Infective endocarditis is an infectious disease usually caused by bacteria, including streptococci, staphylococci, and enterococci. In this report, a case of infective endocarditis in which Pseudoclavibacter spp. detected as the causative agent was presented. A 66-year-old female patient was admitted to our hospital with weight loss, malaise, and lumbar pain. A 2/6 murmur was detected in the physical examination of the patient, who had a history of mitral valve surgery nine years earlier. Blood sample was collected for culture and vancomycin [1 g/24 hours, intravenously (IV)] and gentamicin (8

INTRODUCTION AND OBJECTIVES: The aim of our study is to assess changes in the epidemiologic features of patients with native valve infective endocarditis.
METHODS: We analyzed a prospective series of 228 cases of native valve infective endocarditis in non-intravenous drug users attending our center between 1987 and 2009. We compared three subperiods: 1987-1994 (67 cases), 1995-2002 (74 cases) and 2003-2009 (87 cases).
RESULTS: The mean age of patients has progressively increased (38±22 years in the first subperiod vs 60±16 years in the third; P<.001), as has the proportion of cases without p

BACKGROUND: Conventional diagnosis of infective endocarditis (IE) is based mainly on culture-dependent methods that may fail because of antibiotic therapy or fastidious microorganisms.
OBJECTIVES: We aimed to evaluate the added values of serological and molecular methods for diagnosis of infective endocarditis.
PATIENTS AND METHODS: One hundred and fifty-six cases of suspected endocarditis were enrolled in the study. For each patient, three sets of blood culture were withdrawn and serum sample was collected for Brucella, Bartonella and Coxiella burnetii antibody testing. Galactomannan antige

Non-bacterial thrombotic endocarditis (NBTE), also known as marantic endocarditis, is a rare entity characterized by sterile vegetation on cardiac valves. It is typically associated with underlying hypercoagulable conditions, such as a patient with malignancy or autoimmune disease. We report a 70-year-old patient who presented with chronic cough, intermittent fever, progressive dyspnea, and significant weight loss. Imaging revealed left-sided pleural effusion with lung collapse, and further evaluation identified a central bronchial mass consistent with squamous cell carcinoma of the lung with

Brucellosis is a rare zoonotic infection with a low annual incidence in the United States. Infective endocarditis secondary to brucellosis involving native or prosthetic valves is contemplated to be an extremely rare entity. As Brucella can present with non-specific sign and symptoms, clinicians need to have a higher degree of suspicion of Brucella endocarditis in culture-negative endocarditis patients, particularly those who have a history of exposure to farm animals. Timely diagnosis with appropriate management using antibiotics can prevent valvular damage and restore the valve's structural

A case of severe endocarditis, with complications of multiple infarction, meningitis, and ruptured mitral chordae tendineae, caused by Streptococcus agalactiae in a healthy man, is reported. Emergency cardiovascular surgery was performed on the day of admission. Infective endocarditis caused by S. agalactiae is very rare, particularly in a healthy adult. In addition, microbiological analysis revealed that S. agalactiae of sequence type (ST) 19, which belongs to serotype III, was present in the patient's vegetation, mitral valve, and blood culture. It was therefore concluded that the endocardit

BACKGROUND AND OBJECTIVES: There is a rising incidence of infective endocarditis-related stroke (IERS) in the United States attributed to the opioid epidemic. A contemporary epidemiologic description is necessary to understand the impact of the opioid epidemic on clinical characteristics of IERS. We describe and analyze trends in the demographics, risk factors, and clinical features of IERS.
METHODS: This is a retrospective cohort study within a biracial population of 1.3 million in the Greater Cincinnati/Northern Kentucky region. All hospitalized patients with hemorrhagic or ischemic stroke

OBJECTIVE: Standard treatment of patients with infective endocarditis is radical debridement and valve replacement, in cases with advanced pathology the treatment is usually root replacement with either a composite graft or a homograft. Enthusiasm for the use of the Ross operation in non-infective aortic valve disease is increasing, but use of the pulmonary autograft in the treatment of aortic valve endocarditis has been limited. The objective of this prospective study is to present the technique and results of our experience with aortic valve endocarditis treated with the Ross operation.

Cutaneous vasculitis can pose a significant diagnostic challenge due to its broad range of potential causes, including immune-mediated conditions, drug reactions, and infection-driven processes. Systemic infections can sometimes manifest initially with skin findings, including palpable purpura, petechiae, or necrotic lesions, which mimic primary vasculitic disorders. Distinguishing between infectious and non-infectious vasculitis is critical, as empirical immunosuppressive therapy in patients with an underlying infection can worsen outcomes and delay appropriate treatment. We report the case o

Background: Mitral valve infective endocarditis (IE) still has a high mortality. Minimally invasive mitral valve surgery (MIMVS) is technically more challenging, especially in patients with endocarditis. Here, we compare the early postoperative outcome of patients with endocarditis and other indications for MIMVS. Methods: Two groups were formed, one consisting of patients who underwent surgery because of mitral valve endocarditis (IE group: n = 75) and the other group consisting of patients who had another indication for MIMVS (non-IE group: n = 862). Patients were observed for 30 postoperati

Infective endocarditis is a life-threatening infection of heart valves and adjacent structures characterized by vegetations on valves and other endocardial surfaces, with tissue destruction and risk of embolization. We used high-resolution mass spectrometry to define the proteome of staphylococcal and non-staphylococcal vegetations and Terminal Amine Isotopic Labeling of Substrates (TAILS) to define their proteolytic landscapes. These approaches identified over 2000 human proteins in staphylococcal and non-staphylococcal vegetations. Individual vegetation proteomes demonstrated comparable prof

Bartonella quintana is a louse-borne Gram-negative bacterium first discovered as the cause of trench fever during World War I. As B. quintana is not identified using routine culture with 5-day incubation, the pathogen is an agent of blood culture-negative endocarditis, particularly among individuals experiencing homelessness. Here, we describe a case of an 80-year-old man who presented in the winter of 2017 with severe aortic valve regurgitation resulting in exercise intolerance, requiring aortic valve replacement. Pathological assessment of the explanted aortic valve revealed changes of infec

Transthoracic and transoesophageal doppler echocardiography has become an essential tool for the diagnosis of infective endocarditis. It detects valvular vegetations and destructions, evaluates regurgitations, and appreciates haemodynamic tolerance. Recognition of endocarditis, early identification of high-risk patients, and detection of mechanical complications are the most accurate informations derived from this technique. Transoesophageal echocardiography has the potential to avoid the difficulties encountered using the transthoracic approach, and it must be used particularly in the presenc

The diagnosis and management of pacemaker-related infective endocarditis present significant challenges, with limited available data. Accurately attributing a systemic infection to pacemaker endocarditis can be difficult, particularly in identifying vegetations and obtaining positive blood cultures from patients who have undergone non-specific antibiotic therapy. Moreover, such infections may manifest long after pacemaker implantation. Herein, we present a male patient in his 70s, with a history of pacemaker placement, who was admitted with a three-month history of fever and chills, having alr

INTRODUCTION: Infective endocarditis (IE) remains a severe infection associated with substantial morbidity and mortality. Given the central role of platelets in its pathogenesis, antithrombotic therapy has been hypothesized to exert a preventive effect, though clinical evidence remains inconclusive.
METHODS: We conducted a retrospective, population-based study using the TriNetX Global Collaborative Network to assess the incidence of IE among individuals exposed to antiplatelet or anticoagulant therapy versus untreated controls. Propensity score matching accounted for major cardiovascular and

We present a case of a 42-year-old Caucasian man that came to our facility complaining of dyspnea and subjective fevers and was found to have infective endocarditis. His blood cultures initially grew Peptostreptococcus spp and he was started on penicillin and gentamicin. Repeat blood cultures, drawn 48 h later, became positive for Granulicatella adiacens. Vancomycin was added at this point, given reports of growing penicillin resistance of Granulicatella spp. Transthoracic echocardiogram showed multiple vegetations on the aortic, mitral and tricuspid valves. The patient's final antibacterial r

Infective endocarditis and liver cirrhosis is an infrequent association. A retrospective study was performed in order to characterize predisposing factors, microbial causes and evolution. Medical records between 1995 and 2008 (June) were searched. Four cases were identified. In three cases liver cirrhosis was in stage Child A, and in all 4 there was a predisposing cardiac disease. Clinical manifestations were classical in 3 cases and in one presented as hepatic failure. Only in one case a typical agent was recovered. Other cases were associated to a nosocomial agent or Corynebacterium sp. [cor

This report deals with a 25-year-old female patient, who was admitted to a medical centre with complaints of fatigue and arthralgia. A non-homogeneous mass originating from the posterior papillary muscle and reaching the posterior and anterior cusps of the mitral valve was evident on the patient's echocardiogram. Her intractable fever, despite adequate antibiotic therapy, led us to consult with the cardiac surgeons. A mitral valve replacement was performed on the 8th day of admission. The pathological examination of the mitral valve and the vegetative lesion revealed the presence of organized

We report the case of a 64-year-old man with Arcanobacterium pyogenes endocarditis. The patient presented with dyspnea and asymmetrical progressive quadriparesis. A transthoracic echocardiogram revealed mobile vegetations on both leaflets of his mitral valve measuring 0.5 x 3 cm, thickening of the mitral valve with severe mitral regurgitation due to dehiscence of the papillary muscle to the posterior mitral leaflet. He also had aortic sclerosis with a vegetation measuring 0.5 x 1 cm causing aortic valve dehiscence and free flow aortic regurgitation. An initial hemoculture grew out pleomorphic,
