The evolution of Ebola virus: Insights from the 2013-2016 epidemic.

The 2013-2016 epidemic of Ebola virus disease in West Africa was of unprecedented magnitude and changed our perspective on this lethal but sporadically emerging virus. This outbreak also marked the beginning of large-scale real-time molecular epidemiology. Here, we show how evolutionary analyses of Ebola virus genome sequences provided key insights into virus origins, evolution and spread during the epidemic. We provide basic scientists, epidemiologists, medical practitioners and other outbreak responders with an enhanced understanding of the utility and limitations of pathogen genomic sequenc

Short report: transient antibody response in Taenia solium infection in field conditions-a major contributor to high seroprevalence.

The discordance between extremely high seroprevalence of Taenia solium antibodies in disease-endemic populations, relatively few symptomatic cases of neurocysticercosis, and high background levels of putatively inactive brain lesions (mainly calcifications) in seronegative controls have confused researchers, clinicians, and epidemiologists in the last decade. We reviewed longitudinal serologic data from general population serosurveys in 3 different disease-endemic areas of Peru and Colombia and found that approximately 40% of seropositive people were seronegative when resampled after 1 year (3

Baromania: A contrarian epidemic.

As the world grapples with a rapidly increasing prevalence of obesity, epidemiologists, health care professionals and policy makers are working together to increase awareness about its prevention and management. However, what is also increasingly visible in a subset of not very obese individuals, is an undue concern about their weight, a condition we call Baromania. Akin to orthorexia nervosa, anorexia and bulimia. we define baromania as "a state of extreme attention towards one's own weight, accompanied by elation and excitement regarding weight loss and maintenance". This paper describes the

Current Recommendations and Practice of Oxygen Therapy in Preterm Infants.

This narrative review identified 23 publications in 2011 to 2019 discussing randomized trials of oxygen saturation targets of 85% to 89% versus 91% to 95% in infants below 28 weeks' gestation. Of 18 commentaries or consensus statements, 17 recommended saturation targets above 89%. Five systematic reviews reported that the 85% to 89% target increased mortality but not the composite of death or disability. The evidence for increased mortality was assessed as of "high", "moderate," or "low," quality, reflecting substantial differences in interpreting the GRADE guidelines. Systematic reviews and g

West Nile virus (lineage 2) in mosquitoes in southern Moravia - awaiting the first autochthonous human cases.

Here we report repeated detection of lineage 2 West Nile virus (WNV-2) from Culex modestus and Cx. pipiens mosquitoes collected at fishponds in the Lednice-Valtice Area during the mosquito seasons 2015 and 2016. The WNV strains recovered share genetic homology with WNV strains isolated during an extensive monitoring in 2013 as well as with strains circulating in southern and central Europe at the same time. Repeated detection of WNV indicates its establishment in the area and also warns infection specialists and epidemiologists about possible emergence of human cases or even outbreaks of West

The delayed appearance of a mercurial warning.

The publication of a 1971 study from Japan on chronic methylmercury poisoning in adults provides an occasion to reexamine the history of Minamata disease and its social and political repercussions. Research findings were suppressed or held back due to controversies that were tearing the Japanese scientific community apart. Similar controversies occurred outside of Japan as well. Only now are the long-term adverse effects of environmental methylmercury exposure becoming clear. The delayed release of the 1971 results therefore reminds us as epidemiologists of our obligation, even in the presence

Physiology, pathophysiology, and anthropology/epidemiology of human earcanal secretions.

Two types of glands are found in the outer third of the human earcanal: sebaceous glands that produce sebum and modified apocrine glands that produce apocrine sweat. Together, these substances make up cerumen, which serves to clean, lubricate, and, to some extent, protect the earcanal from bacteria and fungus. Excessive/impacted cerumen can cause tinnitus, vertigo, itching, pain, external otitis, and hearing loss. Two populations are known to have a high incidence of excessive/impacted cerumen: individuals with mental retardation and the elderly. Anthropologists have used cerumen type to tract

Plague mortality and demographic depression in later medieval England.

Both direct and indirect evidence implies that England experienced a lengthy period of stagnant or declining population during the later fourteenth and fifteenth centuries. The Black Death of 1348-1349 had brought about profound changes in England's agrarian economy, and this subsequent demographic depression is most commonly interpreted by historians as the result of plague mortality, recurring in severe outbreaks after the disease's introduction into the country. This paper reviews the evidence and assumptions behind this interpretation, in light of recent research by historical demographers

Prematurity.

Perinatal epidemiologists no longer speak much of "prematurity". Instead the World Health Organization recommends that infants born before 37 completed weeks of gestation should be designated "preterm" and those of birthweight less than 2500 g "low birthweight". Both definitions have obvious drawbacks--the former because menstrual dates are often unknown or unreliable, the latter because it does not separate preterm from small-for-gestational-age infants. Whatever the classification of these small infants there is no dispute about their high mortality and morbidity; in the United Kingdom they

A clinical and epidemiological evaluation of tonometry as a screening test.

Because of their differences in emphasis, clinical practitioners and clinical epidemiologists often employ different terms and concepts in comparing and rating health screening tests. The apparent inconsistency of a high error rate (96 to 99% over-referrals) yet good test sensitivity and specificity (40% and 95% respectively) for tonometry can be explained by a complete analysis of tonometric screening, encompassing both the clinical and epidemiological view-points. This paper reviews basic terminology, explains the clinical and epidemiological approaches, and demonstrates both of these method

John L. Cameron, can be linked to 16 prospective clinical trials over his career thus far, starting with alternate-allocation trials and transitioning to prospective, randomized, placebo-controlled trials. These trials studied various topics in surgery-from pancreatitis to surgical site infections, to drain trials, a trial in Crohn disease and multiple trials in pancreatic surgery and cancer. Bayesian prediction of an epidemic curve.

An epidemic curve is a graph in which the number of new cases of an outbreak disease is plotted against time. Epidemic curves are ordinarily constructed after the disease outbreak is over. However, a good estimate of the epidemic curve early in an outbreak would be invaluable to health care officials. Currently, techniques for predicting the severity of an outbreak are very limited. As far as predicting the number of future cases, ordinarily epidemiologists simply make an educated guess as to how many people might become affected. We develop a model for estimating an epidemic curve early in an

Smallpox models as policy tools.

Mathematical models can help prepare for and respond to bioterrorism attacks, provided that their strengths and weaknesses are clearly understood. A series of initiatives within the Department of Health and Human Services brought modelers together with biologists and epidemiologists who specialize in smallpox and experts in bioterrorism response and health policy and has led to the parallel development of models with different technical approaches but standardized scenarios, parameter ranges, and outcome measures. Cross-disciplinary interactions throughout the process supported the development

[Active epidemiologic surveillance in the poliomyelitis eradication program in Serbia].

The main strategy of the worldwide Program of Poliomyelitis Eradication is based on immunization with oral poliovirus vaccine and active epidemiological surveillance aimed to demonstrate the absence of wild poliovirus circulation. The specification of the surveillance in the program, reporting and investigation of certain syndrome--the acute flaccid paralysis--as a specific feature of surveillance of poliomyelitis, is a new experience both for clinicians and epidemiologists. Along with the achieved results, problems in conducting the active epidemiological surveillance in Serbia, applied measu

CDC Guidance for Use of a Third Dose of MMR Vaccine During Mumps Outbreaks.

CONTEXT: In response to numerous mumps outbreaks reported throughout the United States in 2016 and 2017, the Advisory Committee on Immunization Practices (ACIP) recommended a third dose of measles, mumps, and rubella (MMR) vaccine for groups of persons determined by public health authorities to be at increased risk for acquiring mumps because of an outbreak. OBJECTIVE: To provide guidance for health departments when implementing the ACIP recommendation. DESIGN: Draft guidance was developed by Centers for Disease Control and Prevention subject matter experts based on technical consultations w

Communicable Diseases Prioritized According to Their Public Health Relevance, Sweden, 2013.

To establish strategic priorities for the Public Health Agency of Sweden we prioritized pathogens according to their public health relevance in Sweden in order to guide resource allocation. We then compared the outcome to ongoing surveillance. We used a modified prioritization method developed at the Robert Koch Institute in Germany. In a Delphi process experts scored pathogens according to ten variables. We ranked the pathogens according to the total score and divided them into four priority groups. We then compared the priority groups to self-reported time spent on surveillance by epidemiolo

Recent advances in the study of Kaposi's sarcoma-associated herpesvirus replication and pathogenesis.

It has now been over twenty years since a novel herpesviral genome was identified in Kaposi's sarcoma biopsies. Since then, the cumulative research effort by molecular biologists, virologists, clinicians, and epidemiologists alike has led to the extensive characterization of this tumor virus, Kaposi's sarcoma-associated herpesvirus (KSHV; also known as human herpesvirus 8 (HHV-8)), and its associated diseases. Here we review the current knowledge of KSHV biology and pathogenesis, with a particular emphasis on new and exciting advances in the field of epigenetics. We also discuss the developmen

Methods of outbreak investigation in the "era of bacteriology" 1880-1920.

The advent of bacteriological methods in the later 19th century has been seen, on the examples of America and Germany, to have been followed by a new laboratory-based, contact-tracing method of investigating outbreaks of epidemic disease. In Britain, however, this new approach never took firm root, and practising epidemiologists continued to follow an observational and deductive tradition in field investigations, rejecting any primary dependence on bacteriological methods. Alongside this persistent observational practice, there emerged a new statistical approach, based in Pearsonian biometrics

Clonal distribution and associated characteristics of Escherichia coli clinical and surveillance isolates from a military medical center.

Antimicrobial-resistant Escherichia coli are a concern for military health services. We studied 100 extended-spectrum beta-lactamase (ESBL)-producing and non-producing E. coli clinical and surveillance isolates from military personnel and civilians at Brooke Army Medical Center (2007-2011). Major E. coli lineages, most prominently ST10 (24%), ST131 (16%), and ST648 (8%), were distributed much as reported for other North American populations. ST131, represented mainly by its resistance-associated ST131-H30 clonal subset, was uniquely associated with a clinical origin, regardless of ESBL status.

Investigation of outbreaks: epidemiology.

Outbreaks of Legionnaires' disease create high levels of public anxiety and media interest and inevitably consume a great deal of public health resources. Investigations should begin as early as possible in order to rapidly identify suspected sources of infection, control the outbreak and prevent further cases occurring. The investigations should be coordinated by an outbreak control team who work collaboratively within local/national/international public health guidelines and with clear terms of reference. The actions carried out by epidemiologists when investigating community-, hospital-, or
