Changing patterns of treatment and survival in neonates with meconium ileus.

This report describes 51 neonates with meconium ileus and emphasizes a changing pattern of treatment and improved survival. Twenty-four neonates had uncomplicated meconium ileus due to inspissated meconium obstructing the distal ileum. Twenty-seven neonates had 41 complications of meconium ileus including volvulus (18), bowel atresia (13), perforation (5), and giant cystic meconium peritonitis (5). Nine patients with uncomplicated cases responded to nonoperative clearing of meconium using a meglumine diatrizoate (Gastrografin) enema. Six of 7 patients with enema failures underwent laparotomy,

Primary anastomosis is the preferred surgical approach for proximal intestinal atresia: a retrospective 20-year analysis.

PURPOSE: We aimed to compare the outcomes of primary anastomosis (PA) and enterostomy as treatments for intestinal atresia in neonates to identify the factors influencing the choice of modality. METHODS: We conducted a retrospective single-centre analysis of all neonates with intestinal atresia between 2000 and 2020 and measured the clinical outcomes. We performed logistic regression to identify factors that influenced the choice of surgical approach. RESULTS: Of 62 intestinal atresia neonates, 71% received PA. There were no significant differences in gestation, gender, age at operation, bir

Choledochal cyst in the adult.

OBJECTIVE: The authors examined the natural history of choledochal cysts in adults treated surgically. BACKGROUND: An initial diagnosis of choledochal cyst is uncommon in adults. The recommended treatment is excision, rather than bypass, to achieve effective biliary drainage and because of the risk of cancer. METHODS: A retrospective study of 27 adult patients was completed to determine the frequency of anastomotic complications and the incidence of cancer. RESULTS: Fifteen patients were treated by cyst excision, and one developed an anastomotic stricture, treated by percutaneous dilation.

Biliary and other gastrointestinal interventions.

Over the past several years, renewed interest in biliary endoprostheses has developed among interventional radiologists. The experimental, technical, and clinical progress that produced this enthusiasm is reviewed, with emphasis on recently published reports. The application of percutaneous enterostomy for gastric decompression or feeding has become widely practiced, and recent reports of percutaneous and endoscopic gastrostomy are compared, with emphasis on controversial points. The technique of percutaneous enterostomy has been adapted for jejunal and colonic application, and these technical

How to manage low gut obstruction in neonates with immature ganglion cells in the colonic wall?

BACKGROUND: Neonates with immature ganglion cells in the colonic wall may have the clinical picture similar to Hirschsprung's disease, especially total colonic aganglionosis. Management of this entity depends on the judgment of each clinician. OBJECTIVE: The aim of this study was to review management of clinical low gut obstruction in neonates with presence of immature ganglion cells in the colon. MATERIAL AND METHOD: A retrospective study of neonates with clinical low gut obstruction due to presence of immature ganglion cells treated between 2007 and 2012 was reviewed. RESULTS: Six patient

[Preventive operations in intestinal abnormalities].

In intestinal anomalies, preventive surgical procedures include gastrostomies and enterostomies as well as some special operations such as ligature of the cardia and associated appendectomies. General indications for enterostomies are seen in life-threatening circumstances, i.e. prematurity, surfactant deficiency, major associated malformations and complications. Preventive surgical procedures are necessary in certain cases of esophageal atresia, complicated small bowel atresia, meconium ileus, anorectal malformations, Hirschsprung disease, neuronal intestinal dysplasia and necrotizing enteroc

The surgeon's role in the treatment of chronic intestinal pseudoobstruction.

OBJECTIVE: The surgeon's role in the treatment of patients with chronic intestinal pseudoobstruction (CIP) is under-appreciated. Our aim was to determine the effects of operative treatment on symptomatic relief of CIP. METHODS: Records of all 21 patients who underwent surgery for CIP from 1980 to 1990 were reviewed. CIP was diagnosed by a combination of manometric, radiological, and/or histological examinations. RESULTS: Six of the nine patients who underwent resection or bypass of presumably localized disease are currently maintained on oral intake; one patient with multiple sclerosis who c

[Nontraumatic perforations of the small intestine].

From 1970 to 1978, 14 patients with nontraumatic perforations of the small intestine were treated in the Department of General and Abdominal Surgery of the University Frankfurt am Main. In eight cases suture and bypass and/or interval operation with enterostomy was performed. Seven patients survived, one died. In the six other patients resection with primary anastomosis or suture were performed. One patient survived and five patients died following peritonitis due to insufficiency of the anastomosis. The problem of nontraumatic perforation of the small intestine is discussed and the literature

Primary anastomosis as a valid alternative for extremely low birth weight infants with spontaneous intestinal perforation.

The aim was to assess the results of primary anastomosis (PA) compared to enterostomy (ES) in infants with spontaneous intestinal perforation (SIP) and a weight below 1000 g. Between 2014 and 2016, enterostomy was routinely carried out on extremely low birth weight (ELBW) patients with SIP. From 2016 until 2019, all patients underwent anastomosis without stoma formation. We compared outcome and complications in both groups. Forty-two patients with a median gestational age of 24.3 weeks and a birth weight of 640 g with SIP were included. Thirty patients underwent PA; ES was performed in 12 pati

[Principles and parallels of prevention and repair of parastomal hernia with meshes].

After formation of a permanent terminal stoma by enterostomy, parastomal hernia (PSH) occurs in up to 80% of cases and leads to a wide variety of symptoms and complications with a high rate of emergency operations due to incarceration (ca. 15%). Consequently, greater consideration should be given to PSH prevention even as early as the time of enterostomy and generously applied indications for elective repair of manifest PSH. The aim of this article is to summarize and evaluate the current evidence for PSH repair and prevention. Poor postoperative results after attempted repair of manifest PSH

Are secondary bile acids in choledochal cysts important as a risk factor in biliary tract carcinoma?

Since secondary bile acids have mutagenic potency and choledochal cyst patients with or without cyst-enterostomy have a high risk of developing bile duct carcinoma, we examined the hypothesis that secondary bile acids are elevated in the choledochal cyst and possibly cause biliary tract carcinoma. Eleven choledochal cyst patients with or without previous cyst-enterostomy or biliary tract carcinoma, seven patients with biliary tract carcinoma not associated with choledochal cyst and five patients with cholecystolithiasis were examined. Samples were directly needle aspirated from the cyst or by

Hazard and outcome of retreated choledochal cyst patients.

Thirty-nine patients initially treated for choledochal cyst (25: cyst-enterostomy for 15 type I and 10 type IV A, 13: cyst excision for 8 type I and 5 type IV A, 1: sphincterotomy for type III) have been completely followed up for a mean period of 17 years to examine the effect of surgical management on their lives. Eighteen of 25 cases with cyst-enterostomy (72%) needed retreatment due to the complications but one could not be retreated because of the presence of advanced bile duct carcinoma. Fourteen of 17 cases were retreated with cyst excision, of whom 12 showed an excellent outcome but ot

Parastomal herniation of the gallbladder.

Parastomal hernias can occur in up to 31% of patients following an enterostomy (Cheung in Aust N Z J Surg 65:808-811, 1995). This type of hernia develops through an intentional fascial defect. Commonly, most parastomal hernias involve a reducible segment of omentum, small bowel, or colon. Typically, these hernias are asymptomatic and associated rarely with strangulation or obstruction. Patient preference and clinical scenario may dictate management of these hernias. Non-operative management of parastomal hernias includes abdominal binders and enterostomy belts. Operative management includes a

Historical Cohort Study of Congenital Isolated Hypoganglionosis of the Intestine: Determining the Best Surgical Interventions.

No standard diagnostic method or surgical treatment for congenital isolated hypoganglionosis (CIHG) has been established. This study aimed to analyze the clinical outcomes of patients with CIHG and identify the best surgical interventions provided thus far. Data on surgical interventions in 19 patients were collected between 1992 and 2020, including the type of enterostomy, type of revision, and length of the intestines. Ganglion cells in the myenteric plexus were enumerated using Hu C/D staining. The ratio of the length of the small intestine to its height was defined as the intestinal ratio

Pancreatico-Gastrostomy: A Modified Two-Layered Technique.

INTRODUCTION: Pancreato-duodenal resections are commonly done for periampullary carcinomas. The outcome of the procedure is decided by pancreato-enterostomy. Pancreato-jejunostomy (PJ) has been employed largely as pancreato-enterostomy. Recently, there has been a renewed interest in pancreato-gastrostomy (PG). The debate continues on the choice of reconstruction. METHODS: A hundred cases of periampullary carcinoma were subjected to modified pancreatico-gastrostomy. The pancreatico-gastrectomy was evaluated by drain fluid amylase done on days 1, 3, and 5 post-operatively and clinical findings.

Timing of ostomy reversal in neonates with necrotizing enterocolitis.

INTRODUCTION: Most neonates with necrotizing enterocolitis (NEC) requiring laparotomy have bowel resection and intestinal diversion. At present, there is no consensus regarding the best time for enterostomy reversal. Our aim is to determine if there is any difference in outcomes of infants whose enterostomy was reversed early versus late. MATERIALS AND METHODS: We retrospectively reviewed medical records of patients with NEC admitted to the neonatal intensive care unit at a large urban children's hospital from 2002 to 2010. The patients underwent operative intervention with creation and subse

Long-term follow-up after surgical management of necrotizing enterocolitis: sixty-three cases.

One hundred twenty-five infants underwent surgical intervention for necrotizing enterocolitis between 1972 and 1984. Sixty-three infants, who survived more than 30 days postoperatively, were evaluated for long-term complications. There were 28 girls and 35 boys (mean birth weight 1,725 +/- 890 g; gestational age 32 +/- 4 weeks). Associated problems included hyaline membrane disease (43), cardiac anomalies (25), and trisomy 21(2). Thirty-six survivors required long-term ventilatory support. Fifty-nine infants underwent bowel resection and enterostomy, 3 decompressing enterostomies without resec
