OBJECTIVES: Motor evoked potentials (MEPs) were recorded in 7 cases of Creutzfeldt-Jakob disease (CJD) to asses the involvement of pyramidal motor pathways in these cases. The diagnosis of CJD was confirmed by autopsy in 5 cases and based on clinical data in two cases.
METHODS: Transcranial (MEP-cortex), root magnetic (MEP-root) and electrical stimulation of peripheral nerves (F-wave, direct M-response) were performed. The cortical excitability threshold, F-wave frequency, MEP amplitudes, peripheral motor conduction velocity, standardized distal latencies and central, root, and F-wave conduct

To document evoked potential changes in brainstem strokes, motor evoked potential (MEP) to abductor digiti minimi and median somatosensory evoked potential (SEP) were studied in 10 patients with medial and four patients with lateral brainstem strokes. All the patients with lateral brainstem stroke had normal MEP; and SEP was abnormal in one only. In medial brainstem stroke patients, MEP was abnormal in seven; unrecordable in 4 and prolonged central motor conduction time in 3 patients. SEP was abnormal in four; unrecordable in 3 and prolonged central sensory conduction in 2 patients. In one of

Motor-evoked potentials (MEPs) from forearm muscles were recorded in response to single-shock electrical stimulation of motor cortex of rats (n = 15) under pentobarbital anesthesia and controlled room air ventilation. In addition, electroencephalograms (EEGs) were recorded for all animals. Following baseline MEP recording in room air (21% O2), animals were subjected to graded hypoxia of either 15.75%, 10.5%, or 5.25% oxygen for 10 minutes, then followed by room air ventilation for 15 minutes. The mean baseline latency, amplitude, and duration of the evoked muscle response were 4.3 +/- 0.4 msec

We are reporting 98 cases of motor evoked potential (MEP) monitoring performed between 1982 and 1986. These were divided into supratentorial, posterior fossa, and spinal cord categories. We observed in this sample that the peripheral nerve or electromyographic response was substantially more sensitive than the spinal cord response to injury and hypotension during operation and was often present bilaterally. Reversible weakening or loss of the peripheral nerve response was not associated with a deficit. However, weakening of the peripheral nerve response without recovery could warn of a motor d

OBJECTIVE: The role of ipsilateral descending motor pathways in voluntary movement of humans is still a matter of debate, with partly contradictory results. The aim of our study therefore was to examine the excitability of ipsilateral motor evoked potentials (iMEPs) regarding site and the specificity for unilateral and bilateral elbow flexion extension tasks.
METHODS: MR-navigated transcranial magnetic stimulation mapping of the dominant hemisphere was performed in twenty healthy participants during tonic unilateral (iBB), bilateral homologous (bBB) or bilateral antagonistic elbow flexion-ext

BACKGROUND: Various modalities have been used to confirm the blood flow through parent arteries or surrounding perforating arteries during surgical aneurysm clipping, including motor-evoked potentials (MEPs), Doppler ultrasound, and indocyanine green videoangiography. Nonetheless, contralateral hemiparesis due to arterial blood flow insufficiency may arise because of false-positive or false-negative errors. By performing controlled intraoperative awakening during aneurysm clipping, we compared patients' voluntary movements with simultaneous MEP.
METHODS: Four patients with anterior choroidal

BACKGROUND: The description of the natural course of recovery from a spinal cord injury (SCI) with spontaneous improvement of neurological, neurophysiological, and functional measures is an important prerequisite in appraising effects of upcoming interventional therapies.
OBJECTIVE: To describe the spontaneous evolution of motor-evoked potentials of the anterior tibial muscle (TA-MEP) and their relation to outcomes of lower extremity motor scores (LEMS) and walking function in patients recovering from an acute SCI.
METHODS: TA-MEPs were assessed in 255 SCI subjects within 5 time intervals th

BACKGROUND: The objective was to evaluate whether motor nervous pathways are affected when patients are treated for childhood acute lymphoblastic leukemia (ALL).
PROCEDURE: Thirty-two children with ALL were studied at the end of treatment by means of motor evoked potentials (MEPs) elicited by magnetic stimulation (MS) transcranially and peripherally and underwent a detailed neurological examination. Thirty-two healthy children matched with them for age, sex, and height served as a control group.
RESULTS: The latencies of the MEPs were significantly prolonged along the entire motor nervous pa

Although theta-burst stimulation (TBS) is known to differentially modify motor cortical excitability according to stimulus conditions in humans, whether similar effects can be seen in animals, in particular rats, remains to be defined. Given the importance of experimental rat models for humans, this study explored this stimulation paradigm in rats. Specifically, this study aimed to explore corticospinal excitability after TBS in anesthetized animals to confirm its comparability with human results. Both inhibition-facilitation configurations using paired electrical stimulation protocols and the

OBJECTIVE: The primary aim of this study was to compare the predictive value of motor evoked potentials (MEPs) and early clinical assessment with regard to long-term hand motor recovery in patients with profound hemiplegia after stroke.
METHODS: The sample was an inception cohort of 39 stroke patients with an acute, ischemic, supratentorial stroke and an initial upper-extremity paralysis admitted to an academic hospital. Hand motor function recovery was defined at 26 weeks poststroke as a Fugl-Meyer Motor Assessment (FMA) hand score>3 points. The following prognostic factors were compared at

Objective We aimed to develop and validate a personalized neural field theory (NFT) model of the motor cortex incorporating recurrent connections from layer 5 to layers 2/3. Methods We extended the established feedforward model of Wilson et al. (2021) by adding recurrent feedback connections. Synaptic coupling coefficients were derived using a bottom-up approach from microanatomical data and calibrated via Particle Swarm Optimization (PSO) against experimental input-output (IO) curves. The model was then personalized for individuals by optimizing three recurrent weighted gains ([Formula: see t

BACKGROUND: Radiology, particularly MRI, is critical for the diagnosis and timely management of spinal cord injury. In 20–25% of patients with clinical signs of myelopathy MR imaging is normal; this can be challenging diagnostically and delay the instigation of life-changing therapeutic interventions. Investigations that specifically probe the physiology of spinal white matter tracts, both the ascending pathways (somatosensory evoked potentials; SEPs) and descending pathways (motor cortical evoked potentials; MEPs), could provide additional information but their application in this context has

OBJECTIVE: Insular gliomas are surgically challenging. Intraoperative neurophysiological monitoring assists surgical excision. Although still controversial, a reduction of Motor Evoked Potential (MEPs) signal>50% correlates with postoperative impairment. However, this reduction often occurs too late. Identifying pre-warning indicators, anticipating MEPs drop>50%, could optimize surgery of insular gliomas.
METHODS: Monocentric retrospective study, collecting clinical, surgical and neurophysiological data of patients harboring insular intra-axial lesions. I) MEPs tracking included the baseline,

Paraplegia remains the most devastating complication following thoracoabdominal aortic surgery. Motor-evoked potential (MEP) monitoring has been widely used to assess intraoperative motor function. MEP amplitude is affected by various factors, including anesthetic agents and measurement time; however, there are no reports regarding MEP monitoring using remimazolam in thoracoabdominal aortic surgery. A 57-year-old woman underwent open repair of a thoracic descending aorta for a chronic dissecting aortic aneurysm under remimazolam and remifentanil anesthesia. The administration rate of remimazol

A schwannoma is a benign nerve sheath tumor treated by enucleation, which carries the risk of intraoperative nerve injury that is observed after awakening. Transcranial motor evoked potential (TcMEP) monitoring has been established as an effective method to predict and prevent intraoperative neurological complications during brain and spinal surgery. However, there have been few reports on its application in head and neck surgeries. We performed enucleation to relieve the symptoms due to schwannomas in the neck of two women, aged 25 years and 70 years. Both women presented with a left cervical

The role of the ipsilaterally descending motor pathways in the recovery mechanisms after unilateral hemispheric damage is still poorly understood. Motor output reorganization was investigated in a 56-year-old male patient with acquired unilateral hemispheric atrophy due to Rasmussen encephalitis. In particular, the ipsilateral corticospinal pathways were explored using focal transcranial magnetic stimulation. In the first dorsal interosseous and wrist extensors muscles, the median amplitudes of the ipsilateral motor evoked potentials induced by transcranial magnetic stimulation in the patient

PURPOSE: To present a case of loss of motor-evoked potentials (MEPs) to the left foot in the supine position after a partial reduction and instrumented fusion from L4 to pelvis which was managed successfully without revision or removal of implants.
METHODS: We report a patient with high-grade spondylolisthesis who demonstrated loss of motor-evoked potentials after posterior spinal fusion and transfer to supine position. The patient's knees were flexed to 90° and signals were immediately restored. Systemic steroids were administered and circumferential fusion was delayed 21 days. Anterior-inte

OBJECTIVE: To evaluate whether spinal cord intraoperative monitoring (IOM) with somatosensory and transcranial electrical motor evoked potentials (EPs) predict adverse surgical outcomes.
METHODS: A panel of experts reviewed the results of a comprehensive literature search and identified published studies relevant to the clinical question. These studies were classified according to the evidence-based methodology of the American Academy of Neurology. Objective outcomes of postoperative onset of paraparesis, paraplegia, and quadriplegia were used because no randomized or masked studies were avai

OBJECTIVE: Myogenic transcranial motor-evoked potentials (tc-MEPs) were applied to monitor spinal cord ischemia in the repairs of thoracoabdominal aortic aneurysms. We investigated whether tc-MEPs after spinal cord ischemia/reperfusion could be used to predict neurologic outcome in leporine model.
METHODS: Tc-MEPs were measured at 30-second intervals before, during, and after spinal cord ischemia (SCI) induced by balloon occlusion of the infrarenal aorta. Twenty rabbits were divided into five groups. Four groups (n = 4 animals in each group) had transient ischemia induced for 10, 15, 20, or 3
