Acute disseminated encephalomyelitis: current understanding and controversies.

Acute disseminated encephalomyelitis (ADEM) is an uncommon monophasic idiopathic inflammatory demyelinating disease. Available diagnostic criteria do not reliably distinguish it from first presentations of relapsing diseases such as multiple sclerosis (MS) and neuromyelitis optica (NMO). In this article, we review current concepts about ADEM and what distinguishes it from other idiopathic inflammatory demyelinating disease, and we highlight controversial aspects and diagnostic problems. We review pathological differences between ADEM and MS in terms of their utility in the diagnosis of ADEM. F

Acute disseminated encephalomyelitis associated with enteroviral infection.

A case of acute disseminated encephalomyelitis in 13-year-old boy associated with enterovirus is described. The patient had symptoms of severe headache and photophobia for 2 days. Diagnosis was made on the basis of diffuse high intensity white matter lesions in the left frontoparietal region seen on magnetic resonance imaging, and positive enterovirus polymerase chain reaction in cerebrospinal fluid. His symptoms improved substantially without specific therapy, and he recovered without neurologic sequelae.

Viliuisk encephalomyelitis: intrathecal synthesis of oligoclonal IgG.

Viliuisk encephalomyelitis (VE) is a neurodegenerative disorder expressed as subacute meningo-encephalitis progressing to a more prolonged pan-encephalitic syndrome with a fatal outcome within 1 to 10 years. Some patients survive to a steady state of global dementia and severe spasticity that may last for over 20 years. Multiple micronecrotic foci surrounded by inflammatory infiltrates are observed throughout the cerebral cortex and other gray matter areas. Infectious etiology of VE is strongly suspected, but the causative agent has not been identified. We conducted a search for assays that mi

Acute disseminated encephalomyelitis following Plasmodium vivax malaria.

A 24-year-old Japanese man showed neurological disturbances 2 weeks after complete recovery from Plasmodium vivax infection. Magnetic resonance (MR) images of the brain showed multiple high-intensity spotty lesions in the left cerebral cortex and subcortex. Cerebrospinal fluid examination, including polymerase chain reaction analysis for viruses, revealed no sign of active infection. Repeated blood smears were negative for malaria. We diagnosed acute disseminated encephalomyelitis (ADEM) following Plasmodium vivax malaria from the clinical course and MR images. ADEM should be regarded as one o

Multiphasic disseminated encephalomyelitis associated with streptococcal infection.

Multiphasic disseminated encephalomyelitis (MDEM) is a rare peculiar diagnosis which is defined as acute demyelinating central nervous system disease with relapses occurring only within 4 weeks of initial manifestation. This report describes the case of a 6-year-old boy with MDEM diagnosed by clinical findings and magnetic resonance imaging. The disease had a biphasic evolution, and with a second course of high-dose corticosteroids a complete recovery without further relapse was obtained during the following 18 months. Serological evidence of streptococcal infection as specific trigger for MDE

Encephalomyelitis due to human parechovirus type 1.

OBJECTIVES: The implication of a viral agent in encephalomyelitis has been reported for several years. In the present study we wanted to demonstrate the presence of human parechovirus type 1 (HPEV1) in a patient diagnosed with encephalomyelitis. STUDY DESIGN: Clinical samples (throat and rectal swabs, acute and convalescent sera, cerebrospinal fluid) were collected from a 10-month-old boy diagnosed with encephalomyelitis. The appropriated samples were tested for cytomegalovirus, varicella zona virus, mumps virus and enteroviruses with specific culture, and serological and molecular biological

Acute disseminated encephalomyelitis after live rubella vaccination.

We report here a case involving a 14-year-old boy who developed acute disseminated encephalomyelitis following live rubella vaccination. The patient became febrile and began to experience nuchal pain 16 days after the immunization. By 22 days after immunization, he experienced difficulty in walking. By 24 days, he had developed tetraparesis with retention of urine, and total sensory loss below the Th1 dermatomal level. He was febrile at this point and showed nuchal rigidity and Lhermitte's sign. Cerebrospinal fluid examination revealed elevated cell counts, protein level, and myelin basic prot

Intravenous immunoglobulin therapy in acute disseminated encephalomyelitis.

Three children ranging in age from 2 to 5 years with acute disseminated encephalomyelitis (ADEM) were successfully treated with high-dose intravenous immunoglobulin (IVIG). Their symptoms were somnolence, fever, headache, vomiting, and resting tremor. In all of these patients, it was difficult to distinguish the condition from viral encephalitis before analyzing the myelin basic protein. ADEM was diagnosed because of increased levels of myelin basic protein in their cerebrospinal fluid and abnormal high-signal intensity on T2-weighted magnetic resonance imaging. All patients were given IVIG at

Acute disseminated encephalomyelitis: MR and CT features.

We describe three cases of acute disseminated encephalomyelitis (an immune-mediated inflammatory demyelinating disease of the central nervous system) and their histology, showing different radiological features. One appearance is a few ring-shaped enhancing lesions, which are found predominantly in the supratentorial white matter, the other is solid disseminated lesions.

Eastern equine encephalomyelitis outbreak in coturnix quail.

Eastern equine encephalomyelitis (EEE) was the cause of heavy mortality in coturnix quail (Coturnix coturnix) reared commercially in South Carolina. The birds showed depression, tremor, and partial paralysis that advanced into complete paralysis, torticollis, and death within a few hours. The only consistent lesion on necropsy was a catarrhal enteritis in the duodenal area. The disease spread rapidly to all pens throughout the two houses on the farm in all birds over 2 weeks old, and mortality ranged from 40 to 90% in the various pens within the house. Total mortality exceeded 90,000 birds. Ag

St. Louis encephalomyelitis in Hermosillo, Sonora, Mexico.

This report, which describes an epidemic of St. Louis encephalitis in northwestern Mexico, was prompted by the apparent infrequency of previous Mexican outbreaks of this disease. The site of the epidemic was the Municipality of Hermosillo in the State of Sonora. This municipality, with a population of about 273,000, is situated in a hot flat area which is naturally dry but which is also well-irrigated. Here, over an eithy-week period, 51 persons became ill and were hospitalized with symptoms including fever, vomiting, stiff neck, lethargy, and increased cell/protein levels in their cerebrospin

Nitric oxide and experimental autoimmune encephalomyelitis review.

Immunisation with neuroantigen in complete Freund's adjuvant (CFA) results in a range from severe experimental autoimmune encephalomyelitis (EAE) to no EAE in various strains and sexes of rodents. When CFA was substituted for carbonyl iron, all were EAE-susceptible. One of the differences between the two adjuvants was the strong induction of inducible nitric oxide synthase in EAE-resistant strains by CFA. The questions discussed in this review are: 1/ Could exaggerated production of nitric oxide protect against development of autoimmunity? 2/ Could non-susceptible strains be rendered susceptib

Seronegative Autoimmune Encephalomyelitis with Area Postrema Symptoms.

INTRODUCTION: Patients presenting with encephalopathy and longitudinally extensive myelitis pose a significant diagnostic challenge. Area postrema-related symptoms, such as intractable hiccoughs, can aid in narrowing the differential diagnosis. Neuromyelitis optica spectrum disorders and glial fibrillary acidic protein (GFAP) autoimmune encephalitis are known causes; however, some cases remain seronegative, suggesting the presence of unidentified autoantibodies or immune targets. CASE PRESENTATION: A previously healthy man in his 70s presented with headache, fever, and confusion, followed by

Interferon Lambda Signaling Restrains Experimental Autoimmune Encephalomyelitis.

IFN-λ is a type III interferon (IFN) with pleiotropic functions in modulating immune responses. To address its function in autoimmune neuroinflammation, we evaluated the development and progression of experimental autoimmune encephalitis (EAE) in IFNLR1KO (Ifnlr1-/-) and C57Bl/6 (WT) mice following immunization with MOG35-55 peptide. The results show that Ifnlr1-/-mice developed significantly more severe EAE than WT littermates with a similar day of onset, suggesting the potential of IFN-λ in reducing disease severity. We next interrogated whether IFN-λ differentially modulates EAE induced by

Acute Disseminated Encephalomyelitis Following COVID-19 Infection.

Acute disseminated encephalomyelitis (ADEM) is a relatively rare, post-inflammatory, immune-mediated demyelinating central nervous system disease that is predominantly reported in pediatric populations. Following the emergence of severe acute respiratory syndrome coronavirus 2, cases of ADEM are being reported following infection with this virus. Our case report describes a male patient in his early 40s who developed severe coronavirus disease 2019 (COVID-19) that rapidly progressed to a critical disease requiring invasive mechanical ventilation and high positive end-expiratory pressure, which
