Intraoperative mitomycin C in endolymphatic sac surgery for Ménière's disease: A pilot study.

The long-term results of endolymphatic sac (ES) shunt surgery in Ménière's disease have been reported to be unsatisfactory compared to the short-term results, probably because of fibrosis and/or reclosure of the incised ES. To solve this problem, we tried to apply mitomycin C (MMC) intraoperatively to the incised ES as used in trabeculectomy for glaucoma. MMC has an antiproliferative effect as well as an antineoplastic effect. Thus, it is expected that MMC would have a benefit to prevent rapid fibrosis and/or reclosure of the ES. MMC did not show any ototoxicity in our animal study and we star

Retrolabyrinthine vestibular neurectomy with simultaneous monitoring of eighth nerve action potentials and electrocochleography.

Since November 1978, we have used retrolabyrinthine vestibular neurectomy in 29 of 42 cases as the primary procedure to relieve vertigo in Meniere's disease. The results indicate that 67% of patients had no vertigo postoperatively, while 21% were much improved. Hearing was maintained within 20 dB of the preoperative level in 78%. Simultaneous intraoperative electrocochleography and eighth nerve action potentials were used to monitor auditory function in the last fourteen cases using an Amplaid Mark V evoked potential signal processor. It appears that the intraoperative electrocochleography aft

Surgical opening of the endolymphatic sac in Ménière's disease--our experience from 1962-80.

Ten per cent of patients referred to the authors' clinic for vertigo prove to have Ménière's disease, and 93% of these are managed conservatively, leaving less than 1% of the entire population of vertigo patients to undergo an operation for Ménière's disease. Incision of the endolymphatic sac without any shunt is the operation performed by the authors. Results are given for a total of 157 patients and also in a smaller group of 101, all having a follow-up of greater than three years. Vertigo was eliminated or diminished in 78% during the first 12 months after surgery in the former group, and i

Analysis of surgical procedures in patients with vertigo.

The majority of patients with vertigo secondary to inner ear problems can be cured by surgery (80%). The results after neurectomy procedures such as cochleovestibular neurectomy or vestibular neurectomy appear to be much better (85%) than after the endolymphatic subarachnoid shunt procedure (70%). If hearing is worth saving, a conservative procedure should be used first in an effort to preserve the patient's hearing. If this fails, a cochleovestibular neurectomy will usually resolve the problem. At present we are using the retrolabyrinthine vestibular neurectomy as a primary procedure for Meni

Volume-rendered computed tomography images of the surgical field for endolymphatic sac surgery.

PURPOSE: Identification of the endolymphatic sac has failed occasionally. Postoperative complications have also rarely been reported. Given a safer and more reliable surgery, preoperative anatomical assessments are valuable, however, the vestibular aqueduct has seldom been seen with multi-planar reconstruction (MPR) computed tomography (CT) images yet. Our study aimed to determine the significance and utility of volume-rendered (VR) CT images of the surgical field for identifying the vestibular aqueduct, compared with MPR CT images. SUBJECTS AND METHODS: 14 patients with Meniere's disease who

Menière's disease--1983-1989.

A MEDLINE review of Menière's disease for the years 1983 through June, 1989 identified over 300 articles published in the English language. These articles have been reviewed, and those which represent significantly new or different findings from a previously published review article are summarized. Specific studies have been selected to highlight new areas of interest as well as new areas of controversy. Major trends in the period from 1983 through 1989 include a new AAO-HNS criteria for reporting results in Menière's disease, expanding interest in the relationship between immune disorders and

Summating potential in Meniere's disease.

This study assesses several electrocochleographic (ECoG) duration and amplitude measures in a clinically defined Meniere's group and compares the results with those from a normal hearing control group and a hearing loss group (cochlear). The summating potential (SP)/action potential (AP) amplitude ratio was the most efficient diagnostic measure, with 62% of the Meniere's group demonstrating abnormal ratios compared to 4% of the normal control group and 17% of the cochlear group. The SP changes were then studied from three angles: 1. Comparison of SP changes with glycerol test results; 2. Compa

Tomography of the vestibular aqueduct in ear disease.

A controversy exists concerning whether or not roentgenographic narrowing or nonvisualization of the vestibular aqueduct is a specific sign for Meniere's disease. Of 190 ears that were evaluated, abnormal aqueducts were seen in 42.9% of ears with Meniere's disease, 45.4% of contralateral, noninvolved ears from patients with Meniere's disease, 41.3% of ears with diseases other than Meniere's disease, 30.4% of ears with no disease, and 51.6% of normal ears. Narrowing or nonvisualization of the vestibular aqueduct is a nonspecific roentgenographic sign that is seen in diseased, as well as in norm

The long-term outcome of Meniere's disease patients following sac and shunt operation assessed by Hydrops MRI.

PURPOSE: The aim of this study was to evaluate the long-term outcome of Meniere's disease (MD) patients who had previously undergone sac and shunt operation using Hydrops MRI for assessment. METHODS: A total of 17 unilateral MD patients who had previously undergone sac and shunt operation underwent an inner ear test battery, followed by Hydrops MRI. The 34 ears were classified into two groups. Group A consisted of 17 operated ears, which were further subdivided into Group A1 (with endolymphatic hydrops, EH), and Group A2 (without EH). In contrast, Group B comprised 17 non-operated (opposite)

Intraoperative electrocochleography of endolymphatic hydrops surgery using clicks and tone bursts.

Seventy-seven ears of patients with Menière's disease (n = 69) and other ears with secondary endolymphatic hydrops (n = 8) underwent valved shunt surgery, and were monitored with intraoperative electrocochleography (ECoG) using clicks stimuli (n = 77), and both clicks and tone bursts (n = 37). Of the 41 ears in which the "baseline" measurement was > 35% summating potential to action potential (SP/AP) amplitude click ratio, 23 ears (56%) showed a significant ratio decrease (improvement). Twenty-six ears out of the 37 that were monitored by both clicks and tone bursts had abnormal absolute SP to

Endolymphatic sac surgery for Meniére's disease. A cumulative study of twelve years' experience.

This paper analyzes a total of 861 endolymphatic-mastoid shunt procedures (ELS) during 12 years. The review of long-term results following surgery for patients with intractable Meniére's disease indicates that ELS often provides temporary remission of severe disabling symptoms. Overall results, according to AAOO criteria, showed long-term vertigo control in 71.90% of cases. Using AAO-HNS guidelines, vertigo was completely or substantially controlled in 85.89% of the cases. In addition, hearing gains were frequently sustained and hearing deterioration often prevented over the years. A positive

Rear Admiral (Astronaut) Alan Shepard: Ménière's disease and the race to the moon.

On May 5, 1961, Alan B. Shepard Jr. piloted the Freedom 7 craft into a suborbital flight to become the first American man in space. His promising astronautical career was soon scuttled by spells of dizziness and tinnitus later diagnosed as Ménière's disease, until William F. House-considered the father of neurotology and a pioneer in surgery for vestibular schwannomas-intervened. In 1968 House implanted an endolymphatic-subarachnoid shunt, which at the time was a virtually experimental procedure. Shepard's debilitating Ménière's disease was cured, but not quite in time for him to pilot the doo

Endolymphatic sac surgery for Meniere's disease: a composite study of 339 cases.

This paper analyzes 339 consecutively performed endolymphatic mastoid sac shunts. Regardless of the method used, vertigo was controlled and hearing either improved or stabilized in approximately 70% of the cases. Overall, vertigo was controlled in approximately 80% of the cases. Similar results were obtained in cases of Meniere's syndrome and vestibular Meniere's disease. It is suggested here that, regardless of the method employed, there remains a high likelihood that the symptoms will recur, and the question is raised as to whether sac surgery may merely induce a temporary remission of the d

Endolymphatic sac surgery for hearing conservation in Meniere disease.

Thirty-five patients with Meniere disease underwent endolymphatic sac decompression and drainage with a Silastic sac-mastoid shunt. On the basis of patients' responses to a questionnaire and on analysis of preoperative and postoperative audiometric studies, we found the method to be an effective means for conservation or improvement of hearing and for relief of vertigo in patients with positive glycerol tests. Of the 35 patients, eight had negative glycerol tests and did not benefit from surgery (P less than .001). Of the 27 patients with positive glycerol tests, 70.4% (19) had no further vert

Bilateral Menière's disease in surgical versus nonsurgical patients.

In patients with Menière's disease, the possibility of developing Menière's disease in the uninvolved ear is of great concern. In this study, the incidence of bilateral Menière's disease (BMD) in medically treated patients was found to be 17 percent, while in surgically treated patients it was significantly lower, 5.9 percent (p less than 0.01). The incidence of BMD for each surgical procedure was as follows: 9 percent after endolymphatic subarachnoid shunt (n = 101), 7 percent after cochleovestibular neurectomy (n = 100), 6 percent after cochleosacculotomy (n = 18), and 0 percent after vestib

Perilymphatic ATP plays a critical role in modulating cochlear function to protect from loud sound induced hearing loss.

BACKGROUND: Extracellular adenosine triphosphate (ATP) signalling via purinergic receptors plays a key role in cochlear adaptation to loud sound. Traditionally, ATP activation of purinergic receptor P2X receptors has been proposed to induce a cation shunt, reducing the endolymphatic potential and the driving force for sound transduction. However, direct evidence for this protective mechanism remains limited. Here, we provide direct experimental evidence identifying a distinct, compartment-specific ATP signalling pathway that modulates cochlear function. METHODS: In mature Dunkin-Hartley guine
