The diagnosis and treatment of psychiatric emergencies.

Although the majority of psychiatric patients present to the ER in chronic crisis, the clinician should also be familiar with the assessment and management of acute emergencies. Because emergency patients reflect the entire spectrum of psychiatric psychopathology, the primary care physician should consult with a mental health clinician whenever possible. Following a medical model, a thorough diagnostic evaluation should be completed and specific treatment initiated. Most often, the treatment consists of some combination of psychological, pharmacologic, and physical approaches.

Prophylaxis and treatment of pregnant women for emerging infections and bioterrorism emergencies.

Emerging infectious disease outbreaks and bioterrorism attacks warrant urgent public health and medical responses. Response plans for these events may include use of medications and vaccines for which the effects on pregnant women and fetuses are unknown. Healthcare providers must be able to discuss the benefits and risks of these interventions with their pregnant patients. Recent experiences with outbreaks of severe acute respiratory syndrome, monkeypox, and anthrax, as well as response planning for bioterrorism and pandemic influenza, illustrate the challenges of making recommendations about

Treatment of nail diseases in the emergency department.

BACKGROUND: Although Americans seek care for their nail diseases on an outpatient basis, they also seek nail care in the setting of the emergency department (ED). PURPOSE: This study aims to characterize the treatment of nail diseases at ED visits in the United States from 2009-2018. METHODS: Data from 2009-2018 was collected from the National Hospital Ambulatory Medical Care Survey for EDs. RESULTS: There was an estimated 1.54 (95% CI [1.35, 1.74]) million nail visits to the ED from 2009-2018. Whites accounted for the most visits (57%), followed by African Americans (21%). 25-44-year-olds

Early hemoperfusion for emergency treatment of carbamazepine poisoning.

OBJECTIVES: To investigate the clinical value of early hemoperfusion (HP) in emergency treatment of carbamazepine (CBZ) poisoning. METHODS: 104 patients with acute CBZ poisoning treated from August 2004 to October 2015 in the Emergency Department were reviewed. Patients were categorized into three groups: group A, who received HP treatment in the Emergency Department; group B, who received HP treatment in the blood purification room; and group C, who did not received HP treatment. Pharmacokinetic parameters of CBZ and remission of complications were compared among the three groups. RESULTS:

Emergency department evaluation and treatment of wrist injuries.

Correct diagnosis of wrist injuries is critical in preventing prolonged pain and dysfunction. Radiographs cannot diagnose a large percentage of injuries. Wrist sprain is considered one of the most common yet most treacherous emergency department (ED) diagnoses because radiographs do not always rule out all acute injuries. Knowledge of the anatomy, normal physical examination findings, and physical examination abnormalities associated with different pathological conditions, is paramount in making the correct diagnosis. This article focuses on the anatomy, diagnosis, and ED management of acute w

Treatment of pediatric migraine in the emergency room.

Migraine constitutes a relatively common reason for pediatric emergency room visits. Given the paucity of randomized trials involving pediatric migraineurs in the emergency department setting compared with adults, recommendations for managing these children are largely extrapolated from adult migraine emergency room studies and trials involving outpatient home pediatric migraine therapy. We review current knowledge about pediatric migraineurs presenting at the emergency room and their management, and summarize the best evidence available to guide clinical decision-making.

Emergency treatment options for pediatric traumatic brain injury.

Traumatic brain injury is a leading killer of children and is a major public health problem around the world. Using general principles of neurocritical care, various treatment strategies have been developed to attempt to restore homeostasis to the brain and allow brain healing, including mechanical factors, cerebrospinal fluid diversion, hyperventilation, hyperosmolar therapies, barbiturates and hypothermia. Careful application of these therapies, normally in a step-wise fashion as intracranial injuries evolve, is necessary in order to attain maximal neurological outcome for these children. It

Emergency percutaneous treatment of arterial iliac axis ruptures.

The objective of this paper is to assess the feasibility and effectiveness of emergency percutaneous treatment of ruptures of the iliac axis. In 5 years, we observed 13 patients (mean age, 62.1 years), 11 with rupture of the external iliac artery and two with rupture of the common iliac artery (six traumatic and seven iatrogenic). All patients were treated with stent grafts. A follow-up was performed with a color Doppler ultrasound at 1, 3, 6, and 12 months during the first year and then yearly. Immediate technical success was obtained in all cases. During a mean follow-up of 22.3 months, one

Treatment failure in emergency department patients with cellulitis.

OBJECTIVE: To identify the rate of treatment failure in emergency department patients with cellulitis. METHODS: This prospective observational convenience study enrolled adult patients with uncomplicated cellulitis. Physicians performed a standardized assessment prior to treatment. To calculate the interrater reliability of the assessment, duplicate data collection forms were completed on a small subsample of patients. Treatment failure was defined as the occurrence of any one of the following events after the initial emergency department visit: incision and drainage of abscess; change in ant

Homeopathic treatment in emergency medicine: a case series.

Following a multiple-casualty construction disaster in Israel, members of The Center of Integrated Complementary Medicine joined in the emergency activity of the Shaare Zedek Medical Center. They administered homeopathic treatment to injured patients to supplement conventional orthopaedic treatment. This was to our knowledge the first time that complementary medicine had been used officially in conjunction with conventional medicine in an emergency situation. Our objective is to report and summarize the rationale, procedures and outcome of the complementary medicine intervention. Fifteen ortho

[Psychopharmacological treatment in the pre-clinical emergency medicine].

Psychiatric emergencies are a common cause for a call for the pre-clinical emergency physician (EP). However, previous studies reported serious problems in diagnosing and particularly treating these patients. Although evidence-based treatment guidelines and algorithms exist for the most important somatic disorders the EP has to deal with, they do not for psychiatric emergencies. It is the aim of this review, subsequent to an extensive overview on the available literature, to provide the reader with differentiated suggestions for pharmacological crisis intervention as well as with treatment rec

Emergency department evaluation and treatment of wrist injuries.

This article provides the emergency physician a thorough understanding of the basic and surface anatomy required to accurately diagnose wrist injuries in the emergency department. Functional and neurologic examinations are discussed in detail as they increase precision in diagnosis. Special radiographic studies, guided by exam findings, are often crucial in adequately visualizing an otherwise occult injury. Radiographic evidence of injury is not present at time of initial evaluation in a small but noteworthy percentage of significant injuries. For these reasons, the appropriate use of radiogra

Emergency esophagogastrectomy for treatment of hydrochloric acid injury.

Eight patients with severe hydrochloric acid injury of the esophagus and stomach had early esophagogastrectomy. Four patients survived and 6 to 8 weeks later underwent successful substernal colon replacement. The value of early diagnosis and subsequent aggressive surgical management of patients who have ingested hydrochloric acid is stressed.

Treatment of the endodontic emergency: a decade later.

A questionnaire was sent to diplomates of the American Board of Endodontics to determine changing trends in the treatment of endodontic emergencies since Dorn's survey 10 yr ago. There is a definitely greater trend toward complete instrumentation of the root canals to the apex regardless of the emergency condition than there was a decade ago. The number of respondents who leave teeth open when the pulp is nonvital has dropped dramatically in the past decade. When there is no periapical involvement, over one third of the respondents are completing treatment in one visit. A decrease in the use o

[Treatment of thoracic trauma at an emergency hospital].

Thoracic trauma was treated in 159 hospitalised patients within a five-year period, with polytrauma being recorded from 40 per cent of the patients. Application of thorax suction drainage proved to be the most effective first treatment for 50 per cent of all cases. Emergency thoracotomy was necessary in 7.6 per cent. The lethality rate recorded from these cases of thorax injury amounted to twelve per cent.
