Numerous experimantal data show that melatonin secreted by the pineal gland and enterochromaffin cells (apudocites) of the gastrointestinal tract plays an universal protective role in cases of various disorders of the pancreatic gland. In experiments, this hormone restricts the manifestations of acute pancreatitis and diabetes mellitus. It is suggested that, after reliable clinical justification, melatonin can probably be used for therapeutic aims.

71 stomachs removed because of ulcer were examined. It was confirmed that atrophic changes and intestinal metaplasia of the stomach mucous membrane are more pronounced in the stomach ulcer than in duodenal ulcer. There were no significant quantitative differences in the state of glandular apparatus in the anterior wall and small curvature of the stomach. The mean content of the border enterocytes in stomach ulcer was 78.8%, duodenal ulcer-86.9%: goblet cells-15.3% and 9.9%, Panet cells-2.7 and 1.14%, respectively, enterochromaffin cells-3.2 and 2.06%, respectively.

Administration of somatostatin induced changes of functional activity in all the central and peripheral structures under study. Neuronal activity in the paraventricular nucleus decreased by the 3rd and 15th min after somatostatin injection whereas by the 20th min the firing rate increased. Pancreata of guinea-pigs were investigated for the presence and cellular distribution of biogenic amines. Tissue concentrations of 5-HT have been measured in the duodenal mucosa of adults and children with coeliac disease and were found to be significantly higher than those from a control group. The molecular elucidation of two human skeletal dysplasias revealed that they are caused by an increase or a decrease in the synthesis of serotonin by enterochromaffin cells of the gut. This observation revealed a novel and powerful endocrine means to regulate bone mass. Exploiting these findings in the pharmacological arena led to the demonstration that inhibiting synthesis of gut-derived serotonin could be an effective means to treat low-bone-mass diseases such as osteoporosis.

Due to its potent and long-lasting antisecretory properties is omeprazole, the first clinically used H+/K+-adenosine triphosphatase inhibitor, highly effective in healing of duodenal and gastric ulcers and reflux oesophagitis. Omeprazole is superior to all other presently available antiulcer drugs in the treatment of Zollinger-Ellison syndrome and refractory ulcers. Short-term administration of the drug is safe. However, serum gastrin and gastric enterochromaffin-like cells should be carefully monitored during long-term treatment with the drug.
