Evidence-based medicine evaluation of electrophysiological studies of the anxiety disorders.

We provide a systematic, evidence-based medicine (EBM) review of the field of electrophysiology in the anxiety disorders. Presently, electrophysiological studies of anxiety focus primarily on etiological aspects of brain dysfunction. The review highlights many functional similarities across studies, but also identifies patterns that clearly differentiate disorder classifications. Such measures offer clinical utility as reliable and objective indicators of brain dysfunction in individuals and indicate potential as biomarkers for the improvement of diagnostic specificity and for informing treatm

An evidence-based medicine strategy for achieving remission in bipolar disorder.

Controlled trials have demonstrated the efficacy of several classes of drugs for achieving acute response in bipolar mania and depression. For many years, clinical response has been the primary outcome in the majority of short-term efficacy studies. However, there is a growing consensus that the optimal goal in the long-term management of bipolar disorder is remission. The purpose of this article is to briefly summarize the clinical importance of remission in bipolar disorder and to review data on the effectiveness of available treatments for achieving and sustaining remission.

Treating the initial anterior shoulder dislocation--an evidence-based medicine approach.

This manuscript presents the best available evidence to answer questions regarding the treatment of the patient with an initial anterior shoulder dislocation. The highest levels of evidence available offer the following conclusions: (1) of the many methods to reduce the dislocated shoulder, little data exist to identify the best method. Recommendations are based on low levels of evidence (levels 4 and 5). (2) Premedication with intra-articular lidocaine has fewer complications and requires a shorter time in the emergency room than intravenous sedation with no detectable differences in reductio

Treatment of lichen planus. An evidence-based medicine analysis of efficacy.

OBJECTIVE: To critically appraise the body of literature concerning treatment of lichen planus (LP). DESIGN: Review of MEDLINE and BIOSIS databases to identify articles published with at least an English abstract before March 1998 that examined treatment of LP. MAIN OUTCOME MEASURES: Forming a primary database on which most recommendations are based. We thus selected 83 clinical trials or small series of patients in the medical literature that referenced clinical data on patients treated for LP. RESULTS: There are no large randomized trials with definitive results in the medical literature

The use of evidence-based medicine in the management of acutely ill children.

OBJECTIVES: To determine the extent to which emergency medicine physicians are exposed to and implement evidence-based medicine (EBM). METHODS: A 2-part, self-administered questionnaire was sent to 375 randomly selected emergency physicians (125 PEM Fellows, 125 AAP Section of Emergency Medicine, 125 American College of Emergency Physicians). The first part focused on the physician's experience with EBM and the second part included 2 clinical scenarios to determine their decision-making process. Respondents were considered either junior physicians (practicing <5 years since residency training

Guidelines for treatment of neonatal jaundice. Is there a place for evidence-based medicine?

UNLABELLED: Treatment of neonatal jaundice continues to be a controversial issue. Arguments that traditional practice results in over-treatment have led to the adoption of more liberal guidelines in some countries. The importation of liberal guidelines from one country to the next, however, is fraught with danger, because differences in epidemiology, sociology and healthcare delivery systems between countries may not be adequately reflected. The unreflected extension of liberalization to non-target groups of patients can expose the latter to significant risk. It is not clear that the evidence

Evidence based medicine on the use of botulinum toxin for headache disorders.

Botulinum toxin blocks the release of acetylcholine from motor nerve terminals and other cholinergic synapses. In animal studies botulinum toxin also reduces the release of neuropeptides involved in pain perception. The implications of these observations are not clear. Based on the personal experiences of headache therapists, botulinum toxin injections have been studied in patients with primary headaches, namely tension-type headache (TTH), chronic migraine (CM) and chronic daily headache (CDH). So far, the results of randomized, double-blind, placebo controlled trials on botulinum toxin in a

Claude Bernard was a 19th century proponent of medicine based on evidence.

The French physiologist Claude Bernard (1813-1878) has the unfair reputation of being ferociously opposed to the use of probabilities and statistics in medicine. In the 19th century, he would have been included among those who opposed the emergence of what would eventually become clinical epidemiology. The truth is that Bernard valued the role of medical statistics in clinical medicine but viewed it as potentially misleading in laboratory-based physiology. He posited that clinical medicine had to be guided by probabilistic evidence as long as physiological mechanisms remained unknown. Bernard

Beyond the Average Patient: The Ergodicity Problem in Evidence-Based Medicine and a Proposal for Individual-Dynamic Medicine.

RATIONALE: Evidence-based medicine (EBM) replaced authority with appraisal and has reduced harm. Its own methodologists have documented a persistent gap between what its methods conclude and what its practice does: average effects that are marginal for most treated patients, guidelines that misfit the individual, widespread difficulty converting relative into absolute effects, and no built-in trigger for stopping a treatment that is not helping. AIMS AND OBJECTIVES: To identify a common source of these problems in the application of population-level estimates to individuals, and to propose a

DeepER-Med: Advancing Deep Evidence-Based Research in Medicine Through Agentic AI.

Trustworthiness and transparency are essential for the clinical adoption of artificial intelligence (AI) in healthcare and biomedical research. Recent deep research systems aim to accelerate evidence-grounded scientific discovery by integrating AI agents with multi-hop information retrieval, reasoning, and synthesis. However, most existing systems lack explicit and inspectable criteria for evidence appraisal, creating a risk of compounding errors and making it difficult for researchers and clinicians to assess the reliability of their outputs. In parallel, current benchmarking approaches rarel

[Lewy body dementia: therapeutic propositions according to evidence based medicine and practice].

Lewy body dementia (LBD) may present with two clinical forms: dementia with Lewy bodies (DLB) and Parkinson's disease with dementia. LBD is characterized by a profound deficiency of acetylcholine and a deficiency of dopamine. The cholinergic deficit is implicated in major attention disorders and fluctuations. Acetylcholinesterase inhibitors (donepezil or rivastigmine) were studied in several double-blind placebo-controlled studies. The meta-analyzes show a moderate benefit on the cognitive level and on some psychiatric manifestations including hallucinations. A disabling parkinsonian syndrome

Emergency Medicine Evaluation and Management of Small Bowel Obstruction: Evidence-Based Recommendations.

BACKGROUND: Small bowel obstruction (SBO) is a commonly diagnosed disease in the emergency department (ED). Recent literature has evaluated the ED investigation and management of SBO. OBJECTIVE: This review evaluates the ED investigation and management of adult SBO based on the current literature. DISCUSSION: SBO is most commonly due to occlusion of the small intestine, resulting in fluid and gas accumulation. This may progress to mucosal ischemia, necrosis, and perforation. A variety of etiologies are present, but in adults, adhesions are the most common cause. Several classification system

Evidence-based medicine: An update on treatments for peritoneal dialysis-related peritonitis.

Peritonitis continues to be a major complication of peritoneal dialysis (PD), and adequate treatment is crucial for a favorable outcome. There is no consensus regarding the optimal therapeutic regimen, and few prospective controlled studies have been published. The objective of this manuscript is to review the results of PD peritonitis treatment reported in narrative reviews, systematic reviews, and proportional meta-analyses. Two narrative reviews, the only existing systematic review and its update published between 1991 and 2014 were included. In addition, we reported the results of a propor
