Congenital combined deficiency of coagulation factors VII and X--different genetic mechanisms.

Combined coagulation factor VII (FVII) and factor X (FX) deficiency (combined FVII/FX deficiency) belongs to the group of bleeding disorders in which both factors show reduced plasma activity. It may arise from coincidental inheritance of separate coagulation factor deficiencies or a common cause as large deletions comprising both gene loci. The F7 and F10 genes are located on the long arm of chromosome 13. Here, we describe 10 cases with combined FVII/FX deficiency representing both genetic mechanisms of occurrence. Genetic analyses included direct sequencing of the F7 and F10 genes and MLPA

The use of activated recombinant coagulation factor VII during haemarthroses and synovectomy in a patient with congenital severe factor V deficiency.

Factor V deficiency is a rare hereditary bleeding disorder. Currently, FV concentrates are not available, and the treatment of spontaneous bleeding or bleeding associated with invasive procedures is transfusion of fresh frozen plasma (FFP). However, FFP transfusion can lead to the development of inhibitor to FV, and is associated with several potential transfusion reactions including allergic reactions. We report a patient with congenital severe FV deficiency with repeated haemarthroses of a shoulder joint, and progressively severe allergic reactions to FFP transfusions. In addition, the patie

Spontaneous Splenic Rupture in a Patient With Combined Congenital Factor XIII and Factor VII Deficiencies: A Case Report.

Spontaneous (atraumatic) splenic rupture (SSR) is an uncommon and potentially fatal emergency, most frequently linked to infection, cancer, or hematologic disorders. Although clotting factor XIII (FXIII) and factor VII (FVII) deficiencies are uncommon bleeding disorders on their own, their coexistence and association with spontaneous splenic rupture have not been reported before.  A 26-year-old adult male with known congenital FXIII deficiency, epilepsy, right-sided hemiparesis, and developmental delay presented with diffuse abdominal pain and vomiting. He was first evaluated at another hospit

Anesthetic management with labor epidural analgesia of the parturient with severe factor VII deficiency: a case report.

BACKGROUND: Factor VII deficiency is considered a contraindication to neuraxial anesthesia due to the risk of an epidural hematoma. CASE REPORT: A 32 year old G1P0 parturient with severe factor VII deficiency presented for an anesthesiology consultation at 32 weeks gestation. Initial coagulation studies were significant for an elevated INR (2.0) and a low factor VII level of 6%. After interdisciplinary discussion, it was decided that neuraxial analgesia could be offered if her coagulation studies corrected after administration of recombinant activated factor VII (rFVIIa). The patient presente

Analysis of clinical manifestations and molecular pathogenesis of six patients with hereditary blood coagulation factor VII deficiency.

BACKGROUND: Hereditary blood coagulation factor VII (FVII) deficiency is a rare hemorrhagic disorder inherited in an autosomal recessive pattern, involving variants in the gene encoding FVII (F7). The sites and types of F7 mutations could influence the coagulation activity of plasma FVII (FVII: C) and the severity of hemorrhage symptoms. However, the specific molecular mechanisms of FVII deficiency are still unclear. OBJECTIVE: To analyze clinical manifestations and coagulation functions of six patients of hereditary FVII deficiency and explore specific molecular mechanisms of the disease. M

Double heterozygous mutations Gln100Leu and His348Gln of the F7 gene in a patient with factor VII deficiency.

A 25-year-old Chinese woman who had a history of easy bruising was admitted to hospital due to uncontrolled epistaxis. She showed factor VII activity level of 2% and factor VII antigen level of 4% of the normal value. We detected a novel missense mutation g.8355 A>T (p.Gln100Leu) in the second epidermal growth factor-like (EGF) domain and a g.11482 T>G (p.His348Gln) in the catalytic domain. Although the Gln100 residue is close to the junction of EGF-2 domain with the serine protease domain, we infer that the substitution of polar negatively charged Gln residue at the position 100 with introduc

Coagulation Factor VII Deficiency: A Review of the Clinical Implications and Approaches to Replacement Therapy and Prophylaxis.

Factor VII (FVII) deficiency is a rare bleeding disorder that can present as either inherited or acquired. Inherited FVII deficiency arise from mutations in the F7 gene, resulting in a highly heterogeneous and wide range of phenotypic expressions from asymptomatic individuals to those experiencing severe and life-threatening hemorrhagic episodes. Notably, the severity of symptoms is typically not correlated with plasma FVII levels. Diagnosis generally involves a combination of coagulation assays, including prothrombin time (PT), assessment of FVII activity and antigenic levels, and a thorough

Acquired Factor VII Deficiency Presenting with Bleeding Diathesis in a 52-Year-Old Black Man in Botswana.

BACKGROUND: Factor VII is one of the vitamin K-dependent coagulation factors synthesized in the liver and has a short circulating half-life of 4 - 5 hours. METHODS: We report a case of a 52-year-old black man who presented with life-threatening bleeding from multiple sites. RESULTS: We determined that it was caused by acquired factor VII deficiency of less than 5%. He had a septic pelvic focus which was managed empirically with antibiotics. The bleeding was stopped by fresh frozen plasma and factor VII plasma levels gradually increased to normal levels over the course of 4 months. CONCLUSIO

[Combined hereditary deficiency in factors VII and X revealed by a prolonged partial thromboplastin time].

BACKGROUND: Congenital factors VII and X deficiency is rare. Association of both deficiencies is exceptional. CASE REPORT: A 3 year-old boy, born to consanguinous Moroccan parents, had a prolonged partial thromboplastin time discovered fortuitously. This finding led to the diagnosis of combined factors VII and X deficiency. His siblings had the same deficiencies. CONCLUSION: Profound deficiencies in factors VII and X are inherited following an autosomal-recessive mode. These deficiencies may be asymptomatic, only discovered by prolonged partial thromboplastin time. They may also be revealed

Successful treatment of acquired factor VIII deficiency in a child using activated factor VII concentrates: case report and review of the literature.

Acquired factor VIII deficiency is a rare life-threatening disorder that should be suspected in individuals without a prior bleeding history who present with mucous membrane, muscle, and/or urinary tract bleeding. The authors describe a 5-year-old girl with epistaxis, intramuscular bleeding, and forearm compartment syndrome requiring emergent fasciotomy. Coagulation studies showed a factor VIII level of less than 1%. The prolonged activated partial thromboplastin time corrected immediately when mixed with normal plasma at a 1:1 ratio but became prolonged again following incubation at 37 degree

A Phenotype-driven strategy for bilateral VATS in severe factor VII deficiency: securing hemostasis without complete parameter correction.

BACKGROUND: Perioperative management for severe Factor VII (FVII) deficiency in thoracic surgery typically aims to reduce bleeding risk by correcting coagulation parameters with factor replacement—a strategy limited by cost, thrombotic risk, and availability. This report describes a phenotype-driven strategy, guided by a prior uneventful invasive procedure, that enabled safe surgery without routine factor replacement, offering a viable approach despite profound laboratory abnormalities. CASE PRESENTATION: A 58-year-old male with severe congenital FVII deficiency (FVII: C < 8%) and symptomatic

Factor VII deficiency: Unveiling the cellular and molecular mechanisms underlying three model alterations of the enzyme catalytic domain.

Activated factor (F) VII is a vitamin K-dependent glycoprotein that initiates blood coagulation upon interaction with tissue factor. FVII deficiency is the most common of the rare congenital bleeding disorders. While the mutational pattern has been extensively characterized, the pathogenic molecular mechanisms of mutations, particularly at the intracellular level, have been poorly defined. Here, we aimed at elucidating the mechanisms underlying altered FVII biosynthesis in the presence of three mutation types in the catalytic domain: a missense change, a microdeletion and a frameshift/elongati
