Comprehensive anatomical dissection procedure with special reference to the layer-structured facial muscles and fasciae and mouth floor.

OBJECTIVES: Conventional head and neck practice procedures fail to address several clinical issues. Anatomical structures were removed from the surface layers in that order, and the dissection was shifted to the deeper layer. This approach makes it difficult for medical and dental students to understand the relationships among various anatomical structures. Furthermore, it is difficult to determine the risk on the mandibular nerves and blood vessels during conventional mandible removal procedures. This study aims to develop an anatomical technique that maintains the skin, superficial facial mu

Macroscopic Anatomy of the Layered Structures of Facial Muscles and Fasciae in the Temporal-Malar-Mandible-Neck Region.

The layered structures of facial muscles and their topographical relationship with facial fasciae are still not fully understood. This study aimed to clarify the layered structures of facial muscles and fasciae in the temporal-malar-mandible-neck region. Thirty-four human cadavers were examined during gross anatomy courses at Niigata University (2017-2020). The face was composed of 3-layered (deep, middle, and superficial) fasciae and 4-layered facial muscles (first superficial, second superficial, third, and fourth muscle layers) according to the attachment of muscles and their topographical

Mechanical stimulation of paralyzed vibrissal muscles following facial nerve injury in adult rat promotes full recovery of whisking.

Many patients suffer lifelong disabilities after peripheral nerve injury. Insufficient recovery has been attributed to excessive axonal branching, axonal regrowth to improper targets and polyneuronal reinnervation of motor endplates. We used the rat facial nerve transection/suture model to quantify the effects of mechanical stimulation on the paralyzed whisker musculature. "Manual" stimulation involved briskly stroking the whiskers by hand in a manner that specifically mimicked normal whisker movement. "Environmental" stimulation involved enhanced whisker use as rats encountered objects in an

Facial asymmetry with severe unilateral hypoplasia of the muscles of mastication: a report of clinical and electromyographic findings.

Mild degrees of asymmetry of the human body have been recognised both by classical sculptors and more recently by anatomists as both normal and widely occurring. Morphologic asymmetries of the face of a more severe nature are, however, quite rare. When such deformities occur, both the hard and soft tissues may be involved and the abnormality may be acquired as a result of trauma, infection, neoplasia or surgery or be of a developmental origin as in the syndromes affecting the first pharyngeal arch. Developmental asymmetries of the jaws may also arise as a result of unilateral disruption of man

Electromyographic activities of the jaw and facial muscles in subjects with different vertical skeletal patterns and breathing modes.

BACKGROUND: Mouth breathing (MB) can affect morphological changes in the craniofacial structures, electromyography is widely used for quantitative analysis of muscle function. OBJECTIVE: The aim was to evaluate the electromyographic (EMG) activities of the anterior temporalis (TA), masseter muscle (MM), orbicularis oris superior (OOS) and mentalis muscle (MT) in children with different vertical skeletal patterns and breathing modes during rest and various functional mandibular movements. METHODS: BioEMG III was used to measure the variations in EMG activities of TA, MM, OOS, and MT in 185 su

High-resolution surface electromyographic activities of facial muscles during mimic movements in healthy adults: A prospective observational study.

OBJECTIVES: Surface electromyography (sEMG) is a standard tool in clinical routine and clinical or psychosocial experiments also including speech research and orthodontics to measure the activity of selected facial muscles to objectify facial movements during specific facial exercises or experiments with emotional expressions. Such muscle-specific approaches neglect that facial muscles act more as an interconnected network than as single facial muscles for specific movements. What is missing is an optimal sEMG setting allowing a synchronous measurement of the activity of all facial muscles as

Ultrasound Imaging of the Facial Muscles and Relevance with Botulinum Toxin Injections: A Pictorial Essay and Narrative Review.

High-resolution ultrasound is preferred as the first-line imaging modality for evaluation of superficial soft tissues, such as the facial muscles. In contrast to magnetic resonance imaging and computed tomography, which require specifically designated planes (axial, coronal and sagittal) for imaging, the ultrasound transducer can be navigated based on the alignment of facial muscles. Botulinum toxin injections are widely used in facial cosmetic procedures in recent times. Ultrasonography is recognized as a useful tool for pre-procedure localization of target muscles. In this pictorial review,

Optimizing muscle inset in free functional muscle transfer for facial reanimation: Technical refinements for efficiency and stability.

Free functional muscle transfer is a well-established technique for dynamic facial reanimation; however, the inset phase remains technically demanding due to limited operative space and time constraints related to ischemia. We describe a series of technical refinements focusing on muscle preparation, suture organization, and inset strategy. Key elements include determining muscle length in the neutral facial position, use of preplaced multilayer fixation sutures, creation of a load-distributing mesh at the muscle ends, and systematic organization of sutures using numbered instruments to preven

Facial Electrical Muscle Stimulation for Soft-tissue Lifting and Symmetry Enhancement.

BACKGROUND: Facial neuromuscular electrical stimulation (NMES) has been used to support facial muscle recruitment in functional settings; however, evidence for aesthetic applications remains limited. This exploratory case series descriptively documents soft-tissue appearance changes and treatment tolerance following a standardized anatomical treatment approach with individualized stimulation settings targeting the lip elevator complex and orbicularis oculi. METHODS: Seven adults (5 women, 2 men; age range 24-39 y) underwent 10 NMES sessions at 1-2 week intervals using a commercially available

Occipitalis muscle: using for repetitive facial nerve stimulation in myasthenia gravis.

Repetitive nerve stimulation (RNS) is a highly sensitive electrophysiological test used for diagnosing myasthenia gravis (MG). Here, we evaluated electrodiagnostic value of RNS using facial nerve and occipitalis muscle. Patients with generalized MG were included. Clinical findings were assessed. RNS test was performed on occipitalis, nasalis, trapezius, abductor policis brevis (APB) and abductor digiti minimi (ADM) muscles. Twenty-five (78%) patients had relevant decrement during RNS test of at least one muscle. Nasalis muscle had the highest diagnostic potential (68.8%), followed by trapezius

Use of the masseter motor nerve in facial animation with free muscle transfer.

Facial paralysis is either congenital or acquired, and of varying severity, which leads to an asymmetrical or absent facial expression. It is an important disability both from the aesthetic and functional points of view. Between 2003 and 2008, at the Department of Maxillofacial Surgery, University of Parma, Italy, 21 patients with facial paralysis had their faces reanimated with a gracilis transplant reinnervated by the masseter motor nerve. All free-muscle transplants survived the transfer, and no flap was lost. Facial symmetry at rest and while smiling was excellent or good in most cases, an

Development of an intraoral device for facial muscle retraining and its clinical application.

The aim of this work was to develop a facial neuromuscular retraining technique for cases of facial palsy, involving an easy-to use intraoral device that allows correcting abnormal neuromuscular patterns and improving esthetics. Facial palsy is a motor alteration of multiple origin that results in facial asymmetry. Treatment remains controversial to date and includes, surgical anastomosis and decompressive and plastic surgery, corticosteroids therapy, injection of botulinum toxin, and administration of other medicinal drugs. Physical therapy involving electrical stimulation and feedback are al

[The restoration of chronic facial paralysis with a modified technique of sternocleidomastoid muscle transposition].

OBJECTIVE: To evaluate a modified technique of sternocleidomastoid muscle transposition for the dynamic reanimation of the chronic facial paralysis. METHODS: Both of the sternal and cleidus extremities of the sternocleidomastoid muscle were elevated from their bony attachment. The mastoid extremity was left in the place as a pedicle for the blood supply. The muscle stripes were then transposed into the face through the tunnel under the skin and the ends were sutured to the opposite orbicularis oris muscle of the upper and lower lips to correct the deformity of the mouth and nose. RESULTS: Fo

[Transposition of pedicled sternocleidomastoid muscle for repair of facial paralysis in late stage].

OBJECTIVE: To explore a new surgical approach to repair facial paralysis in late stage, using regional transposition of pedicled sternocleidomastoid muscle for the dynamic reanimation of the paralyzed face. METHODS: Seven cases with facial paralysis in late stage from December 1999 were treated and followed up for 10 months before clinical evaluation. In all of the cases, the sternal and clavicular branches of the sternocleidomastoid muscle were both elevated from their bony attachments, with the mastoid insertion left in situ as the pedicle for blood supply and accessory nerve maintained in

[Rehabilitation of facial paralysis by temporal muscle flap and implantation of gold weights].

Facial paralysis is a severe disability that often produces major ocular disorders, cosmetic deformities, and, in many cases, functional incapacity for something as characteristic of human beings as facial expression. For these reasons, it is necessary to correct this defect as completely as possible to produce the best physiological result. Of the many methods available for the repair of the lesion, most authors agree that direct repair of the nerve is the most reliable technique. When this is not feasible, the use of gold weights for the eyelid and a temporalis muscle flap for the mouth are

Temporalis Muscle Transposition in Irreversible Facial Nerve Palsy:‎ A Vestibular Approach.

Background The facial nerve plays a crucial role in innervating the motor supply of facial muscles, enabling essential facial expressions that facilitate human communication. Defects or damages to this nerve can have significant consequences, leading to functional, emotional, and social difficulties caused by the immobility of facial muscles. Patients suffering from irreversible facial nerve palsy often experience functional symptoms such as eyelid closure defects, mouth deviation, and limited movement. Methods This study aims to address these challenges and offer potential solutions for patie
