Navigating existential conversations about life and death with families in paediatric palliative care: a qualitative interview study with healthcare professionals.

BACKGROUND: When a child has a life-threatening illness, guidelines emphasise the importance of providing holistic support to the whole family. Conversations about existential issues are essential in this support, yet staff often lack formal training in communication, particularly when addressing such issues as life and death. This qualitative study explored how paediatric healthcare professionals in Sweden experience and approachconversations about existential issues with children and their families facing life-threatening illness. METHODS: Eight professionals representing different discipli

"You're really walking along the razor's edge": A meta-synthesis on the existential cost of breast cancer related to financial toxicity.

Cancer-related financial toxicity (FT) is a challenge of living with and beyond breast cancer (BC). A systematic review and meta-synthesis was conducted to report the experience of individuals with BC regarding cancer-related FT. Data were searched in MEDLINE, Embase, Cinahl, Scopus, PsycINFO, and Web of Science from inception. Eligibility was restricted to original qualitative studies. We performed a meta-synthesis by generating interpretative themes and a model of cancer-related FT. Thirty-two studies were included, encompassing 1080 individuals with BC. Of these studies, 17 were conducted i

Neurobiological and Existential Profiles in Posttraumatic Stress Disorder: The Role of Serotonin, Cortisol, Noradrenaline, and IL-12 Across Chronicity and Age.

Posttraumatic Stress Disorder (PTSD) is characterized by disruptions in central nervous system functioning and existential crises, yet the mechanistic links between neurobiological processes and dimensions of life meaning and identity remain underexplored. The aim of this study was to examine the relationships between stress biomarkers (serotonin, cortisol, noradrenaline, and interleukin-12 [IL-12]) and existential attitudes (measured using the Life Attitude Profile (Revised) [LAP-R]) in mining rescuers, considering PTSD duration and participant age. This cross-sectional study included 92 men

Meditation and psychedelics facilitate similar types of mystical, psychological, and philosophical-existential insights predictive of wellbeing: a qualitative-quantitative approach.

Both psychedelic substances and meditation have been proposed to facilitate personally meaningful and transformative experiences, with insights playing a central role. However, previous research has mainly relied on questionnaires, limiting the range of insights that can be identified. In this study, we recruited participants who provided narrative reports of insights in personally meaningful psychedelic (n = 147) or meditation (n = 66) experiences. Psychedelic experiences were facilitated both by classic (e.g., LSD, psilocybin, DMT) as well as non-classic (e.g., MDMA, ketamine, cannabis) psyc

Suffering and Salutogenesis: A Conceptual Analysis of Lessons for Psychiatry From Existential Positive Psychology (PP2.0) in the Setting of the COVID-19 Pandemic.

The COVID-19 pandemic has had a widespread effect on the thoughts, emotions and behavior of millions of people all around the world. In this context, a large body of scientific literature examining the mental health impact of this global crisis has emerged. The majority of these studies have framed this impact in terms of pre-defined categories derived from psychiatric nosology, such as anxiety disorders, depression or post-traumatic stress disorder. These constructs often fail to capture the complexity of the actual experiences of the individuals being studied; more specifically, they describ

Being acknowledged by others and bracketing negative thoughts and feelings: Frail older people's narrations of how existential loneliness is eased.

AIMS AND OBJECTIVES: The aim of this study was to describe how EL was eased, as narrated by frail older people. BACKGROUND: Existential loneliness (EL) is an unavoidable part of the human condition. It is a complex phenomenon that has been described as disconnection from life. If EL is acknowledged in the care of older people, the experience of EL can be reduced. DESIGN: In this qualitative study, we used an exploratory and descriptive design. METHODS: The study was based on 22 narrative interviews with frail older people, 76 to 101 years old, who were receiving long-term care and services.

Existential suffering and the extent of the right to physician-assisted suicide in Switzerland: Gross v Switzerland [2013] ECHR 67810/10.

In Gross v Switzerland, the European Court of Human Rights held by 4-3 majority that Switzerland had violated the right to decide when and how to die included in the right to respect for private and family life under Article 8 of the European Convention on Human Rights. To comply with the ruling, Switzerland must issue guidance detailing the circumstances (if any) under which physicians may lawfully prescribe lethal medication to competent individuals who have a voluntary and settled wish to die, yet whose suffering is not the product of a medical condition likely to result in death in the nea

Multimedia-assisted narrative intervention for existential distress among patients with advanced cancer: a one-group pre-post study.

PURPOSE: Patients with advanced cancer often experience challenges related to life meaning, coping confidence, and death-related concerns, which may impair quality of life. This preliminary one-group quasi-experimental pre-post study examined whether meaning in life, terminal self-efficacy, and death anxiety changed after a multimedia-assisted narrative psychosocial intervention. The uncontrolled design was intended to identify preliminary within-participant signals and cannot establish intervention efficacy. METHODS: A one-group quasi-experimental pre-post study was conducted in three hospic

Psilocybin-assisted Existential, Attachment and RelationaL (PEARL) therapy for patients with advanced cancer: protocol for a multi-method feasibility trial.

BACKGROUND: Individuals with advanced cancer often experience high levels of distress for which there are few standardized treatment approaches. Our multidisciplinary team has combined existing evidence-based approaches into Psilocybin-assisted Existential, Attachment, and RelationaL (PEARL) therapy. PEARL therapy combines elements from psilocybin-assisted psychotherapy, including preparatory therapy sessions, a high-dose psilocybin session, and integration sessions, with important elements from evidence-based psychotherapies designed for patients with advanced cancer. METHOD: This open-label

Lived Experiences of Learning to Use a Long Cane: The Importance of Integrating Perceptual, Existential, and Social Dimensions for Active Cane Use.

How do people who are blind or visually impaired experience learning to use a long cane? This question is of paramount importance for planning and delivering rehabilitation programs and orientation and mobility (O&M) training. Until now, research into learning to use a long cane has focused primarily on technical and professional aspects, paying little attention to the lived experience of the learning activities that are offered in the field of O&M. This extensive qualitative study adopts a lifeworld phenomenological approach and sets out to examine the pedagogical processes within rehabilitat

Working with chronic and relentless self-hatred, self-harm, and existential shame: a clinical study and reflections (Paper 2 of 2).

This second of two papers focuses on the shame which emerged in the first 14 years of analysis of a woman who was bulimic, self-harmed, and repeatedly described herself as 'feeling like a piece of shit'. To explore this intense and pervasive shame I draw on Jung's and Laplanche's emphasis on experiences of unresolvable, non-pathological 'foreignness' or 'otherness' at the heart of the psyche. Images, metaphors, elements of clinical experience, and working hypotheses from a number of analytic traditions are used to flesh out this exploration. These include Kilborne's use of Pirandello's image o

Meaning-Centered Psychotherapy for Existential Distress and Demoralization in Advanced Cancer: A Narrative Review of Clinical Evidence and Implications for End-of-Life Care.

Existential distress and demoralization are common among patients with advanced cancer and are associated with impaired psychological well-being, reduced quality of life and difficulties in end-of-life adjustment. Meaning-Centered Psychotherapy (MCP), grounded in Viktor Frankl's logotherapy, was developed to address these existential dimensions of suffering. This narrative review synthesizes evidence from randomized and controlled studies evaluating MCP in adults with advanced cancer and discusses its clinical relevance within supportive and palliative care. Across studies, MCP demonstrated co

Addressing Existential Distress Through Compassion: Feasibility and Preliminary Effects of Compassion With You (COMwithU), a Compassion-Based Intervention for Patients With Cancer.

BACKGROUND: Cancer-related distress extends beyond anxiety and depression to existential suffering, including loss of meaning, helplessness, uncertainty, and relational strain. Compassion-based interventions in psycho-oncology are promising, but existential outcomes such as demoralization and spiritual well-being have rarely been examined. Compassion With You (COMwithU) is a brief, five-session compassion-based intervention that centers on cultivating self-compassion, supported by the therapist's compassionate presence and by practices of extending compassion toward others. METHODS: This sing

Living Well With Uncertainty in Advanced, Metastatic or Incurable Cancers: A Pragmatic Feasibility Study of the Adapting to Life With Cancer Cognitive ExisteNtial Therapy (ACCENT).

BACKGROUND: New treatments are contributing to individuals living longer with advanced, metastatic, or incurable (AMI) cancers. The impact of these treatments is unpredictable, resulting in considerable uncertainty for these patients. Currently, there are no interventions that effectively address uncertainty in AMI cancers. To fill this gap, we designed the Adapting to life with Cancer Cognitive ExisteNtial Therapy (ACCENT) intervention. AIM: To evaluate the feasibility, acceptability, and preliminary efficacy of ACCENT in AMI cancers. METHODS: ACCENT was delivered online for 6 consecutive w

"One must know things, but above all, one must know how to be": an existential-phenomenological study of professional identity in palliative care.

BACKGROUND: Palliative care-while overlapping with but distinct from end-of-life care-involves multidisciplinary teams providing medical, psychosocial, and existential support to patients and families facing serious illness. While prior research has examined professional identity in palliative care, the existential dimensions of identity formation-what it means to be a palliative care professional-remain largely underexplored. AIM: To explore the lived experiences of palliative care professionals, focussing on how their professional identity is constructed through the existential notion of "b

From existential uncertainty to a new mindset promoting recovery: Exploring the development of uncertainty experience in women with vulvar neoplasia - A qualitative longitudinal study.

BACKGROUND: Women with vulvar neoplasia continue to experience uncertainty up to six months post-surgery. Uncertainty in illness is considered a significant psychosocial stressor, that negatively influences symptom distress, self-management strategies and quality of life. According to the Reconceptualized Uncertainty in Illness Theory, the appraisal of uncertainty changes positively over time in chronic illness. We aimed at exploring whether and how the experience of uncertainty develops in women with vulvar neoplasia. METHODS: We selected a purposive sample of seven women diagnosed with vulv

An update on the myriad antifungal resistance mechanisms in dermatophytes and the place of experimental and existential therapeutic agents for Trichophyton complex implicated in tinea corporis and cruris.

INTRODUCTION: There is an epidemic emergence of increased resistance in dermatophytes with to antifungal drugs with ergosterol1 (Erg1) and Erg11 mutations to terbinafine and azoles. Apart from mutations, mechanisms that predict clinical failure include efflux pumps, cellular kinases, heat shock proteins (Hsp), and biofilms. Apart from itraconazole and SUBATM (Super-Bioavailable) itraconazole, measures that can be used in terbinafine failure include efflux-pump inhibitors, Hsp inhibitors and judicious use of antifungal drugs (topical + systemic) combinations. Beyond the Boundaries of the Psyche: An In-Depth Examination of the Recovery Processes From Existential Vacuum and Coping Strategies of Psychologically Traumatised Individuals-A Qualitative Study.

AIM: This study aimed to gain an in-depth understanding of the posttraumatic search for meaning and inner transformation experiences of psychologically traumatised individuals by examining their coping strategies and recovery from the existential vacuum through a phenomenological approach. INTRODUCTION: It is known that trauma creates not only a psychological but also an existential crisis in individuals and triggers a search for meaning; however, studies on how this process is reflected in the subjective experiences of individuals are limited. METHOD: The study was conducted with a qualitat

Death anxiety, life's meaninglessness, and mental resilience among women with symptoms of behavioral addictions and alcohol use disorder: Using the existential approach.

In recent decades, a growing body of research has emphasized the unique nature of substance abuse among women, necessitating a gender-specific approach and thus individualized therapeutic interventions. The purpose of this study, based on the existential approach, was to examine whether symptoms of several behavioral addictions (shopping, sex, gambling and betting, eating) and alcohol use disorder correlate with death anxiety, lack of meaning in life, and levels of mental resilience in a convenience sample of women. A total of 3,176 women participated in this online study and completed a demogr
