[30% intralipid shortens the duration of anesthesia induced by etomidate in rats].

OBJECTIVE: To examine whether 30% intralipid could shorten the duration of anesthesia induced by etomidate and its underlying mechanism. METHODS: Male Sprague-Dawley rats were anesthetized by etomidate (2.85 mg/kg) through caudal vein. After loss of righting reflex, 40 rats were randomly divided into four groups, each being administered intravenously with 2 mL 30% intralipid, 2 mL normal saline, 0.5 mL 30% intralipid, and 0.5 mL normal saline, respectively. The durations from loss of righting reflex to revival of righting reflex and to normal activities were recorded. In the study in vitro, t

Medical suppression of hypercortisolemia in Cushing's syndrome with particular consideration of etomidate.

Cushing's syndrome is associated with excessive cortisol secretion by the adrenal gland or ectopic tumours and may result in diabetes, hypertension, and life-threatening infections with high mortality rates especially in the case of surgical resection. Although surgical resection is the treatment of choice, patients may benefit from preceding medical therapy. This may especially be useful as an adjunctive approach in emergency settings, if patients cannot undergo surgery, if surgery or radiotherapy fails, or if the tumour recurs. Medical therapy can be categorized in three different groups-inh

Prevention of etomidate-related myoclonus in anesthetic induction by pretreatment with magnesium.

BACKGROUND: Etomidate frequently leads to myoclonic jerks during anesthetic induction. This study aimed to detect if pretreatment with magnesium decreases myoclonus incidence. METHODS: A hundred ASA (the American Society of Anesthesiologists) I-II patients were included and randomized into 2 groups. Three minutes before etomidate induction of anesthesia (by 0.3 mg/kg intravenous etomidate), Group M received 2.48 mmol (60 mg) intravenous magnesium sulphate and Group P received equal volume of intravenous saline. Myoclonus was evaluated as "present/absent". RESULTS: The rate of myoclonus was s

Population pharmacokinetics and pharmacodynamics of brief etomidate infusion in healthy volunteers.

This study established the pharmacokinetic and pharmacodynamic relationships of the bispectral index (BIS) and Observer's Assessment of Alertness/Sedation (OAA/S) scale with effect site drug concentrations during and after brief etomidate infusion. Eighteen American Society of Anesthesiologists status I or II volunteers received etomidate (0.2%) infusion at 5 mg/min until the loss of eyelash reflexes, and spontaneous recovery was allowed. Data for plasma etomidate concentrations, BIS, and OAA/S were collected every minute and analyzed by NONMEM. A 2-compartment pharmacokinetic model and a phar

Etomidate and propofol inhibit the neurotransmitter release machinery at different sites.

The mechanism of general anaesthetic action is only partially understood. Facilitation of inhibitory GABAA receptors plays an important role in the action of most anaesthetics, but is thought to be especially relevant in the case of intravenous anaesthetics, like etomidate and propofol. Recent evidence suggests that anaesthetics also inhibit excitatory synaptic transmission via a presynaptic mechanism(s), but it has been difficult to determine whether these agents act on the neurotransmitter release machinery itself. In the present study we sought to determine whether the intravenous anaesthet

Effect of thiopental, propofol, and etomidate on vincristine toxicity in PC12 cells.

Neurotoxicity is the dose-limiting side-effect of vincristine in cancer therapy. Using the nerve growth factor (NGF)-dependent neurite outgrowth and cell proliferation of the PC12 pheochromocytoma cell line as an in vitro assay, the protective effect of different intravenous anesthetics was assessed. Vincristine (1 nmol/L) significantly decreased the percentage of neurite-forming cells from 68% +/- 9% to 27% +/- 7% within a 3-day incubation period. The longer neurites (> 2 x cell body) in particular proved to be extremely sensitive to vincristine (from 17% +/- 4% to 0% of total neurite-express

The effect of etomidate on rabbit liver microsomal drug metabolism in vitro.

Etomidate, an imidazole-containing anesthetic agent, is shown to be a reversible inhibitor of rabbit liver microsomal enzymes in vitro. Inhibition of aniline hydroxylation by etomidate follows competitive kinetics, while inhibition of microsomal N-demethylase and O-demethylase activities is of the mixed type. The concentrations of etomidate required to cause 50% inhibition of these enzyme activities are in the 7-10 microM range. NADPH-cytochrome c reductase is not inhibited by concentrations of etomidate below 100 microM. Spectrophotometric studies show that the addition of etomidate to liver

Paradoxical Agitation Following Etomidate Sedation for Electrical Cardioversion: A Case Report.

Etomidate is commonly used for procedural sedation in the emergency department due to its rapid onset, short duration of action, and favorable hemodynamic profile. Adverse effects are generally limited to pain at the injection site, myoclonus, nausea, and transient adrenal suppression. Paradoxical agitation following etomidate administration is rarely described. We report a case of severe, transient agitation following low-dose etomidate used for procedural sedation to facilitate electrical cardioversion in a healthy young adult presenting with new-onset atrial fibrillation.

Plasma concentrations of etomidate during an intravenous infusion over 48 hours.

Two groups of six patients who had undergone major maxillofacial surgery and who required intermittent positive pressure ventilation, analgesia and sedation for about 48 hours postoperatively were studied. Analgesia in the postoperative period was maintained by an infusion of fentanyl 0.034 micrograms/kg/minute. Sedation was maintained with an intravenous infusion of etomidate such that the patients slept but opened their eyes when addressed and obeyed commands. An assay method was developed which enabled the measurement of plasma concentration of etomidate for periods of up to 24 hours after
