The problematic storage of etretinate in fat during chronic etretinate therapy prompted us to search for other "high-affinity" tissues in 3 patients studied at autopsy. Specimens from eleven organs were analysed for etretinate and its main metabolite, etretin, by high-performance liquid chromatography. The results confirmed an accumulation of etretinate in fat and to a lesser degree in liver. High levels of etretinate were also found in the adrenals and, in one case, this value exceeded that of the fat tissue. This study reports comparative results of the effects of an aromatic retinoid, etretinate (RO 10-9359), on various disorders of the skin and mucous membranes. One hundred thirteen patients suffering from psoriasis, lichen planus, keratosis follicularis, and various other disorders were treated and examined. Forty-five patients with cutaneous T-cell lymphomas (CTCL), 32 with mycosis fungoides (MF) and 13 with Sézary syndrome (SS), were treated with interferon-alpha 2a (IFN-alpha 2a) (6-9 x 10(6) IU daily) for 3 months. A six-year experience in 20 patients with advanced mycosis fungoides treated with combination chemotherapy with bleomycin, cyclophosphamide, prednisone and etretinate (BCPE) in advanced mycosis fungoides showed initial complete remissions in 16 patients (85%). Serum vitamin A and E concentrations were determined in six patients with vulvar epithelial carcinoma and in 25 patients with lichen sclerosus and atrophicus (LSA) of the vulva. No difference could be found in these levels between the two groups. LSA patients received oral etretinate for two months at least; improvement was observed in 93% of the patients. Twenty patients with oral lichen planus (OLP) were divided into two groups. The first group (8 reticulated OLP, 2 hyperkeratotic OLP) was treated with topical tretinoin. The second group (4 reticulated OLP, 6 eroding OLP) was treated with oral etretinate. All patients were regularly followed in order to observe over a period of five years the evolution of the lesions under treatment, the side-effects and recurrences when the treatment was discontinued.

A family with a clinical variant of Papillon-Lefèvre syndrome (PLS), associated with recurrent pyogenic infections, has been followed up for more than 20 years. Etretinate 0.5 mg/kg body weight combined with 0.1% triamcinolone acetonide and 5% salicylic acid in an O/W cream gave more than 70% improvement in 62% of 75 patients. Of this satisfactorily improved group, at least 41% were still in the same condition after 3 years on a maintenance dose of, on average, 0.3 mg/kg body weight etretinate daily and 20 g weekly of a relatively strong corticosteroid cream. The side effects were acceptable and the convenience for the patients is great as compared with other treatment.

Thirty-nine patients with mycosis fungoides in various stages or Sézary syndrome were treated with isotretinoin and 29 with etretinate as single drug therapy. Complete remission within 2 months was obtained with isotretinoin in 8 cases (21%) and partial remission in another 15 cases (38%). An open clinical trial was used to evaluate the synthetic retinoid, etretinate, for treatment of idiopathic seborrhea in Cocker Spaniels, West Highland White Terriers, and Basset Hounds. Clinical and histologic improvement was seen in the Cocker Spaniels. Etretinate had no beneficial effect on the skin disease of the West Highland White Terriers or the Basset Hounds. Etretinate treatment did not alter the type or degree of otitis externa. Clinical side effects were minimal. Relevant laboratory abnormalities were not detected.
