Role of periodontal endoscopy in clinical decision-making for teeth with suspected vertical root fractures.

BACKGROUND: This study aimed to evaluate the role of periodontal endoscopy in clinical decision-making for teeth with suspected vertical root fracture (S-VRF) and to assess the diagnostic accuracy of this procedure with respect to vertical root fracture (VRF). METHODS: A prospective cohort of 115 S-VRFs meeting the inclusion criteria was analysed. The teeth were independently evaluated by two endodontists and subsequently categorized into three groups based on the type of pre-endoscopic treatment: flap surgery (S group, n = 56), endodontic treatment (R group, n = 48), and extraction (E group,

Factors influencing treatment decisions in permanent mature teeth with irreversible pulpitis: a questionnaire-based study.

BACKGROUND: Decision-making in dentistry is a complex process, and this study evaluated factors that influence dentists' approaches in permanent mature teeth with irreversible pulpitis. METHODS: An online questionnaire was distributed to a group of dental practitioners. The questionnaire surveyed dentists' opinions about the management of vital permanent teeth with irreversible pulpitis including a case scenario. RESULTS: Data from 262 respondents were analysed. Barriers to perform vital pulp therapy (VPT) included presuming it an inappropriate long-term treatment (29.7%), lack of knowledge,

Retrospective Evaluation of Periradicular Biopsies: An 18-Year Study.

INTRODUCTION: Radiographic findings in periradicular areas are repeatedly associated with infected root canal systems. Although non-odontogenic lesions in teeth are reported to be low, they often mimic periapical pathoses, and consequently, histopathologic examinations after surgical revisions are nurtured. METHODS: Biopsies submitted to the College of Dentistry between 2003 and 2021 were reviewed. Clinicopathologic characteristics were collected, including age, sex, medical history, location, sensibility tests, and clinic impressions from each specimen. Histopathologic diagnosis and gross de

Accuracy of dentists in the clinical diagnosis of oral lesions.

OBJECTIVE: Dentists play an important role in the early diagnosis and treatment of oral lesions. However, treatment based solely on a clinical impression of the diagnosis, without histologic confirmation, can result in serious consequences, particularly when the lesion is precancerous or cancerous. The purpose of this study was to determine the overall accuracy of clinical diagnoses made by dentists as well as to compare the diagnostic ability of general practitioners with members of various dental specialties. METHOD AND MATERIALS: The biopsy reports of 976 specimens submitted to the Departm

Paresthesia and Dysesthesia after Root Canal Therapy of a Mandibular Molar: Diagnosis and Management in a Clinical Case Report.

Inferior alveolar nerve (IAN) injury is a rare but serious complication of root canal treatment (RCT), typically caused by mechanical over-instrumentation, chemical irritation from extruded sealers, or thermal damage. We report the case of a 42-year-old woman who developed persistent burning pain, numbness, and dysesthesia of the right lower lip and gingiva after RCT of the right mandibular second molar. Initial management by a general dentist involved retreatment and calcium hydroxide medicament; however, early CBCT was misinterpreted, and referral was delayed. High-quality CBCT and clinical

Nonsurgical management of periapical lesions: a prospective study.

A total of 93 cases comprising 132 teeth with signs and symptoms or radiographic evidence of periapical pathoses were treated endodontically and were followed up for a period of 2 years. Nonsurgical management was successful in 84.4% of the cases. Histopathologic examination of tissue specimens from cases in which treatment failed revealed that only 35.7% of the cases involved apical cysts, while the remaining 64.3% involved chronic inflammatory tissue. Interestingly, 50% of the failures were observed at or 1 year after completion of treatment, which stresses the need for long-term follow-up o

Histopathological Study of Regenerative Endodontic Therapy on an Immature Mandibular Second Premolar With Pulp Necrosis: A Case Report.

Regenerative endodontic therapy (revascularization) for immature permanent teeth with pulp necrosis and/or apical periodontitis is an effective treatment to promote root maturation. Previous histological studies have reported the formation of cementoid or osteoid tissue and periodontal ligament-like tissue within the root canals. This case report presents the histopathological findings of a human immature permanent tooth with pulp necrosis following revascularization. A 11-year-old male patient presented with tenderness on biting and the formation of a sinus tract in the mandibular right secon

Treatment of an Endo-Perio Lesion with Ozone Gas in a Patient with Aggressive Periodontitis: A Clinical Case Report and Literature Review.

The pulp and periodontium have obvious relationships that have been described in many studies. Pulp infections may affect periodontal tissues and vice versa. Teeth with endo-perio lesions have a worse prognosis than isolated endodontic or periodontal lesions. Elimination of endodontic and periodontal infections is essential for successful treatment, so co-operation between endodontists and periodontists is necessary. In this clinical case, a 44-year-old male presented with primary periodontal disease with secondary endodontic involvement in his lower right canine because of aggressive periodon
