Sporadic endolymphatic sac tumor--a diagnostic and therapeutic challenge.

Endolymphatic sac tumor (ELST) is a rare low-grade locally aggressive neoplasm of the inner ear that may occur sporadically or in the setting of von Hippel-Lindau syndrome. We herein present a case of sporadic ELST in a 39-year-old man, treated using an interdisciplinary approach (surgery+radiotherapy), with a 10-year follow-up. The patient presented with hearing loss of sudden onset. The treatment of choice for ELST is radical tumor resection, which is associated with a good long-term prognosis. Remission may last for years, but there may be local recurrences, probably as a result of incomple

Gamma knife radiosurgery in jugular foramen endolymphatic sac adenocarcinoma.

A 41-year-old male presented to us with a history of right-sided temporal headache and ear discharge. MRI revealed an extra-axial space-occupying lesion in the region of the right jugular foramen. The patient was operated upon and a radical excision of the tumor was performed. Histopathology revealed an adenocarcinoma, probably of endolymphatic sac origin. The patient had a recurrence of tumor at 2-year follow-up and was subjected to gamma knife radiosurgery. The patient was subsequently asymptomatic at a 2.5-year follow-up and imaging revealed regression of the tumor size. Stereotactic radios

Revision of endolymphatic sac surgery for recurrent Meniere's disease.

Understanding the pathogenesis of Meniere's disease can result from studying its natural history; however, a marvelous opportunity for this understanding exists in surgical revisions of endolymphatic sac enhancement. In addition to avoiding destructive procedures, revision of endolymphatic sac enhancement has restored health to many patients. The procedures and findings from such revisions, which are briefly described in this article, have provided the best opportunity for the author to understand and observe the pathogenesis of Meniere's disease.

Electrocochleographic monitoring in endolymphatic sac surgery for Menière's disease.

OBJECTIVE: To determine the efficacy of intraoperative electrocochleographic monitoring during endolymphatic sac-ballooning surgery for Menière's disease. DESIGN: At each of five surgical steps, the eighth nerve action potential (AP) amplitude and latency, summating potential (SP) amplitude, and SP/AP amplitude ratio were recorded. Data were analyzed using repeated measure analysis of variance. PATIENTS: Electrocochleographs were successfully recorded in 97 patients during endolymphatic sac-ballooning surgery. Forty patients had an abnormal baseline ratio (group 1), and 57 patients had a nor

Endolymphatic sac: possible role in autoimmune sensorineural hearing loss.

Histologic examination of the temporal bone of a patient with Wegener's granulomatosis showed involvement of the vessels of the endolymphatic sac (ELS) but no involvement of the vasculature of the rest of the inner ear. Autoimmune sensorineural hearing loss in this patient may have been due to interference of antigens and antibodies with the filtering capacity of the ELS, leading to imbalance of inner ear fluids. Histologic findings reemphasize that the cochlea and ELS have different blood supplies.

Endolymphatic sac drainage for intractable vertigo (method and experiences).

This paper describes a method of endolymphatic sac decompression and drainage. The method emphasizes widely exposing dura, avoiding skeletonization of the posterior semicircular canal and draining the sac via a T tube. The results of this procedure in 75 patients, including statistics in 46 ears are discussed. In patients with unilateral disease good results were obtained in control of vertigo in 94 percent of patients and significant improvement in cochlear function in over 30 percent.

Relationship between changes in hearing function and volumes of endolymphatic hydrops after endolymphatic sac drainage.

Background: Endolymphatic sac drainage (ELSD) may have a positive effect on endolymphatic hydrops (EH) and may help to preserve inner ear function. However, the relationship between changes in EH volumes and hearing function after ELSD has not been described. Objectives: We aimed to reveal the factors related to changes in hearing and EH following ELSD. Material and Methods: Twenty-one patients who received ELSD were enrolled. Pure tone audiometry and 3-T magnetic resonance imaging (MRI) 4 h after intravenous injection of gadolinium enhancement were performed just before surgery and 2 years la

Case report: endolymphatic sac tumor masquerading as Meniere's disease.

INTRODUCTION: Endolymphatic sac tumors are rare, indolent tumors with an initially innocuous presentation of audiovestibular dysfunction mimicking benign, less sinister aetiologies. The objective of this study was to describe the clinical and pathologic features of endolymphatic sac tumors, highlighted by one such case presenting to a tertiary Neuro-otology and Skull Base center. PRESENTATION OF CASE: We report a case of a destructive endolymphatic sac tumor of the middle ear resulting in cranial neuropathies of the facial, vestibulocochlear, and vagus nerves. The acute clinical decline along

Evaluation of Tumor-Infiltrating Leukocytes in Endolymphatic Sac Tumor.

OBJECTIVE: Endolymphatic sac tumors (ELSTs), as rare low-grade neoplasms, are primarily treated with surgery. This study analyzes the characteristics of tumor-infiltrating leukocytes (TILs) in ELSTs and their relationships with clinical features to explore the potential for immunotherapy in ELSTs. METHODS: Clinical data and tumor specimens of 10 ELSTs patients who underwent surgery were retrieved. Immune expression levels of CD3, CD4, CD8, and CD66b were assessed by immunohistochemical staining and expressed as the integral optical density (IOD). Hematoxylin and eosin (HE) staining were perfo

Endolymphatic sac tumor in an 8-month-old cat.

BACKGROUND: The endolymphatic sac is an organ devoid of sensory receptors. It is connected with the endolymphatic compartment and contains endolymph. Endolymphatic sac tumor (ELST) is a rare neoplasm involving the middle and inner ear described in humans and dogs that does not show cellular characteristics of malignancy, but can be locally invasive and involve destruction of the temporal bone and adjacent structures. CASE DESCRIPTION: An 8-month-old female cat was referred because of sudden onset of vestibular signs starting 3 days prior to referral. On clinical examination, the patient showe

Low-tone air-bone gaps after endolymphatic sac surgery.

OBJECTIVES: We detected chronic low-tone air-bone gaps (LTABGs) in some patients with Meniere's disease after endolymphatic sac surgery. The aim of the present study was to elucidate the mechanism of LTABGs after endolymphatic sac surgery. METHODS: We investigated 50 patients with Meniere's disease, who underwent surgery more than two years prior. LTABGs were defined as the three-tone-average=20dB formulated by (a+b+c)/3, where a, b, and c are ABGs at 0.25, 0.5, and 1kHz, respectively (ABG ±). The intra-operative finding was focused on identifying operculum (OPC ±). RESULTS: The ratio of pos

Endolymphatic sac tumor demonstrated by intralabyrinthine hemorrhage. Case report.

Endolymphatic sac tumors (ELSTs) are locally invasive neoplasms that arise in the posterior petrous bone and are associated with von Hippel-Lindau (VHL) disease. These tumors cause symptoms even when microscopic in size (below the threshold for detectability on imaging studies) and can lead to symptoms such as hearing loss, tinnitus, vertigo, and facial nerve dysfunction. While the mechanisms of audiovestibular dysfunction in patients harboring ELSTs are incompletely understood, they have critical implications for management. The authors present the case of a 33-year-old man with VHL disease a

Endolymphatic sac tumour: a rare cause of recurrent vertigo.

Endolymphatic sac tumour occurring in a 32-year-old man presenting with Meniere's like symptoms of recurrent vertigo, hearing loss and tinnitus is described. Magnetic resonance imaging and computed tomography showed a vascular bone tumour centred over the retrolabyrinthine aspect of the temporal bone where the endolymphatic sac was located. Surgical excision via a translabyrinthine approach was performed. Endolymphatic sac tumours are rare papillary adenocarcinomas that arise from the endolympatic sac. It can be mistaken both on radiology and histology for other tumours such as paragangliomas,
