Implementing immediate postpartum contraception: a comparative case study at 11 hospitals.

BACKGROUND: Immediate postpartum long-acting reversible contraception (LARC) is an evidence-based practice, but hospitals face significant barriers to its adoption. Our objective was to examine how organizational context (e.g., size, employee attitudes toward the clinical practice) and implementation strategies (i.e., the actions taken to routinize a clinical practice) drive successful implementation of immediate postpartum LARC services, with a goal of informing the design of future implementation interventions. METHODS: We conducted a comparative case study of the implementation of inpatien

Paracetamol-associated knowledge, attitudes and practices of the New Zealand public: an online survey.

INTRODUCTION: Although paracetamol is the most widely used prescription and over-the-counter drug globally, it also has a higher toxicity than other common non-opioid analgesics and is a leading preventable cause of acute liver failure and liver transplants. International evidence suggests that the public may have inadequate knowledge about paracetamol; however, in Aotearoa New Zealand (NZ), paracetamol is relatively cheap and readily available in large quantities at pharmacy and non-pharmacy outlets. AIM: To identify paracetamol-associated knowledge, attitudes and beliefs of the NZ public.

How are community pharmacist-led health screening services evaluated? A scoping review using the RE-AIM framework.

BACKGROUND: Community pharmacists increasingly deliver health screening services that expand access to preventive care and early disease detection. However, evaluation practices vary substantially, limiting comparison across studies and the development of implementation-ready evidence. OBJECTIVE: To describe how community pharmacy-based screening services have been evaluated and to propose priorities for more complete, sustainable, and transparent evaluation. METHODS: Medline, APA PsycINFO, and International Pharmaceutical Abstracts were searched for English-language reports published from J

Prevalence and Factors Associated With Pill-Swallowing Difficulties Among Outpatient Pharmacy Attendees at a Malaysian Teaching Hospital: A Cross-Sectional Study.

BACKGROUND: Pill-swallowing difficulty is a common yet under-recognised barrier to effective pharmacotherapy, with potential consequences for adherence, therapeutic efficacy, and patient safety. Malaysian evidence is scarce, particularly in outpatient populations where long-term oral medication use predominates. OBJECTIVE: To determine the prevalence, identify associated factors, and examine the consequences of pill-swallowing difficulties among outpatient pharmacy attendees in a Malaysian teaching hospital. METHODS: A cross-sectional online survey was conducted at the outpatient pharmacy of

Knowledge and Attitudes Regarding Pharmacogenetics of Pharmacists in the Netherlands: A National Online Survey Study.

RATIONALE, AIMS AND OBJECTIVES: Pharmacogenetics is considered a promising method to improve pharmacotherapy, yet its implementation in clinical practice is hampered, limiting its potential benefits to the public. Previously, its uptake has been investigated within a variety of healthcare professional groups. Commonly cited barriers by prescribing physicians are lack of knowledge and insurance coverage. An important occupational group who may support physicians due to their expert knowledge are pharmacists. However, they have been understudied in research on pharmacogenetics implementation thu

A Practice Based Behavioral Health Management Registry (BHMR): Implementation, Structure and Content.

Previous commentaries in the Formulary Evaluation section of INNOVATIONS in Pharmacy have pointed to the difficulties of establishing the credibility of trial-based and modeled claims for therapy interventions. Claims for interventions in behavioral health are no exception. A recent report by The Kennedy Forum emphasizes the need for a system of measurement-based care using validated scales to assess the need for and response to therapy. To accomplish this, The Kennedy Forum proposed a core set of outcomes measures for behavioral health interventions. This core set provides the context for a p

Lessons Learned: Approaches to Delabeling Penicillin Allergy-A Clinical Management Review of Population-Level Strategies.

Penicillin allergy labels are a major barrier to optimal antimicrobial stewardship, yet most reported reactions are inaccurate and can be safely removed after evaluation. This Clinical Management Review article summarizes lessons from population-level delabeling initiatives, highlighting the value of incorporating nonallergist perspectives and practical implementation tools. Effective strategies extend beyond specialist settings and rely on multidisciplinary engagement-including primary care, pharmacy, and nursing-to scale interventions. A practical toolkit offering standardized risk-stratific

Addressing challenges in pediatric thrombosis: a comprehensive guideline development.

BACKGROUND: Pediatric thrombosis is a relatively rare but severe condition in the field of pediatrics, with far-reaching consequences. Recent studies have indicated a rising incidence of this disease in children over the years. Additionally, the pharmacological treatment of thrombotic diseases in children faces numerous challenges. Due to significant physiological differences between children and adults, guidelines for the prevention and treatment of thrombotic diseases in adults cannot be directly applied to pediatric patients. PURPOSE: A systematic review of the existing evidence-based medi

Modernizing antibiotic regimens for obstetric infections through antimicrobial stewardship.

Maternal sepsis remains a major and largely preventable contributor to maternal morbidity and mortality, with intra-amniotic infection and postpartum endometritis remaining common obstetric sources of sepsis. Intra-amniotic infection complicates approximately 2%-5% of term deliveries, and postpartum endometritis occurs in up to 7% of births. Both conditions are typically polymicrobial, arising from ascending infection involving gram-positive, gram-negative, and anaerobic organisms. Despite rising antimicrobial resistance, the recommended antibiotic regimens for obstetric infections have remain
