying a new entity. As an example, we report nine Japanese patients with unilateral atrophy of the distal upper limb and the cervical cord. Symptoms typically began insideously in young adulthood, progressed for a few years and then stabilized. Distribution of the muscle weakness usually corre

Electrodiagnosis for mitigating false-negative non-responsiveness in electrical evoked contractions: A case series exploring probable polyneuromyopathy induced by nonuse.

INTRODUCTION: The revised international standards for neurological classification of spinal cord injury (ISNCSCI) have facilitated the documentation of non-spinal cord injury-related impairments, such as chronic peripheral nerve injuries and muscle weakness due to immobility. This advancement addresses potential biases in muscle strength examinations. Utilizing electrically evoked contractions from paralyzed muscles, enhanced by electrodiagnosis, holds promise in identifying false-negative diagnoses of non-responsiveness to neuromuscular electrical stimulation. This concept prompts the explora

Variability of Sural SNAP and Peroneal CMAP Amplitudes Limits Consistent Electrodiagnosis of Chronic Axonal Polyneuropathy: A Cross-Sectional Multicenter Study.

INTRODUCTION/AIMS: Current consensus guidelines for the electrodiagnosis of polyneuropathy derive from highly standardized investigations performed in specialist centers, raising the question of whether they are equally applicable in everyday general neurology outpatient settings. This multicenter study evaluated the consistency of electrodiagnostic findings in chronic axonal polyneuropathy across different clinics. METHODS: We retrospectively collected sural sensory nerve action potential (SNAP) and peroneal compound muscle action potential (CMAP) amplitude data from 62 patients with cryptog

Ultrasonography versus electrodiagnosis of median nerve before and after open carpal tunnel release surgery: which can better diagnose, predict and assess postoperative improvement?

Diagnosis of Carpal Tunnel Syndrome (CTS) relies on a structured clinical approach with Electrodiagnostic Studies (EDX) accepted as the gold standard in the diagnosis. Yet, Ultrasound (US) has emerged as a non-invasive alternative especially in assessing morphological changes. And so, to accurately diagnose CTS by a valid non-painful simple tool, objectively assess postoperative improvement and finally to predict postoperative clinical outcome from preoperative data; clinical, EDX, and US data were collected, compared and correlated pre- and 1 month postoperative. While US achieved diagnostic

Use of quantitative intra-operative electrodiagnosis during partial ulnar nerve transfer to restore elbow flexion: the treatment of eight patients following a brachial plexus injury.

The transfer of part of the ulnar nerve to the musculocutaneous nerve, first described by Oberlin, can restore flexion of the elbow following brachial plexus injury. In this study we evaluated the additional benefits and effectiveness of quantitative electrodiagnosis to select a donor fascicle. Eight patients who had undergone transfer of a simple fascicle of the ulnar nerve to the motor branch of the musculocutaneous nerve were evaluated. In two early patients electrodiagnosis had not been used. In the remaining six patients, however, all fascicles of the ulnar nerve were separated and electr

Electromyographic and magnetic resonance imaging to predict lumbar stenosis, low-back pain, and no back symptoms.

BACKGROUND: Magnetic resonance imaging is commonly used to diagnose lumbar spinal stenosis. Some persons without symptoms have a small lumbar spinal canal. Electrodiagnosis has been used to diagnose spinal stenosis for over sixty years, but we are aware of no masked, controlled trials of the use of electrodiagnosis for that purpose. This study was performed to evaluate the relationships of magnetic resonance imaging measures and electrodiagnostic data with the clinical syndrome of spinal stenosis. METHODS: One hundred and fifty persons between the ages of fifty-five and eighty years old, incl
