Diabetic foot complicated by vertebral osteomyelitis and epidural abscess.

UNLABELLED: Vertebral osteomyelitis (or spondylodiscitis) is steadily increasing in Western countries and often results from hematogenous seeding, direct inoculation during spinal surgery, or contiguous spread from an infection in the adjacent soft tissue. We present the case of a 67-year-old white patient with type 2 diabetes who went to Hospital for high fever, back pain, and worsening of known infected ulcers in the left foot. Despite intravenous antibiotic treatment and surgical debridement of the foot infection, high fever and lower back pain continued. Bone biopsy and two consecutive blo

Spontaneous methicillin-resistant Staphylococcus aureus epidural abscess in pregnancy.

BACKGROUND: Epidural abscess is a rare complication of regional anesthesia, and spontaneous formation is even more uncommon. Diabetes mellitus, concomitant infection, intravenous drug use, and immune suppression are risk factors for spontaneous epidural abscess. CASE: A 29-year-old white woman presented at 28 weeks of estimated gestational age reporting an intermittent headache. She had Horner syndrome and was hospitalized. A cervicothoracic epidural abscess was diagnosed. Surgical decompression and parenteral antibiotics resulted in complete resolution of neurologic symptoms. Cultures were p

Tubercular epidural abscess in children: report of two cases.

Tubercular spinal epidural abscess (SEA) is an uncommon infectious occurrence. SEA was first described in 1761. Clinical prospects of tubercular SEA are graver, if not promptly diagnosed and treated appropriately. We present two cases of spinal epidural abscess of tubercular etiology who progressed to paraplegia over the course of disease. MRI pointed towards an epidural abscess in the dorsal spine. Histopathological analysis revealed SEA, tubercular in nature in both cases. Patients responded to surgical decompression and anti-tubercular therapy (ATT). Such type of clinical entities has least

Spinal surgery: an unusual case of spinal epidural abscess.

Southeast Missouri Hospital in Cape Girardeau, Missouri, admitted a patient from an outside hospital with back pain and progressive quadriparesis. Imaging revealed an extensive spinal epidural abscess that extended from the craniocervical junction to the sacral spine. Because of the patient's critical condition and the need to minimize the duration of surgery and anesthesia, two surgical teams simultaneously operated on the patient on the day of admission. One team performed a decompressive laminectomy of C1 through C7 and T1 through T6, while the other team performed, through a separate skin

Spinal epidural abscess: a rare complication of olecranon bursitis.

Spinal epidural abscess is a rare but potentially fatal condition if left untreated. We report the case of a 67-year old man who presented to the Accident and Emergency department complaining of acute onset of inter-scapular back pain, left leg weakness and loss of sensation in the left foot. On examination he was found to be pyrexial with long tract signs in the left lower leg. In addition he had a left sided olecranon bursitis of three weeks duration. Blood tests revealed raised inflammatory markers and a staphylococcal bacteremia. Magnetic resonance imaging (MRI) confirmed the diagnosis of

Pott's puffy tumor and epidural abscess arising from pansinusitis.

BACKGROUND: Sinusitis is a common disorder that can result in rare but serious complications including periorbital or orbital cellulitis, intracranial abscess or meningitis, subperiosteal scalp abscess ("Pott's puffy tumor"), osteomyelitis, and cavernous sinus thrombosis. CASE REPORT: We report a case of a 41-year-old man who presented to our Emergency Department with pansinusitis. He did not obtain recommended follow-up treatment after discharge and 26 days later returned with a persistence of sinusitis, Pott's puffy tumor, and an intracranial abscess caused by Streptococcus intermedius. Posterior spinal epidural abscess: an unusual complication of vertebroplasty.

OBJECTIVE: Complications after vertebroplasty are rare. There are few reported infectious complications requiring surgical management such as corpectomy with anterior reconstruction and posterior stabilization, although we have not seen any reports about epidural abscess in the literature. We present a patient in whom posterior epidural abscess developed after vertebroplasty in which drainage and antibiotherapy were required for treatment. METHODS: A 70-year-old female with a painful T12 osteoporotic compression fracture underwent percutaneous vertebroplasty using polymethylmethacrylate witho

Spinal epidural abscess caused by brucellosis. Two case reports.

Brucellosis is still an important public health problem in the Mediterranean countries, including Turkey, and is most probably underdiagnosed or underreported. Two rare cases of extradural brucellar granuloma causing spinal cord compression in the thoracic and cervical regions were identified by magnetic resonance imaging. The abscesses were totally excised surgically. Medical treatment was given immediately after diagnosis, but delayed neurosurgical intervention resulted in partial neurological recovery in one patient although the other showed complete neurological recovery. Abscess formation

Chronic osteomyelitis with epidural abscess: CT and MR findings.

A case of chronic osteomyelitis in the occipital bone associated with underlying chronic epidural abscess was diagnosed by skull radiography, CT, and MR. Skull radiography showed diffuse sclerosis of the occipital bone and dural calcifications. Computed tomography demonstrated thickened sclerotic changes of the osteomyelitic area, dural calcifications, and epidural fluid collection. The area of chronic osteomyelitis displayed, essentially, no signal intensity in the T1-weighted and T2-weighted MR images. The chronic epidural abscess had heterogeneous signal intensity on T1-weighted images and

[Lumbar epidural hematoma and spinal abscess following peridural anesthesia].

Epidural application of local anesthetics or opiates is commonly used for treatment of severe pain or arterial obstruction. We discuss two cases of complications following peridural anesthesia. As an acute complication, a lumbar epidural hematoma developed hours after placement of the catheter and caused sciatic pain and nearly complete paraplegia. The hemorrhage might have been promoted by heparinization. After decompressive hemilaminectomy the patient recovered but was disabled by a persistent paresis. The second patient suffered from renal failure, arterial obstruction, and phantom limb pai

Hidden Threat: Unveiling the Dangers of Cervical Epidural Abscess.

Cervical epidural abscess (CEA) is an unusual but severe condition. In most cases, it can be related to infections such as the methicillin-sensitive Staphylococcus aureus at the sites of intravenous cannulae. The current case report describes a patient who developed a right epidural abscess at C₅-C₇, diagnosed following persistent chest pain and fever. He eventually recovered after surgical decompression with intravenous antibiotics. The diagnosis of spinal epidural abscess (SEA) is very challenging because of the nonspecific character of symptoms. Early diagnosis using leukocytosis, elevated

Streptococcus pneumoniae Spinal Epidural Abscess in an Immunocompetent Patient.

Epidural abscesses are a rare diagnosis in immunocompetent patients. The most common risk factors linked with spinal epidural abscesses are intravenous drug use, diabetes mellitus, hepatitis, and iatrogenic factors like prior surgery and catheter usage. The thoracic and lumbar spine are the most common sites of these abscesses. Clinical presentation can include back pain, fever, and neurologic deterioration, with back pain occurring in almost two-thirds of patients. Staphylococcus aureus is the most common causative pathogen. We present a 50 male with no significant past medical or family hist

Cervical spinal cord stimulator trial complicated by epidural abscess.

BACK GROUND: Spinal cord stimulation (SCS) is a growing interventional treatment modality in patients experiencing intractable pain refractory to conservative treatments. Many patients with chronic low back and leg pain that persists after surgery have found pain relief, and more evidence is suggesting that chronic upper limb and neck pain may respond just as well to this therapy. However, the placement of foreign body, for instance SCS leads, in the epidural space can become the source for deep intra-spinal infection. CASE REPORT: We present a 49-year-old robust male who underwent a temporar

Spinal epidural abscess in children: a case-based review.

BACKGROUND: Pediatric spinal epidural abscess is a major suppurative infection of the central nervous system. It is an extremely rare pathology carrying serious risk of permanent neurological sequelae if is not properly treated. METHODS AND RESULTS: All the pertinent literature was analyzed, focused on pediatric cases of spinal epidural abscess and its peculiar features. Two illustrative cases are also presented. The first case is that of a 9-year old girl who took medical attention, when she was already paraplegic. Despite prompt surgical evacuation and good neuroradiological outcome and int

Management of cervical spine epidural abscess: a systematic review.

BackgroundCervical spinal epidural abscess (CSEA) is a localized infection between the thecal sac and cervical spinal column which may result in neurological deficit and death if inadequately treated. Two treatment options exist: medical management and surgical intervention. Our objective was to analyze CSEA patient outcomes in order to determine the optimal method of treatment.MethodsAn electronic literature search for relevant case series and retrospective reviews was conducted through June 2016. Data abstraction and study quality assessment were performed by two independent reviewers. A lac
