Phenotypic and Endotypic Determinants of Atopic Dermatitis Severity From the Atopic Dermatitis Research Network (ADRN) Registry.

BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin condition with a highly variable clinical phenotype. OBJECTIVE: This study aimed to identify historical and clinical features and biomarkers associated with AD severity. METHODS: A US registry of extensively phenotyped AD participants (aged 0.73-80 years) were enrolled at 9 academic centers. Information on family and personal medical history, examination, skin swabs (culture), and serum biomarkers was collected to evaluate their association with AD severity. RESULTS: Participants with AD (N = 2862) whose disease was categoriz

Self-Injurious Behaviour in SCA17: A New Clinical Observation.

BACKGROUND: Self-injurious behaviour has historically been associated with borderline personality disorder. Nevertheless, over recent years, it has been reported in numerous neurological syndromes, especially hyperkinesias. CASE REPORT: Two cases of SCA17 manifested self-injurious behaviour, namely repetitive scratching of the skin resulting in severe excoriations. In one of them, the abnormal behaviour was associated with the inability to resist the impulse to commit the act along with relief following the damage. DISCUSSION: This is the first report describing self-injurious behaviour in S

Social Anxiety Symptoms and Body-Focused Repetitive Behaviour Severity in Adolescents: A Serial Mediation Analysis of Shame and Emotion Regulation Difficulties.

Body-focused repetitive behaviour disorders (BFRBDs), including trichotillomania (TTM) and skin-picking disorder (SPD), typically emerge during adolescence. These disorders have been conceptualized within emotion regulation models, in which body-focused repetitive behaviours (BFRBs) may function as attempts to regulate aversive emotional states. Although anxiety frequently co-occurs with BFRBDs, its association with BFRB severity is generally modest, highlighting the need to examine affective and regulatory processes. This cross-sectional study examined the associations among social anxiety, s

Improving skin picking diagnosis among Brazilians: validation of the Skin Picking Impact Scale and development of a photographic instrument.

BACKGROUND: Skin picking disorder is a prevalent disorder frequently comorbid with depression and anxiety, which is underdiagnosed mainly by dermatologists. Assessment of skin picking disorder is based on instruments influenced by the awareness about skin picking disorder and comorbid symptoms. To date, there is no validated instrument for Brazilian individuals nor an instrument to evaluate the severity of skin lesions in an objective way. OBJECTIVES: Validate the Skin Picking Impact Scale for Brazilian Portuguese and create a photographic measurement to assess skin lesions. METHODS: The sam

A Hierarchical Personality Approach Toward a Fuller Understanding of Onychophagia and Compulsive Buying.

PURPOSE: Employing a hierarchical model of personality, prior research suggests that cardinal traits such as conscientiousness and agreeability predict central traits such as materialism and need for arousal that in turn impact surface traits such as onychophagia and compulsive buying. More research is needed to explore additional central traits and their effect on onychophagia and compulsive buying. Thus, the goal of the current research is to examine how another central trait-negative perfectionism-impacts onychophagia and compulsive buying. DESIGN/METHODOLOGY/APPROACH: A survey was adminis

Compulsive Skin Picking in Anorexia Nervosa: A Qualitative Study of Emotional and Interoceptive Dysregulation.

BACKGROUND: Body-focused repetitive behaviors (BFRB), including compulsive skin picking, are observed in patients with anorexia nervosa (AN). However, their functional significance remains unclear. AN is characterized by chronic energy restriction and altered interoceptive processing, which may contribute to persistent internal tension beyond overt eating symptoms. This study aimed to explore the functional meaning of compulsive skin picking as a potential behavioral marker of emotional and interoceptive dysregulation relevant to clinical assessment and treatment planning. METHODS: A qualitat

A randomized crossover trial with experience sampling to test placebo effects on pathological skin-picking.

Emotional distress is a typical trigger for pathological skin-picking (PSP). Given that placebo treatments can reduce emotional distress, the present study sought to investigate placebo responsiveness in individuals with PSP. In this randomized crossover trial, 69 participants (90% female, M = 31 years) with self-reported PSP underwent two weeks with daily placebo treatment (pills introduced as 'N-acetylcysteine' to reduce skin-picking) and two weeks without placebo treatment. Participants used a smartphone application to provide daily ratings for their skin-picking symptoms, as well as their

Psychologic trauma, posttraumatic stress disorder, and dermatology.

Psychologic trauma refers to events (such as sexual assault, major earthquake, or plane crashes) that overwhelm an individual's capacity to cope. Psychologic trauma can result in chronic and recurring dermatologic symptoms that persist after the trauma subsides. Examples are cutaneous sensory flashbacks (which may be fragments of the sensory component of the traumatic experience), autonomic hyperarousal (with symptoms such as profuse sweating or flare-up of an underlying stress-reactive dermatosis), conversion symptoms (such as numbness, pain, or other medically unexplained cutaneous symptoms)

[Anxiety, obsessive-compulsive disorder and trauma in the DSM-5].

BACKGROUND: The 12 anxiety disorders which were defined in the DSM-IV, have been increased to 27 disorders in the DSM-5. These 27 disorders have been subdivided into three chapters: 1. anxiety disorders, 2. obsessive-compulsive and related disorders (OCRD) and 3. trauma and stressor-related disorders. AIM: To describe the most important differences between the DSM-IV and DSM-5 classifications of the above-mentioned disorders. METHOD: Survey of the relevant literature. RESULTS: Within the chapter on anxiety disorders in DSM-5 the classifications of 'panic disorder' and 'agoraphobia' have bee

Obsessive-compulsive and related disorder symptoms in the perinatal period: prevalence and associations with postpartum functioning.

Obsessive-compulsive disorder (OCD) symptoms are more likely to develop or be exacerbated during pregnancy and the postpartum period, which can cause significant distress and impairment. However, the disorders grouped with OCD in the DSM-5, obsessive-compulsive and related disorders (OCRD; e.g., hoarding disorder (HD), body dysmorphic disorder (BDD), trichotillomania (TTM), excoriation disorder (ED)), have rarely been examined in the perinatal period. This study aimed to explore (1) the prevalence of all clinically significant OCRD symptoms in pregnancy and the postpartum period and (2) the co

Rates and Clinical Correlates of Cannabis Use in Trichotillomania and Skin Picking Disorder.

Despite rising rates of cannabis use across the United States, and in psychiatric populations especially, relatively limited attention has been paid to rates of cannabis use in individuals with body focused repetitive behaviors (BFRBs) such as trichotillomania and skin picking disorder. Using data from an online survey, we collected rates of past-year cannabis use among individuals with BFRBs and investigated whether cannabis use is associated with the clinical presentation of trichotillomania and/or skin picking disorder. We found that approximately 33% of individuals reported past-year canna

Self-induced dermatoses: A great imitator.

The self-induced dermatoses represent about 2% of dermatology patient visits, and include the recurrent body-focused repetitive behaviors (BFRB) (skin-picking or excoriation disorder, trichotillomania, onychophagia and onychotillomania), dermatitis artefacta, and features of other psychiatric disorders, for example, secondary to excessive grooming in body dysmorphic disorder, skin picking in delusional infestation, or secondary to self-harm in depressive disease. Among the BFRBs, onychophagia and onychotillomania are most likely to be associated with lesions that mimic other dermatologic condi

Use of antipsychotic drugs in dermatology.

Antipsychotic drugs can be beneficial in dermatology because of their both central nervous system and peripheral effects. All antipsychotic drugs have a central postsynaptic dopamine D2 receptor blocking effect, which underlies their antipsychotic action. The antipsychotic drugs have varying degrees of histamine H1-receptor, cholinergic muscarinic receptor, and α1-adrenergic receptor blocking effects, which can affect cutaneous perception and the autonomic reactivity of the skin and can be potentially beneficial in the management of certain histamine or sympathetically mediated dermatologic ma

Treatment of facial scarring and ulceration resulting from acne excoriée with 585-nm pulsed dye laser irradiation and cognitive psychotherapy.

BACKGROUND: Self-inflicted skin ulcers and scars are often observed in patients with compulsive skin picking. The term "neurotic excoriation" has been used to describe this condition and may or may not coexist with other true skin pathologies, such as acne. The condition poses a diagnostic and treatment challenge because patients often also have an undiagnosed underlying psychologic disorder. CASE REPORTS: Two patients with numerous linear and stellate facial ulcers and hypertrophic and erythematous scars were diagnosed with acne excoriée in the setting of an obsessive-compulsive disorder lin

Skin changes in primary psychiatric disorders.

Primary psychiatric disorders where skin changes appear most frequently include: delusions of parasitosis, body dysmorphic disorder, neurotic excoriations, dermatitis artefacta, and trichotillomania. In all these diseases the primary pathologic condition is of psychiatric nature, and the skin changes are secondary and self-induced. In this review we wanted to present the epidemiology, clinical pictures, and treatment options for these disorders. These patients are more frequently seen in dermatology clinics, as they may be unwilling to acknowledge a psychiatric basis for their psychical sympto

The structure of genetic and environmental risk factors for dimensional representations of DSM-5 obsessive-compulsive spectrum disorders.

IMPORTANCE: The new DSM-5 "Obsessive-Compulsive and Related Disorders" chapter contains a series of conditions thought to be etiologically related to obsessive-compulsive disorder (OCD). However, the evidence to support this relatedness remains incomplete. OBJECTIVE: To estimate the degree to which genetic and environmental risk factors are shared and/or unique to dimensionally scored OCD, body dysmorphic disorder (BDD), hoarding disorder (HD), trichotillomania (hair-pulling disorder) (TTM), and excoriation (skin-picking) disorder (SPD). Skin Picking Disorder: A Canadian Retrospective Study of 83 Patients.

BACKGROUND: Skin picking disorder (SPD) is classified as a primary psychodermatologic disorder, in which lesions are self-induced. It is frequently encountered by dermatologists, but the management is still a source of discomfort for the majority. OBJECTIVES: The first objective is to determine the characteristics of the SPD patients in our centre: the demographics, the psychiatric comorbidities, clinical and histopathological characteristics of SPD patients, treatments and follow-up. The second objective is to demonstrate the need for education in the dermatologic community. METHODS: We ret

Efficacy of dupilumab in prurigo nodularis in elderly patient.

Prurigo nodularis (PN) is a chronic disorder, affecting adults, with multiple nodules, typically on the limbs. The treatment is challenging, especially in elderly patients. An 85-year-old woman had developed in the last 3 years itch with nodular lesions and erythematous scaly patches and excoriations. The extension of the lesions was evaluated by body surface area (BSA) score and the patient's itch and disease-related sleep disorders by a Numeric Rating Scale (NRS) from 0 to 10. The Dermatology Life Quality Index (DLQI) and blood chemistry were performed before and during the therapy. At the b
