Accessory nerve injury during carotid endarterectomy.

Injury to the accessory nerve (cranial nerve XI) during carotid endarterectomy is rare; to date only three cases have been reported in the literature. Traction on the sternocleido-mastoid muscle was the proposed mechanism of injury in all three cases. Four cases of accessory nerve palsy occurred in 850 carotid endarterectomies performed between 1978 and 1986 at this institution, an incidence of 0.47%. All four patients had classic signs and symptoms of accessory nerve injury, which developed between 20 and 60 days after operation. The three most recent cases were examined specifically for acce

Cranial nerve paralysis following carotid endarterectomy.

During the past seven years 347 patients have been entered into a data bank at the Duke University Medical Center for evaluation of transient neurologic ischemia. One hundred fifty eight of these patients had carotid endarterectomies of whom 24 (15.1%) developed 26 (16.4%) peripheral cranial nerve palsies. Injury to the peripheral portion of the hypoglossal nerve was noted in 13 patients, to the cervical branch of the facial nerve in five and to the recurrent laryngeal nerve branch of the vagus in eight. Complete recovery of nerve function usually occurred within four months but residual defic

Motor speech malfunction following carotid endarterectomy.

Cranial nerve injury during carotid endarterectomy is a well-recognized complication of this procedure. The evaluation of the extent of the injury and the persistence of the malfunction is difficult to accomplish with conventional methods. A protocol designed to evaluate motor speech function was administered to 36 patients prior to carotid endarterectomy, 2 days after surgery, and 6 weeks postoperatively. The protocol included assessments of hypoglossal nerve function, superior-recurrent laryngeal function, glossopharyngeal nerve function, and integrated motor speech ability. Direct laryngosc

Carotid artery stenting outcomes are equivalent to carotid endarterectomy outcomes for patients with post-carotid endarterectomy stenosis.

BACKGROUND: Carotid artery stenting (CAS) has been advocated as an alternative to redo surgery for patients with post-carotid endarterectomy (CEA) stenosis. This study compares early and late clinical outcomes for both groups. METHODS: This study analyzes 192 patients: 72 had reoperation (Group A) and 120 had CAS for post-CEA stenosis (Group B). Patients were followed prospectively and had duplex ultrasounds at 1 month, and every 6 to 12 months thereafter. The perioperative complications (perioperative stroke, myocardial infarction/death, cranial nerve injury) and 4-year end points were analy

Comparison of standard carotid endarterectomy with Dacron patch angioplasty versus eversion carotid endarterectomy during a 4-year period.

Currently, the two primary approaches to carotid endarterectomy for extracranial carotid stenosis are carotid endarterectomy with patch angioplasty and eversion carotid endarterectomy. In a retrospective study over a 4-year period from 1998 to 2002, we had an opportunity to compare the two approaches as two surgeons utilized carotid endarterectomy with Dacron patch angioplasty and two other surgeons utilized eversion carotid endarterectomy. During the 4-year period, 189 carotid endarterectomies were performed, 125 with Dacron patch angioplasty (CE-P) and 64 with eversion (EE) endarterectomy. T

Timing of carotid endarterectomy and clinical outcomes.

The timing of carotid endarterectomy (CEA) for symptomatic ipsilateral carotid artery stenosis has evolved in practice over time. Key landmark trials outlined the benefit of performing CEA in the recently symptomatic carotid artery stenosis, defined as revascularisation within 6 months of the index neurological event. Further evidence and sub-analysis demonstrate that performing CEA within 2 weeks of symptoms has the maximal benefit in reducing stroke free survival and is associated with a safe perioperative complication profile. This has translated into guideline recommendations and widesprea

Carotid endarterectomy in patients with high plaque.

BACKGROUND: Difficult cephalad exposure during carotid endarterectomy in patients with high plaque (HP) may lead to increased incidence of complications after carotid endarterectomy. We report on our experience of carotid endarterectomy in patients with HP. METHODS: This is a retrospective review of 1,233 consecutive patients who underwent carotid endarterectomy by a single surgeon at 2 teaching hospitals between January 1989 to December 2018. Group A consisted of patients with HP (n = 100) diagnosed by computed tomography angiography of the neck in 90, catheter-based arteriography in 8, and

Coronary endarterectomy: The current state of knowledge.

Coronary artery endarterectomy (CE) is a procedure performed adjunctive to coronary artery bypass grafting in patients with diffuse coronary artery disease (CAD). This review was conducted in order to assess the safety of the procedure, the associated complications as well as short- and long-term patient outcomes. A literature search was performed via Pubmed, and 50 articles were included in the review based on our inclusion and exclusion criteria. The mortality and morbidity rates were found to be acceptable in a highly selected group of patients. Therefore, CE may have an important role to p

[Carotid Endarterectomy in Patients with Antiaggregation Therapy].

Carotid endarterectomy is a common way of surgical treatment of extracranial carotid artery disease caused by atherosclerosis. Patients are often operated on under local anesthesia with intraoperative application of heparin. Postoperative bleeding occurs in up to 8% of cases, and up to 4.7% of patients need reoperation due to bleeding. TachoSil is a medical sponge consisting of collagen with added human coagulation factors (fibrinogen and thrombin). In contact with water, blood or bodily fluids, it forms a clot that adheres to the surface. The hypothesis and aim of our study was to show that T

Postoperative management of pulmonary endarterectomy and outcome.

Pulmonary artery thromboendarterectomy (PTE) has been regarded as a promising, potentially curative surgical procedure. However, PTE is associated with specific postoperative complications, such as reperfusion pulmonary edema and right heart failure leading to a considerable mortality of 7-24%. Despite its limitations PTE is a better surgical alternative to lung transplantation which carries high morbidity and mortality. The aim of the study is to analyze the efficacy, safety, morbidity and survival associated in the postoperative period and quality of life after six months of PTE in Indian pa

MR angiographic and sonographic indications for endarterectomy.

PURPOSE: Our objective was to determine whether appropriate criteria could be developed for performing an endarterectomy on the basis of sonographic and MR angiographic findings. METHODS: Fifty patients were examined prospectively with sonography, MR angiography, and conventional angiography. All three imaging studies were performed within 2 weeks of one another, and conventional angiography served as the reference standard. RESULTS: All 10 carotid occlusions were detected with sonography and MR angiography. Sonography accurately showed flow in two arteries, and MR angiography showed flow in

The changing indications for carotid endarterectomy in the UK.

In early series the majority of carotid endarterectomies were performed in patients with amaurosis fugax (AFx) or transient ischaemic attacks (TIAs) who were thought to have atheromatous ulcers of the carotid bifurcation or the internal carotid artery (ICA). The degree of stenosis was considered to be of secondary importance. We compared our own data with two British series undertaken in the early and late 80s/early 90s. This reflects the broadening of indications and the change of practice for carotid endarterectomy over the years, on the one hand towards including patients who are at greater

Fate of the carotid artery contralateral to endarterectomy.

The management of internal carotid artery disease contralateral to endarterectomy is highly controversial. At our institution we have adopted an approach by which patients are followed with serial duplex scanning after unilateral carotid endarterectomy. Surgery on the contralateral carotid artery is recommended for patients who exhibit ischemic neurologic symptoms or develop an 80% to 99% carotid stenosis. This strategy is based on previous reports that have documented an increased incidence of strokes in these two groups of patients. As a result, 40 patients among a study population of 200 un

[Carotid endarterectomy. Risks and benefits of treatment].

From 1980 to 1985, 210 endarterectomies of the carotid artery were performed in 187 patients (50 female, 127 male). In 55 cases there were asymptomatic filiform stenoses with concomitant high grade stenosis or occlusion of the contralateral artery. 155 stenoses of the carotid artery of mostly the highest degree were symptomatic. Patients were examined by means of the direct CW-doppler ultrasound immediately postoperatively and every six months up to six years. In 9 cases (4.3%) there was a perioperative (central) neurologic incident, which in 5 (2.3%) cases was followed by permanent neurologic

Results of coronary artery endarterectomy and reconstruction.

Since 1978, 5005 patients have had coronary artery bypass operations: 50% had conventional grafts only (group A), 25.1% required one coronary artery endarterectomy (group B), and 24.9% required multiple endarterectomies (group C). Operative mortality and long-term survival were stratified within each group according to the presence of additional risk factors: severe left ventricular dysfunction, repeat operation, insulin-dependent diabetes mellitus, female sex, and age over 70 years. Operative mortality was 4.0% in group A patients, 6.3% in group B, and 10.4% in group C; it increased in each g

Carotid endarterectomy in a small community hospital.

Over a 10 year period carotid endarterectomy was performed in a total of 177 operations in 145 patients at a small community hospital. Mortality and morbidity rates were similar to those reported from larger medical centers. The use and comparison of intracerebral protective techniques of hypercarbia and shunting are discussed. Routine use of the Silastic shunt appears preferable to more elaborate means of cerebral protection. The noninvasive Dopscan and periorbital Doppler examination are effective tools in the selection of patients for surgery and follow-up of patients with asymptomatic brui
